Dear Mr. Secor: Per your request, ACC Environmental Consultants, Inc. (ACC) performed limited asbestos surveys of The Central Plant, Med Sci 1B, Olson Hall and Wright Hall, located at University of California, One Shields Avenue, Davis, California, in October and November of 2013. The surveys of the referenced sites were performed to identify suspect asbestos-containing building materials and loose & peeling lead-containing paint that may be impacted during the planned preliminary investigation project. Common suspect asbestos-containing building materials that may be impacted by typical projects may include but are not limited to, the following materials:
• Flooring & Associated Adhesives • Baseboard & Associated Adhesives • Gypsum Wallboard, Taping/Joint Compounds, & Texturing Compounds • Plaster Wall & Ceiling Systems • Spray Applied Acoustical Material • Fireproofing • Pipe, Pipe Elbows & Pipe Fitting Insulation • Boiler & Flue Insulation • HVAC Duct Insulation • HVAC Joint Mudding & Duct Tape • Sink Undercoating • Asbestos-cement (Transite) Flues, Panels & Shingles • Ceramic Tile Grout and Underlayment • Interior & Exterior Stucco • Brick Mortar • Roofing Materials
Building components not associated with the planned investigation project were not sampled. Furthermore, limited lead sampling was conducted to identify suspect lead-containing coatings that may be disturbed by project activities for the purpose of compliance with Cal-OSHA’s Lead in Construction Standard and is not intended to

Asbestos Bulk Sample Results
Mr. Chris Yama, a Cal/OSHA Certified Asbestos Consultant (CAC #98-2356) with ACC, performed bulk sampling of suspect asbestos-containing materials during October and November of 2013. Two Hundred ninety-eight (298) samples of suspect materials were submitted for analysis. Copies of the laboratory results are attached. The samples were delivered to Forensic Analytical Laboratories, of Hayward, California; an independent laboratory that participates in the bulk sample proficiency analysis program conducted by the United States Environmental Protection Agency (EPA) and is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP). The samples were analyzed using Polarized Light Microscopy (PLM) with dispersion staining to estimate percent composition by volume. Samples with less than 1% (<1%) asbestos are designated as “Trace asbestos.” Samples with no observable asbestiform minerals are designated as “no asbestos detected.”

1EPA’s NESHAPS regulations define categories of asbestos-containing materials (ACM) based on their potential of asbestos fiber release when disturbed:
• Friable - Any material containing more than 1 percent asbestos that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure.
• Category I Non-friable ACM (Cat 1 NF) - Asbestos-containing packings, gaskets, resilient floor covering and asphalt roofing products containing more than 1 percent asbestos.
• Category II Non-friable ACM (Cat II NF) - Any material, excluding Category I non-friable ACM containing more than 1 percent asbestos as determined using the methods specified under AHERA, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
2OSHA’s Asbestos in Construction Standard (Federal - 29 CFR 1926.1101 and California – 8 CCR 1529) define specific “Classes” of work based on the risk of exposure to employees with the potential for disturbance of asbestos-containing materials. The classes of work are defined as
• Class 1 - Asbestos-related activities involving the removal of thermal systems insulation (TSI) and surfacing ACM or presumed ACM.
• Class 2 - Asbestos-related activities involving the removal of ACM, which are not TSI, or surfacing ACM. Based on the sample results, the planned investigation project will impact asbestos-containing materials. The final scope of work should be reviewed against the asbestos survey information. If asbestos-containing materials will be disturbed during the projects, licensed asbestos abatement contractors working under standard University of California asbestos program requirements should perform all work. A review of all other asbestos survey information available for the property in conjunction with these results should be conducted prior to proceeding with project activities and when a change in the project scope is developed. If suspect asbestos-containing materials will be impacted that are not addressed in this survey or in the historical survey records, additional sampling should be conducted prior to disturbance. Historical records (typically predating 1995) have limited value for project planning and should be verified with confirmatory inspection and additional sampling.
Lead Sample Results Mr. Chris Yama, a California Department of Public Health Lead Inspector/Assessor/Monitor (I/A/M-3814) with ACC, collected fifty-five (55) bulk samples to establish lead-paint concentration for clean up and disposal requirements. Samples with detectable amounts of lead must be properly removed and disposed of according to local, state and federal regulations. Lead sampling was conducted to identify suspect lead-containing coatings that

The bulk samples were obtained from suspect lead-containing building materials identified at the building. Paint colors and/or descriptions are identified based on the surface color observed by ACC at the time of the survey and does not necessarily identify paint descriptions underlying the surface coat. The sample was delivered to Forensic Analytical Laboratories, of Hayward, California; an independent American Industrial Hygiene Association (AIHA) accredited laboratories, for analysis and was analyzed by Atomic Absorption (AA) Spectroscopy in accordance with the EPA 3050B/7420 Method. The colors, locations, and lead contents of these paints are listed below.

