Success with Endeavors Depends on Strong Activism of Members

President Todd Zeno’s Report to the PPMA House of Delegates in June.

Good evening Ladies and Gentlemen. As you know, I have the honor of serving as President of the Association this year, and I can tell you that the state of the Pennsylvania Podiatric Medical Association is good. The House of Delegates has many functions, but one of its main functions is to allow us to work back to the grassroot membership by combining the House with the Board of Governors meeting. Structurally, we need you to take what we do and say here this weekend back to your Division membership.

Let me speak first about the state of our Association’s financial condition. Thanks to Dr. Pinker and the Board, you will receive a Finance Report which details the fact that we have continued our long history of responsibly employing your dues to the benefit of our membership and profession. For the past two decades, we have proposed responsible budgets and we have stayed pretty closely to those figures annually. Through that approach, we have accumulated reserves that keep us within all nationally recommended financial standards applied to not-for-profit Associations.

Membership & Student Applicant Pool Must Grow Next, you will receive a membership report which shows that we, like most Associations of all types, need to do a better job at attracting young members. You will see that our member-ship numbers are level, but within that level number our members are aging.

The generation of mature members has done a great job in forming and growing our Association. But we are facing the challenge of convincing young members of the importance of an Association, which represents their business interests. Last year, we spoke of needing to convince members of the relevance of Association membership. This year, I feel that our relevance tag line has to be usurped by the importance tag line.

Let me explain why I feel that we need to convince our peers of our “importance.” While

the state of the Association is good and strong, the state of the profession is in peril of not being able to make the same statement. As you will hear in this House, our student applicant pool is down. We have plans to remedy that, however, the effectiveness of those plans depend on us, here in this House.

A fight for more applicants MUST be coordi-nated with a renewed effort to maintain and expand our residencies. That effort depends on the support and actions that we must decide to take here, this weekend.

Title XIXFederal Legislation in the VA, which has been successfully passed, is now being followed by our Title XIX legislation that has been entered into the House of Representatives. This Title XIX Bill is a much harder challenge than the VA Parity Act, as Title XIX has an identifiable “cost.” While the VA Bill had a politically palatable subject matter, relating to our Veterans, we need to link our Title XIX Bill to the politically important cost related to the epidemic of diabetes.

This topic brings me to the core concern that I spoke about when I was inaugurated in November. Our success in all of these endeavors depends on our own activism, and our resolve to spend some personal time in support of our profession. Let us look at Title XIX. It is a simple Bill, which conforms the definition of physician in the Medicaid statute (Title XIX) to the existing definition of physician in the Medicare statute (Title XVIII). Seems simple, doesn’t it?

Well, nothing in Washington is simple. Each Bill in Washington is “scored.” That score is the amount that the Bill will cost the system, not reflecting any savings that the Bill will produce. In your House Book, you will see the names of the members of the House of Representatives who serve you in Pennsylvania. You will also see copies of the last two articles that we placed in our newsletter. The March/April newsletter instructed you in how to determine your Congressional Representative’s name and local address. Our May/June newsletter asked you to begin the process of contacting your Representative to ask for a meeting.

I cannot tell you how important this process is. We have a Bill, and that Bill has a budgetary

APMA’s Study Gives Valuable Insight on Student Recruitment

If you Present the Opportunities Students will Come!According to a recent APMA study on student recruitment, conducted by Edge Research, a career in podiatry is not foremost on the minds of students. The good news is that part of these findings “suggested that if you create opportuni-ties, students will find you.” It’s time for PPMA/APMA members to join their peers and impact a student’s decision more aggressively in favor of podiatry.

Create the Opportunities to EngageStudentsIncreasing student enrollment in podiatric schools won’t happen on its own. You must create the occasion by calling your local elementary/high school guidance counselors and offer avenues to connect with those looking at careers in science or medicine. Another finding from the APMA study concluded that “Interest in medicine among Prospects occurs earlier than for Podiatry students,” a signal that educating students before and during high school will make the most impact.

There are many variables to consider with your action plan:1) Set up speaking engagements at local schools. a. APMA is able to provide members with presentations for prospective students. This study is part of several initiatives currently underway, watch for more to come. b. The American Association of Colleges of Podiatric Medicine (AACPM) can connect you with students and resources. Join the AACPM Mentoring Network. Registration is free, go to https://tinyurl.com/ybpzz67m.2) 2) Clinch the deal, schedule “shadowing” to pre-med or science students frequently. 3) Tell students the “Why” of Podiatry: a. Podiatry’s range of practice settings b. Salary-to-work and life-to-balance ratios c. Residency opportunities d. Ability to form meaningful patient relationships4) The earlier the better to reach prospects, according to the APMA study; as well as focusing on ways to increase online Search Engine Optimization (SEO) and Word of Mouth (WOM). 5) According to APMA’s study, a high percentage of prospects were unaware or misinformed about certain key aspects of podiatry including: • DPMs Perform Surgery • The Costs of Podiatric Medical School vs. Medical School

Podiatry’s Best Kept Secret is Podiatry ...

