You can get support to volunteer or get and keep a job, to get involved in new activities, to meet people andmake new friends.

The social inclusion team includes a jobcoach, a community connector andvolunteers to help you achieve your goals.

If you live on your own or share withothers who are being supported to liveindependently, you can get supportfrom our Supported Living team.

The team works 24 hours a day, 365 days a year. You can get supportdaytime, evenings, at night, atweekends, on bank holidays – andwhile you are on holiday.

If you live on your own, with yourfamily, a partner or someone whocares for you, you can buy support forliving from the Community Supportteam. This team works daytime,evening and weekends, including bankholidays.

iPlanitAt Three Cs, person-centred planning issupported by iPlanit.Everyone we support has a user-friendly account online where outcomes and actions agreed in person-centred planning are logged. It helps makesure that the outcomes most important to the personare kept in view until they have been achieved.

Evidence from iPlanit is used towork out who has achieved theirgoals and made successfulcontributions to their own livesor the lives of others. Peoplereceive Independence andCommunity awards and prizesat Celebrating Success events.

Instead of getting agencies ortemps in, we offer paid jobs in ourEmployment Pool to people wesupport. As well as earningmoney, it helps people gainexperience and learn skills to getinto permanent paid employment.

Andrew has two hours support. This ismostly about readingletters and bills andbeing nudged aboutappointments. He gives many more hours back to other people and to Three Cs.

Like Donovan, most people thrive on being active andinvolved or using their skills to make adifference. As well as job coaching and anemployment pool, Three Cs offers a rangeof day opportunities and activities, and thechance to contributethrough volunteeringand mutual support.

You can get support to volunteer or get and keep a job, to get involved in new activities, to meet people andmake new friends.

The social inclusion team includes a jobcoach, a community connector andvolunteers to help you achieve your goals.

If you live on your own or share withothers who are being supported to liveindependently, you can get supportfrom our Supported Living team.

The team works 24 hours a day, 365 days a year. You can get supportdaytime, evenings, at night, atweekends, on bank holidays – andwhile you are on holiday.

If you live on your own, with yourfamily, a partner or someone whocares for you, you can buy support forliving from the Community Supportteam. This team works daytime,evening and weekends, including bankholidays.

Who are we?Three Cs stands for ‘Control and Choice in theCommunity’ for people with learning disabilities,autism and/or mental health challenges.

We support people to control and choose their ownlives and contribute to the community as equal citizens.

What do we do?We provide support for living in East and South EastLondon. We do this through Community Support,Supported Living, and Social Inclusion and DayServices. Our approach is based on the social modelof disability and the recovery model of mental health.

What is important to us?We focus on people’s strengths and interests to buildconfidence, skills and independence. We promotesocial inclusion, including social activities, paid jobsand volunteering.

We expect staff, volunteers and Trustees to be HAPPIin their work for Three Cs. That means:
