Quality & Safety/ Patient Engagement/ Impact on patient services:

The training aims to improve awareness of health and social care professionals in identifying and managing potential risk of suicide and most importantly to reduce the incidence of suicide for patients of Central London CCG, West London CCG and Hammersmith and Fulham CCG. As part of the identification and development the Suicide Awareness and Intervention Training and e-learning module there was input and consultation with key stakeholders including primary care, third sector and patients.

Finance

The resources required to support this project are mostly in the form of oversight and monitoring of the implementation of the ‘face to face’ training modules and development of the e-learning module for the on-line training.

Risk Mitigating actions

There is a risk that not all organisations will be able to release funds over the entire contract period of three years.

The contract will be on a call off basis, so if there is a decrease in available funds fewer courses can be called off the contract.

Quotation Process

1. Purpose

The purpose of this report is to inform the Governing Body of the approval of contract

award by the Finance and Performance Committee. The evaluation was carried out by

representatives appointed by Central London CCG, who formed the evaluation panel to

evaluate bids received in response to the Request for Quotation issued on 05 August 2015

for the Suicide Awareness Training, Intervention Training and e-learning module.

2. Introduction

The report has been compiled on behalf of the evaluation panel following completion of

responses to the Request for Quotation for the provision of Suicide Awareness, Intervention

Training and e-learning module.

3. Background and Context

Central London Clinical Commissioning Group (CLCCG) is the lead organisation in implementing the Tri-Borough CCGs’ Suicide Prevention Strategy 2013-18 and this work stream is supported and underpinned by the Inner London Suicide Prevention Working Group. Rates of suicide are higher across the three boroughs than most other London boroughs and particularly in the borough of Westminster. During 2011-2013 approximately 150 people died due to suicide/undetermined injuries across the three- boroughs. The profile of these deaths is comparable with the national picture, with the most common group being men aged between 30 and 60. The cost of completed suicides in England costs approx. £1.7m and includes the cost of funerals, police time, individual pain and suffering of relatives. Although suicide rates are unlikely to drop to zero it is suggested that many deaths by suicide are preventable The Suicide Prevention Working Group operates across Westminster, the Royal Borough of Kensington and Chelsea and Hammersmith & Fulham was established in 2012. The group meets on a quarterly basis to promote effective inter-agency working in communicating, managing and ultimately reducing the number deaths from suicide in the boroughs. The group’s members are drawn from a range of NHS, local authority, police and voluntary sector organisations.

In August 2014 (final paper presented on 13/08/2014) the Suicide Prevention Working Group undertook a training needs assessment exercise, which identified the need to increase the level of training available for health and social care professionals. The recommendation from the needs assessment was to secure funding from NHS and Local Authority commissioners to commission a training package for professionals across the three- borough providers to include awareness and interventions, e-learning modules and train-the-trainer sessions for in-house training.

4. The Procurement Process

Following approval by CLCCG Finance and Performance Committee on 25th March 2015 and the other organisations, an Open Tender was advertised on the NHS Supply2Health and Pro-Contract e-procurement portals (www.supplying2nhs.com) on 5th August 2015. Table 1 outlines the procurement process timetable:

5. The Evaluation Panel

The CCG evaluation panel was established at the start of the process prior to the

advertisement being issued. The panel met on 7th September 2015 to review and moderate

the responses for Lots 1 and 2.

6. Process of Evaluation

Initially 37 organisations expressed an interest in delivering this service; however 2

organisations (lot 1) and 8 organisations (lot 2) submitted quotations by the deadline of 1st

September 2015.

Evaluation criteria was agreed together with evaluation weighting and scoring mechanisms

(appendix 2).

The contract term is for 2 years with the possibility to extend for a further 12 months and

will be a standard NHS contract on a call off basis. A format incorporating the usual

safeguards for both parties regarding notice, termination and performance management.

Key Performance Indicators (KPIs) are incorporated into the contract, to ensure delivery of

pre-defined levels of performance in line with the service specification, there are currently

draft KPIs included in the service specification and these are expected to be finalised with

the successful bidders when finalising the contract terms.

The CL CCG Mental Health Project Manager will provide contract and performance

management and regular reports to the Suicide Prevention Working Group and other

committees as required.

