Physical activity, appropriate nu-trition, and access to health care
services are the ingredients for bet-ter health and wellness in Franklin County—that is what a comprehen-sive community health assessment determined. To help our communi-ties achieve healthier lifestyles, Alice Hyde Medical Center (AHMC), in partnership with the Franklin County Department of Health, are building on our existing programs, services, and educational activities.
The five AHMC health centers—in Chateaugay, Fort Covington, Moira, St. Regis Falls, and, most recently Malone—offer primary care services such as routine physicals, treatment of illnesses and injuries, and man-agement of chronic conditions like diabetes.
To help our community members who do not have health insurance, AHMC partners with various or-ganizations, including the Franklin County Department of Social Ser-vices, Public Health, and Excellus, to provide accurate information and resources. This program assists individuals in applying for state-
sponsored health insurance includ-ing Medicaid, Child Health Plus, and Family Health Plus. And, recently AHMC redesigned our financial as-sistance programs and we now have a professional dedicated to helping our patients with financial counsel-ing and assistance—more details about this are on page 9.
There are also many programs to help you and your family reach and maintain your optimal level of health. For example, the AHMC In Touch with Health community well-ness program offers a free 6-week chronic disease self-management course and we cultivate healthy life-styles through the “Eat Well Play Hard” initiative that encourages physi-cal activity and targets dietary practices with children over the age of two. And, we are plan-ning to be a “We Can!” site (Ways to Enhance Children’s Activity and Nutrition)—stay tuned for details on this pro-gram.

IIt is no secret that our health care system is facing very challenging times. Na-tional health reform appears stalled and more people are uninsured. The state and federal governments are calling for dramatic cuts in health care spending—which
has an impact on both our Medical Center and our community. The troubled economy makes it difficult to pay for increasing premiums and, at times, even copayments and deductibles. On top of this, there are growing shortages of health care professionals.
But, here at Alice Hyde, in spite of all of these major challenges, we are making changes to improve coordinated, high quality health care for our community and our patients. One new program designed to improve care for the patients in the Medical Center is the Hospitalist Program, which began on March 1st. Through this program, we have “hospitalists”—physicians and Nurse Practitioners—whose job is to provide all care to patients admitted to Alice Hyde. These Hospitalists will work closely with patients’ doctors to develop a care plan, both for the hospital stay and for care once a patient leaves the hospital.
Hospitalist programs are being introduced at hospitals across the country with more frequency due to the many benefits they provide to patients. Because members of the Hospitalist team are in the hospital, 24/7, they are able to respond quickly to any change in a patient’s condition. Another advantage for our patients will be a shortened wait time to be admitted—they will not need to wait for their provider to admit them to the hospital—the Hospitalist facilitates this so care and treatment can start immediately.
Patients are not the only ones who benefit from having a doctor available at all times while in the hos-pital. The Hospitalist program also gives doctors more time to focus on their patients during office visits rather than dividing their time between the hospital and their offices. Our Medical Staff is very supportive of the Hospitalist Program because it means they can spend more time with patients in the office.
To provide this 24/7 Hospitalist program, AHMC is contracting with the nationally renowned Apogee Physi-cians. This company employs highly skilled Hospitalists to care for our patients and work with our doc-tors—and they help to keep the hospitalists’ skills up-to-date through extensive education.
We are confident that this change will not only improve care, but increase patient and family satisfaction with our inpatient care.

Thinking Spring? Why not think about ways you can increase you and your family’s physical activity. Here’s some suggestions.
Outdoor activities have the same benefit as indoor activities, but also
provide a chance to enjoy the spring weather. You can hear the birds singing and smell the new flowers bloom. It is relaxing and will help increase the stress relief that exercise provides. Some other benefits you and your family could get from being physically active include:

control• Decreased risk of heart disease, high
blood pressure, type 2 diabetes, colon cancer, and osteoporosis
• Decreased feelings of anxiety and depression

• Find a safe place to exercise – backyard, church yard, school playground or park, are possibilities.
• Check out the area to make sure the ground is safe and free from dangers such as limbs and rocks.
• Wear tennis shoes and safe, comfy clothes in layers. Remove earrings, rings, and necklaces.
• Avoid the hottest time of the day or try to exercise in the shade. Wear sun glasses and a hat.

Have you ever been watch-ing TV and eating popcorn
or chips and realize that you’ve eaten the whole bowl or bag—without even knowing it? How about nibbling at a party, standing at the buffet table, and not knowing how much you’ve really eaten?
These are examples of what an author and expert from Cornell University, Dr. Brian Wansink, calls ‘Mindless Eating.’ Dr. Wansink has written the book Mindless Eating: Why We Eat More Than We Think and you may have seen him on TV or quoted in leading magazines and newspapers. Here he of-fers some tips for becoming a “mindful eater.”

MINDFUL EATING TIPS:
• SLOW DOWN. Savor your food, and enjoy your meal—don’t eat standing up at the kitchen counter.
• PORTION CONTROL. Use a smaller plate to serve your meals.
• REMOVE THE THREAT. Put meats and carbohy-drates on your plate in the kitchen—don’t put these dishes on the table, leave only vegetable dishes on the table while you eat.
• PAY ATTENTION. Read food labels carefully—don’t be seduced by “healthy” or “low fat” labels, read the nutritional label closely—paying close attention to serving size.
Become MINDFUL about EATING
EATING with the SEASONSEating with the seasons is the best way to consume delicious, healthy, and low cost food. Local produce is chosen for good flavor instead of long distance shipping quali-ties—therefore, less time and money is spent preserving the food. Buying locally will also support area farms and neighbors, help conserve farmland and local communi-ties, and result in fewer miles traveled—reducing air pollution and fossil fuel con-sumption, and fewer nutrients are lost between harvest and consumption.

INSOMNIA - Insomnia is often a symptom of other problems. Typical patterns of insomnia include the in-ability to fall asleep or stay asleep at night, waking up earlier than usual, and daytime fatigue. Most people with insomnia don’t fall asleep in in-appropriate situations, like driving. If this does occur, it may signal that a medical disorder (such as sleep apnea) is the cause of insomnia.
SLEEP APNEA - Excessive daytime sleepiness is the primary symptom. Some people will deny sleepiness but feel fatigued. Other symptoms are snoring, snorting, and gasping
sounds when you sleep -- often first noticed by a sleeping partner. Restless or unrefreshing sleep is also typical, as are headaches in the morning.
NARCOLEPSY - Excessive sleepiness during the day, alleviated by naps, is a symptom of narcolepsy. Dream-ing during naps and experiencing dream-like hallucinations as you fall asleep are also warning signs. Loss of muscle control that occurs with emotion, such as laughing or anger, and the inability to move as you’re going to sleep or waking up (called sleep paralysis) are also symptoms.
RESTLESS LEG SYNDROME - The primary warning sign is the irresist-ible urge to move your legs shortly after you get into bed, in the middle of the night after awakening, or even when wide awake during the day. It usually feels better if you get up to walk around or rub your leg. “Creepy-crawly” or twitching feel-ing in your calves, feet, thighs, or arms are symptoms of restless leg syndrome -- the sensations of dis-comfort can be quite varied. Kicking or twitching leg movements during sleep, and sometimes while awake, may be warning signs.

