GREATER RICHMOND AGE WAVE READINESS
TOOLKIT
Objective
To develop an Age Wave Readiness Toolkit to empower individuals to take ownership of their own aging and community engagement, as well as, provide a mechanism to build broader awareness of resources available across the region. This work includes: identifying evidenced-based or practice-based content, sources for content, methods of dissemination and product development.

Regional information (inclusive of all PD15) Benefits the individual (elders and caregivers) and is person-centered.
At least 12 months in existence. Contents without an expiration date. Responds to a Community Need or
Opportunity Supports positive and optimal aging. Vetted by the Committee (for relevance and
appropriateness) Evidence-based materials
