There is a long way from the first idea till the realization.The designing of each device is preceded by surveying ofthe beauticians' demands: in addition to defining thetreatment objection, we pay attention to the cosmeticians' other requirements as well. The blood vessels in the skin surface treatedwith galvanic current dilate and blood flow isstimulated. Oxygen and nutritional supply to thecells improves, as well as the removal of wastematerials and skin functions. Owing to thevasodilating effect, the speed of absorption ofactive ingredients may be multiplied. As a resultof these, the iontophoresis device has been pertaining to the “basic devices” for decades.Though the use of iontophoresis has been limitedby the appearance of ultrasound, but eventoday, the iontophoresis devices can be appliedthe more effectively to take in water-soluble,charged ingredients.
However, the use of galvanic current has variousside effects as well: it causes the migration ofions in the cells, which tilts over the normalequilibrium of ions in the cells and alters theircharge, beyond that causes a shift of the skinpH value. With the exception of the fact that weuse this effect during the deep-cleaning (desincustration), this is to be seen as a non-desirable side effect of the iontophoresistreatment, exceptionally because these changesinfluence the intake of active ingredients andcause itching.
Another side effect of the use of galvanic current is the stimulation of the nerves responsible for vascular reactions. The blood ves-sels in treated area dilate, thus skin types proneto reddening (rosacea) must not be treated withgalvanic current.

We have developed a newform of current which keepsthe advantages of galvanic current but eliminates its sideeffects. Therefore the microderm® 2000throws new light upon the cosmetic treatment done with galvanic current.

After selecting the microphoresis treatment program,the microderm® 2000 device carries out a treatment section consisting of three steps.
The first phase of the treatment is LYMPHATICMASSAGE: the handpieces oscillate at a constantspeed, and the movements of the electrodesfollow the usual directions of lymphatic massage - simultaneously on both cheeks, fromthe centerline outwards, to the lymph nodes.The fine vibration triggers the lymph flow in thelymphatic vessels of the face and the neck, thusreducing the oedema of the face and the eyeregion, stimulating the removal of waste materials, therefore the healing of problem skin.
The second phase of the treatment is STIMULATING-NUTRITION: we have to set onlythe polarity according to the type of active ingredient applied during the treatment. Thehandpieces oscillate at a constant speed andconduct autopolar® current into the skin as well.First we apply gel ampoule and moisturising gelon the skin, and then we work these active ingredients entirely into the skin by conductingthe handpieces with circular, slow and smoothing movements on the surface of theskin.
The third and last phase of the treatment isREFLEX STIMULATION: the handpieces oscillateat a continuously changing speed but don't conductcurrent to the skin. The viofrim® is a combination of two treatment instruments, which is designed for disinfecting with highfrequency current and face cleaning with a circular brush.
The high frequency glass tube of the instrument containsrarefied inert gas, which starts ionizing by the effect ofhigh frequency. The ions start glowing upon contactingthemselves and the wall of the glass tube. Approachingthe glass electrode to the skin, sparks jump to the skinsurface.

Vital processes depending on temperature
The metabolism of the skin and musclesdepends on temperature: an increase of 1oCresults in 13% increase in the speed of themetabolism. Oxygen consumption of cells ismultiplied by 2-3 if the temperature is increasedby 10oC. The skin responses to the fluctuationsof temperature through its regulatory mechanisms:blood flow through increases or decreases, theblood vessels dilate, and more sweat is secreted.
Changes of temperature are perceived by thequite special “antennas” of the skin, by the thermoreceptors. The points of cold and hotperception are present in the skin in differentnumbers: there are 1-5 cold points in an area of1 cm2, while the number of hot points is only 1in an area of 2.5 cm2.
The face skin is the most sensitive area of thewhole body in respect of heat-effects: here are16-19 cold points in a unit of skin surface. That'swhy the cold treatment carried out with thethermoderm® 2000 device has such a spectaculareffect.
Heat receptors are active between 30-45oC -their maximum sensitivity is somewherebetween 37-43oC. That's why the temperatureof the skin-ironing program of our instrumentcan be set between 36-44oC.
The heating up of the skin
Due to perspiration, superficial thermal therapyresults in removal of waste materials from theskin. The thermal effect makes the vesselsdilate, thus the blood flow through of the skinis improved, as well as the nutrition of the skinand the removal of waste materials. The dilatedpores facilitate the penetration of active ingredients: the emulsions, ointments andserums penetrate more effectively than withoutthe thermal effect.

