INDICATIONS AND RESULTS
Abstract: The first clinical affemRt at orthotopic liver transplantation was performed 32 years afJo. Although the initial results were poor, /tver transplantation has now become the treatmentofchoice for most (non-neop.lastic) endstage liver diseases, and possibly for a Few well-selected neoplastic conditions. The longest survivor recently celebrated her quartercentury anniversary of receiving her transplanted liver. Almost 5,000 liver transplants, representing one-fifth of the overall world experience, have been performed at the Pittsburgh Transplantation Institute_ Based on this uni9,ue single experience, unlikely to ever be duplICated, the authors describe the indications and results of liver transplantation. In the first p,arl of this article all possible indications for liver replacement are analyzed, with an extensive review of the results obtained with liver transplantation. The second part is dedicated to the results of ] 50 1 liver transplants performed at the Pittsburgh Transplantation Institute during 4 years, with specific affention to the patient and graft survival according to the primary diagnosis and the severity of the disease.

The potential usefulness of orthotopic liver transplantation (OLTx) in the treotmen/ofendstage liver diseoses was recognized in the 1950s, and the first clinical attempt was carried out at the University of Colorado in Denvert on March 1, 7963 [7]. It was unsuccessfu: the 3 year old patient, affected by extrahepatic biliary atresia, exsanguinated on the operating table. Among the next 8 patients, transplanted in Denver (6 patients) [2J,

riconosciuta negli anni '50, ed il primo tentativo clinico venne eseguito 0110 University of Colorado, a Denver, ill 0 marzo 1963 [1]. The late 7 960' s and early 7 970' s saw very slow /?rogress in this field, with patients suFFering tram Frequent and disablmg complications, and an overall one-y'ear patient survival oFonly35%[6}, By then, ihelechnicalstepsoF the standara procedure had already been precisely described, and the surgica{ technique that is currently used at the Pittsburgh Transplantation Institute, and in most of the world transplant centers, is essentially the one developed at the University of Colorado in the 1960's [/ ,71- On the other hand, the indications forOLTxweremuch more limited in those early days, and the list of the diseases bein9 treated with this Rrocedure has grawn significantly in the past 30 years, For example, postnecrotic cirrhosis of any etiology is ioday the most common indication forOLTx in most liver transplant centers, while only one patient in the oriflinal Denver series had post-necrotic cirrhOSIS [8]. The reason for tlie paucity of cirrhotic patients {other than biliary atresia} in the early trials was that, while the Fate of patients with a liver malignancy or extrahepatic biliary atresia was easy to predict, the prognosIs of a stable patient with cirrhosis courd not be predicted as readily. Therefore, given the uncertainty surrounding the operation, most candidates were first considered only when they were already moribund, usually as a consequence of recentor ongoing variceal hemorrhage and most died while awaiting for a suitable donor_ It was the routine achievement of long-term survival {the longest surviving recipient was transplanted 25 years ago}, several maior immunological advances [9-20], and the progress in donor surgery [2] -23J and organ preservation [24-26}, wliat led to an expansion of the indications for OLTx. As a conse~uence, diseases that were not considered before are now successfully treated with OLTx, and the procedure could be used for virtually any patient suffering from a lethal hep'atic disease. In 1982 it was estimated that ihe annual need For OLTx was 15 per million population [/2J, but this is probably now hIgher, as tlie advances of the past decade allow us to treat patients that not long ago would have been considered not transplantable, In fact, depending on the criteria used to determine what are appropriate indications, we can estimate that oetween 4,000 and 50,000 OLTx may be needed in the United States every year, with a similar number in Europe [ 14]. Transplantation, held in June 1983 [31] There are four main groups: p'arenchymal liver diseases, cholestatic lIVer disease; congenital errors of metabolism, and hepatic tumors (Table 1),_ Parenchimal Liver Diseases There are many causes of parenchymal liver iniury that may lead to acute or chronic liver failure that grossly impairs the quality of life or poses an acute obstacle to survival. The choice of OLTx is predicated on the knowledge that other tlierapies are ineffective in decreasing morbidity, or in simply rescuing the patient's life. Post Necrotic Cirrhosis The most common indication forOLTx in adults is post necrotic cirrhosis. The main etioloc;Jy is chronic viral hepatitis. which may result trom hepatitis B virus (HBV), with or without coinfection with hepatitis delta virus (HOV), from hepatitis C virus (HCV), or from other, not yet characterized viral agents, The other large group is formed by p'atients with cryptogenic disease; which is a heterogeneous group in whom no defined etiology can be identified, /I should be noted that the true incidence ot hepatitis C (and the risk for reinfection I in the population presented below IS not known. because the test for the antibody' against this agent was not routinely available until May, 1990, In many areas of the world infection with HBV is endemic, and the most common cause of advanced liver disease and hepatocellular carcinoma. Recurrence of infection after transplantation is high in HBsAg+ patients. Efforts to reduce the incidence and severity of reinfection, with the use of T-interferon and active and passive immunization, have been done at many institutions. A large series of patients were analy'zed in a retrospective multicenter study performed in Europe. Three hundred and seventy-/vvo HBsAg positive patients underwent OLTx at 17 centers [32]. Patients were stratified as to the ser070gic markers for hepatitis B; acute fulminant versus chronic, co-existent HOV inFection, and the length of hepatitis B immune globulin Ireotment. The mean three year actuarial risk of recurrence of HBV was 50%. Eighty three percent of HBII DNA positive developed recurrence; 6696 of the HBeAg positive, DNA negative recurring; and 58% of HBV DNA negative, HbeAg n~ative recurring (usually wifhin 24-30 months after surgery). Patients who had co-existent HDV infection in chronic phose, had a 32% incidence of recurrence, and those with fulminant hepatic failure rela· ted to hepatitis B virus had a 17% recurrence rate. Recurrence as related to hepatitis B immune globulin therapy reveoled that no therapy was associated with 75% rate of recurrence, while those with short term therapy' (less than 6 months of hepatitis B immune globulin) had a 7496 recurrence rate. Those who had longer than 6 months of hepatitis B immune globulin hod a 36% recurrence rate, At the Pittsburgh Transplantation Institute, 69 patients who underwent OLTx belween 1988 and 1992, where analyzed accordin~ to the HBeAg status, and the length of hepatitis B immune grobulin treatment. In effetti, e possibile stimare che siano necessari ogni anna negli Stati Uniti da 4.000 a 50.000 OLT x, a seconda dei criteri utilizzati per stabilire Ie indicazioni appropriate, con necessita dello stesso ordine di grandezza anche in Europa [14]. Ma, mentre in passato la decisione di procedere al trapianto si basava prevalentemente su fattori tecnici e medici dipendenti dalle ca' ratteristiche del ricevente, oggi e 10 disponibilita degli organi che sempre pi~ spesso condiziona questa scelta. Se Sl vuole realizzare un incremento sostanziale delI' attivita di trapianto, si dovra senza dubbio procedere alia revisione delle attuali strategie riguardanti 10 do nazi one di organi ['L7·30].
INOICAZIONI PER IL TRAPIANTO 01 FEGATO
La maggior parte delle indicazioni aWOLTx sono state definite dalla Nationallnstitutes of Health (NIH) Consensus Development Conference on Liver Transplantation, svoltasi nel giugno del 1983 [31]. Vengono distinti quattro gruppi princip'ali; patologia epGtica parenchimale, potologla epatica colestalica, errori congenili del metabolismo e tumori epatici (Tab. 1).
PATOlOGIA EPATICA PARENCHIMALE