*Quantity for “paint” reflects approximate area of loose & peeling only, not all painted surfaces Of the fifty-five (55) samples collected, ten were reported to contain lead above 0.5%, 5,000 parts per million (weight by weight), or 1.0 mg/cm2 which is the definition for lead based paint by the Environmental Protection Agency (EPA) and the California Department of Public Health (DPH). Of the fifty-five samples taken forty-three samples were found to have detectable amounts of lead. The OSHA Lead in Construction Standard requires the use of special work practices during the disturbance of paint with any detectable amounts of lead. See attached OSHA Lead Regulation Summary. Lead containing waste materials with a concentration greater than 0.1%, for total lead, is considered hazardous waste in the State of California. Lead containing waste materials with a total lead concentration between 0.005% (50 ppm) and 0.10% (1000 ppm) must be re-analyzed using the waste extraction test (WET) method to determine the soluble lead content for waste disposal requirements. The EPA – Renovation, Repair and Painting Final Rule (40 CFR 745) requires that renovations conducted for compensation (where lead-based paint will be disturbed), must be performed by Certified Firms using Certified Renovators following the requirements set forth in the regulation. Contractors are also required to notify the Division of Occupational Safety and Health (DOSH) prior to disturbing greater than 100 square feet or l00 linear feet of material containing lead greater than 0.5% by weight, 5000 parts per million (ppm) or 1.0 milligram per square centimeter (mg/cm2).
OSHA Lead Regulation Summary
The Federal Occupational Safety and Health Administration (OSHA), has enacted a lead standard, which was adopted by the Cal/OSHA as 8 CCR 1532.1. The purpose of both standards is to protect construction workers from exposure to lead. OSHA is primarily concerned with activities that disturb paints with any detectable amounts of lead. Lead was used in most paints until the mid 1950’s and was banned in amounts in excess of 0.06% by weight in 1978 for most non-industrial paints by the Consumer Product Safety Commission (CPSC). The Cal/OSHA standard requires contractors and employers to notify the State of California Division of Occupational Safety and Health (DOSH) prior to disturbing greater than 100 square feet or 100 linear feet of material containing lead greater than 0.5%, 5,000 parts per million (weight by weight), or 1.0 mg/cm2. The Cal/OSHA standard also requires contractors and employers who perform paint removal activities to monitor their employees to determine whether they are being exposed in excess of the action level of 30 micrograms per cubic meter of air (µg/m3) over an eight-hour time weighted average (TWA) or the “Permissible Exposure Limit” (PEL) of 50 µg/m3 TWA. Monitoring is performed by personal air sampling.
UC Davis Seismic V; Asbestos & Lead Paint Surveys UC Davis DCM November 2013 Page 15 of 18 Even when concentrations are below the action level, an employer must provide employees with High Efficiency Particulate Air (HEPA) filtered vacuums, wetting agents and hand-washing facilities. If the exposure exceeds the action level or the PEL, other procedures such as containing the area, local exhaust ventilation, respiratory and worker protection, worker training, decontamination facilities and medical monitoring are required. OSHA has identified several work practices that pose varying levels of lead exposure to laborers disturbing lead-containing paint. Estimated exposure levels of lead are founded on the activity itself, rather than the concentrations of lead present in paint. Therefore, as an example, paint that contains 0.5% versus 15% of lead by weight or 0.8 mg/cm2 versus 3.5 mg/cm2 of lead in paint could pose the same exposure levels to workers depending on the activities that cause the disturbance and the administrative and engineering controls that are followed. The following is a summary of work activities that disturb paint, the expected exposure and the respiratory protection requirements that result as outlined in the OSHA standards:

Polychlorinated Biphenyls (PCB) Sampling ACC conducted sampling of caulking typically suspected to contain Polychlorinated Biphenyls (PCBs). The samples were delivered to EMSL, an independent accredited laboratory for analysis by the EPA 8082 Method. Copies of the laboratory results are attached for reference. PCBs are mixtures of synthetic organic chemicals with the same basic chemical structure and similar physical properties ranging from oily liquids to waxy solids. Due to their non-flammability, chemical stability, high boiling point and electrical insulating properties, PCBs were used in hundreds of industrial and commercial applications including electrical, heat transfer, and hydraulic equipment; as plasticizers in paints, plastics and rubber products; in pigments, dyes and carbonless copy paper and many other applications. More than 1.5 billion pounds of PCBs were manufactured in the United States prior to cessation of production in 1977. PCBs have been demonstrated to cause a variety of adverse health effects, including cancer in animals. PCBs have also been shown to cause a number of serious non-cancer health effects in animals, including effects on the immune system, reproductive system, nervous system, endocrine system and other health effects. Studies in humans provide supportive evidence for potential carcinogenic and non-carcinogenic effects of PCBs. The different health effects of PCBs may be interrelated, as alterations in one system may have significant implications for the other systems of the body. The proper identification and handling of PCB-contaminated equipment and wastes is critical to the prevention of

All equipment or waste containing PCBs should be properly labeled, alerting people to the requirement for special handling procedures. While procedures may vary depending on the industry or specific operation, as a general rule, any time there is a risk of contact with PCBs, appropriate protective equipment should be worn to limit contact with the skin and eyes and to protect against inhalation of PCB fumes. Such equipment may include plastic or rubber gloves, boots, overalls, aprons, face shields or self-contained breathing apparatus. For workers cleaning up a major spill containing high concentrations of PCBs, a full suit of non-porous material should be worn. Clothing that has become contaminated should be disposed of along with other PCB wastes. PCB manufacture, use, storage and disposal are regulated by U.S. EPA under TSCA and Part 761, Title 40 of the Code of Federal Regulations (40 CFR Part 761). TSCA regulates any materials or wastes that contain PCBs at concentrations of 50 ppm (parts per million) or greater. PCB wastes are also regulated as hazardous waste by DTSC under the Health and Safety Code (HSC) and Title 22 of the California Code of Regulations (22 CCR). Criteria for determining PCB wastes are:

Limitations ACC conducted the limited survey with the standard of care ordinarily exercised by qualified and reputable members of the environmental/industrial hygiene profession based on conditions and practices observed at the property and information provided to ACC related to the project and/or purpose of the survey at the time of the investigation. The survey was limited to specific project areas and was not intended to identify all suspect asbestos-containing materials within the building. Areas and materials not included in the survey should be inspected and sampled prior to any renovation, maintenance, demolition or other activity that may cause disturbance to the materials. This report does not intend to identify all hazards or unsafe practices, nor to indicate that other hazards or unsafe conditions do not exist at the property. ACC encountered the following inaccessible areas in addition to general concealed conditions (i.e. within wall cavities, above/below solid ceilings or flooring/sub-flooring materials, etc.) and are excluded from the scope of the survey. These areas should be inspected and any suspect materials and sampled accordingly prior to any renovation, maintenance, demolition or other activity that may cause disturbance to the materials. • No inaccessible areas/equipment were identified within the provided project areas Materials that would require intrusive or destructive sampling were generally not sampled as part of the project unless written direction was provided to ACC to perform intrusive and/or destructive sampling on specific building systems, the area was unoccupied at the time of the survey and by performing intrusive/destructive sampling would not create an unsafe condition. Furthermore, ACC shall not be responsible for identifying and/or sampling suspect materials concealed within walls, columns, beneath flooring, above solid ceilings, underground or in any other concealed areas. ACC shall not be responsible for identification, sampling and/or characterization of lead-containing materials, PCB and lighting/mercury wastes, and water or mold impacted materials. General observations may be noted if ACC observed suspect conditions to the client either separately or within this report. ACC excludes sampling concrete and asphalt paving as suspect asbestos-containing materials. Aggregate found in these materials may contain asbestos if supplied from quarries located in known ultra-mafic areas. It is possible that prior to recycling and/or disposal, recycling agents or landfills may require sampling of these materials to determine the presence of asbestos prior to acceptance.
ACC excludes characterization of soils in areas on known ultramafic rock (where naturally occurring asbestos may be found in soils) as part of the scope of work. If the project area is located within a known ultramafic rock area, provisions should be made to address regulatory requirements for any planned excavation and grading as part of the project. ACC can provide further detail on regulatory requirements related to naturally occurring asbestos in soils.