Understanding the SymptomsSleep Problems
• Your sleep does not improve with self-help techniques, such as es-tablishing good sleep hygiene, cut-ting down on caffeine, exercising, and using relaxation techniques.
• You think your sleep problems may be related to an underlying condition, such as depression or
heart failure.
• You snore loudly or make snorting or gasping noises while you sleep -- or your partner observes these things while you’re asleep.
• You fall asleep doing normal ac-tivities, such as talking or driving.
• You regularly feel unrefreshed on awakening and are constantly fatigued. Sleep disorders are among the many possible causes for fatigue.
• You suspect your medication is causing your sleep problems.

at the forefront of the latest tech-nology for superior patient care with the purchase of the Philips In-telliVue patient monitoring system for all operating rooms.
The patient monitors are designed to match the pace and unique needs of anesthesia and peri-op-erative care. The system provides continuous monitoring of patient
vital signs and anesthesia gas levels, with easy to use and view color coded one-touch screen functions. In addition, the Anesthesiologist, at a seconds notice, can have an immediate response to a patient’s needs due to early-warning alarms known as ‘Smart Alerts’. The alerts are triggered by deviations in a patient’s vital signs based on their admitting or current diagnosis.
The monitors also have a “pig-gyback” function—a smaller de-tachable monitor travels with the patient after surgery to recovery or the Intensive Care Unit, giving health care providers access to timely and comprehensive patient information at the point-of-care.

PROGRAMIn November 2009, the Medical Center launched a Total Joint Replacement
Program, a comprehensive pre and post-operative treatment plan for ortho-pedic patients. The treatment plan, beginning four to six weeks before surgery, is tailored to the needs of each individual patient.
The program begins with the scheduling of surgery, a facility tour, a pre and post-operative needs assessment, and a comprehensive “day of education”—a presentation to patients and their family members by orthopedic surgeons, anesthesiologists, nurses, physical and occupational therapists, case coordina-tors, and discharge planners.
Once surgery is complete, the team works with patients to maximize comfort and begins the rehabilitation and recovery process. In an effort to help patients recover safely and in a timely manner, the Program has dedicated in-patient rehabilitation beds in the Alice Hyde Nursing Home.

Alice Hyde Medical Center recently upgraded its Emergency Department (ED) with the installation of MEDHOST’s full-suite Emergency Department Information System (EDIS). MEDHOST is designed to help AHMC better serve
the community with decreased patient wait times and improved patient safety and care. Using touch screen technol-ogy, MEDHOST makes it easier for doctors and nurses to care for their patients, while allowing quick access to impor-tant medical information to save time in an environment where lost seconds can mean lost lives.
Designed by clinicians, MEDHOST is easy-to-use and was created to suit the unique needs of a medical center’s ED. MEDHOST allows doctors and nurses to create clear and accurate charts, order medication, and provide patient care all from the patient’s bedside or anywhere else in the ED. Patients no longer have to spend time waiting for prescriptions or for tests to be ordered, and because charts and test results are electronic, information is received into MEDHOST in real time. But MEDHOST goes beyond simplifying the charting process by helping prevent the possibility of human error with intuitive software that can alert clinicians to allergies or drug interactions. MEDHOST features a unique ToolKit application, so the system can be quickly custom-ized to fit new demands and clinical, financial, regulatory, or technical changes.

If you find yourself with an unexpected overnight
stay at the Medical Center or an unplanned visit to the Emergency Department or outpatient service, you and your family will meet Margie, Alice Hyde’s Patient Financial Advocate. “When people are sick, the last thing they need to be concerned about is ‘how am
I going to pay?” said Margie. “But they do worry—and I can help ease their minds.”
Margie helps patients and their families determine what their “out of pocket” costs will be for their hospi-tal care—that is, the copayments and deductibles that the patient will be responsible for paying. If the patient may have difficulty paying the full amounts, she works with them to set up payment arrangements. And, for those who cannot afford their care, Margie helps to de-termine if the patient is eligible for “charity care” that is available through the Medical Center—it is important
to note that the charity care policy is not hospital insur-ance. It is the payment of last resort, when patients have been denied Medicaid coverage and don’t have the money to pay for care, they are considered for the funds available through Alice Hyde.
“Patients and families seem to appreciate that we ad-dress their financial concerns and offer options that they may not be aware of,” said Margie. “Then, they can spend their time on what’s most important—get-ting better.”