Cirrosi Post-Necrotica
L'indicazione pill comune per l'OLTx negli adulti e la cirrosi post-necrotica. L' eziorogia principale e raRpresentata ~all' epatite virale cronica, die puC> derlvare dal virus dell' epatite B (HBVL con 0 senza
coinfezione dal virus dell' epatite delta (HDV), dal virus dell'epatite C (HCV), 0 da altri agenti virali non ancora definiti. L' altro ampio gruppo e rappresentato dai pazienti con malattia criptogenetica e costituisce una categoria eterogenea nella quale non e possibile identihcare alcuna eziologia specifica, E; da sottolineare che I'incidenza delI' epatite C (e i rischi di reinfezione) non e nota can certezza, poiche il test per la determinazione dell' anticorpo contro questo agente non era disponibile nella routine clinica fino al ma,.ggio 1990. In molte aree geogratlche I'infezione da HBV e endemica e rappresenta la causa pill frequente di malania epatica avanzata e di carcinoma epatocellulare. La recidiva dell'infezione dopo trapianto e elevata nei pazienti positivi per la presenza dell'antigene di superficie circolante (HBsAg). In molti istituti sono stati fatti sforzi per ridurre I'incidenza e la gravita della reinfezione, con I'utilizzo di T-interferone e I'immunizzazione attiva e passiva. Un' ampia serie di pazienti e stata ana lizzata in uno studio retrospettivo multicentrico condotto in Europa. Trecentosettantadue pazienti HBsAg positivi sono stati sottoposti a OLT x in 17 centri [32]. I pazienti sono stati stratificati a secondo dei marcatori sierologici per I' epatite B, dell'infezione acuta fulminante 0 cronica, della concomitante infezione da HDV, e della durata del trattamento con immunoglobuline anti-epatite B. II rischio medio attuariale di ricorrenza dell'HBV a 3 anni dal trapianto e stato del 50%. L'83% dei pazienti HBV-DNA positivi ha sviluppato recidiva di epatite, mentre I'incidenza di ricorrenza e stata del 66% nei pazienti HBeAg positivi/DNA negativi, e del 58% nei pazienti DNA-HBV negativi/HbeAg negativi (di solito entre 24-30 mesi dalI'intervento). I pazienti con coesistente infezione da HDV; in fase cronica hanno avulo un'incidenza di ricorrenza deI32~~, e quelli con insufficienza ep'atica fulminante da virus dell' epatite B hanno avuto un'incidenza di ricorrenza del 17%. L'analisi della ricorrenza di epatite in relazione 0110 terapia con immunoglobuline antiepatite B ha rivelato chel'incidenza di recidiva e stata del 75% in assenza di ogni terapia, mentre trattamenti a breve termine (meno di 6 mesi di immunoglobuline anti-epatite B) si sono accompagnati 0174% di recidiva. Nei casi di trattamento con immunoglobuline per pill di 6 mesi I'incidenza di recidiva e stata del 36%. Presso il Pittsbur[Jh Transplantation Institute, 69 p,azienh sottoposti a OLT x tra il 1988 e Ii 1992 so no stati analizzati in relazione alia presenza 0 menD di HBeAg ed alia durata del trattamento can immunoglobuline anti-epatite B. L'incidenza media di ricorrenza a 2 anni in ~uesto gruppo di pazienti e stata del 67%. I pazienti sottoposti a trattamento a lunge
termine con immunoglobuline anti-epatite B hanno avuto una incidenza di flcorrenza del 60%, mentre quelli can trattamento inferiore ai 6 mesi hanno avuto una incidenza di ricorrenza del 75%. Un ristretto gruppo, non sottoposto a trattamento con immunoglobuline anti-epatite B (9 pazienti), ha presentato una incidenza dl ricorrenza dell 00%. L'incidenza di ricorrenza a 3 anni nei pazienti HBeAg positivi e HBeAg negativi e stata rispettivamente dell'80% e del 46%. Nonostante I' elevata frequenza di ricorrenza, non crediamo che sia valido abolire di principio il trapianto di fegato nei pazienti aHetti da infezione HBV. Allo state attuale riteniamo che il trapianto sia giustificato per quei pazienti die presentano un basso rischio di recidiva di infezione da HBV (vale a dire HBeAg e HBVDNA negativi). Per quanta riguarda i pazienti ad elevato rischio di recidiva, piuttosto che accettare la sconfitta, riteniamo si debbano studiare gli effetti di nuovi e tlromettenti agenti farmacologici, quali il 3-TC, la timosina 0 il famciclovir, conlinuando gli sforzi per sviluppare nuove strategie per la prevenzione 0 il trattamento della recidiva di epatite B.

presenta un tipico esempio di malattia Rer 10 quale, solo 10 anni ~a, i'OLTx sarebbe stato escluso 0 fortemente controindicato. Con un approccio multidiscipinare ail' abuso alcolico, casi ben selezionati di cirrosi alcolica presentano risultati successivi al trapianto paragonabili 0 quelli di altre malattie benigne [34]. Presso il Pittsburgh Transplantation Institute, I' accettazione di questi pazienti come candidati per trapianto richiede il completamento di un programma di riabilitazione con documentata astinenza. T uttavia, non riteniamo che si possa lasciare procedere verso I' exitus pazienti troppo compromessi per aHrontare un periodo arbltrariamente stabilito di astinenza ed in circostanze simili valutiamo Ie strulture sociali e familiari di supp'orto prima di prendere una decisione definitiva. Questo processo decisionale richiede il coinvolgimento attivo di assistenti sociali, psicoloQi, psichiatri ed altre competenze professlonali.
Epatite Autoimmune
L' epatite autoimmune si presenta piG comunemente nelle donne eli eta compresa tra i 15 e i 25 anni, con un secondo picco dopo la menopausa. La terapia corticoste

immune globulin treahnent had a recurrence rate of 60%, while those who had a treatment lasting for less than 6 months had a recurrence rate of 75%, and a small subset that did not receive any hepatitis B immune globulin treatment (9 patients) had a recurrence rate of 100%. Patients that were HBeAgpositive had a three year recurrence rate or 80%, while those who were HBeAg negative had a three year recurrence rate of 46 %. Despite the high rate of recurrence, we do not believe that there should be an across the board ban on liver transplantation in patients with HBV infection. At this time, we believe it is iustiFied to transplant those patients that are at low risk for recurrent HBV infection (i.e., HBeAg and HBV DNA nefJative). As for those pptients that are at high nsk for recurrence, rather than simply' declaring defeotwe should study the effects of new ana promising afJents, such as 3-TC, thymosin or Famciclovlr, as we continue to strive to develop new strategies for the prevention or treatment of recurrent hepatitis B. Alcoholic Cirrhosis About 10% of the adult poRulation in the United States have a history of alcohol abuse [33J, and alcoholic liver disease is the most common cause of chronic liver disease in western society. Alcohol-induced liver injury is a p'rime example ofadisease in which OLTx might have been precluded, or strongly discouraged, 10 years ago. With a murtidisciplinarr approach to substance abuse, properly selected cases of alcoholic cirrhosis nave outcomes after transplantation that are comparable to those of other non-malignant diseases [34]. At our institution, the acceptance of these patients as candidates for transplantation usually requires a previous successful rehabilitatIon, with documented abstinence. However, we do not believe that patients should be allowed to die if they are too sick to meet an arbitrary period of sobriety and in circumstances such as these we shoufd evaluate the patient's familial and social sUP/20rl structures before makinfl a determination. This process requires an acftve involvement on the part of social workers, psychologists, psychiatrists, and other professionals. Autoimmune Hepatitis Autoimmune hepatitis mos~ commonly presents in women between the ages of T 5 and 25, with a second peak in eost-menopause. Corticosteroid therapy is effective in pro/onginfJ life (5-year survival of 85% in treated patIents, vs. 40% in the control group) [35]. However when liver fundion lias been seriously c~mpromised, and qualifY. of liFe i.s significantly affected, OLTx should be consIdered. We should mention that recently we described a new appro.ach for. th!s group of patients with encouragmg preltmmary results. A gr~up of 21 pa~ients ,,:,ith biops(:proven autoimmune chrOniC adlve hepatItIs were given the new immunosupp'ressive drug tocrolimus (formerly FK506) [16]. After 3 months of therapy the serum AL T level was reduced by 80%, and the AST by 70% [36 J. These data suggest that tacrolimus should be compared to prednisolone in a randomized trial, to identifY. the best available medical treatment. If these preliminary results are conFirmed, it is conceivable that the natural history
of autoimmune hepatitis will be changed such that liver Failure might be averted. Fulminant HeP.9tic Failure A diagnosis of fulminant hepatic failure can be made when jaundice and hepatic encEphalopathy develop within 8 weeks ot onset of the illness in a patient that was previously healthy [37,38]. Some patients may develop encephalopathy after 8 weeks from the onset of symptoms, in the absence of pre-existing liver disease. This subset of patients has been described as "late onset hepotic Failurel/by the English literature [39/, or "subfulminant hepatic failure" by the French literature [38]. The prognosis is very grim in both conditions, with a mortality greater than 70% when the patient progresses 10 grade 4 encephalopathy [.d0-44]. The leading causes are hepatitis viruses and drug hepatotoxicity caused by a variety or agents. The prognosIs changes in relation to the etiology, with acetaminophen-induced liver injury having the best rate of recovery with medICal treatment alone (approximately 50% survival in a series of 32 cases) [44], and fulminant non-A non-B viral hepatitis and other drug hepatotoxicity (i.e. halothane) having the worst outcome. Before 1982, results witli OLT x could not justify advocating it For this patient population [12J, especially since recovery without liver replacement occurred in 5 to 20% of cases [37, 38J. However, the results with transplantation have steadily improved in the past decade, and in a recently' reported series of more than 600 cases of OLTx for fulminant hepatic fai/urer the one-year survival rate was 63?1o [45]. In tight of this progress, OLTx has become WIdely accepted as therapy For this indication (46-49]. The decision to replace an acutely Fo'/lng liver is a most difficult one, and must often be made within a Few hours. A systematic assessment can help establish whether the p.otient belongs to the subset that has a good chance of recovery, or identify those who have a h!gh probability of dying without transplantatIon [50,51 J. As mentioned before, the etIology IS an important Rfognostic determinant /39), with fUlminant hepatitis B and non A non B (most of which, at least in Western countries, have later been shown not to be due to HCV) having the worst prognosis. Other features that predict imminent Cleath include relentless progression to grade 3 or 4 coma, severe coagulop.othy, rapid liver necro~is,. acid,?~is, renal failure, and hemodynamIC Instabt/,ty. The perioperative mortality is often related to brain-stem herniation during or just after the OLTx, sometimes desp'ite the continuous monitoring of intracranial pressure. In general, reFerral to a liver transplant pr99ram should be done os early as possible, because is the only treatment modality for a significant pe~centage of patients with fulminant hepatIC failure {45}. Budd-Chiari Syndrome The Budd-Chiari syndrome, caused by obstruction of the main hepatic veins, can be Found in association with a wide variety of disorders, such as paroxysmal nocturnal he-

