Introduction

TheTriathlon®KneeSystemInstrumentationhasbeendevelopedbasedonStryker’s30-yearorthopaedichistory.ThesystemcombinestheexpertiseoforthopaedicandhumanfactorsengineerswiththatofsurgeonsandORstaffworldwide.

Indications

TheTriathlon®TotalKneeSystemcomponentsareintendedforuseinprimarytotalkneearthroplastyandrevisionofauni-kneearthroplastyorHighTibialOsteotomytoalleviatepainandrestorefunctionforpatientssufferingfrom:

> Painful,disablingjointdiseaseofthekneeresultingfrom:degenerativearthritis,rheumatoidarthritisor post-traumaticarthritis.

> Post-traumaticlossofkneejointconfigurationandfunction.

> Moderatevarus,valgus,orflexiondeformityinwhichtheligamentousstructurescanbereturnedto adequatefunctionandstability.

> Revisionofpreviousunsuccessfuluni-kneereplacementorotherprocedure

> Ligamentousinstabilityrequiringimplantbearingsurfacegeometrieswithincreasedconstraint.

> Absentornon-functioningposteriorcruciateligament.

Contraindications

> Anyactiveorsuspectedlatentinfectioninoraboutthekneejoint.

> Distantfociofinfectionwhichmaycausehematogenousspreadtotheimplantsite.

> Anymentalorneuromusculardisorderwhichwouldcreateanunacceptableriskofprosthesisinstability, prosthesisfixationfailure,orcomplicationsinpostoperativecare.

> Bonestockcompromisedbydisease,infectionorpriorimplantationwhichcannotprovideadequate supportand/orfixationtotheprosthesis.

> Skeletalimmaturity.

> Severeinstabilityofthekneejointsecondarytotheabsenceofcollateralligamentintegrityandfunction.

> Obesity.

Warnings and Precautions

Thepatientmustbeadvisedofboththelimitationsofthereconstructionandtheneedforprotectionoftheimplantfromfullweightbearinguntiladequatefixationandhealinghaveoccurred.Excessiveactivityandtraumaaffectingthejointreplacementhavebeenimplicatedinfailureofthereconstructionbyloosening,fractureand/orwearoftheprostheticimplants.Looseningofthecomponentscanresultinincreasedproductionofwearparticles,aswellasdamagetothebone,makingsuccessfulrevisionsurgerymoredifficult.

Thepatientshouldbecautionedtolimitactivitiesandprotectthereplacedjointfromunreasonablestresses,andtofollowtheinstructionsofthephysicianwithrespecttofollow-upcareandtreatment.

Thepatientshouldbewarnedofsurgicalrisks,andmadeawareofpossibleadverseeffects.Thepatientshouldbewarnedthatthedevicedoesnotreplicatetheflexibility,strength,reliability,ordurabilityofanormalhealthyjoint,thattheimplantcanbreakorbecomedamagedasaresultofstrenuousactivityortrauma,andthatthedevicehasafiniteservicelifeandmayneedtobereplacedinthefuture.

Appropriateselection,placementandfixationofthetotalkneecomponentsarecriticalfactorswhichaffectimplantservicelife.Asinthecaseofallprostheticimplants,thedurabilityofthesecomponentsisaffectedbynumerousbiologic,biomechanicandotherextrinsicfactors,whichlimittheirservicelife.Accordingly,strictadherencetotheindications,contraindications,precautionsandwarningsforthisproductisessentialtopotentiallymaximizeservicelife.

ManyoftheTriathlon®KneeSystemInstrumentshaveuniquemechanismsincorporatedtoassistsurgeonsandORstaff,inasimplified,efficientsurgicalexperience.Therefore,assemblyinstructionshavebeenincludedinthefirstsectionofthissurgicaltechniquetoassistwithinstrumentsthatmaybepre-assembledonthebacktable,aswellasotherinstrumentsthatneedtobeassembled.

Allofthemechanismsthatallowinstrumentstobeadjustedand/orassembledhavebeenpolished.

Tibial Preparation > Therearetwooptionsfortibialpreparation: extramedullary(EM)referencingalignmentand intramedullary(IM)referencingalignment.

> TheRevisionTibialResectionGuide,availablein LeftandRightconfigurations,aswellasCaptured andOpenconfigurations,isdesignedtoavoidsoft tissueimpingement.

Option 1 – ExtramedullaryReferencing> Thetibialresectionassemblyhasfiveparts:the appropriateRevisionTibialResectionGuide,the AnkleClamp,theDistalAssembly,theProximalRod andtheTibialAdjustmentHousing.Theseare assembledfirst.
