All food is low sodium; diabetic options available. Memory support with manageable behaviors, no elopement risk, no exit seeking patterns Coordinating MD appointments and transport for medical needs (dialysis, chemo etc.) Diabetic management (blood glucose checks, insulin management) Stage 1 and 2 decubitis ulcers Wound management End of life care with hospice support (palliative) PT/OT/Speech therapy Respiratory therapy Catheter care and urostomy care Colostomy care Ileostomy care Social worker for long-term planning assistance The Wellington Senior Living Community Care Capabilities The Wellington can provide care to your patients who may not need 24-hour nursing supervision. We offer both temporary respite care and full time oversight in a home-like community setting. Our residents enjoy the privacy of their own apartment along with the peace of mind knowing nurses are on staff and trained caregivers are available to provide assistance and oversight. Please note that prior to your patient’s transfer to our community, state regulations require an assess- ment or re-assessment by our Director of Health Services/RN. As you prepare for discharge, please call us to plan a time for the assessment.