Come menzionato in precedenzo, I'eziologia costituisce un fattore importante [?er determinare 10 prognosi [39J: I' epatite hJlminante do virusS e non-A non-S (10 maggior parte delle quali, nei Raesi occidentali, si sonG rivelate non legate 01-I'HCV) e caratterizzata dalla prognosi peg~iore. Altri elementi predittivi di exitus Immlnente includono la progressione irreversibile ai gradi III 0 IV di coma, 10 coagulopatia grave, 10 rapida necrosi epatica, I'acidosi, I'insufficienza rena Ie e I'instabilita emodinamica. La mortalita perioperatoria e in genere secondaria ad erniazione del tronco encefalico durante 0 nelI'immediato post-OLTx, talvolta nonostante il monitoraggio continuo della pressione endocranica. In genera Ie, il ricovero del paziente presso un centro di trapianto di
fegato dovrebbe verificarsi il piu precocemente possibile, rappresentando I'unica terapia per una percentuale consistente di pazlenh con insufficienza epatica fulminante [45].

The number ot different surgIcal procedures that hove been proposed to treat Budd-Chiari syndrome has been reported to be as high as 23 [54/, and the results vary greatly r 55} The (Jrocedures most commonly used in ihe United States are the side-to-side portocaval and mesocaval shunts, when the inferior vena cava and portal vein are Ratent, and the mesoatrial shunt when the inferior vena cava is stenosed or thrombosed and the portal vein is potent. Success rates with these procedures range from 30<;; to 92%, with the maiorily being in the 60°610 75% range [56]. However, it there is progressive liver fciilure, and no concomitant neaplastic disease, Budd·Chiari syndrome can be successfullx managed by OLT x. The first liver transplant for this disorder was performed by our group on November 28, 7974 [57]. This patient is still olive and well 21 years after Ihe procedure, and has hod two children. Since that time, a number of patients with Ihis condition have been treated with liver transplantation at many institutions r 56,58). Based on our eXF?erience, and that of the Cambridge group {58}, it is clear that liver tronsplantation is indicated for the Budd-Chiari syndrome once it has progressed to end-stage liver disease. Whether to transplant or shunt those patients with intermediate degrees of liver Failure is currently uncertain. An interesting point is that the Budd-Chiari syndrome can recur after liver transplantation, as it happened to 3 of the patients we reported recently, all of whom died (56]. Other Parenchymal Diseases There are many other hepatic p'arenchymal diseases for which liver transplantation has been successfully performed, including, but not limited to, cystic Fibrasis [591, congenital hepatic fibrosis, and neanatol hepatitis. Liver transplantation once seemed so drastic a
measure that it was used only as a lost resort For "benign" hepatic disease (60). Today, on the contrary, aI/owing a patient to deteriorate to the point that life-support devices are required before considering transplantation is unacceptable. Cholestetic Liver Diseases Cho/estatic liver diseases are a group of conditions characterized by bile ductin;u'Y. which results in a severe impairment of bile excretion and hepatoce/{ular dysfunction. These events may ultimately lead to cirrhosis. Livertransp{antation has provided patients with these disorders a chance for near-normallong-Ierm survival. Primary Biliary Cirrhosis Primary biliary cirrhosis [PBCT, or chronic non~upp'urative de.strucfive Cholangitis is a d,soraer that primarily affects middleaged women, with a ratio of women to men of approximately 10: I [6 I]. The 5-year surv!va'. of patients with PBC, not treated WIth ',ver transplantotion, ranges fram 30% to 70% [62,63}. The fact that the natural history of PBC is reasonably

well characterized has allowed to study the influence of the stage of the disease on the outcome after liver transplantation [64,65]. In one such investigation [64}, the survival following liver transplantation was markedly b~fter than that predicted (assuming no transplantation) using survival analxsis [66/. It is clear that in PBC the timing of transp7antation is critical, and it shoufCl be carried out before the stage of catastrophic deterioration is reachecf The quality of liFe of individuals transplanted for PBC has been very good, with ninety eercent of the long-term survivors being fully rehabilitated [67]. However, we should note here that, while the I-year survival rates after transplantation were 83%, 75%, and 58%, according to the candidate risk category (low, medium, and high, respectively), the actual gain in survival is particularly significant in tfie highrisk candidates {64]. In fact, if we compare these survival rates with the ones predicted by the Mayo prognostic model For PBC [66}, there is a 58% gam in survival at I-year in the high-risk candidates, 55% in the mediumrisk candidates, and only 14% in theiow-risk candidates [68]. These data should be kept in mind when establishing candidacy and liver transplantation priority for this category of patients. Biliary Atresia Biliary atresia is by For the most common indication for liver transplantation in children {69,70]. The disease is already obvious in the neonate, characterized by mcreasingly severe chorestasis during the first 90 days of life and, unless treated, it is universally fatal. Children with this anomaly are often treated with a portoenterostomy (Kasai operation) [71}. However, only a small subset has been actually cured by this procedure, and most children die of liver failure that continues to progress after surgery. It is estimated that 7556 of the patients suffering from biliary atresia will eventually require a liver transplant, regardless of any previous surgical treatment 133]. Long term survival after liver transplantation is excellent, ranging between 8mb and 85% in most series, with an essentially flat survival curve after the first 6 months. Tfie quality of life of the lon9-term survivors is excellent, and the majoflty of these children have a very satisfactory growth and intellectual development [72}. Sclerosing Cholangitis Prim or;: sclerosing cholangitis [PSC] is a chronic cholestatic liver disease which is extremely difficult to define accurately [73,74]. It 15
recognized by liver dysfunction and a characteristic radiologic appearance (with multiple localized strictures and dilatation, in the absence of known causes of cholangitis, such as operative trauma, calculus disease, or congenital anomalies), which is related to portal tract inflammation, bile duct proliferation, and periductal fibrosis involving small intrahepptic and large extrahepatic ducts. The etiology is still unknown, and no curative medical treatment has been identified [74]. In contrast with PBC, PSC affects mainly men [sex ratio 4: I] who are 30 to 50 years old. Until recently, patients with PSC offen unde-

Non e tuttora definito se sottoporre a trapianto 0 a derivazione porto-sistemica quei pazienti che presentano gradi intermedi di insufficienza epatica. Un elemento interessante e rappresentato dalla nozione che la sindrome di Budd-Chiari puc ricorrere do po trapianto di fegato, come in 3 casi della sene do noi recentemente descritta, tutti evoluti successivamente all'exitus [56J.
Altre Malattie Parenchimali
Numerose so no altre potologie epatiche parenchimali per Ie quali il trapianto di tegato e stato eseguito con successo, inclusela fibrosi cistica [59], la fibrosi epatica congenita e I' epatite neonatale. In passato, il trapianto di fegato appariva come una misura COS! drastlca do essere utilizzata, nelle malottie epatiche "benigne", solo come risorso estrema r 60]. Attualmente, 01 controrio, e inaccettabile lasciare che un paziente possa deteriorare fino 01 ricorso al supporto mecconico delle funzioni vitali prima che il trapianto vengo preso in considerazione,