... But it doesn’t have to be! What happens to secrets is that they eventually take on a life of their own. Tell students what a podiatric career is really all about; one they can add to their options. The following summarizes a few key points from APMA’s student recruitment market research.

A Career in Podiatry is Not Top of Mind: Prospects start out comparatively unfamiliar with Podiatry; 2-in-10 would consider it as a career. The podiatry consideration funnel is on par with Osteopathic Medicine, a field which has grown exponentially in recent years (85% in the past 10 years; 6% of Prospects already considering a career in Podiatry).

Earlier Exposure to Podiatry Makes a Difference: Among Prospects considering Podiatry (n=33), 67% became interested in the field before college. Among current Podiatry students, this drops to 11%, and 39% of PMS students say they got interested after college. Before/during high school is a key time to raise awareness of career options.

Prospects Say Online Resources and Word Of Mouth (WOM) are Key: Prospects are using a range of resources to research medical careers, including a mix of online search and word of mouth—the latter of which ranks as most influential on the list.

Prospects’ Priorities Align with Podiatry: Most Prospects prioritize providing relief to patients, work-to-life balance, patient variety, and quality time with patients—all elements of podiatric practice.

Prospects Are Uninformed on a Few Key Aspects of Podiatry: The greatest misunderstanding or confusion exists around the opportunity to perform surgery (a key benefit); the costs of medical school; and a podiatrist’s salary. Many Prospects are also unaware of the variety of patients, conditions, and practice settings.

Our PPMA members can attest to the APMA survey findings on the best methods for recruiting:

PPMA Member Simon Tabchi, DPM, had a recent Moravian College grad shadow him who just got accepted into the first-year class at Temple University School of Podiatric Medicine (TUSPM). “Many people think it is all foot care and ingrown toenails,” says Dr. Tabchi. His student was unaware that podia-trists treated and evaluated tendon and bony abnormalities, both in office and surgically; and that podiatrists treat ankle pathology. (From the APMA survey, “48% of Podiatry students identify shadowing a professional as their most important resource for learning about Podiatry.”)

PPMA Vice President Laura Virtue, DPM, has been recruiting students for the past 15 years on her own. “I see tons of kids every day because there’s lots of pediatricians in my group,” she says. So she opens the conversation whenever she has the opportunity. Recently a student who shadowed her is graduating this year from Dr. Guido LaPorta’s residency program. According to Dr. Virtue, “When a student does shadow me, I try to bring them to the Wound Center and to my office.” This way they experience what a typical day is like as a podiatrist.

PPMA Past President Joseph Smith, DPM, says, “I will say that words cannot express the satisfaction and pride that you feel when you get to see that student/doctor you have mentored and who has shadowed you in your office stand up and receive their lab coat during the White Coat Ceremony at Temple University School of Podiatric Medicine. It is very rewarding, on a personal level, to know that you have changed someone’s entire life forever. You have given that person a chance to have a wonderful career. You have helped to create another healthcare provider, and you have helped the profession of podiatric medicine grow. There is no other feeling like it.”

APMA’s Recruitment Event at the National 2018 Received A+As instructed by the APMA House of Delegates to take a leading role in Student Recruitment efforts, APMA launched its first “Step into Podiatry” event during the 2018 National to expose local pre-health students and advisors to podiatry. Upon arrival, prospective students were invited to interact with a panel that consisted of a new practitioner, third-year resident, and third-year student at the Barry University School of Podiatric Medicine. Later these students shared lunch with AACPM members who answered questions about podiatry and their experiences entering a podiatric career. In addition, these prospects were able to observe surgical workshops and tour the exhibit hall.

The event was such a huge success that APMA has sched-uled a Second Annual “Step into Podiatry” event on July 12

during the 2019 National in Salt Lake City, Utah. APMA’s Career Development Manager Tiffany Kildale says, “We

encourage any member to reach out to us with questions regarding recruitment presentations and support, and as always, to let us know what they’re doing individually so we can help promote them and their recruitment efforts!”

The next step for APMA will be to engage its Stakeholder Advisory Group in the coming months to start developing the brand neutral recruitment website, and launch the Student Recruitment toolkit early next year.

2019 Clinical Conference Recruitment Event PlannedThe Goldfarb Foundation is also setting its sights on a Student Recruitment event at this year’s Clinical Conference, November 7–10, at Valley Forge Casino Resort. The Foundation recognizes the need to enhance student recruitment in both the quantity and quality of applicants to guarantee the continued success of our profession. To do this, Goldfarb Foundation Board Members Edwin Hart III, DPM, and Joseph C. Smith, DPM, are coordinating these efforts with the Temple University School of Podiatric Medicine (TUSPM) to offer a group of undergraduate students and their advisors the chance to interact with PPMA Board members and ask questions. The students will also be able to sample a lecture or two and have lunch with our members.