The analyzing laboratory quantifies asbestos concentrations by calibrated visual estimation using standard PLM methodology, with detection of asbestos is material/matrix dependent. Detection of trace asbestos (<1%) may not be reliable or reproducible by PLM and percentage of asbestos weight cannot be determined with standard PLM methodology. Confirmation of asbestos concentrations within complex matrices (i.e. plaster, gypsum wallboard/taping/joint compounds, stucco, resilient flooring, roofing) or when asbestos concentrations are 1% or less may warrant additional analysis by PLM point counting, gravimetric reduction or Transmission Electron Microscopy for proper characterization of asbestos-containing materials and/or waste-stream analysis. This report is prepared for the express use of University of California at Davis, its agents and employees. The NESHAP Final Rule does not define the preparation method for multi-layered samples. In order to determine the composite quantity ofasbestos, the volume percent of each layer is determined, the asbestos containing layers are analyzed by point counting and the composite quantity ofasbestos is calculated. The NESHAP Final Rule can not be applied to matrices that dissolve in refractive index liquid. This includes tar, mastic oradhesive typically found on the back of floor tiles. According to the NESHAP Final Rule, point count data is only necessary when the visual estimateof asbestos is below 10%.

(ACC) performed a comprehensive asbestos and lead paint survey of the above referenced site from January 13th 2014 through February 7th 2014. The survey of the site was performed to identify suspect asbestos-containing building materials, loose & peeling lead-containing paint, mercury light tubes, mercury thermostat switches, PCB-light ballasts, and PCB caulkings that may be impacted during the planned Renovation/Demolition project. Common suspect asbestos-containing building materials that may be impacted by typical renovation/demolition projects may include but are not limited to, the following materials:

ACC conducted the survey of the building according to the scope of work as outlined in the proposal dated December 8, 2013. Building components not associated with the planned Renovation/Demolition project were not

Furthermore, lead sampling was conducted to identify suspect lead-containing coatings that may be disturbed by project activities for the purpose of compliance with Cal-OSHA’s Lead in Construction Standard and is not intended to be a “Lead Inspection” or “Lead Risk Assessment” as defined by the California Department of Public Health (CDPH). ACC did not conduct destructive sampling (i.e. opening wall cavities, removal of sub-floor assemblies, etc.) and as such, if any planned renovation, maintenance or demolition activities will impact concealed materials, additional investigation and sampling should be conducted prior to disturbance of these systems.
Asbestos Bulk Sample Results
Mr. Chris Yama, a Certified Asbestos Consultant (#98-2356) with ACC, performed bulk sampling of suspect asbestos-containing materials from January 13th 2014 through February 7th 2014. One hundred ninety two samples of suspect materials were submitted for analysis. Copies of the laboratory results are attached. The samples were delivered to Forensic Analytical Laboratories, of Hayward, California, an independent laboratory that participates in the bulk sample proficiency analysis program conducted by the United States Environmental Protection Agency (EPA) and is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP). The samples were analyzed using Polarized Light Microscopy (PLM) with dispersion staining to estimate percent composition by volume. Samples with less than 1% (<1%) asbestos are designated as “Trace asbestos.” Samples with no observable asbestiform minerals are designated as “no asbestos detected.”

• Friable - Any material containing more than 1 percent asbestos that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure.
• Category I Non-friable ACM (Cat 1 NF) - Asbestos-containing packings, gaskets, resilient floor covering and asphalt roofing products containing more than 1 percent asbestos.
• Category II Non-friable ACM (Cat II NF) - Any material, excluding Category I non-friable ACM containing more than 1 percent asbestos as determined using the methods specified under AHERA, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
2OSHA’s Asbestos in Construction Standard (Federal - 29 CFR 1910.126 and California – 8 CCR 1529) define specific “Classes” of work based on the risk of exposure to employees with the potential for disturbance of asbestos-containing materials. The classes of work are defined as

Asbestos Sampling Conclusions & Recommendations Based on the sample results, the planned Renovation/Demolition project will impact asbestos-containing materials. The final scope of work should be reviewed against the asbestos survey information. If asbestos-containing materials will be disturbed during the projects, all work should be performed by licensed asbestos abatement contractors working under standard asbestos program requirements. A review of all other asbestos survey information available for the property in conjunction with these results should be conducted prior to proceeding with project activities and when a change in the project scope is developed. If suspect asbestos-containing materials will be impacted that are not addressed in this survey or in the historical survey records, additional sampling should be conducted prior to disturbance. Historical records (typically predating 1995) have limited value for project planning and should be verified with confirmatory inspection and additional sampling as necessary prior to project planning.
Lead Sample Results Mr. Chris Yama, a California Department of Public Health Lead Inspector/Assessor/Monitor (I/A/M-3814) with ACC, collected twenty-six (26) bulk samples to establish lead-paint concentration for clean up and disposal requirements. Samples with detectable amounts of lead must be properly removed and disposed of according to local, state and federal regulations. Lead sampling was conducted to identify suspect lead-containing coatings that may be disturbed by project activities for the purpose of compliance with Cal-OSHA’s Lead in Construction Standard and is not intended to be a “Lead Inspection” or “Lead Risk Assessment” as defined by the California Department of Public Health. The bulk samples were obtained from suspect lead-containing building materials identified at the building. Paint sampling was limited to major paint colors on interior and exterior walls, including the predominant trim color throughout the building and may not represent all accent or trim colors found at the property. Paint colors and/or descriptions are identified based on the surface color observed by ACC at the time of the survey and does not necessarily identify paint descriptions underlying the surface coat. The samples were delivered to Forensic Analytical Laboratories, of Hayward, California; an independent American Industrial Hygiene Association (AIHA) accredited laboratory, for analysis. Samples were analyzed by Atomic Absorption (AA) Spectroscopy in accordance with the EPA 3050B/7420 Method. The colors, locations, and lead contents of these paints are listed below.