Cirrosi Biliare Primaria

tropianto di fegato, varia dal 30% 0170% [62,63]. II fotto che 10 storia noturale della PBC sia abbastanza ben carotterizzata, ha consentito di studiore I'influenza dello stadio d~lIa malattia sui risultati dopo trapianto dl fegato [64,65]. In uno di questi studi f641,-la soprawivenza do po trapianto di fegato e multata significativamente migllore rispetto a 9uanto stimato con I' analisi della soprawlvenza in assenza di trapianto J66J. E' chiaro che nella PBC il timing el trapianto assume un'importanza cntica e questa non dovrebbe essere effettuoto prima che 10 malattio abbia raggiunto uno stadio molto avanzato di compromissione funzionale. La qualita della vita dei pazienti sottoposti a trapianto (Jer PBC e risultata molto buona, con il 90% dei soprawissuti a lunge termine
totalmente riabilitati [67). T uttavia, e do notare che, mentre Ie percentuali di sopravvivenza ad 1 anna dal trapianto era no dell'83%, 75% e 58%, a secondo della categoria di rischio del candidato (rispettivamente basso, medio e alto), I' effettivo miglioramento in termini di sopravvivenza risulta particolarmente significativo nei candidati ad alto rischio [64]. In effetti, conFrontando queste percentuali di sopravvivenza con quelle previste dol modello prognostico "Mayo" per 10 PBC [66], si evidenzia un incremento della sopravvivenza ad 1 anna del 58% nei candidati ad elevato rischio, del 559b nei candidati a media rischio, e solo del 14% nei candidati a basso rischio [68]. Questi dati vanna considerati nella definizione della candidatura e della priarita del. trapiantc? di. fegato per que$ta categona di pazlentl.
Atresia Biliere
L' atresia biliare costituisce di gran lunga I'indicazione piu frequente 01 trapianto di fegato in eta pediatrica [69, 70]. La malattia e gia evidente nel neonata, caratterizzata do colestasi in~Jfavescente durante i primi 90 giorni di vita e se nan tra tta 10 , risulta inevitabilmente (atale. I bambini con questa anomalia vengono spessa trattati con portoenterostomia (operazione di Kasai) [71]. Tuttavia, solo una percentuale minore di casi e stata efficacemente curata can questa pracedura, mentre 10 ma~giar parte dei bambini muoiona per Imsufficienza epatica che continua a progredire dopa I'mterventa. E' stimato che il 75% dei p'azienti affetti do atresia biliare richieaano infine il trapianto di fegata, indipendentemente do agni precedente trattamento chirurgico [33]. La sopravvivenza a lunga termine dopa trapianta di fegata e eccellente, variando dall'80% a1l'85% nella maggior parte dei casi, con una curva di sapravvivenza essenzialmente piatta dapo i primi 6 mesi. La qualita delia vita del sopravvissuti a lunge termine e eccellente e 10 maggior parte di questi bambini hanna una crescita e uno sviluppo intellettuale molto soddisfacenti [72].
Colangite Sclerosante Primaria
La colangile sclerosante primaria [Prima!)' Sclerosing Cholangitis -PSC-] e una malattia epatica cronica colestatica che risulta estremamente difficile da definire accuratamenle [73,74]. Viene riconasciuto per 10 disfu~zione eRotica secondoria e per un asr ''J radialo.9ico caratteristico (con str . losi multiple ~ocali alternate a dilate .ioni, in assenza di cause note di colangite( quali traumi oReretori, calcoli 0 anoma ie congenitel, che e secondo ria a flogosi dello spazio porta Ie, proliferazione outtale e fibrosi peridutta-
Ie a carico dei piccoli dotti intraepatici e dei dotti extraepatici maggiori. L eziologia e ancora sconosciuta e non e state Identificato alcun trattamento medico curativo [74). Contrariamente 0110 PBC, 10 PSC colfisce principalmente maschi [rapporto 4: L di eta compresa tra i 30 e i 50 anni. Fino a poco tempo fa, i pazienti affelti do PSC venivano spesso sottoposti a vari interventi sullo via biliare a scopo diagnostico 0
Ralliativo (quali I' esplorazione chirurgica dell' albere biliare, posizionamento di orenaggio a T, il posizionamento di stent endobiliare e/o 10 derivazione biliodigestiva). T utte queste procedure chirurgiche non hanno determinato particolari 5enefici a lungo termine [75], mentre hanno incrementato 10 sviluppo di episodi colangitici e di tenaci aderenze a livello dell'ilo epatico. II trapianto di fegato offre I' unica prospettiva dl cura e sopravvivenza a lunge termine per i pazienti affetti do PSC nella fase terminale della malattia, e tutti ali altri approcci invasivi dovrebbero essere abbandonati (ad eccezione della colangiografia endoscopica retrograda e della biopsia epatica percutanea). Recentemente, 10 sopravvivenza di 216 pazienti trepianlati per PSC e stata confrontata con un modello di previsione statistica del decorso della malattia (anche in questa coso considerando I' assenza di trapianto) [76). II modello, che considera reperti fisici obiettivi, caratteristiche biochimlche e istopatologiche della PSC, e state sviluppato in uno studio su 426 pazienti affetti do PSC e condotto in 5 istituti in Europa ed in Nord America [77]. La sopravvivenza attuariale a 5 anni (KaplanMeier) dopo trapianto di fegato e stata del 73% rispetto do una sopravvivenza prevista del 28% secondo il modello menzionato piu sopra. Analogamente a quanta osservato nei pazienti affetti da PBC [68], I'incremento pili rilevante nella soprovvivenza dei pazienti affetti da PSC si e verificato nel gruppo a rischio piu elevato. Se paragoniamo Ie percentuali di sORravvivenza con 9uelle ,previste dol modello prognostico 'Mayo per la PSC [77], I'incremento nella sopravvivenza ad un anna successiva a trapianto di fegato e solo del 7% nei f'azienti a basso rischio e raggiunge il 40% nel gruppo a rischio elevato l68). Anche in questa coso, questa elemento dovrebbe essere tenuto in serio considerozione 01 momenta della definizione delle strategie per 10 selezione del candidato e dei criteri oi priorita per il trapianto.
Altre Malattie Colestatiche
II trapianto di fegato e indicato come trattamento chirurgico anche per altre patologie colestatiche. La cirrosi biliare secondaria, 10 malattia di Caroli {condizione caratterizzata do dilotazione biliare intraepatica congenita associata a colan-
rwent different biliary operations for diagnostic or palliative purposes (such as suraical exploration of the btliary tree, T-tube pTacement, biliary stenting or reconstruction, or both). All tnese procedures failed to achieve long-term benefits [75/, and iust served to promote episodes of cholangitis and strong adhesions in the hepatic hilum. Liver transplantation offers the only prospect for cure and lonG term survival to patients with PSC in the terminal stage of the disease, and 0/1 other surgical approaches {with the exception of endoscopic retrograde cholangiography and percutaneaus liver biopsy} should be abandoned. Recently, the survival of 216 patients transplanted for PSC was compared to the pred,ctions made by' survival anotsis (again, assuming no transplantation) {76 . The model incorporates physical finding? iochemical and histopathoiogic features ot PSC, and was developed in a study of 426 patients affected by PSC and managed conservatively at 5 institutions from Europe and North America [77]. The 5-year Kaplan-Meier actuarial survival after liver transplantation was 73~6, compared to 28% expected survival according to the above mentioned model. Similarly to what is observed with PBC patients {68}, the most important gain in survival in patients with PSC corresponds to the high-risk group. If we compare the survival rates with the ones predicted by the Mayo prognostic model for PSC [77J, the gain in 7 -year survival rate with liver transplantation is only 7% in the low-risk patients, and as high as 40% in the high-risk group {68]. Again, this information should be seriously consIdered when Formulating policies for candidate selection and transplantation priority criteria. Other Cholestatic Diseases Liver transplantation is indicated as surgical treatment of a number of other cholestatic diseases. Secondary biliary cirrhosis, Caroli's disease {a condition charoclerized by congenital intrahepatic biliary dilatation associated either with recur' rent cholangitis or cirrhosis}, familial cholestasis, and Alogillii's syndrome are the most important diseases in this group. In a series of 70 fX!tients transplanted For Caroli' 5 disease at the Pittsburgh Transplantation Institute, 7 patients were alive from 2. 1 ta 7. 6 rec!rs after liver transplantation (mean =4.8 yea"rs) [78]. TheroleoFtransp'lanta· tion was also studied in a series of 75 children affected by familial cholestasis. Eleven children were alive andgrowing well From 7.8ta6.2years aftertronsplantation [79]. A/q/ille's disease is on autosomic disorder charoctertzed by intrahepatic cholestasis and peripheral pulmonary artery stenosis. Liver transplantation is indicaied in a subset of these patients {usually about 12%} who develop cirrhosis, and in the ones with prr;>gressi' veporlo/ fibrosis, severeandpermanentchOlestasis pror:;ressive liver dvsfunction, and growth retardation. In a series of 13 potients transplanted for A/ogille's disease, the 1 and 7 year survival rate was 52%. Of the 6 mortalities 4 occurred within 11 doys of transplant, and the other 2 before the efeventh postoperative month. All survivinf} children were growing well and participating In normal childliOOd a--tivities [80]. Congenital Errors of Metabvlism Several studies showed in the early 7 960' s that liver allografts retain their metabolic spe-

