Overview of Internship:
• Two-Year, Compensated Experience • Begins summer following PY1 year

Goal Statement:
“By the end of this internship, a Clinic-Based Ambulatory Care intern will be provided exposure to the complexities of Specialty and Primary Care (SPC) ambulatory care team practice sites and the role of a Clinical Pharmacist Practitioner (CPP) in a unique clinic setting, which may have a specialty pharmacy component.”
The intern will be able to:● Effectively collaborate with a CPP along with other
healthcare providers● Confidently educate and counsel patients● Advance clinical research skills ● Explore various areas of ambulatory care pharmacy

Clinical Pharmacist Practitioner (CPP):
• A CPP is a licensed pharmacist approved to provide drug therapy management, including controlled substances, under the direction of, or under the supervision of a licensed physician
• Only a pharmacist approved by the Pharmacy Board may legally identify himself/herself as a CPP

Year by Year Commitment:● Each intern is designated to a clinic as a longitudinal experience throughout the 2-year program
● Start: May/July following PY1 year (depending on immersion schedule) ● End: April of PY3 year● Time Commitment:
○ PY2/PY3: 8-12 hours/week○ Immersion: 16 hours/month○ Summer: 40 hours/week (full-time)

Ellen Montgomery - PY2:
• Primary Patient Population: Family Medicine (Chronic Conditions)
Intern Duties
• Engage in direct/indirect care• Identify areas for pharmacy
intervention in the clinic for short-term/long-term projects
• Attend monthly CPP meetings and take minutes
• Participate in AmbCare POD• Train new intern in the summer• Recruit and assist in
application process for internship
Clinic-Specific Duties
• Review patient charts and participate in CPP visits
• Maintain frequent follow-up with high-risk patients
• Conduct medication reviews and education
• Improve the clinic’s quality metric for uncontrolled COPD by MMRC assessments and inhaler counseling
• Coordinate with population health team

• Primary Patient Population: Gastroenterology (Hepatitis C Virus)
Intern Duties
• Specialty Clinic: Provide back-up assistance in refill calls to UNC SSC Assist patients with resources and access to treatment & care– in person, via phone, via email
• Longitudinal Research Projects• Assist other CPPs with duties/projects
Clinic-Specific Duties
• Assess appropriate HCV treatment option given patient characteristics and history
• HCV Clinical Initial & Follow-up Assessment
• HCV medication side effect management
• CoverMyMeds: Completing PA requests/extensions