Of the twenty-six samples collected, fourteen (14) were found to have detectable amounts of lead. The OSHA Lead in Construction Standard requires the use of special work practices during the disturbance of paint with any detectable amounts of lead. See OSHA Lead Regulation Summary below. Lead containing waste materials with a concentration greater than 0.1%, for total lead, is considered hazardous waste in the State of California. Lead containing waste materials with a total lead concentration between 0.005% (50 ppm) and 0.10% (1000 ppm) must be re-analyzed using the waste extraction test (WET) method to determine the soluble lead content for waste disposal requirements. The EPA – Renovation, Repair and Painting Final Rule (40 CFR 745) requires that renovations conducted for compensation (where lead-based paint will be disturbed) in Target Housing or Child-Occupied facilities, must be performed by Certified Firms using Certified Renovators following the requirements set forth in the regulation. Contractors are also required to notify the Division of Occupational Safety and Health (DOSH) prior to disturbing greater than 100 square feet or l00 linear feet of material containing lead greater than 0.5% by weight, 5000 parts per million (ppm) or 1.0 milligram per square centimeter (mg/cm2).
OSHA Lead Regulation Summary
The Federal Occupational Safety and Health Administration (OSHA), has enacted a lead standard, which was adopted by the Cal/OSHA as 8 CCR 1532.1. The purpose of both standards is to protect construction workers from exposure to lead. OSHA is primarily concerned with activities that disturb paints with any detectable amounts of lead. Lead was used in most paints until the mid 1950’s and was banned in amounts in excess of 0.06% by weight in 1978 for most non-industrial paints by the Consumer Product Safety Commission (CPSC). The Cal/OSHA standard requires contractors and employers to notify the State of California Division of Occupational Safety and Health (DOSH) prior to disturbing greater than 100 square feet or 100 linear feet of material containing lead greater than 0.5%, 5,000 parts per million (weight by weight), or 1.0 mg/cm2. The Cal/OSHA standard also requires contractors and employers who perform paint removal activities to monitor their employees to determine whether they are being exposed in excess of the action level of 30 micrograms per cubic meter of air (µg/m3) over an eight-hour time weighted average (TWA) or the “Permissible Exposure Limit” (PEL) of 50 µg/m3 TWA. Monitoring is performed by personal air sampling. Even when concentrations are below the action level, an employer must provide employees with High Efficiency Particulate Air (HEPA) filtered vacuums, wetting agents and hand-washing facilities. If the exposure exceeds the action level or the PEL, other procedures such as containing the area, local exhaust ventilation, respiratory and worker protection, worker training, decontamination facilities and medical monitoring are required. OSHA has identified several work practices that pose varying levels of lead exposure to laborers disturbing lead-containing paint. Estimated exposure levels of lead are founded on the activity itself, rather than the concentrations of lead present in paint. Therefore, as an example, paint that contains 0.5% versus 15% of lead by weight or 0.8 mg/cm2 versus 3.5 mg/cm2 of lead in paint could pose the same exposure levels to workers depending on the activities that cause the disturbance and the administrative and engineering controls that are followed. The following is a summary of work activities that disturb paint, the expected exposure and the respiratory protection requirements that result as outlined in the OSHA standards:

Polychlorinated Biphenyls (PCB) Sampling
Mr. Chris Yama with ACC conducted sampling of caulkings typically suspected to contain Polychlorinated Biphenyls (PCBs) on February 12, 2014. Three (3) samples were collected and delivered to EMSL Analytical, Inc., of San Leandro, California, an independent accredited laboratory for analysis by the EPA 8082 Method. Copies of the laboratory results are attached for reference. PCBs are mixtures of synthetic organic chemicals with the same basic chemical structure and similar physical properties ranging from oily liquids to waxy solids. Due to their non-flammability, chemical stability, high boiling point and electrical insulating properties, PCBs were used in hundreds of industrial and commercial applications including electrical, heat transfer, and hydraulic equipment; as plasticizers in paints, plastics and rubber products; in pigments, dyes and carbonless copy paper and many other applications. More than 1.5 billion pounds of PCBs were manufactured in the United States prior to cessation of production in 1977. PCBs have been demonstrated to cause a variety of adverse health effects, including cancer in animals. PCBs have also been shown to cause a number of serious non-cancer health effects in animals, including effects on the immune system, reproductive system, nervous system, endocrine system and other health effects. Studies in humans provide supportive evidence for potential carcinogenic and non-carcinogenic effects of PCBs. The different health effects of PCBs may be interrelated, as alterations in one system may have significant implications for the other systems of the body. Any single, unique, well-defined chemical compound in the PCB category is called a "Congener". The name of a congener specifies the total number of chlorine substituents and the position of each chlorine. PCBs were manufactured as a complex mixture of congeners, through progressive chlorination of batches of Biphenyl until a certain target percentage of Chlorine by weight was achieved. Commercial mixtures with higher percentages of Chlorine contained higher proportions of the more heavily chlorinated congeners, but all congeners could be expected to be present at some level in all mixtures. Once released into the environment and subjected to "weathering", or taken in by plants or animals and partially stored/metabolized/excreted, substantial changes in the congener ratios occurred, and continue to occur. Thus, determination of the parent mixture(s) ultimately resulting in a given environmental sample may be difficult or impossible. While PCB was manufactured and sold under many names, the most common were the "Aroclor" series, in many of which a numerical identifier included the percentage of Chlorine (e.g., "Aroclor 1254", with 54 percent Chlorine). Laboratory reports typically reflect equivalent Aroclor series for each sample. The proper identification and handling of PCB-contaminated equipment and wastes is critical to the prevention of future waste management problems. All equipment or waste containing PCBs should be properly labeled, alerting people to the requirement for special handling procedures. While procedures may vary depending on the industry or specific operation, as a general rule, any time there is a risk of contact with PCBs, appropriate protective equipment should be worn to limit contact with the skin and eyes and to protect against inhalation of PCB fumes.