cificity offer transfer to a new host fBI ,82]. It was obvious already at that time that a p'ractical implication of this foctis that certain liverbased metabolic disorders can be cured by liver transplantation. The resolution of gout, naturallypresenf in Dalmatian dogs, after liver transplantation using mongrel canine donors conclusively proved/his hypothesis [83]. The fact that the graft maintained its phenotype resulted in tlie cure of congenital errors of metabolism in many patients treated by liver transplantation for end-stage liver diseases f59,84-94/, and anatomically normal livers have also been replaced to correct congenital metabolic defects [95-99]. As the other side of the coin, there is at least one reported case where a coagulation defect of the donor was transferred to the recifJient by liver transplantation {1 00 I. At least 16 distinct inborn errors of metabofism have been treated with liver transplantation, including T-I-antitrypsin deficiency', Wilson's disease, tyrosinemia and hemochromatosis [14]. Manyother cannot be cured by liver transplantation and might benefit from allogeneic bone marrow transplantation [701/. The guidelines for decision making in speciFic metabolic errors became increasingly clear in the past decode, allowing better care of these patients [93, 10 I]. Tumors

La malattia di Alagille e un disturbo autosomico caratterizzato do colestasi intraepatica e stenosi all' arteria polmonare. II trapianto di fegato e indicato nel gruppo di questi pazienti (circa il 12%) che sviluf?pano cirrosi ed in quelli che presentano fibrosi portale progressiva, colestasi grave permanente, dlsfunzione epatica progressiva e ritardo nella crescita. In un gruppo di 13 pazienti trapiantati per malattla di Alagille, Ie percentuali dl sopravvivenza sono risultate del 52% (can un follow up do 1 a 7 anni). Dei 6 decessi, 4 si sono verificati entr~ i primi 11 giorni dal trapianto, e gli altri 2 prima dell'undicesimo mese postoperatorlo. Tutti i bambini sopravvissuti sono cresciuti normalmente e

Primary hepatic malignancies were originally considered "the indication" for liver transplantation. Removal of the diseased organ was seen as the best treatment for hepatic lesions that could not be treated with conventional fechniques of subtotal liver resection [60]. As a consequence of this concept, II out of the first 25 patients transplanted at University of Colorado [8] had a hepatocellular carcinoma. However, the resufts with malignant tumors did not fulfill these expectations, with high mortality rates due to recurrence [2]. The prognosis proved to be different in small, incidental tumors. The longest survivor in this group is a patient who underwent liver transplantation for biliaIY. atresia on January 22, 1970. The experience with liver transf?lantation for benign tumors of the liver is stdllimited, and has yet to be defined. We have previously reported a small series of patients transplanted for hepatocellular adenomatosis and focal nodular hyperfJlasia, with 4 out of 5 patients alive from 4. I to 9.6 years after liver transplantation. All of these p'atients were activated as transplant candidates because of progressive liver failure related to multiple lesions that occupied at least 80% of the hver parenchyma, and that were not resectable by subtotol hepatectomy [102]. Primary Malignant Tumors In spite of disapP<?intments due to the poor long-term results, liver malignancies are still treated with transp'lantation in many centers. We believe ihat two points should be made clear when treating these patients. First, the uncertainty of the proenosis must be openlr.discussed with the p,c!hent. Secondly, possible metastases should be aggressi-