Thermophoresis
The device has been designed to carry out oneof the three electro-cosmetic methods for intakeof active ingredients, the thermophoresis withheat, thus this is its main function.
This method is suitable for the intake of everywater or oil-soluble preparation, especially forthe “dissolution” and more uniform spreadingon the skin surface of oilier and denser creammasks.
This method without direct current effectmeans a safety alternative in all cases when thepatient couldn't be treated with current(pregnancy, pacemaker, afraid of treatmentscarried out with current), and when the use ofultrasound is contraindicated (for example thetreatment of tissues of the breast or over sensitiveareas), or when the ultrasound can't penetrateinto the treatment products (for examplepreparations consisting powder).
The handpiece is evidently made from such typeof material that doesn't get into chemicalreactions with the active ingredients, thereforedoesn't influence their biological activity.

Skin-ironing and trap of active ingredients
The essence of the most complex treatment programof the device is that in the hot phase (36-44oC) and inthe cold phase (18-25oC) - using the optimal ranges oftemperature for the function of the heat receptors ofthe skin - can influence the metabolic processes. Theheating up is objected to stimulate the metabolism inthe skin, the cooling down prevents from this.
During the first phase the pores dilate, the activeingredients can penetrate more intensively and moreeffectively. Due to the full-bloodedness effect, the skinreceives more oxygen, that stimulates its metabolismand triggers the removal of waste products.
During the second phase of the treatment the porescontract as a result of the cooling down, thus the skintraps the introduced active ingredients. The decreaseof the temperature reduces the activity of the enzymesas well, making metabolism to return to its originallevel, and by the cooling down the reddening of theskin ceases as well.
Special heating-cooling handpieceThe thermoderm® 2000 device treats the skin gently. Ifthe handpiece touches the skin surface, the heat is conducted by the tissues, thus the temperature of thetreating area alters. Careful dosing of the heating andcooling effect is ensured by the automatic adjustmentof the temperature values programmed in the software of the instrument. The device controls continuously these fluctuations, and sets the intensityof heating or cooling from moment to moment according to it.
The cooling down of the skin surface - similar to thefunction of a fridge - causes formation of heat.Effective cooling and fast changing of temperature canbe provoked only if the created heat is conductedaway effectively. In the centre: the P. acnes
The chronic inflammation of the sebaceousgland and the hair follicle belonging to it (theso-called pilosebaceus unit) causes the acne.Under normal circumstances the oily materialproduced by the sebaceous glands gets to theskin surface along the fine hair present in thehair follicle, and there, by spreading, contributes to the building up of the protectivefilm in the skin surface - the so-called hidrolipidfilm.
The acne process begins when a comedo buildsup in the common way leading out from thepilosebaceus unit, thus blocking the clearing ofsebum.
Thus the sebum gets stuck and the bacteriumresponsible for the inflammation - the Propioni-bacterium - multiplies in it.
The bacterium makes the sebum to become irritating fatty acids, which have a role in thedeveloping of inflammation. Due to differentchemotactic factors, leukocytes are gatheringtogether as well, which cause the inflamedinfiltration evolving around the sebaceousgland.
Finally: Acne develops with comedos, pus blisters and hurting nodules, which lasts for along time and normally causes scars.

Photoinactivation
The extirpation of the Propioni-bacterium, whichplays the main role in the pathophysiologicprocess, causes the stopping of the acne process,thus the mitigation of subjective symptoms. Ourobjective was to introduce an electro-cosmeticmethod that without the well-known side effectsof medicines, with direct or indirect electriceffect could reduce the mild form of acne.
In order to be able to understand the joint effectof the three different methods used in our newdevice, we have to learn about the characteristicsof the pathogen bacterium before.
During its biological activity, as many other creatures, the P. acnes produces porphyrins, anatural group of compounds, which occur in thehuman organism as well. If the porfirins areexposed to light of a certain wavelength, free-radicals develop, which extirpate the bacterium.
The porphyrins can absorb the energy the mosteffectively in the spectrum of visible blue light(between 400-430 nm), thus the developing offree-radicals and the extirpation of bacteria canbe stimulated the most effectively with this typeof light. This means the photoinactivation, whichcan ruin the multiplying bacteria in the interiorof the sebaceous gland - in an area where thecosmetics can't penetrate - with the help of light.