9. Recommendations

The recommendation is to award the contract to: Connecting with People for Lot 1 and

Virtual College for Lot 2 with a contract start date of 02 November 2015.

Central London CCG Finance and Performance Committee approved the Contract Award on

Wednesday 23 September 2015.

Appendix 1

Suicide Awareness, Intervention Training & e-Learning Resource- Specification

1.1 Context The rates of suicide in the City of Westminster, Royal Borough of Kensington & Chelsea and London Borough of Hammersmith & Fulham, collectively referred to as the three- boroughs, have historically been higher than in most London Boroughs, and in particular in Westminster. Based on the North East Public Health Observatory: Community Mental Health Profiles 2013, around 150 people died due to suicide and undetermined injuries in 2011-13 in the three boroughs, and the profile of these deaths fits the national picture: women account for approximately 25% of all suicides and the most common group is men aged between 30-60. It is only in recent years that even Westminster has fallen into line with England (though rates remains higher than London) indicating that there is still more work to do to support this vulnerable population. Although the total numbers of suicides are low, they are significant contributors to Potential Years of Life Lost due to the average age of suicides being between 15 and 44. In addition, it is estimated that a complete suicide in England costs c. £1.7m, with costs clustered into (a) intangible costs, such as loss of life to the individual pain and suffering of relatives, (b) lost output (waged and unwaged) and related health and social costs (c) cost of recovery of bodies, funeral and emergency services time [No health without mental health Outcomes Strategy for People of All Ages].

The costs of incomplete suicides and/or non-fatal suicide attempts are also substantial. While suicide rates are unlikely to drop to zero, evidence suggests that many deaths by suicide are preventable [Appleby L, et al, the National Confidential Enquiry into Suicide and Homicide by People with Mental Illness 2014]. Locally, the Suicide Prevention Working Group operating across Westminster, the Royal Borough and Kensington and Chelsea and Hammersmith & Fulham is a ‘coalition of the willing’ that brings together local representatives from award-winning national charities and key public sector organisations. It meets quarterly to promote effective inter-agency working in communicating, managing and ultimately reducing the number deaths from suicide in our boroughs. A key recommendation based on the work of the group to date is the commissioning of a training programme in suicide awareness and intervention for front-line professionals. To inform this, a training needs analysis was completed with input from key public and voluntary sector organisations to- understand the demand and components of a learning programme to be commissioned. The responses indicated that there is demand across the three -borough CCGs and Local Authorities for both awareness and intervention courses for front line staff across primary care, local authority and voluntary sector organisations. The main focus of this training will be primary care health staff, on the basis of the results of the suicide audit in Westminster and Kensington & Chelsea, which found that of suicides in the sample period: • 95% were registered with a GP in inner north west London, • 96% had a previous suicide attempt; and • 83% were at home rather than in public or other places. • 40% had been seen by a mental health professional within the month prior to the

suicide event, either at GP practice, in the community or at an acute setting.

This specification aims to address all of the NHS Outcomes Framework Domains and Indicators. 2.2 Local defined outcomes It is anticipated that by the end of the learning interventions, participants:

will have increased knowledge of suicide and its effects,

are more confident and able to distinguish people at risk of suicide,

are able to perform (and understand the results of) risk assessments,

are able to recognise early stages of suicide risk and are able to signpost patients to the right services within the three- boroughs,

understand the appropriate responses to suicide and appreciate the roles of other professionals who will form the frontline of the “suicide prevention” including: GPs, primary and secondary care nurses, social workers, pharmacists, specialist staff such as physiotherapists, occupational therapists, mental health and LD professionals, community matrons and others directly involved in the care of the individual,

will have a clear understanding of what makes a good care plan, what quantifies an “excellent” versus “inadequate” care plan; and

will have identified communication methods of sharing “best practice” with colleagues as a way of fostering wider improvements.

3. Scope

3.1 Service description Core service components The supplier will provide the following services from a variety of locations across Westminster, Kensington and Chelsea and Hammersmith and Fulham:

A) c. 440 delegates to have suicide awareness training courses, to be delivered over

three years

Target audience: General Practice staff (e.g. GPs, Practice Nurses, reception staff), Local

Authority staff, Voluntary Sector organisation staff

The aims of this general awareness learning will be to:

• Give participants a clear understanding of suicide prevalence and its underlining causes, including the effects of alcohol abuse.