For workers cleaning up a major spill containing high concentrations of PCBs, a full suit of non-porous material should be worn. Clothing that has become contaminated should be disposed of along with other PCB wastes. PCB manufacture, use, storage and disposal are regulated by U.S. EPA under TSCA and Part 761, Title 40 of the Code of Federal Regulations (40 CFR Part 761). TSCA regulates any materials or wastes that contain PCBs at concentrations of 50 ppm (parts per million) or greater. PCB wastes are also regulated as hazardous waste by DTSC under the Health and Safety Code (HSC) and Title 22 of the California Code of Regulations (22 CCR). Criteria for determining PCB wastes are: • total threshold limit concentration (TTLC) of 50 ppm of PCBs, and/or • soluble threshold limit concentration (STLC) of 5 ppm of PCBs as oily liquid.

Areas and materials not included in the survey should be inspected and sampled prior to any renovation, maintenance, demolition or other activity that may cause disturbance to the materials. This report does not intend to identify all hazards or unsafe practices, nor to indicate that other hazards or unsafe conditions do not exist at the property. ACC encountered the following inaccessible areas in addition to general concealed conditions (i.e. within wall cavities, above/below solid ceilings or flooring/sub-flooring materials, etc.) and are excluded from the scope of the survey. These areas should be inspected and any suspect asbestos-containing materials sampled for the presence of asbestos prior to any renovation, maintenance, demolition or other activity that may cause disturbance to the materials. Inaccessible Areas
• No inaccessible areas/equipment were identified within the provided project areas Materials that would require intrusive or destructive sampling were generally not sampled as part of the project unless written direction was provided to ACC to perform intrusive and/or destructive sampling on specific building systems, the area was unoccupied at the time of the survey and by performing intrusive/destructive sampling would not create an unsafe condition. Furthermore, ACC shall not be responsible for identifying and/or sampling suspect materials concealed within walls, columns, beneath flooring, above solid ceilings, underground or in any other concealed areas. ACC shall not be responsible for identification, sampling and/or characterization of water or mold impacted materials. General observations may be noted if ACC observed suspect conditions to the client either separately or within this report. ACC excludes sampling concrete and asphalt paving as suspect asbestos-containing materials. Aggregate found in these materials may contain asbestos if supplied from quarries located in known ultra-mafic areas. It is possible that prior to recycling and/or disposal, recycling agents or landfills may require sampling of these materials to determine the presence of asbestos prior to acceptance.
ACC excludes characterization of soils in areas on known ultramafic rock (where naturally occurring asbestos may be found in soils) as part of the scope of work. If the project area is located within a known ultramafic rock area, provisions should be made to address regulatory requirements for any planned excavation and grading as part of the project. ACC can provide further detail on regulatory requirements related to naturally occurring asbestos in soils. Quantities identified may not represent entire quantities of each material in the building based on the scope of the survey. The analyzing laboratory quantifies asbestos concentrations by calibrated visual estimation using standard PLM methodology, with detection of asbestos is material/matrix dependent. Detection of trace asbestos (<1%) may not be reliable or reproducible by PLM and percentage of asbestos weight cannot be determined with standard PLM methodology. Confirmation of asbestos concentrations within complex matrices (i.e. plaster, gypsum wallboard/taping/joint compounds, stucco, resilient flooring, roofing) or when asbestos concentrations are 1% or less may warrant additional analysis by PLM point counting, gravimetric reduction or Transmission Electron Microscopy for proper characterization of asbestos-containing materials and/or waste-stream analysis.

The aforementioned documents are associated with Olson Hall HVAC System Upgrades, Phase II, in Room 0040, Basement Mechanical Room (University Project Number 2503800). Two Rivers Demolition commenced site work associated with the aforementioned project on June 16, 2014 and the University accepted said site work as complete on June 20, 2014. Olson Hall is located on the UC Davis Campus, in Davis, California. Deviations from University Asbestos and Lead Remediation specifications (Sections 028200 & 028300), and the Contractor’s approved Work and Disposal Plans have complicated Project Close Out. This is due in part to the University’s pre-demolition activities which resulted in a lead dust release, and ultimately changed the Contractor’s lead work classification from non-trigger task work to lead trigger task work. The aforementioned deviations include but are not limited to the following: Pre-Job Start Submittals; Contractor’s approved Work Plan (means & methods); Contractor’s approved waste disposal plans; and University clearance criteria. This letter identifies pre-job start and post-job completion submittal deficiencies and/or omissions related to the University’s Asbestos Remediation Specification (Section 028200) and Lead Remediation Specification (Section 028300). Said deficiencies and/or omissions listed below must be corrected and accepted by the University’s prior to issuing the Contractor final payment. Also, please let me know if additional documentation is needed to address mid-project modifications to requirements found in the University’s Asbestos and Lead Remediation Specifications, including lead clearance criteria and asbestos / lead disposal. Contractor shall provide Asbestos Project Close Out information identified in Project Close-Out Requirements, Section 028200 Asbestos Remediation Specification, 3.15. Close Out documentation shall pertain to actual work performed, not proposed work, and clarify deviations from project specifications and approved Work and Disposal Plans, including means and methods, and waste packaging / transportation / disposal.
4. Contractor’s statement titled “3.15 Project Close-Out”, which was provided on Two Rivers Demolition’s letterhead, states “All asbestos was turned over to the University for disposal per John Nichols of Delta Development.” The aforementioned statement is false and must be removed from the Contractor’s Close Out documentation. Neither John Nichols or Delta Development directed the Contractor to deviate from the approved waste disposal plan. Furthermore, Asbestos Project Close Out documentation should accurately reflect waste disposal and any deviations from approved Exhibit 21 Asbestos Waste Disposal Plan.

