A history of exercise programs, including receipts for memberships in health clubs

Many carriers also require a nutritional consult and psychological evaluation.

A number of carriers now require detailed documentation of participation in a physician-supervised diet.

Most require the submission of at least six months' worth of office notes from the supervising physician, including proof of dietary supervision and recorded weigh-ins.

Gastric Bypass Procedures• Roux-en-Y gastric bypass• Laparoscopic adjustable gastric band• Sleeve gastrectomy

• The bypass typically causes more weight loss, has a greater impact on diabetes, but it also has a greater risk associated with it, and so as a result, for people who need less of an operation, they need a less severe operation and a less effective operation, then the band is the way to go.

• For people who need more of an operation, the bypass is the way to go, and maybe the sleeve gastrectomy is something in between.

Sleeve Gastrectomy• Several surgeons worldwide have adopted the

procedure and have offered it to low BMI and low risk patients as an alternative to laparoscopic banding of the stomach.

• It generates weight loss by restricting the amount of food (and therefore calories) that can be eaten by removing 85% or more of the stomach without bypassing the intestines or causing any gastrointestinal malabsorption.  It is a purely restrictive operation.  It is currently indicated as an alternative to the Lap-Band® procedure for low weight individuals and as a safe option for higher weight individuals.

Advantages of Laparoscopic Gastric Sleeve Resection

• Laparoscopic gastric sleeve resection is simpler than gastric bypass surgery. Unlike gastric bypass, this procedure does not re-route the passage of food.

• The procedure is less likely to cause complications such as the malabsorption of vitamins.

• It allows for weight loss similar to what is experienced with gastric bypass surgery.

• It allows for the resolution or cure of medically related illnesses that are secondary to obesity.

• Most insurance companies cover this procedure

Disadvantages of Laparoscopic Gastric Sleeve Resection• Although the procedure has achieved excellent

results, it is the newest of the bariatric procedures and has a history of about three years.

• Because a long, narrow tube is created out of the stomach, this creates resistance and limits the amount of food that a patient can swallow at any one sitting.

• Rare complications can include blockages in the gastric sleeve or post-operative leaks from the staple line.