• Dispel some of the incorrect stereotypes that surround suicide. • The barriers that prevent those at risk from seeking help. • Promote a holistic view of health with awareness of the role of biological (somatic),

psychological (thoughts, emotions and behaviour) and social (relationships, stress) factors.

• To explore attitudes and perceptions towards suicide and mental illness. • Signpost the type of interventions that can mitigate the risks of suicide. B) C. 250 delegates to have suicide intervention training courses, to be delivered over

three years

Target audience: Front-line General Practice staff, Voluntary Sector organisation staff

In addition to delivering the aims of the awareness training course, the aims of the

intervention training courses will be to:

• Give participants a clear understanding of suicide risk assessment methodologies. • The key interventions and responses that can be made to mitigate the risk of suicide. • An understanding of how to build emotional resilience through the promotion of well-

being; building social capital through relationships and psychological coping strategies. • Build a local network of peer support to continue to learn together and for mutual

support. Due to limited capacity for staff to be released from their workplace, it may be appropriate for participants to undertake intervention training as part self-directed work. The supplier will request participants to evaluate courses as they are delivered, and will follow up with course participants six months following the course to check that learning outcomes are sustained in the working lives of course participants.

C) e-Learning training module content and online solution/platform to be delivered in

year one of the contract, support to be delivered over three years.

The successful candidate for lot 1 will be required to create the e-learning training module

content, and work with the successful candidate in lot 2 to ensure the content can be

developed, programmed and integrated into the solution/platform provided by the successful

candidate of lot 2.

The successful candidate in lot 2 will programme the content, and integrate this into their

proposed online audio-visual e-Learning solution/platform, which must be user friendly, easily

accessible online, and developed with an assessment at the end of the module with

certification on passing. The solution/platform must be username and passwords enabled and

have robust data security encryption to ensure information is secure at all times. The

successful candidate(s) will be required to work collaboratively and in partnership to ensure

the e-Learning content can be firstly developed, and then delivered effectively through an e-

Learning solution/platform.

The aim of the e-Learning package will be to convey key messages for a potentially very wide

range of staff in all partner organisations, ensuring participants receive a basic level of

knowledge, based on established evidence based best practice. The online training module, at

the least, will cover the essential knowledge covered in the general awareness session. This

may include hyperlinks to ensure easy access to additional resources as further reading.

On-going support will be required across the full contractual period. This will include; updating

content on annual basis and resolving any technical issues within two weeks of being logged

with the e-learning developer. The technical support will need to be provided over the course

of the contract.

As part of developing the e-Learning Solution the successful candidate(s) of lots 1 and 2 are

required to work in partnership and engage with stakeholders listed below to ensure the

solution is appropriate for a varied audience, using plain English and with the needs of the

varied stakeholders in mind.

• at least three hospital training and development teams

• at least three GP practices at least two voluntary sector organisations

It is anticipated that the awareness component of the e-learning package be

incorporated into mandatory training ensuring that all staff are trained as part of the

mandatory training cycle.

D) Two train the trainer courses to be delivered in year one of the contract

The supplier is required to provide two courses to train trainers to deliver suicide awareness training as part of in-house training. This should enable, for example, hospital based ‘trainers’ to run organisation-based general awareness sessions. This would allow these local trainers to reflect local working practices and work styles. It will also provide a legacy approach to the training programme rather than a one-off training commission.

E) Service promotion activities The supplier is required actively to promote the training courses to the target audiences using a variety of materials and methods (at least printed and electronic, but innovate approaches based on experience are welcome) to ensure that courses are attended. The supplier is responsible for all administration aspects of the courses; this will include seeking venue space. CLCCG will be responsible for authorising venue costs, and wherever possible the supplier should try to secure free space, either at 15 Marylebone Road or in the respective organisations premises.

The service will be provided from appropriate premises in community settings across Westminster, Kensington and Chelsea and Hammersmith and Fulham which the supplier is required to identify.

7. Contract Duration

The contract will be for a period of 2 years with the option to extend for a further 12 month period subject to performance.