ERRORI CONGENm DEl METABOUSMO
Nei primi anni '60, parecchi studi avevano dimostrato che i fegati trapiantati mantengono 10 loro speci~icita metabolica dopo il trasferimento nel nu~vo ospite [81,82]. Gia a quel tempo risulto chiaro che un'implicazione pratica di questa dato cansiste nel fatto che alcuni disturbi del metabolismo dipendenti dol fegato possono essere curati can il trapianto di fegato. La guarigione della gotta, malattia congenita nei coni Dalmata, dopo trapianto di fegato usando donatori canini di razza mista, ha dimostrato in maniera conclusiva questa ipotesi [83]. La conservazione do parte dell' organo trapiantato del fenotipo nativo ha portato 0110 cura di errori congeniti del metabolismo in molti pazienri soltoposti a trapianto di fegato per insufficienza epatico terminale [59,84-94] ed anche ~ati anatomicamente normali sonO stati soshtuiti per correggere difetti congeniti del metabolismo [95-99]. D'altra parte, e stata descritto almeno un coso in cui un difelto di coagulazione del donatore e state trasferito 01 ricevente mediante un trapianto di fegato [100]. Almeno 16 diversi tipi di errori congeniti del metabolismo sono stati curati mediante trapianto di fegatot inclusi il deficit di T-l-antltripsina, la malattia di Wilson, 10 tirosinemla e I'emocromatosi [14]. Molte altre condizioni non J?qssono essere curate mediante trapianto di fegato e potrebbero beneficiare del trapianto di mldollo osseo [101). tendo un migliore traltamento di questi pazienti [93,101].
TUMORI
I tumori primitivi del fegato sona stati considerati originariamente I'indicazione aI trapianta di fegato. L' aSRortazione delI' organo malata era consiClerata come la miglior cura per lesioni epatiche che non potevano essere traltate can tecniche convenzionali di reseziane subtatale del fegato [60]. Come conseguenza di questa concelta, 11 dei primi 25 pazienti sottaposti a trapianta alia University of Colorado [8) erano affelti da carcinoma epatocellulare. T uttavia, i risultati del trapianto eRotica per tumori maligni non hanno sodClisfatto Ie aspettative, a causa dell' elevata incidenza di mortalita secandaria a recidiva neaplastica [2]. La prognosi e risultata invece molto migliore per i tumori incidentali di piccole dlmensioni. La piG lunga soprawivenza in questo gruppo e stata registrata in una paziente soltoposta a trapianto di fegato p'er atresia biliare il 22 gennaio 1970. II fegato rimosso presentava un epatoma di 3 em di diametro: la paziente e tultora viva e sana, 25 anni do po il trapianto.
T umori Benigni
L' esperienza con il trapianto di fegato per tumori benigni e limitata e deve ancora essere definlta. Abbiamo in precedenza descritto una serie di pazienti sottoposti a trapianto di fegato per adenomatosi epatocellulare e iperplasia nodulare focale, con 4 pazienti su 5 vivi do 4.1 a 9.6 anni dopo il trapianto. T ulti questi pazienti erano candidati a trapianto per insufficienza epatica progressiva secondaria a lesioni multiple che sostituivano almeno 1'80% del parenchima epatico e non erano susceltibili di resezione mediante epatectomia subtotale [102].
Tumori Maligni Primitivi
Nonostante il disappunto dovuto agli scarsi risultati a lunge termine, in numerosi centri i tumori primitivi del fegato vengono ancora traltati con il trapianto. Ritenlamo che due elementi dovrebbero risultare ch iari nel traltamento di questi pazienti. Innanzitulto, I'incertezza della prognosi deve essere discussa apertamente con il paziente. In secondo luogo, 10 presenza di eventuali metastasi deveessere indagata in maniera aggressiva prima della candidature 01 traplanto. I pazienti affelti do tumore del fegato con fUnzione epatica conservata possono essere spesso trattati con resezioni epatiche. In uno studio che ha analizzato I' outcome di pazienti affetti da epatoma, 76 dei quali trettati con resezione epatica subtotale e 105 sottoposti a trapianto di fegato, Ie
percentuali di soprawivenza ad 1 e 5 anni nel gruppo soltoposto a resezione sono state rispettivamente del 71 % e del 33% mentre nel ~ruppo sottopasto a trapianto so no state nspettivamente del 66% e del 36% [103]. Nonostante una aggressiva valutazione preoperatoria per escludere malattia extraepatica, la recidiva neaplastica dopo il trapianta di fegato per tumari malisni e invariabilmente elevata [2, 104-108 j. Allo state attuale associamo routinaria mente il trattamento chirurgico can 10 chemioterapia adiuvante che, in linea di principio, presenta una maggiore efficacia terapeutica con la somministrazione loco-regionale (attraverso il ramo dell' arteria epatica afferente alia neoplasia). II razionale della chemioterapia intra-arterio so epatica e rappresentato dalla considerazione che il parenchima normale dispone di una doppia vascolarizzazione, mentre I' epatoma nceve I' apporto ematico principalmente attraverso la neovascolarizzazione arteriosa [109]. La p'ossibilita che questi vantaggi tearici al:ibiano un impatto nella realta clinica resta ancora do verificare. Alcuni tipi di tumori maligni del fegato presentano un outcome mlgliore rispetto ad altri. Mentre i tumori di Klatskin hanno 10 soprawivenza a lunge termine piG inFausta (con nessuna soprawivenza a 4.5 anni in una serie di 10 pazienti) (109], gli emangiaendoteliomi epitelioidi presentano una soprawivenza attuariale a 5 anni del 67% [11,112].
Tumori metastatici
II trapianto di fegato e stata utilizzato per il trattamento di un numero molto limitato di tumori metastatici a livello epatico [105-107,113-116J. Abbiamo recentemente riesaminato (dati non pubblicati) una serie di 5 pazienti sottoposti a trapianta di fegato presso il Pittsburgh Transplantation institute nel Reriodo 1981 -1987 per tumore neuroendocrino metastatico al Feaato [116]. T re di questi 5 pazienti sono deceCluti in menD di 9 mesi dal trapianta. Gli altri due pazienti, soprawissuti a lunge termine, rianno presentato recidiva neoplastica, con un paziente deceduto 76 mesi dopo il trapianto e I' altro vivo (giugno 1994) a distanza di 9 anni dal trapianto. Ouest'ultimo paziente era stato soltoposto esclusivamente a trapianto di fegato con linfoadenectomia {possibile tumore primitivo a livello pancreaticoj. Ouesta limitata esperienza non consente la definizione di linee-guida.
RISULTATI DEL TRAPIANTO 01 FEGATO AL pmS8URGH TRANSPLANTATION INSTITUTE
Abbiamo analizzato I'outcome di 1501 Razienti consecutivi soltoposti a trapianto CJi fegato in un periodo di 4 anni (1 genna-
vely searched for before candidacy' for transplantation is established. Individuals with hepatic tumors and normal liver function can offen be treated with liver resections. In a study that analyzed the outcome of p?tients with hepatoma, 76 treated by subtotal liver resection and 105 with liver transplantation, one and Five year survival rates In the resection eroup were 71 % and 33%, resp'ectively, and In the transplant group 66% and 36%, respectively [I 03J. In spite of aggressive preop'erative evaluation to rule out extrahepatic disease, recurrence after liver transp.lantation for malienant tumors is invariably high [2, 104-108 j. Today, we routinely combine the surgical treatment with ad;uvant chemotherapy which, theoretically, should work better when administered r~ionally' (through the hepatic artery branch reeding the tumor). The principle underlying this concept is that normal parenchyma has a dual blOod supply' whereas hefX!tomas receive their blood suppfy mainr through the arterial neo-vasculature [/09 . Whefher these theoretical benefits will translate into reality remains to be seen. Certain types of malignant liver tumors have a better outcome than others. While Klatskin tumors have the poorest long-term survival (with no survivors at 4.5 yeors in a previous series of 10 patients) [I 09}, epithelioid hemangioendotheliomas have a 5-year actuarial survival rate of 67% {II 7, 7 72]. Metastatic Tumors Liver transplantation has been used in the treatment of a velY, limited number of tumors metastatic to the liver {lOS-I 07 I 73-1161. We have recently updated (unpub{;shed data) the follow-up at 5 patients who underwent liveriransp'lantation atour institution, between 1981 and 1987, for a neuroendocrine tumor metastatic to the liver{116J. Three out of these 5 patients died in less than 9 months. The other two, long term survivors, also had recurrence, with one patient dy'ing 76 months after transplantation, and the other still alive (June, 1994) 9 years after the transplant. This last pptient had only' a liver transplant plus node dissection (passible primary tumor in the BOncreos). This small experience makes it difficult to derive any guidelines. RESULTS OF UVER TRANSPLANTATION AT THE PITTSBURGH TRANSPlANTATION IN-SnTUTE . We analyzed the outcome of 1,501 consecutive patients who underwent liver transplantation in a 4year period (January I, 1990-December 31, 1993). Actuarial survival curves were estimated using the Kaplan-Meier method. ComPCJrisons between groups were made using fhe Bre~low test, wi/f! a Bonferr~ni ad;ustment for multIple comp~tnsons. The etloIqgy of the liver disease in this papulation is snown in Table 1. There were 1,286 adults and 215 children at the time of their first liver transp.lant(there were 189 retranplants in this series), with 934 males and 567 females. The mean age was 43.3 +/- 19.1 y'ears (range 0.1 to 76.2), and the mean fOllow-up was 2.15 years (range: 0 to 4.5 years). The overall actuarial patient survival rates were 81 %, 77%, 74%. and 73%, at 1 2, 3, and 4 years, res~tiv~fy [Figure ]). The overall actuarial graft surviva rates were 72%,68%,65%, and