“Our goal is to expand and expose the career path of podiatric medicine to undergraduates in local colleges and universities in southeastern PA,” says Dr. Hart. The PPMA delegates have, over the past several years, put forth resolutions at the APMA House of Delegates to resolve the residency shortfall and to improve efforts in student recruitment and increase applications to podiatry schools. This event is only one step toward these goals. Dr. Smith says, “We believe that this is an excellent way to reach out to prospective students and local advisors. On a “grassroots” level, it will allow them to gain firsthand insight into the field of podiatry.”

Students at TUSPM are also returning to their undergraduate institutions to promote the specialty. “It is really an issue that requires efforts by us ALL to strengthen the profession’s bright future,” says Dr. Hart.

The school advisors will be invited to attend on Friday, November 8 of the Clinical Conference with the students being slated for Saturday, November 9.

Do You Play Favorites?By Lynn Homisak, PRT, CHC, SOS Healthcare Management Solutions, LLC

Do you play favorites? How can I answer that without any sugarcoating? DON’T! If you cater to one or more employees over others, it confirms you are intentionally exhibiting some form of neglect and harassment. It may seem very innocent and reasonable to give more atten-tion and offer greater leeway to a staffer you perceive as more reliable and ambitious. However, this favoritism no matter how minor is the fuse to a practice time-bomb! Low morale, costly staff turnover, loss of good staff, resentment, retaliation, reduced productivity, poor management practices, and lack of teamwork. Boom! I won’t mention lawsuit.

You have a responsibility to treat employees fairly and equally across the board. This could apply to a number of incidents. Look in your employee manual. I’m sure the equality/discrimination rules are clearly defined and if disregarded, will certainly come with consequences. If you have issues with underperforming staff, have that discussion with them through evaluations and reviews, opportuni-ties for improvement, and disciplinary write-ups. If their level of performance does not improve after guidance and counseling, then take the necessary termination steps.

Could Podiatric Assistants Evolve into Podiatric Physician Assistants? Podiatric Medical Assistant Josh Dutt, PMAC, believes the future of his profession will see more patient care being placed into the realm of the Podiatric Medical Assistant; and possibly extending to the scope of a physician’s assistant specializing in podiatry. Dutt has pushed himself to take on more patient responsibilities during his 10-plus-years at Martin Foot and Ankle’s Lititz, PA, location, going beyond PMAC certification and completion of state requirements to take X-rays. He has welcomed every opportunity/challenge that came his way, from MRI tech and recently on his way to becoming a certified diabetic shoe-fitter at the practice. “The position now bestowed upon me is site lead of our Lititz office,” he explains.

Even though Dutt says he has had many highpoints in his career as an Assistant, it is at the times when he is “permitted to treat a patient with conservative therapy in order to prevent worsening of an illness” that brings the most satisfaction. The conserva-tive therapy he refers to is when he educates a patient or fits a patient for diabetic shoes, custom diabetic inserts, or does various types of modifications. He notes, “These types of treatments are tolerated very well by patients, and require almost no restriction or negative impact on a patient’s lifestyle.”

Dutt considers the doctors, staff, and patients he works closely with on a daily basis as a kind of family. He credits Martin Foot and Ankle’s core values in keeping them all strong to handle the hardships/struggles that are inevitable within a medical practice setting. UPDATE

Reference the chart on page 8 that lists common, blatant, favoritism behaviors. There is no excuse for any one of them; use it as a self-assessment test and be honest with yourself. If you have had a role in any of those listed, intentional or not, insisting it’s all in jest (not funny), take the necessary steps to rectify things right now—

• Treat everyone equal.• Be aware of your behaviors and actions; innocent as they may seem. • Follow written employee policy and office rules and apply them across the board, equally. • Conduct performance reviews and base-wage increases, perks, etc., on levels of actual performance. • Assign duties based on skills, abilities, and job descriptions. • Enjoy your workplace, with everyone, equally.• Communicate and treat everyone equal.If you believe this is an overreaction, brace yourself. It’s just a matter of time before even the slightest discrimination will cause a

backlash that will play out as an internal disruption or a legal repercussion. I am a huge proponent of joy in the workplace. Have fun. Did I mention treat everyone equal? UPDATE

cost associated with it. One way to balance out a budgetary cost is to find a way to pay for it. That is beyond my skill level. The other way to balance the cost, however, is to show OVERWHELMING support and sponsorship for the Bill. This is within our collective ability to deliver.

