Ted Hart came forward representing Milone & McBroom. Mr. D'Amico reviewed the changes to the plan per Milone & McBroom's suggestions. He noted the pipe size changes to 48" and 36". There will be flared ends on both pipes for the inlet and outlet. No guard rails have been included in this plan. Wes Clow reminded Mr. D'Amico that rip rap on the embankment was discussed at the last meeting. The driveway profile was reviewed. Don Richards suggested that this should include the complete 2000 feet showing tar and gravel areas. Mr. D'Amico made these indications on the map. Chairperson Tyrrell reminded the Agency that this was the last night for the public hearing. All the information should be submitted tonight. a. Chairwoman Tyrrell called this public hearing of the Inland Wetlands and Watercourses Agency to order at 7:30 pm.

144W-1421 / White Deer Rocks, LLC / 350 White Deer Rocks Rd / Install Driveway within a Regulated Area / Map 006 / Lot 003 Mr. Lavertue, Attorney McVerry, and Mr. D'Amico came forward with a plan dated 3/6/15 and page 4 of plan dated 3/9/15. Ted Hart came forward representing Milone & McBroom.

Mr. D'Amico reviewed the changes to the plan per Milone & McBroom's suggestions. He noted the pipe size changes to 48" and 36". There will be flared ends on both pipes for the inlet and outlet. No guard rails have been included in this plan.

Wes Clow reminded Mr. D'Amico that rip rap on the embankment was discussed at the last meeting.

The driveway profile was reviewed. Don Richards suggested that this should include the complete 2000 feet showing tar and gravel areas. Mr. D'Amico made these indications on the map.

Chairperson Tyrrell reminded the Agency that this was the last night for the public hearing. All the information should be submitted tonight. Ted Hart confirmed that all the engineering comments have been addressed.

Kathy Castagnetta reported that she is unaware of any additional information that would be required by the town. The concern is the oversight of the project during construction. She recommended Sean Hayden's oversight during construction.

Marty Newell arrived at 7:45 p.m.

Mr. D'Amico reviewed the soil and erosion control plan. A stilling basin was added to the plan and silt fence was confirmed to be two rows which will sandwich hay bales. Chairperson Tyrrell stressed the importance of this being confirmed in the field. The cut and fill amounts were also reviewed. Charlie Lewis suggested that at least 100 hay bales should be kept on site for erosion control requirements.

The applicant advised that he would like to begin work as soon as feasible.

The pipe sizes within the construction sequence were corrected. Mary Tyrrell noted that many details are missing with regard to the construction. She would feel more comfortable with complete and accurate maps. Wes Clow agreed that he would like the plans to be updated with the changes. Marty Newell reminded the Agency that meetings were missed due to the weather for which extensions were granted. Mary Tyrrell explained that although the statuary timeline will not allot additional extensions, there will be no fee if the application is withdrawn and resubmitted.

Chairperson Tyrrell looked for comments or questions from the public. No one from the public came forward.

The Agency confirmed for Attorney McVerry that the new application would be classified as summary and a new engineering review would not be required. The applicant verbally withdrew this application. Attorney McVerry agreed to forward a written withdrawal.

Mary Tyrrell and Kathy Castagnetta agreed to review the plan in the morning and send Mr. D'Amico the request for suggested changes tomorrow.

WHEREAS, the Agency classified the application as Summary and

WHEREAS, the Agency has carefully considered all the information submitted;

WHEREAS, the Agency finds that the proposed activity is the same as previously approved by the Commission as Application 05-IW-5052;

WHEREAS, the Agency finds that the proposed activities will have minimal environmental impact on wetlands and watercourses, there are no significant offsite impacts, and that possible impacts on wetlands and watercourses outside the area for which activities are proposed can be further mitigated by the conditions listed below and the proper use of soil erosion and sedimentation controls during construction; and

For "Construction of a Single Family Dwelling, Driveway, Septic and all Associated Appurtenances within a Regulated

Area,"246 Tuttle Road, Woodbury, Connecticut (Map 019 / Lot 072), as represented on plans by H.W. hart, PE dated December 27, 2005, revised to February 13, 2006 and March 4, 2006. Subject to the following conditions:

1. There shall be no burying of any tree stumps or tree materials including but not limited to branches, wood chips and the like. The stumps of all significant trees shall be left in place for stabilization of the affected area(s). 2. The applicant shall provide the Land Use Office with 48 hour notice prior to construction and shall not commence approved work until soil erosion and sedimentation control devices have been installed and inspected. 3. Sediment and erosion controls shall be inspected at least once a week or within 24 hours at the end of a storm with a rainfall amount of .5 inches or greater and replaced/repaired as necessary. 4. The construction equipment shall not be washed out in the regulated area. Washout material shall be contained in a washout pit or equal containment system and hardened material removed from the site for disposal at an acceptable location elsewhere. 5. The site shall be kept clean of all loose debris, litter and similar materials to prevent such from entering wetlands or watercourses. 6. All paths used for site access will be reestablished to original condition.

