New CMS Payment Model Aims to Improve Rural Health CareIn conjunction with an August 3 Executive Order issued by President Donald Trump, the Centers for Medicare & Medicaid Services (CMS) announced a new payment model on August 11 with the goal of increasing access to and improving the quality and financial economics of rural health care. This new demonstration model, the Community Health Access and Rural Transformation (CHART) Model, aims to increase rural providers’ financial stability by providing up-front funding and predictable payments that are less dependent on service volume. The CHART model will also relax certain requirements via waiver to reduce regulatory burden and increase flexibilities for model participants. CHART has two tracks: The Community Transformation Track, and the Accountable Care Organization (ACO) Transformation Track.

Under the guidance of a lead agency which will manage funding, recruit hospitals, engage with the state Medicaid agency and ensure compliance with CHART model requirements, participating rural hospitals in the Community Transformation Track will be involved in implementing a health care delivery redesign strategy. Model participants will benefit from regulatory flexibilities to support the redesign process, including cost sharing waivers for Part B services, transportation assistance for beneficiaries with transportation, waiver of the three-day inpatient stay requirement prior to admission to a Skilled Nursing Facility and the Critical Access Hospital 96-hour rule, telehealth expansion, post-discharge and care management home visits, and the waiver of certain Medicare hospital conditions of participation.

Applications for the Community Transformation Track will be available in September, with implementation expected in the Summer of 2021. Up to 15 lead agencies will receive as much as $5 million to implement the redesign, and participating hospitals will receive a prospectively set capitated payment amount each year.

The ACO Transformation Track will provide up-front funding for up to 20 rural-focused ACOs. Participating ACOs will be required to enroll in a two-sided level of the Medicare Shared Savings Program (MSSP) and will have access to all waivers and beneficiary incentive programs available in the MSSP program. ACOs will receive a one-time up-front payment of at least $200,000 plus $36 per beneficiary, and a prospective per-beneficiary, per-month (PBPM) payment of at least $8 for up to 24 months. The actual amount will be based on the level of risk assumed by the ACO. A request for application for the ACO Transformation Track will be available in spring 2021, with implementation expected in January 2022.

Award winners will receive an engraved crystal award presented in person by WHA President and CEO Eric Borgerding, and recognition in WHA’s The Valued Voice newsletter and on the WHA website.

Rogers Behavioral Health Case Study Webinar: Addiction Recovery via Telehealth During COVID-19Rogers Behavioral Health Chief Medical Officer Jerry Halverson, M.D. and Executive Director of Addiction Services Michelle Maloney, Ph.D., will be presenting a case study on their insights from using telehealth for addiction recovery during a September 2 webinar from 2:30-3:30 p.m. The webinar is presented by the American Hospital Association (AHA).

The educational webinar will share their team’s experience in offering virtual outpatient treatment to patients in Wisconsin and across the nation during COVID-19, including insights into clinical outcomes, unique barriers and solutions, and the future outlook for tele-behavioral health.

There is no fee for the webinar, however, participants need to register here with AHA.

Member Quality Spotlight: Froedtert & The Medical College of WisconsinSaving more brain cells by reducing time to alteplase (tPA) on eligible stroke patients With current guidelines emphasizing delivery of intravenous treatment for stroke patients within 60 minutes of hospital arrival, Froedtert sought to reduce this arrival-to-treatment time to 30 minutes. A multidisciplinary team was established to analyze the process and steps needed from emergency department physician assessment, to activating the stroke team, to placing orders, obtaining CT scans and labs, to treatment. Froedtert used a tracking feedback tool for consistent timekeeping and to diagram a workable and efficient, streamlined process. As a result, Froedtert decreased the average door-to-needle time to less than 30 minutes, which may improve outcomes for stroke patients.

This project is important for patient safety because early delivery of intravenous alteplase saves brain cells. Up to 1.9 million brain cells are lost for each minute the brain is not perfused. By saving more neurons, function and recovery may also be facilitated.

In this initiative, we shared the unified vision of keeping the patient as the focus, so silos of care were effectively eliminated, and true teamwork was experienced.

Reflections from Terri Potter, Former CEO, Meriter Hospital; 1994 WHA Chair1994 - A year of transition and experimentation

As I prepared my reflections, two issues stood out to me – 1) It was still possible for members of the two-party system to find common ground, common solutions to common problems with civility (but there was evidence that the door was beginning to close), and 2) Health care reform was the dominant theme of the day.

Health Care Reform was on everyone’s agenda. Differing models were circulated at the state level and

in Washington. Hope was in the air. Health care organizations had begun to change how they operated, and financing models were being tested. WHA had developed Wisconsin HealthNet with these principles – universal coverage, delivery system restructure, and equitable financing. Governor Tommy Thompson was promoting his Health Care Partnership Plan. Insurance market reform proposals were on a parallel path. By late March 1994, the Wisconsin Legislature adjourned without passage of a bill. It would be many years before proposals of this magnitude would

again be considered by the Wisconsin Legislature.

Attention now shifted to Washington (late March). President and Mrs. Clinton were working on their own Health Care Reform proposal. Their system design included universal coverage, capitated provider payments, consumer choice, employer accountability, universal coverage, best insurance practices (e.g. pre-existing condition coverage). As the calendar rolled forward, it became clear that the President and Mrs. Clinton were going to experience great difficulty getting their ideas adopted.

Back in Wisconsin, a new Cost Containment Commission began its short life in January 1994. By the end of April the Commission was embroiled in “meddling” health care structure changes. By year end consideration for repealing the Commission was being seriously considered in Governor Thompson’s 1995-96 budget proposal.

In May WHA appointed its first vice president

of medical and professional affairs. This action reflected the hospital-physician teamwork necessary in the future as well the application of new quality management tools that hospitals were testing.

Other conversations competing for time on leadership’s agenda included the Department of Justice regarding antitrust, OCI regarding insurance markets and Department of Natural Resources regarding medical waste.

A Deloitte & Touche study published in mid-1994 reinforced the view that hospitals would be parts of integrated delivery systems with integrated information systems and new payment structures within two to five years. These predictions not only predicted change but also the pace of change. Dominant questions were:

• What would be the nature of the physician and hospital relationships in the future?• What would the health care be governing boards of the future look like?• What will be the necessary managerial and leadership skill sets of future leaders?

It is clear that 1994 was a special time to be in health care management. WHA’s agenda was vibrant and laid the foundation for future work.

Please submit at least one photo with your story whenever possible, as that helps tell your compelling stories to legislators, the public and media outlets in both the report and on social media.