We have 18 Congressional Districts in Pennsylvania. We have 17 Congressmen and women representing us in those districts. We have four women and 13 men. We need the members of this House to make contact with those 17 offices and ask for 17 appointments to meet with your Representative. At that meet-ing, we need you to make the “ASK.” The ASK is: Representative, will you cosponsor this Bill; because I am a podiatric physician and I treat thousands of citizens within this district; and I need the relief that this Bill provides; and I am your constituent.”

That simple. This is not a heavy lift. It will take time, but it will also be incredibly valuable to the prospect of success. Is this important enough to warrant your time. YES. Let me tell you why.

YOUR Time and Effort CrucialHistorically, Pennsylvania has been immune from the primary threat that the absence of the Title XIX definition carries with ii—being “optioned” out of Medicaid. We are immune from that threat because podiatry is defined, in our Human Service Code, as a service that will be reimbursed by the Department in the Medical Assistance program. However, this year we have seen a second level threat that the absence of the Title XIX definition has presented.

The Pennsylvania Department of Human Services is requiring that the Medicaid patients with diabetic wounds who are under the care of a DPM must go from the wound clinic to their PCP for a face-to-face encounter and a “certification of eligibility,” in order that a prescription relating to Home Health Services or Durable Medical Equipment can be ordered.

The requirement stems from a regional CMS interpretation of ACA Section 6407, requiring that a face-to-face encounter must take place within six months of the prescription, and that the written order must not be prior to the date of the face-to-face encounter. (Medicare Learning Network Matters® Number: MM8304 Revised)

Patients who are referred to a DPM by a PCP within six months still have to go back to the PCP in order to secure a certification of eligibility for the particular DME (as designated by the DPM) appropriate to the patient’s care. Many patients do not come from the PCP but are referred by Emergency Rooms. In the regular course of the patient’s care, the order for DME may change and this interpretation requires an additional PCP visit and an additional certificate of eligibility.

We have approached this problem by working with the Department, the Pennsylvania Medical Society, and our lobbyist Hap Campbell. We will be submitting legislation to include Doctors of Podiatric Medicine in the definition of “Physician” as that term is used in relation to the Medical Assistance program

and the administration of Medicaid in Pennsylvania. This is a cumbersome approach, but it is the only available approach UNLESS we are successful in having our Title XIX Bill passed in Congress.

This is why your time and your effort is important. Neither our staff nor all the PAC money that we have available are as import-ant as your appointment and face-to-face time with your Congressional Representative. Why? Because you are the constituent—You are the most important contact in a successful campaign to pass legislation. All elected officials listen to the voter.

I would like to leave here this weekend with your commitment to connect with your Congressional Representative. We also need

you to become active with your State Representative. As I said a moment ago, we will try to solve our Medicaid problem through State legislation. That will need the same “contact” advocacy that we need on the Federal level. It is much easier on the state level. Representative Valerie Gaydos is here with us tonight and will tell you about the effectiveness of face-to-face communication.

Hap Campbell, our lobbyist will speak with you tonight. We have, through our effort and his assistance, been included in the Telemedicine Bill, which is before the House. We are also working on several other initiatives, which he will discuss—

• The key to any success that we hope to see in each of the challenges that we discuss in this House is YOUR involve-ment.

• You have to work with us in drawing new students into our profession. Our Board will discuss student visits.

• You have to visit the Congressional delegation and make the “ASK” for their support in Title XIX.

• You have to go to the outings and meet your State Repre-sentatives in order to move our profile higher in the consciousness of the House and Senate in Harrisburg.

As your Association, we will continue to do all that we can, but your decisions, made here this weekend, will elevate our ability to defend our profession and expand the future for new podiatric physicians. Thank you. UPDATE

1. What in your opinion helped you make the decision to become a podiatrist?Observing with Edwin Hart, III, DPM, is what helped me decide to be a Podiatrist. I was always interested in the field; however, never fully knew in detail what a Podiatrist does on a day-to-day basis. Being able to shadow with Dr. Hart as a student at Moravian College gave me the insight I needed to make my decision.

2. What are the most important aspects that podiatry has brought to your life?Podiatry has allowed me to be a Physician, but still be able to have a family life. I am able to balance my work and home life by being a Podiatrist.

3. How do you propose to ignite the idea of a podi-atric career to students?Going out and doing talks to college students. Allowing students to shadow with us even as early as high school. Giving students that opportunity will ignite their interest.

4. What has PPMA done for you lately?PPMA, as well as APMA, has kept me up-to-date with issues going on in our field, locally and nationally. This has allowed me to voice my opinion on different matters to the govern-ment that relate to Podiatry and medicine in general. PPMA/APMA have the proper resources in order to help us get in contact with government officials to make appropriate decisions regarding our field.

5. How does podiatry’s future look to you? Podiatry’s future is bright. It continues to grow and grow yearly. People need their feet, and we are the right specialists for that. UPDATE
