IntroductionThis Provider and Pharmacy Directory includes information about the provider and pharmacy types in UCare's MSHO and listings of all the plan’s providers and pharmacies as of the date of this Directory. The listings contain provider and pharmacy address and contact information as well as other details, as applicable, such as days and hours of operation, specialties, and skills. Key terms and their definitions appear in alphabetical order in the last chapter of the Member Handbook.

UCare's MSHO will accept all eligible people who choose the Plan. American Indians can continue or begin to use tribal and Indian Health Services (IHS) clinics. We will not require prior approval or impose any conditions for you to get services at these clinics. For elders age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, we will not require you to see your primary care provider prior to the referral.

Complaints must be in writing and filed within 180 days of the date you discovered the alleged discrimination.The complaint must contain your name and address and describe the discrimination you are complainingabout. l This Directory lists health care professionals(such as doctors, nurse practitioners, andpsychologists), and facilities (such as hospitals orclinics) that you may see as a UCare's MSHOmember. ~B. Background Information about UCare's MSHO's Network Providers

B1. Key Terms

This section explains key terms you’ll see in ourProvider and Pharmacy Directory.

l Providers are health care professionals andsupport providers such as doctors, nurses,pharmacists, therapists, and other people whoprovide care and services. Services includemedical care, long-term services and supports(LTSS), supplies, prescription drugs, equipmentand other services.

− The term providers also includes facilitiessuch as hospitals, clinics, and other placesthat provide medical services, medicalequipment, and long-term services andsupports.

− Providers that are a part of our plan’s networkare called network providers.

l Network providers have contracted with us todeliver services to members of our plan. Networkproviders participate in our plan. That meansthey accept members of our plan and provideservices our plan covers. The providers in ournetwork generally bill us directly for care theygive you. When you see a network provider, youusually pay nothing for covered services.

l A Primary Care Provider (PCP) is a medicalprofessional who gives you routine health care.Your PCP will keep your medical records and getto know your health needs over time.

l Specialists are doctors who provide health careservices for a specific disease or part of the body.There are many kinds of specialists. Here are afew examples:

− Oncologists care for patients with cancer.

− Cardiologists care for patients with heartconditions.

− Orthopedists care for patients with certainbone, joint, or muscle conditions.

l You also have access to a Care Coordinator. ACare Coordinator helps you manage yourmedical providers and services.

B2. How to choose a Primary Care Provider

(PCP)

You can get services from any provider who is in ournetwork and accepting new members.

When you become a member of our plan, you willchoose a Primary Care Provider who is part of ourplan network. This provider will be your PrimaryCare Provider.

If you did not choose a Primary Care Provider orwould like to make a change to your Primary CareProvider, go to the list of providers in section C1 thatstarts on page 15 and choose a provider:

l that you use now, or

l who has been recommended by someone youtrust, or

l whose offices are easy for you to get to.

l If you want help choosing a PCP, please callCustomer Service at the number at thebottom of this page.

l If you have questions about whether we willpay for any medical service or care that youwant or need, call Customer Service and askbefore you get the service or care.

B3. How to get long-term services and

supports (LTSS)

You may be able to get long-term services andsupports (LTSS), such as services provided by theElderly Waiver (EW) program as a UCare's MSHOmember. LTSS help people who need assistance todo everyday tasks like taking a bath, getting dressed,making food, and taking medicine. Most of theseservices are provided at your home or in yourcommunity but could be provided in a nursing homeor hospital.

After you enroll, you will be assigned a UCare CareCoordinator who will work with you to assess yourneeds and preferences, develop and maintain a careplan, and help coordinate your care and services.

Your Care Coordinator will help you identify specificLTSS in your care plan that, if you qualify, will helpyou meet your care goals. Your Care Coordinatorwill also help you identify your needs as youtransition between levels of care.

If you are getting LTSS when you enroll, you will getthese services through your UCare CareCoordinator. If you reside in the community and arenot getting LTSS when you enroll, your firstscreening will assess your need for these services. Ifyou are eligible, your Care Coordinator will workwith you to get these services.

You must have a Long-Term Care Consultation(LTCC) done and be found to be eligible to getadditional services or support. You can ask to havethis assessment in your home, apartment, or facilitywhere you live.

Your care coordinator will meet with you and yourfamily to talk about your care needs if you call to askfor a visit.

Your care coordinator will give you informationabout community services, help you find services tostay in your home or community, and help you find

services to move out of a nursing home or otherfacility.

