Revalidation is a process which will help pharmacists and pharmacy technicians to keep their professional skills and knowledge up to date. It will help to show you how to provide the safe and effective care, patients and the public expect, as set out in the standards for pharmacy professionals. Pharmacy professionals are valued for their knowledge of medicines and their ability to provide advice to patients and medical colleagues about medicines. There are pharmacy technicians and pharmacists in General Practice, Urgent and Emergency Care, care homes as well as specialist roles in hospitals and community care. Pharmacy professionals are spreading out of their traditional roles. Pharmacists are building on their ability to become independent prescribers and pharmacy technicians are increasingly undertaking more clinical roles to support people to optimise their medicines. It is all linked and represents exciting times if you are a pharmacy professional!

Clinical examinations by pharmacy professionals are becoming more commonplace in all sectors. Courses run by Centre for Pharmacy Postgraduate Education (CPPE) focus on clinical observation, assessment and examination skills. Health Education England (HEE) is funding a number of post graduate learning programmes, designed to extend skills.

Pharmacists have swapped the triangle, spatula and pill counter for the thermometer, the otoscope, stethoscope and the ophthalmoscope. The relationship between pharmacists and patients is becoming closer. Pharmacists are examining throats, checking glands and listening to chests. When I visit a pharmacist’s consulting room I expect to see equipment and I expect them to use it.

The pharmacy team are taking advantage of newer diagnostics; leading the way on adoption of point of care tests to enhance their diagnostic and monitoring capability. Rapid Antigen Tests to identify cases of Strep A infection and influenza A and B will become the norm and built upon growing clinical confidence. Pharmacy professionals will assess CRP quickly and efficiently through semi-quantitative point of care measurements before deciding on how a patient will be managed through the system.

Pharmacy teams will take advantage of new digital technology to revolutionise the way that they support people to manage their conditions. Setting patients up with simple apps on their mobile phones will allow patients to complete simple tests at home. Smart phones will become the mechanism to measure results and instantaneously transfer results from a patient’s home to the clinician’s office. Pharmacy will offer complete pathways of care. Pharmacists will observe, assess, examine and use critical clinical skills and point of care diagnostics to make an appropriate diagnosis. They will then decide with the patient on appropriate treatment. Pharmacy technicians will supply that treatment and provide information to support the person to manage their condition. They will build partnerships with the people who use their services in order to optimise medicines and improve patient outcomes. All pharmacy professionals are clinical – but some will deliver complete and complex pathways of care in a variety of settings.

Hear from the Pharmacy Deans/Regional Heads of Pharmacy: The four HEE Pharmacy Deans/Regional Heads will share the key work programmes about what is happening in HEE to

several presentations, including updates on the Electronic Prescription Service, Summary Care Record, medicines data for secondary uses, Electronic Repeat Dispensing and NHSmail.

Tahmina Rokib (a Chief Pharmaceutical Officer’s Clinical Fellow) has joined the team. She’ll be speaking in the Changing Careers Theatre about her time with us so far, sharing what she has learned and how she plans to continue her journey.

This year we’ve also sponsored a ‘Best of Clinical Pharmacy Award’ for Excellence in use of Technology in Pharmacy Practice. The award recognises an outstanding person, team or project that has made a significant digital contribution to clinical pharmacy. For a long time pharmacy professionals have provided assurance of their ability to keep their knowledge and skills up to date by carrying out and recording continuing professional development (CPD) activities. But in the evolving world of health and social care, patients and the public would like to have further assurance that pharmacy professionals remain safe and effective after their initial registration.

The revalidation framework encourages pharmacy professionals to reflect on their learning and practice, and it focuses on the outcomes for the people using the services of pharmacy professionals to provide that assurance.

The General Pharmaceutical Council (GPhC) will be speaking on Revalidation for Pharmacy Professionals at CPC 2018, providing assurance and improving quality on Friday 27 April 2018 from 14:15-15:00 in the Leadership Theatre.

Join us on Saturday 28th April 2018 from 09:45-10:15 in the Strategy and Policy Theatre for our session dedicated to consultation and education training standards for pharmacist independent prescribers.

What is changing?

On 30th March 2018, revalidation replaced the current requirement for registrants to complete CPD. The revalidation framework sets out GPhC expectations of what pharmacy professionals must do each year to meet the requirements for revalidation.

For now, look out for your sign up letter and personalised timeline which will be coming through the post and should be with you by the end of the April 2018.

You will need to record activities using the updated ‘my GPhC’ online system, and to submit your records every year, at the point when you renew your registration. To see what you will need to record and submit and when, take a look at your personal revalidation timeline.

Shortly after revalidation goes live, CPC will open its doors to over 2,900 pharmacy professionals, to assist them with meeting the requirements for revalidation. We are working with The General Pharmaceutical Council to ensure you receive the latest updates on the new framework.

The GPhC has reported that peer discussion generated the most feedback with questions and concerns about the approach. Their biggest concern was how they were going to find a peer to network with and discuss their practice. There were requests for more clarity on how to carry out this discussion and concerns about the time available to complete the activity.