5. Exhibit 25 – Lead As-Built Summary is incomplete with regard to waste disposal and transportation, and states “See attached document “Lead Waste””. However, the aforementioned “Lead Waste” document was not provided. Dear Mr. Slayton, The following report is in regards to the completed air monitoring clearance sampling that was conducted at the above listed address, in Olson Hall Classroom 0101 (CAAN #3803; Zone #C005). Prior to the visual inspection and air clearance sampling being conducted by N.A.L., Allied Environmental, Inc., removed the asbestos containing Floor Tile, Mastic, and Transite Chalkboard materials from the work area. Denis Edwards, Certified Asbestos Consultant, for National Analytical Laboratories, Inc., completed the air clearance sampling on July 29, 2015. Because the visual inspection showed no asbestos containing construction materials (ACCM), asbestos debris, or any other impairment, Mr. Edwards proceeded with the Phase Contrast Microscopy (PCM) air clearance sampling using 25mm cassettes. The cassettes used had a Mixed Cellulose Ester membrane 0.8 µm, micron pore size that is used specifically for PCM sampling. The air monitoring cassette was calibrated with a Dwyer series VFB Visi-Float Flowmeter at 8.33 liters per minute (lpm). The air monitor operated for total of 150 minutes, for a total cassette volume of 1249.50 liters. The total volume fell within the projected range of 400 to 1800 liters for PCM sampling. A total of two (2) samples were collected from the Center of Classroom 0101 and transported to MicroTest Laboratories, Inc., for analysis. The average concentration for airborne fibers found in the two (2) samples was <0.002 f/cc. The clearance result is below the Environmental Protection Agency’s (EPA’s) PCM clearance level of 0.01 f/cc. Laboratory analytical report and chain of custody form attached. Classroom 0101 work area has been cleared for re-occupancy. Individuals may enter the Classroom without any health or safety concerns in regards to the exposure of airborne asbestos fibers. The total volume of both samples fell within the projected range of 400 to 1800 liters for PCM sampling. A total of two (2) samples were collected from the Center of Classrooms 0105 and 0227 and were transported to MicroTest Laboratories, Inc., for analysis. The average concentration for airborne fibers found in the three (3) samples was <0.002 f/cc. Olson Hall Classroom Renovations – Classroom 0118 198 Peter J Shields Avenue Davis, CA 95616
Dear Mr. Slayton, The following report is in regards to the completed air monitoring clearance sampling that was conducted at the above listed address, in Olson Hall Classroom 0118 (CAAN #3803; Zone #C005). Prior to the visual inspection and air clearance sampling being conducted by National Analytical Laboratories, Inc. (N.A.L.), Allied Environmental, Inc., removed the asbestos containing materials from the work area. Denis Edwards, Certified Asbestos Consultant for N.A.L., completed the air clearance sampling on August 03, 2015. Because the visual inspection showed no asbestos containing construction materials (ACCM), asbestos debris, or any other impairment, Mr. Edwards proceeded with the Phase Contrast Microscopy (PCM) air clearance sampling using 25mm cassettes. The cassettes used had a Mixed Cellulose Ester membrane 0.8 µm, micron pore size that is used specifically for PCM sampling. The air monitoring cassette was calibrated with a Dwyer series VFB Visi-Float Flowmeter at 8.33 liters per minute (lpm). The air monitor operated for total of 150 minutes, for a total cassette volume of 1250 liters. The total volume fell within the projected range of 400 to 1800 liters for PCM sampling. A total of two (2) samples were collected from the Center of Classroom 0118 and transported to MicroTest Laboratories, Inc., for analysis. The average concentration for airborne fibers found in the two (2) samples was <0.002 f/cc. The clearance result is below the Environmental Protection Agency’s (EPA’s) PCM clearance level of 0.01 f/cc. Laboratory analytical report and chain of custody form attached. Classroom 0118 work area has been cleared for re-occupancy. Individuals may enter the Classroom without any health or safety concerns in regards to the exposure of airborne asbestos fibers. Prior to the visual inspection and air clearance sampling being conducted by N.A.L., Allied Environmental, Inc., removed the asbestos containing Floor Tile, Mastic, and Transite Chalkboard materials from the work area. Denis Edwards, Certified Asbestos Consultant, for National Analytical Laboratories, Inc., completed the air clearance sampling on July 28, 2015. Because the visual inspection showed no asbestos containing construction materials (ACCM), asbestos debris, or any other impairment, Mr. Edwards proceeded with the Phase Contrast Microscopy (PCM) air clearance sampling using 25mm cassettes. The cassettes used had a Mixed Cellulose Ester membrane 0.8 µm, micron pore size that is used specifically for PCM sampling. The air monitoring cassette was calibrated with a Dwyer series VFB Visi-Float Flowmeter at 8.33 liters per minute (lpm). The air monitor operated for total of 151 minutes, for a total cassette volume of 1257.83 liters. The total volume fell within the projected range of 400 to 1800 liters for PCM sampling. A total of two (2) samples were collected from the Center of Classroom 0163 and transported to MicroTest Laboratories, Inc., for analysis. The average concentration for airborne fibers found in the two (2) samples was <0.002 f/cc. The clearance result is below the Environmental Protection Agency’s (EPA’s) PCM clearance level of 0.01 f/cc. Laboratory analytical report and chain of custody form attached. Because the visual inspection showed no asbestos containing construction materials (ACCM), asbestos debris, or any other impairment, Mr. Edwards proceeded with the Phase Contrast Microscopy (PCM) air clearance sampling using 25mm cassettes. The cassettes used had a Mixed Cellulose Ester membrane 0.8 µm, micron pore size that is used specifically for PCM sampling. The air monitoring cassette was calibrated with a Dwyer series VFB Visi-Float Flowmeter at 10.47 liters per minute (lpm). The air monitor operated for total of 120 minutes, for a total cassette volume of 1256.4 liters. The total volume fell within the projected range of 400 to 1800 liters for PCM sampling. A total of two (2) samples were collected from the Northeast Corner of Classroom 0228 and shipped via FedEx to EMSL Analytical, Inc., for analysis. The average concentration for airborne fibers found in the two (2) samples was <0.002 f/cc. The clearance result is below the Environmental Protection Agency’s (EPA’s) PCM clearance level of 0.01 f/cc. Laboratory analytical report and chain of custody form attached. Classroom 0228 work area has been cleared for re-occupancy. Individuals may enter the Classroom without any health or safety concerns in regards to the exposure of airborne asbestos fibers. July 11, 2013 Astrid Senger-Gibbons UC Davis - Design & Construction Management 225 Cousteau Place Davis, CA 95618 RE: Asbestos & Lead Inspection – Wellman Hall-Room 0027 Dear Ms. Senger-Gibbons, The following report is in regards to the building inspection completed at the Wellman Hall located at CAAN #4050 ZONE #B042, in Davis, California. Of the three (3) suspected asbestos containing samples collected two (2) were found to contain asbestos containing construction materials (ACCM). Of the three (3) suspected lead containing samples collected one (1) was found to contain Lead Containing Material (LCM). Anthony M. De Arcos, Certified Asbestos Consultant and Registered Environmental Property Assessor (REPA), conducted the inspection. The inspection was completed on July 9, 2013. The inspection was completed according to the EPA’s Asbestos Containing Building Materials (ACBM) In-Schools Rule; 40 CFR 763.85 (Inspection and Re-Inspection). Currently, EPA regulations classify ACBM as materials containing more that 1-percent (1%) of asbestos. Cal-OSHA currently regulates asbestos to 1/10th of 1% (0.1%) and requires that a certified asbestos worker conduct this work. Based on the sample results the Texture (Sheetrock-Joint Compound is to be considered homogeneous) were found to contain ACCM. All square footage should be verified by contractor. No destructive sampling was conducted during the site visit, in the event that future renovation and/or demolition work reveals any unforeseen suspect materials; the contractor shall contact the project manager for further testing. The lead suspect samples were collected according to the Housing Urban Development (HUD) Guidelines, the Environmental Protection Agency (EPA) and California Public Health Department (formally DHS), who regulate and require the abatement or in-place management of LCM hazards equal to or greater than 1.0 milligram per square centimeter (1.0 mg/cm2) of lead or more than 0.5% lead by weight. The following regulation shall be adhered to because OSHA considers all surfaces to contain lead: OSHA’s 29 CFR 1926.62, California Occupational Safety and Health Standard, Title 8 (Cal/OSHA 8 CCR 1532.1). The samples from the Multi-Colored Paint surfaces were found to contain LCM levels above the OSHA Limit of Detection. Thus, a certified CPHD (formerly DHS) lead worker and individuals certified by EPA, with Renovator, Repair and Painting (RRP) designation, must complete any work that will disturb the painted surfaces at the site including prep, in-place management, and/or abatement.