If you need transition planning and coordinationservices to help you move to the community, youmay be eligible to get Moving Home Minnesota(MHM) services. MHM services are separate fromEW services, but you must be eligible for EW.

If you are eligible to receive Home and CommunityBased Services (HCBS) you must use providers whoare enrolled with DHS for HCBS/waiver services. Acomplete list of enrolled providers is available onDHS’s MnHelp website. Provider lists are alsoavailable by accessing the DHS licensing look-upwebsite or the Minnesota Department of Health(MDH) Health Care Facility and Provider Database.

UCare contracts directly for some extended waiverservices, such as transportation, personal careassistants (PCA) and home health services coveredthrough the waiver as “extended” services. PCA,Home Health Care or durable medical equipment(DME) provider lists are available through theSearch Network tool on UCare’s website.

Providers must obtain a UCare provider number anduse it when billing for waiver services. Providersmust limit the waiver services they deliver to servicesfor which they are qualified and contracted toprovide. Providers who had a contract with UCarethat included Elderly Waiver Services must stillfollow the re-enrollment process that DHS created toprovide services to UCare members.

B4. How to access UCare's MSHO’s network

providers

You must get all of your covered services fromproviders within our network. If you go to providers

who are not in UCare's MSHO network (withoutprior authorization or approval from us), you willhave to pay the bill.

A prior authorization is an approval fromUCare's MSHO before you can get a specificservice, drug, or see an out-of-networkprovider. UCare's MSHO may not cover theservice or drug if you don’t get approval.

The exceptions to this rule are when you need urgentor emergency care or dialysis and cannot get to aprovider in the plan, such as when you are away fromhome. You can also go outside the plan for othernon-emergency services if UCare's MSHO gives youpermission first.

l You may change providers within thenetwork at any time during the year. If youhave been going to one network provider,you do not have to keep going to that sameprovider.

l UCare's MSHO works with all the providersin our network to accommodate the needs ofpeople with disabilities. If you need to see aprovider and are not sure if they offer theaccommodations you need, UCare's MSHOcan help you. Talk to your care coordinatorfor assistance.

B5. How to find UCare's MSHO providers in

your area

Turn to the Table of Contents that starts on page 7 tosearch for network providers. You can searchaccording to the type of provider you need: UrgentCare Clinics, Nursing Homes, Primary Care Clinics,Health Care Homes, Visiting Clinic Teams, etc.Under each provider type, providers are organizedalphabetically by state, county, and city.

How to access out-of-network providers

In some cases, you may get covered services fromout-of-network providers that are not part of UCare'sMSHO network. You must get a prior authorizationfrom us in order for the services from anout-of-network provider to be covered. Exceptionsto out-of-network restrictions include services suchas emergencies, post-stabilization, urgently neededservices when our network is not available,out-of-area renal dialysis services, and open accessservices.

If you do not get a prior authorization from us whenneeded, the bill may not be paid. If an out-of-network provider bills you directly, submit the bill toUCare, and we will determine if the services arecompletely covered. State and federal laws preventus from paying you back directly for most medicalbills. The exception is if you pay for Part Dprescription drugs. If you paid for a service that youthink we should have covered, or for moreinformation on out-of-network providers, pleasecontact Customer Service.

How to access specialty providers

UCare allows members of our plan to have directaccess to specialty providers in our network withouta referral. Each time you get health services, check tobe sure the provider is a network provider.

We encourage you to work with your primary caredoctor to assess your health needs and recommend aspecialty provider. If you see a specialty providerwithout consulting your primary care doctor, it isimportant to tell your primary care doctor about thespecialty doctor visit.

PCPOs and home care agencies are not listed in thisProvider Directory. Call our Customer ServiceDepartment to get more information on how to get alist of PCPOs and home care agencies. You can alsoget this information at ucare.org.

How to find a network pharmacy

The Pharmacy section that lists pharmacy providersis located in Section E.

How to determine whether a facility is

accessible

Providers were asked to complete a questionnaireabout accessibility in three areas: parking, entry ways

and other relevant spaces, and office exam roomsand equipment. Responses to the questionnaire areincluded in the provider listings.

If you need more information about access options,you can obtain information by calling CustomerService at the number at the bottom of this page.This could include, but is not limited to, informationsuch as availability of flexible hours or wheelchairaccess.

How to access Interpreter Services

Hearing and language interpreter services areavailable to help you get services. Oral interpretationcan be given to you in your language. You may beable to get written information in your language. Formore information call Customer Service.