Due to the large demand from the pharmacy profession we are hosting a dedicated networking hour to help you find a peer over some tea and cake to assist with your peer-to-peer discussion record. In this relaxed environment pharmacy professionals will be discussing tips for best practice and how they can improve patient outcomes. You can then swap contacts and carry on your learning after the show to then submit your record to the GPhC.

The PDA attend CPC to launch Roadmap 2 – a long-term strategy for the pharmacy sectorThe strategy outlines the steps needed to create an intelligent system that focuses on joined-up patient care.

Integrating the work of primary care pharmacists, GP practice based pharmacists, group practice pharmacists and community pharmacists, this strategy is based on pharmaceutical care and provides a professionally fulfilling and sustainable long term future for all pharmacists.

By taking charge of the medicines and pharmaceutical care agenda, it helps to secure the long term health of the public as well as meet important NHS objectives. Developing clinical relationships with patients – moving away from consumers/suppliers and focusing on the unique skills of individual pharmacists

Roadmap 2 will be launched by the PDA Chairman Mark Koziol in the Keynote Theatre on Saturday 28th April at 1.15pm.

This year, the PDA have also sponsored the General Practice Pharmacy (GPP) Theatre, where visitors can attend workshops that will help them better understand the role of pharmacists working in general practice. The GPP theatre is new to CPC and is situated next to the PDA Stand (H10).

On 28th April at noon, Claire Ward, Director of Public Affairs at the PDA will be leading a Women in Pharmacy session in the Leadership Theatre. This session will include a

The PDA are also sponsoring ‘Finishing First’, a pre-reg conference hosted by the BPSA (British Pharmaceutical Students’ Association). The high-quality, two-day conference is taking place on Saturday 28th at CPC and Sunday 29th April, post event, this conference will help students to prepare for their pre-reg assessment. Finishing First is being run in collaboration with pharmacy training provider, ProPharmace and author Alan Nathan. The sessions will cover a range of topics, including:

• A mini-mock calculations paper• A review of the mock calculations paper,

demonstrating how the answers have been worked out

• Hints and tips on how to prepare for the pre-reg assessment • A taster session on ‘responding to symptoms’.

The ART of HIV Pharmacy - Foundation and beyondLucy Hedley, EHRS Principal Trainer - Pharmacy & EPMA, UCLH, Senior Clinical Pharmacist HIV, Tropical Medicine & Infectious Diseases, will be presenting two sessions on behalf of HIVPA at this year’s Clinical Pharmacy Congress.

The first session on Friday at 17:30 in The Clinical Leadership Theatre, “HIV Update – Where are we now?”, is aimed at non-specialists and will cover some Foundation and Advanced level clinical knowledge.

HIV care and outcomes have changed dramatically over the years. It is a long-term manageable condition with multiple options for treatment.

This session will include a summary of the advancements in HIV care and a review of the history of HIV - with a synopsis of where we are both globally and within the UK. It will also include an update on treatment strategies and availability of pre-exposure prophylaxis (PrEP) and treatment as prevention (TAsP) as well as data from key recent trials.

The session will also provide information on the current status of the HIV epidemic worldwide and in the UK, the evolution of ART and how outcomes have changed over time,findings of the START study

and PrEP and TAsP prevention strategies.The main objectives are to understand a brief history of HIV disease.

The second session on Saturday at 9:45 in The Clinical Pharmaceutics Theatre, will be aimed at both specialists and non-specialists in HIV, at an advanced level of clinical knowledge, though everyone is welcome to attend!

“Complex clinical cases” will be a session using case study presentations to cover dealing with complex HIV patients in an inpatient setting.

People are living longer with HIV and many HIV positive patients are admitted to hospital for conditions other than HIV - HIV therefore covers many areas and there is a high potential for drug-drug interactions. Awareness of and how to manage these patients is no longer limited to those specialising in HIV.

The main objectives are to understand the complexities of HIV care and have an awareness of the management of antiretroviral drug-drug interactions, management of NBM scenarios and the impact of treatment interruption.

Supporting Pharmacy Technicians across the UK at CPC 2018The Association of Pharmacy Technicians UK (APTUK) is the professional leadership body for registered Pharmacy Technicians working in the UK, leading pharmacy technicians to deliver professional excellence for patient centred care.

The Clinical Pharmacy Congress provides excellent opportunities for professional development, collaboration, innovation and networking. Scan4Safety – join us to find out what you need to know and do

Scan4Safety is the programme of work resulting from the publication of the NHS eProcurement Strategy in 2014. Scan4Safety aims to tackle the issue of a lack of standardisation in the NHS and in doing so is enabling the delivery of improved patient care and increasing operational and clinical productivity.

Led by the Department of Health and Social Care (DHSC), Scan4Safety achieves its aims by providing NHS trusts and suppliers to the NHS with a structured and consistent approach to the adoption of international standards. In their most tangible form, international standards enable the placing of unique identifiers – frequently barcodes – on every patient, product and place used in the delivery of care.