EMSL Analytical, Inc. (EMSL) in Carle Place, New York, analyzed the bulk suspect asbestos containing samples utilizing Polarized Light Microscopy (PLM) Method and utilizing the SW-846-3050B*/7000B method analyzed the suspect LCM samples. National Voluntary Laboratory Accreditation Program (NVLAP) Certification #10148-10 and California Environmental Laboratory Accreditation Program (CAELAP) Certification #2339, certifies EMSL. SECTION I: ASBESTOS BUILDING INSPECTION- Federal and state regulations require that anyone disturbing asbestos containing materials are properly trained certified and have the required respiratory protection and medical surveillance. Although not all the rooms or materials throughout the site were sampled, the like materials that were not tested will be treated as homogeneous to the materials that were tested and will be considered as containing ACCM. Based on the regulatory requirements by the National Emission Standards for Hazardous Air Pollutants (NESHAP), Regulation 40 CFR, Part 61, Subpart M the following must occur “If the asbestos content is less than 10 percent, verification shall be made using the point counting method specified in Appendix A, Subpart F, 40 CFR Part 763, Section 1.7.2.4, Polarized Light Microscopy, Qualification of Asbestos Content.” Samples found to contain a Trace, <1% amount of Chrysotile asbestos were re-analyzed utilizing the EPA 600/M4-82-020, Point Count Method to determine if the amount of ACCM is less than or greater than 1%. The results for the Material sample were found to contain <0.25% asbestos. Based on the results the surface materials will be treated to contain a Trace <1% Chrysotile asbestos containing construction material (ACCM). Although neither EPA nor Local County Air Quality regulates materials at <1% level, it does fall under the jurisdiction of OSHA in regards to worker protection. OSHA regulates the material at 1/10th of 1% [8 CCR-1529 (r) (1) Asbestos Containing Construction Material (ACCM)]. Based on this, the work must be completed by a certified asbestos contractor. The CONTRACTOR shall be required to call the landfill to verify that they will accept the waste as general construction debris. If the landfill does not accept the material as general construction debris, the material shall be REQUIRED to be disposed of as non-friable/non-hazardous material. The Sheetrock-Joint Compound is to be considered homogeneous and treated as containing Trace <1% Chrysotile. A certified asbestos abatement contractor must be retained to remove the above materials containing the Trace (<1%) amount prior to any renovation or demolition work being completed at the site. During the removal of the materials, the debris will be placed into an open top waste bin. The bin will have ''Asbestos Hazard'' signs posted around the perimeter of the waste bin. Upon completion of the removal process, the bin will be covered, the signs will be removed and the debris will be disposed of as general construction debris at a non-regulated waste facility. NAL recommends that a certified asbestos abatement contractor be retained to remove trace materials prior to any scheduled renovation/demolition work being completed at the site. Prior to the work process starting a work plan or specifications in regards to the abatement process should be completed and distributed to the abatement contractors during the job walk at the site.

On-Site Observation should be conducted by N.A.L’s Certified Asbestos Consultant or Certified Site Surveillance Technician to verify that the work plan/specification is being followed. This will verify that during the abatement work the outside air was clean. Once a certified asbestos contractor has removed the ACCM, following EPA and OSHA requirements; a visual inspection and air clearance sampling should be completed. Clearances will confirm that the general contractor can reoccupy the work area(s), without concern for exposure to asbestos airborne fibers to their employees thus allowing the renovation or demolition work to be completed by the general contractor. Side Ceiling, None Detected SECTION II: LEAD BUILDING INSPECTION- Since the laboratory results are reported by weight percent, during the collection of the suspect LCM samples the paint must be removed down to, but not including, the bare substrate (wood, metal, etc.). Inclusion of the any amount of the substrate material in the paint sample will dilute the sample result(s). Once the determination is made on where the LCM is located, the In-place Management or the Abatement of the LCM can commence. If the In-Place Management method is to be used, prior to the repainting of the effected surface areas, the loose flaky paint must be removed until the remaining paint adheres smoothly to the substrate. Once this task is completed, the surface area can be repainted without the possibility of paint being dislodged and falling to the floor or ground areas. If the Abatement method of all surfaces is to be completed, then the debris and any loose flaky paint must be bagged or burrito wrapped prior to the removal of the debris from the work area(s) and subsequently the site. Because the paint samples listed below were found to contain LCM, all areas where the LCM will be disturbed will require abatement, encapsulation, and/or prep work by a certified lead worker. Although not all the rooms or materials were sampled, the like materials that were tested and their results will be treated as homogeneous and the materials will be treated as containing LCM throughout the site. Prior to the demolition work being completed and/or the transporting of the debris from the site, Health and Safety Code 25157.8 (AB 2784 National Resources) requires that all lead debris be sampled for Waste Characterization. This will assist the Contractor in making a determination of whether or not the material is to be considered Hazardous or Non-Hazardous Lead waste or general construction debris. The sequence of testing to be completed by the Contractor is as follows:
i Total Threshold Limit concentration (TTLC) with a result of 50 mg/kg or more but less than 1,000 mg/kg of lead must be retested using the Soluble Threshold Limit concentration (STLC) method;
i A STLC result of 5.0 mg/L or greater is considered California Hazardous Waste; i Total Characteristic Leaching Procedure (TCLP) testing shall only be accomplished when approved by the
Owners Representative; This procedure shall be generally reserved for out-of-state shipments; and A TCLP result of 5.0 mg/L or more deems the waste Federal RCRA materials; and
i The California hazardous waste threshold for total lead using STLC is 5 mg/L and i Lead paint that is intact on a surface does not permit the material to be classed as non-hazardous. Waste
profiling shall be accomplished if the paint contains more than 350 ppm by Flame AAS. Exception: Metals that are coated with paint are to be recycled.

