ProgramAccountable Care Organizations (ACOs) are groups of health care providers that work as a team to coordinate care for a group of patients, with the goals of providing high-quality, patient-centered care and reducing costs. The conference will focus on the role of care coordinators, case managers and navigators in the ACO through all phases of care delivery including after hospitalization. At the close of this activity, participants will be able to:

Certificate of completion: Following the conference, a link to an electronic evaluation and attendance confirmation will be emailed to program attendees. You have two weeks to complete the program evaluation and attest to the sessions attended in order to receive a certificate. Evaluations submitted after the two-week period will not be honored.