64%, at /, 2, 3, and 4 yeors, respectively (~igure /). Traditionally, patient survival after lIVer transplantation IS reported separately for adults and.. children, with the latter usuallx showing significantly better survival. The 275 children « 7 8 yeors) had overall actuarial survivals of 82%, 87%, 80%, and 80%, at / r 2, 3, and 4 yeors, respectively (Figure 2), wnile the 7,286 adults (> 78 years) hod overall actuarial survival of 87%, 77%, 73%, and 77 %, at 7, 2, 3, and 4 years, respectively (Figure 2). This difference however, is not significant (p = 0.298). When the entire pa' tient population is analyzed according to the diagnosis, after collapsing them into four brood ca~ories (cholestatic, alcoholic, parenchymal and metabolic diseases, hepatitides) as expected, the cholestatic diseases showed superior results (Figure 3). The actuarial survival rates for chorestatic liver diseases were 90%, 88%, 87% and 87~6, at 7, 2, 3 and 4 years, respective& (FffJure 3); for the alcoholic group they were 82?o, 8m6, 75;6, and 70%, at 7,2, 3, and 4 years respectively (Figure 3); for the parenchymal and metabolic disease group, which included all the other parenchymal diseases (excluding tumors), the survival rates were 78%, 75%, 72%, and 77 %, at 7, 2 3, and 4 yearsf respectively (Figure 3); and for the hepotitiaes they were 79%, 73%,70%, and 70%, at 7, 2, 3, and 4 years, respectively (Figure 3J. When compared with each other, patients undergoing lIVer trans~/antation for cholestatic diseases fared signtficantly better (cholestatic vs hepatitides: p <O.OOOT; cholestatic vs alcoholic cirrhosis: p <0.0007; chofestatic vs parenchymal and metabolic diseases: p <0.00/). The patients who underwent transplantation for a malignant primary or secondary liver tumor were also analr.zed separatelY. and their survival compared with the rest of the patients (a number of these patients -see Table /- had on incidental tumor; however, they are not included with the "tumor" patients because their p,rognosis has been found in the post to be similar to that of potients with benign 'disease). The actuarial survival rates for patients undergoing tron~/antation for cholangiocarcinoma were 70%, 50%, 50%, and 50%, at 7, 2, 3, and 4 years respectively (Figure4-only 7 Ratientatriskat4years). The actuarial survival rates for patients with a known hepatoma before liver transplantation were 74%, 65%, 58%, and 58%, at 7, 2, 3, and 4 years, respectively (Figure 4- only 7 patients at risk at 4 years). The actuarial survival rates for patient undergoing OLTx as treatment for a tumor metostatic to the liver were 86%, 68% and 68%, at 7, 2, and 3 years, respectively (Figure 4 -only 2 patients at risk at 3 yeors). La popolazione era composta da 1286 pazienti adulti e 215 pazienti pediatrici 01 momenta del lora primo trapianto di fegato (in questa serie ci sono stati 189 retrapianti), con 934 maschi e 567 femmine. L'eto media era 43.3+1_19.1 anni (range 0.1-76.2 anni) ed il periodo media oi follow up e stato di 2.15 anni (range 0-4.5 anni). La soprawivenza attuariale del paziente e stata del81 %,77%,74% e 73%, rispeltivamente ad 1, 2, 3 e 4 anni (Fig.l). La soprawivenza attuariale del f~ato trapiantatoe statadel72%, 68%, 6576e64%, rispeltivamente ad 1,2,3 e 4 anni (Fig. 1 ). In accordo con una distinzione tradizionalmente effeltuata, 10 soprawivenza del paziente dopo trapianto di fegato viene aescritta separatamente p'er pazienti adulti e pediatrici; questi ultimi, in g.enere, hanno sempre avuto una soprawlvenza significativamente superiore. Nella nostra popolazione in esame, 10 soprawivenza attu£:lfiale dei 215 pazienti pediatrici (eto<18 anni) e stata deIl'82%, 81%, 80% e 80%, rispettivamente a 1, 2, 3 e 4 anni (Fig.2), mentre i 1286 adulti (et~> 18 anni) hanno presentato una sOj?rawlvenza attuariale dell'81 %, 77%, 73% e 71 %, rispettivamentea 1,2,3 e4anni (Fig.2). Questa differenza non e risultata statisticamente significativa (p=0.298). Quando I'intera popolazione e stata analizzata in relazione alia diagnosi, dopo la dassificazione dei p'azienti in quattro categorie (patologie colestatiche, cirrosi aleolica, patologle parenchimali e metaboliche, ep'atiti), Ie malaltie colestatiche, come p-revedibile, hanna avuto i risultati migliori (Fig.3). Per Ie malaltie colestatiche, la soprawivenza altuariale e stata del 90%, 88%, 87% e 87% rispeltivamente ad 1 2 3 e .4 anni (Fig.' 3); per 10 cirrosi afcofico e stata de",S2%, 80%, 75% e 70%, rispettivamente ad 1, 2, 3 e 4 anni (Fig.3)· RSr Ie patologie parenchimali e metaboliche, comprendenti tutte Ie altre malattie parenchimali \esdusi i tumori), la soprawlvenza attuaria e e stata del 78%, 75%, 72% e 71 %, rispettivamente ad 1, ~! ~ e 4 onni (Fig. 3) mentre per Ie epallh e stata del 79% 73% 70% e 70%, rispettivamente ad 1, 2; 3 e 4 onni (Fig.3). Sono stati inoltre analizzati separatamente i pazienti sottoposti a trapianto per neoplasia maligna primitiva a metastatica del ~egato e la loro soprawivenza e stata confrontata con Ie altre categorie di pozienti (alcuni pazienti - come indicato nella Tab. 1 -presentavano un tumore incidentaIe, tuttavia non sana indusi nel gruppo dei pazienti neoplastici poiche la loro prol;lnosi si e rivelata simile a quella dei pazlenti can patologia benigna). La soprawivenza altuariale dei pazienti sottoposti a trapianto di fegato ~er colangiocarcinoma e stata del 70%, 50 Ib, 50% e 50%, rispeltivamente ad 1,2,3 e 4 anni (Fig.4: solo 1 paziente a rischio a 4 ann i). La soprawivenza attuariale dei pazienti con epatoma diagnosticato prima del trapianto e stata del 74%, 65%, 58% e 58%, rlSp'ettivamente ad 1, 2, 3 e 4 anni (Fig.4: solo 7 pazienti a rischio a 4 anni). La soprawivenza attuariale dei pazienti sottoposti a OLTx per il trattamento di una neoBlasia metastatica al fegato e stata dell 86%,68% e 68% rispettivamente ad 1, 2,3 anni (Fig.4: solo 2 pazienti a rischio a 3 anni). Questi risultati, sorprendentemente positivi, potrebbero dipendere dol falto che la maggior parte dei tumori metastal'ici sono stah eli natura neuroendocrina, che e caratterizzata da un comp'ortamento biologico peculiare. Infine, la soprawivenza attuariale dei pazienti senza aleuna evidenza ,ereoperatoria di epatoma e stata dell'sn, 79%, 77% e 75%, risRettivamente ad 1, 2, 3 e 4 anni (Fig.4: 159 pazienti a rischio a 4 onni). Come previsto, i pazienti con tumore epatico primitivo non evidenziabile prima del trapianto hanna presentato percentuali di soprawivenza molto piu elevate. Queste differenze sana risultate significative confrontando Ie percentuali di sopravvivenza di questa gruppo con quelle dei pazienti affelti do epatoma diagnosticato prima del trapianto (p=0.OOO6) e da colangiocarcinoma (p=0.04). La differenza di sOflrawivenza tra i pazienti con tumore primltivo del fegato non evidenziabile prima del trapianto e neoplasia metastatica non e risultata significativa (dato molto probabilmente legato al fatto che il camp'ione di popolazlone in esame e stato di aimensioni molto piccole). L' outcome nella nostra popolazione di Ol T x e stato ineltre analizzato in rapporto alia dassificazione della United Network for argon Sharing (UNOS). L'UNOS e I'organizzazione che gestisce la distribuzione degli organi n~li Stati Uniti. Quando un pazlente e inserito nel sistema come candidato 01 trapianto gli viene assegnato un punteggio che e indice della gravita della progressione della malattia. ONOS 2 indica che il paziente e stabile e puo aspettare il trapianto a casa.

UNOS 4 indica che Ie condizioni cliniche sono cosi compromesse do richiedere il sostegno meccanico delle funzioni vitali. La sORrawivenza attuariale per L pazienti UNOS 2 e risultata de1l'85%, 82%, 79% e 79%, rispettivamentea 1,2, 3 e4anni. Per i pazienti UNOS 3 e stata de1l'86%, 82%, 78% e 78%, rispettivamente a 1, 2, 3 e 4 anni. Infine, per i pazienti UNOS 41a soprawivenza attuariale e stata del 75%, 72%, 69% e 67%, rispettivamente a 1, 2, 3 e 4 anni (Fig.S). Non e stata evidenziata aleuna differenza statisticamente si.fjnificativa tra pazienti UNOS 2 e UNOS 3. CONSIDERAZIONI PARTICOlARI
Ci 50no aleuni altri aspetti riguardanti 10 candidatura 01 trapianto di f~ato che richiedono aleune considerazionl specifiche. Nella maggior parte dei centri in Europa ed in morti neg Ii Stati Uniti, viene attuato II principio di stabilire un limite 0110 candidatura in rapporto all'eta del ricevente, AI Pittsburgh Transplantation Institute non viene conslderato singolarmente un limite superiore d'eta, in quanto nella nostra esperienza e dimostrato che i pazienti piu anziani presentano percentuali di sopravvivenza a 5 anni simili a ~uelle dei gruppi di pazienti piu giovani [117,1181. La trombosl delle vena porta e70 delle vene mesenterica e splenlca, costituivano in passato una controindicazione al traRianto di Feaato. Questi problemi sono stati quasi interamente superati doll' utilizzo di grafts venosi del donatore [119,l22J. Inoltre, una pre-