7. Stockpiles of earth materials shall be stored outside the 100' regulated area. 8. Removal of silt fences shall take place only after the silt build-up along the fence line has been removed. This material shall be removed from the site or placed as acceptable fill in areas that will not allow the material to migrate. 9. The applicant shall complete and return to the Land Use Office a copy of the Connecticut Department of Environmental Protection Statewide Inland Wetlands and Watercourses Activity Reporting Form prior to commencement of construction. 10. The limits of disturbance shall be clearly marked before construction and the Land Use Office will be given notice 48 hours before construction begins 11. Storage of any fuel/lubricants and the refueling/lubrication of any equipment are forbidden within 100 feet of the affected area(s). The Land Use Office is to be notified immediately of any spillage, discharge or loss of oil/petroleum/chemical liquids or solids which have occurred or will likely occur as a result of the activity. 12. Disturbed soils will be stabilized in a timely fashion to minimize erosion. Specifically, if grading operations at a site will be suspended for a period of thirty (30) days or more, the applicant will accomplish seeding or other appropriate measures to stabilize the area within seven (7) days. WHEREAS, the Agency finds that although the work was completed without a permit and is in regulated area, the site is stabilized;

WHEREAS, the Agency finds that the proposed activities will have minimal environmental impact on wetlands and watercourses, there are no significant offsite impacts, and that possible impacts on wetlands and watercourses outside the area for which activities are proposed can be further mitigated by the conditions listed below.

NOW THEREFORE BE IT RESOLVED that the Woodbury Inland Wetland and Watercourses Agency Approves the application submitted by Scott L. Adair (Trustee), Maria Taylor (Agent)

The Land Use Office is to be notified immediately of any spillage, discharge or loss of oil/petroleum/chemical liquids or solids which have occurred or will likely occur as a result of the activity. For "Construction of a Home Addition with all Associated Appurtenances within a Regulated Area," 18 Orton Lane, Woodbury, Connecticut (Map 036 / Lot 048C), as described on the submitted Inland Wetlands and Watercourses Agency application form dated January 31, 2015, Received February 19, 2015. Subject to the following conditions:

1. There shall be no burying of any tree stumps or tree materials including but not limited to branches, wood chips and the like. The stumps of all significant trees shall be left in place for stabilization of the affected area(s). 2. The applicant shall provide the Land Use Office with 48 hour notice prior to construction and shall not commence approved work until soil erosion and sedimentation control devices have been installed and inspected. 3. The construction equipment shall not be washed out in the regulated area. Washout material shall be contained in a washout pit or equal containment system and hardened material removed from the site for disposal at an acceptable location elsewhere. 4. The site shall be kept clean of all loose debris, litter and similar materials to prevent such from entering wetlands or watercourses. 5. All paths used for site access will be reestablished to original condition.

6. Stockpiles of earth materials shall be stored outside the 100' regulated area. 7. Removal of silt fences shall take place only after the silt build-up along the fence line has been removed. This material shall be removed from the site or placed as acceptable fill in areas that will not allow the material to migrate. 8. The applicant shall complete and return to the Land Use Office a copy of the Connecticut Department of Environmental Protection Statewide Inland Wetlands and Watercourses Activity Reporting Form prior to commencement of construction. 9. The limits of disturbance shall be clearly marked before construction and the Land Use Office will be given notice 48 hours before construction begins 10. Storage of any fuel/lubricants and the refueling/lubrication of any equipment are forbidden within 100 feet of the affected area(s). New Application 15-IW-1508 / 2nd CT Volunteer Heavy Artillery, Ray Manzi (agent), Town of Woodbury (owner) / Middle Road Tpke (Strong Preserve) / Create Rope (Hemp) and Log Temporary Foot Bridge for Civil War Reenactment / Map 023 / Lot 018B Ray Manzi came forward and reported that this is the same proposal as what was approved two years ago. He clarified the bridge will be wood and not log. The bridge is hand cut and assembled on site a week before the event. The last day of the event the bridge is disassembled by 6 p.m.

Photos and drawings of the bridge were reviewed along with an overview of the site. The bridge is 80 feet long and 6 feet wide.

Chairperson Tyrrell questioned whether the Agency feels comfortable with the portable toilets, a dumpster, an ice trailer, a generator, a construction trailer and parking within the regulated area. Mr. Manzi reviewed the location of each of these items. The group agreed that they were comfortable with the ancillary equipment.

A draft motion was requested for review at the next meeting.

15-IW-1509 / Allan Cetrone (Owner) / 22 Fairgrounds Road / Construction of Garage and Driveway within a Regulated Area / Map 036 / Lot 026N Allan Cetrone came forward and reviewed a map of the site with the Agency. The applicant agreed to move the proposal outside of the regulated area and will withdraw the application.