BRH® (Blue-Red-Heat) Handpiece
Our development was based on several scientificexaminations, during one of these the blue lightwith low intensity has been found effective in thetreatment of mild or middle-severe acne. In thisresearch, the patients involved in it were treatedfor 15 minutes, twice a week, totally four weekslong. A 60% improvement was observed by the80% of the subjects, which proved to be long-last-ing for 3-8 months.
During another research, the blue light was com-bined with 660nm-wavelength red light, whichproved to be significantly more effective in respectto acne on the whole, than the blue light in itselfor one of the ingredients of the preparations forexternal use applied against acne, the benzoyl-peroxid.The more important is the anti-inflammatory effectof the red light, which derives from the fact thatthis spectrum of light can influence the release ofcytokins coming from the macrofags or other cells.The cytokins control the function of the cells ofconnective tissues and the production of growthfactors, thus the red light stimulates therunning of the inflammation andhealing process.

CDM®
During the ultrasound treatment a contact agent (e.g.a gel) provides the free way of the waves between thehandpiece and the skin. Dr Derm ultrasound devices areequipped with a contact checking mechanism, whichgives a warning beef if contact between the handpieceand the skin is interrupted. This is the so-called CDM®
automatics, which controls the penetration of ultrasound into the skin and prevents the handpiecefrom heating up. The device gives a warning sound signal as well when the treatment material isn't the suitable quality and hinders the penetration of ultrasound, for example contains a lot of powder material, air-bubbles or not enough small vegetableparts, because these don't conduct the vibration ofultrasound. Fountain effect
If the handpiece is turned upwards and water is poured on its surface that touches the skin, theultrasound makes the water-molecules to move and a special effect may be observed. This is theso-called fountain effect, with which the mechanical effect of ultrasound can be demonstrated. Ifthe handpiece emits ultrasound continuously, the “sparkling” and evaporation (cold vapour) of watershould be observed in the whole surface of the handpiece.
In the case of a device with CDM®-automatics, the effect of ultrasound may be observed only whenthere is enough water poured on the surface. The reason for this is that the device perceives theconduction of ultrasound in the treating area, and works with the preset power only when thisconduction is perfect.
Through the fountain effect the proper function ofthe crystal can be controlled as well. Bydevices the handpiece of which doesn'twork adequately, the water is heated upquickly or there isn't any sparklingproduced.
Our laser-ultrasound handpiece emitsthe light only if there is a perfect contactbetween the treating area and the skin,ensuring a safety treatment in this way.If water is poured onto the treatingsurface, by triggering the fountaineffect the light sources are lit as well.** We should take care and wear protective eyewear.
Small handpiece

It's an old observation that lypolsis during bodytreatments is the most effective if the handpieceheats up the tissues through the continuousemission of ultrasound, thus improving therelease of lipids accumulated in the fatty cells.

Stimulating current and interference currentStimulating currents are those current forms,which cause the muscles to contract - holds theold definition.
The temporally interrupted direct current with afrequency between 10-100 Hz really evokes amuscle contraction. However, the skin resistanceto low frequency currents is very high, and thecurrent also stimulates the receptors of pain,which restricts efficiency. The more intensivecurrent you conduct into the skin through theelectrodes, the more effective musclecontractions you can evoke - and the moreuncomfortable sensation, however.
Currents in the middle frequency range (e.g.over 1000 Hz) penetrate into the skin more easily, thus they got to the focus of attention. Iftwo currents of different frequency (e.g 3950 Hzand 4000 Hz) are directed to a given area sothat they are crossing each other, then a resulting vibration of 50 Hz appears, accompaniedby its characteristic physiological effects.
The result is phenomenal: the efficiency of interference current surpasses that of the traditional stimulating current.

The lymph is a fluid located in the intercellular space, which consists of the fluid being secreted in the arteriousside of capillaries and the waste material created during the metabolism of cells. Hyperhydrosis is a state with excessive perspiration originating from the increased excitement of thesympathetic nervous system, the disease may involve the whole body surface, but symptoms occuron hands and feet more frequently. Hyperhydrosis, beyond that causes difficulties in the daily lifeand work, could trigger various dermatologic diseases (e.g. eczema on hand, infections caused byvirus or mycosis).