August 22, 2014 Mr. Fred Torres, Project Manager Facilities, Mechanical / HVAC University of California One Shields Avenue Davis, California 95616 Dear Mr. Torres: Per your request, I have reviewed additional hazardous material (asbestos/lead) pre-job start and post project close-out information you provided August 20, 2014. The aforementioned documents are associated with Olson Hall HVAC System Upgrades, Phase II, in Room 0040, Basement Mechanical Room (University Project Number 2503800). Two Rivers Demolition commenced site work associated with the aforementioned project on June 16, 2014 and the University accepted said site work as complete on June 20, 2014. Olson Hall is located on the UC Davis Campus, in Davis, California. Deviations from University Asbestos and Lead Remediation specifications (Sections 028200 & 028300), and the Contractor’s approved Work and Disposal Plans have complicated Project Close Out. This is due in part to the University’s pre-demolition activities which resulted in a lead dust release, and ultimately changed the Contractor’s lead work classification from non-trigger task work to lead trigger task work. The aforementioned deviations include but are not limited to the following: Pre-Job Start Submittals; Contractor’s approved Work Plan (means & methods); Contractor’s approved waste disposal plans; and University clearance criteria. This letter identifies pre-job start and post-job completion submittal deficiencies and/or omissions related to the University’s Asbestos Remediation Specification (Section 028200) and Lead Remediation Specification (Section 028300). Said deficiencies and/or omissions listed below must be corrected and accepted by the University’s prior to issuing the Contractor final payment. Also, please let me know if additional documentation is needed to address mid-project modifications to requirements found in the University’s Asbestos and Lead Remediation Specifications, including lead clearance criteria and asbestos / lead disposal. A statement titled “3.15 Project Close-Out” was provided on Two Rivers Demolition’s letterhead, and appears to contain information pertaining to both asbestos and lead Project Close-Out documentation. Although Asbestos and Lead Project Close Out documentation may contain some duplicate information, said information must be provided as two independent documents. Contractor shall provide Asbestos Project Close Out information identified in Project Close-Out Requirements, Section 028200 Asbestos Remediation Specification, 3.15. Close Out documentation shall pertain to actual work performed, not proposed work, and clarify deviations from project specifications and approved Work and Disposal Plans, including means and methods, and waste packaging / transportation / disposal.
4. Contractor’s statement titled “3.15 Project Close-Out”, which was provided on Two Rivers Demolition’s letterhead, states “All asbestos was turned over to the University for disposal per John Nichols of Delta Development.” The aforementioned statement is false and must be removed from the Contractor’s Close Out documentation. Neither John Nichols or Delta Development directed the Contractor to deviate from the approved waste disposal plan. Furthermore, Asbestos Project Close Out documentation should accurately reflect waste disposal and any deviations from approved Exhibit 21 Asbestos Waste Disposal Plan.
5. Exhibit 22 – Asbestos As-Built Summary is incomplete with regard to waste disposal and transportation, and states “See attached document “Asbestos Waste””. However, the aforementioned “Asbestos Waste” document was not provided. Contractor shall provide waste disposal and transportation information requested in Exhibit 22 Asbestos As-Built Summary, Section 028200 Asbestos Remediation Specification.
Lead Pre-Job Start & Post Job Completion Submittal Deficiencies and/or Omissions:
1. One electronic copy of supplemental pre-job start submittals was provided. Contractor shall provide required number of hard copies of project submittals per Section 028300, 1.5 Submittals, D.
2. Recycler weight tickets include slips from Sims Metal, however, Sims Metal is not the recycler identified in the Contractor’s approved disposal plan. Additionally, recycler weight tickets appear to include materials not associated with the Olson Hall project. Contractor shall clarify.

Contractor shall provide Lead Project Close Out information identified in Project Close-Out Requirements, Section 028300 Lead Remediation Specification, 3.15. Close Out documentation shall pertain to actual work performed, not proposed work, and clarify deviations from project specifications and approved Work and Disposal Plans, including means and methods and waste handling. Identify all waste streams, and account for waste hazard classification / packaging / transportation / disposal.
4. Contractor’s statement titled “3.15 Project Close-Out” was provided on Two Rivers Demolition’s letterhead, and states “Materials were hauled off by Two Rivers Demolition Inc. and transported to Schnitzer Steel Products. Identify the materials hauled to Schnitzer Steel Products. Additionally, transport vehicles observed on site were marked as Primoris Demolition, not Two Rivers as stated in Project Close Out documentation. Contractor shall clarify.
5. Exhibit 25 – Lead As-Built Summary is incomplete with regard to waste disposal and transportation, and states “See attached document “Lead Waste””. However, the aforementioned “Lead Waste” document was not provided. Contractor shall provide waste disposal and transportation information requested in Exhibit 25 Lead As-Built Summary, Section 028300 Lead Remediation Specification. Be sure to account for all waste streams, including, but not limited to dust, vacuum debris, decontamination materials and supplies, and personal protective equipment.

ASBESTOS WASTE
All of the Asbestos waste was taken by UC Davis EH&S Department to their containers on site where it was properly labeled and manifested with other asbestos waste.