The benefits of adopting international standards across industries are well documented from sectors including retail and manufacturing. To date, Scan4Safety has proved the benefit of standards adoption in healthcare, having worked with six acute NHS trusts in England, one of which is the Royal Cornwall NHS Hospitals Trust (RCHT). Based on the benefits being delivered by these six trusts to date, it is estimated that the programme has the potential to save over £1 billion for the NHS acute sector over the next seven years.

In a breakfast session on Friday at 9:00 in the Strategy and Policy Theatre, you can hear from the DHSC as well as from the Chief Pharmacist at RCHT on what Scan4Safety requires trusts to do and how to find out more and get involved.

This session is aimed at raising awareness amongst pharmacists about treatment strategies for this patient group. We will cover the major prescribing rules for cancer patients

Genomics and Targeted Cancer Treatment - Clinical Leadership Theatre, 28th April at 13:15

This session will focus on genomics and the molecular mechanisms that cause cancer development, growth, and metastasis. The session will also cover targeted cancer therapies used in clinical practice. Jylamvo is a new oral solution that contains 2 mg/ml of methotrexate developed to address the unmet need for a palatable methotrexate solution. Primarily intended for use in the paediatric population but will also offer an alternative oral presentation to adolescents and adults unable to swallow tablets.

The College of Mental Health Pharmacy (CMHP) aims to benefit individual care through advancing education and research in the practice of mental health pharmacy.

Advancing Education and Research in the practice of Mental Health Pharmacy

Using big data to learn from best practice, improve patient outcomes and improve efficiency IQVIA™ has unique access to dispensing data across the UK in both Primary and Secondary Care with 100% of NHS Acute Trust dispensing data. This is being used commercially to predict changes in the market but is also available to reflect insight back to the NHS. In the session we will look at the data and how it is used in more detail and demonstrate the insights gained from a clinical pharmacy perspective.

In the NHS, IQVIA data can be used at a number of levels:

Individual patient experience and outcome

Formulary development and compliance monitoring

Budgetary and financial planning and management

Nationally, regionally and locally (to Trust or Speciality level)

In Primary Care, there is a Moving Annual Total (MAT) decrease in medicines use to December 2017 and in the session, we will look at several hypotheses to explain this.

Biosimilar Uptake and Generic Substitution

82 drugs are losing exclusivity up to December 2019. These include biologics such as imatinib, adalimumab and gefitinib as well as linezolid, voriconazole, caspofungin and tigecycline.

Using UK list price, in the 5 years to 2020, the NHS would spend over £2.5bn on these drugs. Using a managed switch programme of biosimilar and generic use, there are potential savings of between 50% and 80% (assuming 100% switch).

Jeremy Hunt today cited a report that states 237 million errors per year occurs in the NHS every year, of which definitely avoidable ADRs cost the NHS £98.6 million, 712 deaths and contribute to 1708 deaths per annum. The report also highlighted that medication administration error rates account for 78.6% of errors in secondary care1. Using barcoded medication administration technology (BCMA), BD Cato is launching ReadyMed to help hospitals ensure the safe administration of medication: to help close the loop of medication administration.

• Allows each hospital to achieve a HIMSS Europe level six adoption and utilization of EMR functions required to achieve a paperless environment.2

• Reduces the need for a second nurse check.

UKOPG and the Moorfields #knowyourdrops campaignThe UK Ophthalmic Pharmacy Group was established in the early nineties by a small but motivated group of pharmacists with a specialist interest in ophthalmology. Since then our membership has blossomed and at the most recent count we have 211 members across the globe. The group provides support and guidance to specialist and non-specialist pharmacists and pharmacy technicians, from both primary and secondary care. We run a free to access discussion forum via NHS networks to provide support and to share learning and expertise with our peers.

This year two of our members from Moorfields Eye Hospital, Fiona Chiu and Sarah Thomas, will be presenting on the wonderful #knowyourdrops campaign in The Clinical Leadership Theatre, 27th April at 16:30.

Since 2016 the team have run a successful, pharmacist -led consultation clinic to help improve compliance with eye drops. NICE have recently recognised the value and importance of better supporting patients with eye drops to improve patient care, clinical outcomes and quality of life and have published the #knowyourdrops model of pharmacy led support on their sharing learning database. Come and see why supporting eye drop compliance is so important and learn from the experiences of the team, visit us in the Affiliates Lounge (J10).

Our current co-chairs, Clare Faulkner and Priya Boparai, and a number of our members will also be manning a stand at this year’s CPC. Come see us if you have any challenging ophthalmic questions, any experiences you would like to share and to network other ophthalmic pharmacists. The group works closely with other national bodies including NICE, The Royal Pharmaceutical Society, Medusa and The Royal College of Ophthalmologists. We are currently involved in lots of exciting projects, both independently and in conjunction with the groups mentioned above, feel free to come and talk with us about our current and future workstreams if you are interested in getting involved. We look forward to seeing you there!