The difference in survival rate between patients with no known tumor and metastatic disease was not signiFicant (most likely the result of the small sample size). Outcome was also anaJyzed according .to the United Network for Organ Sharing (UNOS) classification. UNOS is the organization managing organ al/ocation in the United States. When a patient is entered into the system as a transplant candidate he is given a score according to the severity to which the disease has progressed. UNOS 2 indicates that the patient is stable and can wait for the transplant at home. UNOS 3 means that the patient is medically unstable and needs continuous hospitalization. UNOS 4 indicates that the potient's condition has deteriorated to the point at which life-support systems are required. Actuarial survival rates for UNOS 2 patients were 85%, 82%, 79%, and 79%, ot T, 2, 3, and 4 years, respectivelv. For UNOS 3 patients they were: 86%, 82%, 78%, and 78%, at I, 2, 3, and 4 years, respedively. Finally, For UNOS 4 patients they' were 75%,72%, 69%,and67%, at I, 2, 3ana 4 years, resp'ectively (Figure 5). There was no statistical difference between UNOS 2 and UNOS 3 p'atients. However, both UNOS 2 and UNOS 3 patients had significantly better outcome than UNOS 4 patients (UNOS 2 vs UNOS 4: p=0.005; UNOS 3 vs UNOS 4: p <0.000 I). SPECIAL CONSIDERAnONS There are few other issues regarding candidacy to liver transplantation that snould be discussed. It is a policy in most centers in Europe, and in many in the United States, to limit candidacy based on the age of the recipient. A simple upper age limit is not used at our Institute, since It has been our experience that older patients have 5-xear survIVal rates that are similar to that of y'ounger groups [117,178J. Thrombosis of the portal vem, and/or the mesenteric and splemc vein, were in the past considered as contraindication ta liver transplantotion. These problems were almost completely eliminated by the use of cadaveric vein grafts {7 19-122}. Also, previous abdominal surgery can seriously complicate the transplant operation, and it was once considered a contraindication to liver transplantation by' many liver transplant centers. Nowadays, these technical issues are not considered an obstacle at any mal'or transplant center. The last issue we wou d like to address is related to the question of candidacy to liver transRlantation for the patients carrying antiboClies to the human immunodeficiencyvirus (HIV). After the screening enZ)'(TIe immunoassax fOr detecting HIV antibOdies became avaIlable in Marcn 1985, a number of positive kidney, heart, and liver recipients were quicklX reported [123, 124 J. However, the extent of the problem was clearlY defined only after a large study' was completed at the Umversity of Pittsburgh. The stored sera of 1,043 trpnsplanted patients were tested for HIV antibodies, and 1.1% were found ta be pqsitive[l25j. Uvertransplantpotientsshowed a higher rislY, with an incidence of 2.6%. One thirdofthese liverpa/fents '.It1;Jrepositive before the transplant. Fifteen liver patients were then foI-
lowed, oIong with 5 heart and 5 kidney transplant recipients [r26]. The survival of the 15 liver HIV+ patIents was compared to a group of 1,303 HIVpatients who undelWent liver transplanlotion ~uring the same years. Kaplan-MeIer aduanal survival was idenncal at one yeqr. Survival rales at 2,4, and 5 years was lower in the HIV+ patients, althaugh it never reached slotistical significance. It is our belief that HIV+ patients should not be excluded a priori From candidacy to liver transplanlo.tion .. f:lowever, in the present organ ~horloge CrISIS, dIS obVIOUS that HIV+ p'atients w'/I not be considered as transplants candidates at many institutions, both in the United Stales and In
Europe. CONCLUSIONS Indications for liver transplantation have been substantially expanded in the last 30 years. Many factors thar were previously consdered as contraindications, like an upper age limit of 50, alcoholic cirrhosis, multiple upper abdominal operations, and HIV antibcidy cc:rrier status are not any longer preventl.ng candIdacy for liver transo/antatlOn. In patIents suffenng from any ena~stage live~ diseas~ that i~ know'} not to recur m the transplanted allogratt candIdacy is no longer debatable. On the other hand, it is controversial what role should liver transplantation have in the treatment of disea· ses with high rec;urrence rates, such as hepa!itis B and most malignant tumors. However, lIver transp/antallon should not arbitrarily be refused to any of these patient groups. Patients at high risk for recurrent disease should be evaluated and entered into new protocols that may, eventually, improve their prognosis. Prime examples are the preoperative treatment of candidates carrying a primary hepatic malignancy with mtra-arterial and/or systemIc chemotherapy, and the use of novel antiviral agents for preven tion or treatment of recurrent vIral hepatitis B. In June, 1983 the Consensus Development Conference of the Nationallnstitutes of Health concluded that: "liver transplantation is a therapeutic modality for end-stage liver disease that deserves broader applIcation". This sentence had a tremendous impact on the expansion of liver transplantation as a routine surgical service. Long-term survival I> 10 years}, which was uncommon until the bfiginning of the 1980s" increased tremendously with the introduction of new immunosuppressive agents IFigure 6), and technical refinements of the procedure [127J. Nevertheless, liver transplantation is still viewed as a hightechnology procedure that society can do without. ThIS view is surprisingly common in the United States and Euro~. In 1991,75 Directors of the British health s)'.stem were interviewed about the priority of 72 given health care treatments. Liver transplantation ranked eleventh, and anly the treatment For advanced lung cancer ranked lower. The treohnent of AlDS and dialysisranked significant higher (seventh and Fourth, respectiVely) (I2B. We believe such notions are wrong, and not supparted by the current long-term resuhs. Liver transplanlotion is cerIoinly a safe procedure, and the only curative treatment for mary people suffering frOm end-sfoge liver disease. The costof the proCedure and the organ shOl'ltme are the two main problems that should be adJressed in the coming years.

Con la d,spon,bd,ta, nel marzo 1985, della determinazione immunoenzimatica degli anticorpi per I'HIV, sana stali presto descritti soggelti HIV positivi trapiantati di rene, cuore e fegato [123 124]. T uttavia Ie dimensioni del problema sana state definite con precisione solo dopo il completamento di un ampio studio presso 10 University of Pittsburgh. I sieri storati di 1043 pazienti trapianlati sana stati analizzati per la rice;ca di anticorpi per.I:f-!I'( ed e stata evidenzlata una posltlvlta nell' 1 ,79b dei sieri [125]. I pazienti sottoposti a lrapianto di fegato mostravano un rischio maggiore, con un'incidenza di positivita del 2,69b. Un terzo di questi pazienti erano HIV-positivi prima del trapianto. Quindici pazienti trapiantati di fegato, con 5 riceventi di cuore e 5 di rene, sono stati successivamente seauiti nel tempo [1 261. La sopravvivenza dei 15 trapiantati di fegato HIV-posilivi e s.tata confro~tat.a con quella di un gruppo dl 1303 pa~lenh HI\(negativi sottoposh a traplanto dl fegato In quegli stessi anni. La sopravvivenza alluanale ad 1 anno, secondo il metodo di Kaplan-Meier, e risultata identica. Le per~ centuali di sopravvivenza a 2, 4 e 5 ann I sono risultate inferiori nei pazienti HIVpositivi, sebbene non si sioraggi.unto una significativila slatistica. NOI crea,amo che i pazienti HIV-positivi non dovrebbero essere esclusi a priori dalla candidatura a trapianto di fegato. T uttavia, in considera~ zione della cmi attuale di moncanza dl organi, e ovvio che i pazienti HIV-positivi non verranno considerati candidati 0 trapianto in molti istituti, sia negli Stati Uniti che in Europa.
CONCLUSIONI
Le indicazioni 01 trapianto di fegato sono state sostanzialmente ampliate negli ultimi 30 anni. Molti fattori che venivano precedentemente considerati controindicazioni, come I'eta superiore ai 50 anni, 10 cirrosi alcolica, 10 presenza di pregress.e operazioni multiple all'addome supenorl7 e 10 p'ositivita per gli anticorpi HIV, n0!1lmpe~ discono p") 10 candidatura al trap'I~!1to d, fegato. Nei pazienti affetti d? ir:suffl.c,en~a epatica terminale di qualslas, eziOlogla n~)O suscettibile d] recidiv~ nell' ?rgano t~ap,antato la candldatura e fuon discusslo-
ne. D' altra parte, e controverso il ruolo che il trapianto di fegalo dovrebbe avere nel trattamenlo di patologie con elevata incidenza di ricorrenza, quoli I'epatite Bela maggior parte dei tumori maligni. T uttavia, il trapianlo di fegato non dovrebbe essere rifiulato di princlpio a nessuna di queste categorie di pazienti. I p'azienti ad elevato riscnio per ncorrenza della malattia originaria dovrebbero essere volutati per I'inserimento in nuovi protocolli di terapia che possano migliorarne la prognosi. Tipici esempi so no rappresentolt dal trattamento preoperatorio di candidati affetti do tumore maligno primitivo del fegato con chemioterapia intra-arteriosa e/o sistemica, e I'utilizzo di nuovi agenti antivirali per la prevenzione 0 il trattamento della ricorrenza di epatite B. Nel giugno 1983 la Nationa/lnstitutes of Healih (NIH) Consensus Development ConFerence on Liver Transoiantatlon ha concluso che: nil trapianto del fegato e una procedura terapeutica per I'insufficienza epotica terminale che merita una piG esteso applicazione". Questa frase na avuto un impotto tremendo sullo diffusione del trapianto di fegato come attivita' chirurgica routinaria. La sopravvivenza a lungo termine (> 10 annil, che era infrequente fino agli inizi degli anni '80, e aumentata considerevolmente con I'introduzione di nuovi agenti immunosoppressori (Fig.6) ed i rerfezionamenti tecnlci della procedura [ 27). \I trapianto di fegato e, peraltro, ancora considerato una procedura di elevata tecnologia della quale la societa puo fare a meno. Quest'opinione e sorprendentemente comune negli Stati Unili ed in Europa. Nel 1991,75 direttori sanitori britannici sono statl intervistati suII' ordine di priorita di 12 diversi trattamenti rimborsoti dol sistema sanitario nozionale britannico. II trapianto di fegato veniva messo all'undicesimo posta, seguito solo dol trattamento del cancro avanzato del polmone. La curo dell' AIDS e la dialisi venivano poste significativamente piu in alto (rispettivamente al settimo e quarto posta) (128). Riteniamo che queste considerazioni siano errate e non supportabili dOfili attuoli risultati a lunge termine del traplanto epatico. II trapianto di fegato e certamente una procedura sicura e rappresenta I' unico trattomento curativo per i molti pozienti affetti do insufficienza epotica terminale. I costi del trapianto e la carenza di organi sonG i due problemi principali da affrontare nei prossimi anni.
