This recall involves battery packs that power heating systems in jackets. Two battery packs were included with the following Omni-Heat™ Electric jacket styles: Omni-Heat™ Electric Wader Widgeon, Omni-Heat™ Electric Big Game and Omni-Heat™ Gale Warning Interchange. The black battery packs are 3.25 inches long by 2.3 inches wide by 0.7 inches deep and marked with "Columbia" on the top and "OMNI-HEAT™" on the bottom of the pack. "Part: 054978-001" is printed on the side label.

HAZARD

The batteries have a cell defect which can cause overheating, posing a fire hazard.

INCIDENTS/INJURIES

The firm has received one report of a battery overheating that was discovered at its distribution facility in France. No incidents or injuries have been reported in the United States.

The affected dryers were manufactured between June 2005 and November 2006. Description: Wall-mounted hand dryer fitted into washrooms of premises such as shops, restaurants, places of work.

ELECTRIC SHOCK

The affected products have nozzles that are electrically isolated from the base of the dryer and therefore not connected to earth. If an electrical malfunction were to occur, causing an electrical component to come into contact with the nozzle, the user could experience an electric shock if he/she touched the nozzle.

Two incidents reported of users receiving a non-fatal electric shock.

The product does not comply with the Low Voltage Directive (LVD).

INCIDENTS/INJURies

CPSC and the firm have received about 14 reports of the battery cases overheating in the United States, including three reports of minor burns to consumers and four reports of minor property damage.

DESCRIPTION

This recall includes the Rocketfish™ Model RF-KL12 Mobile Battery Case. The battery case is made of black lightweight, soft-touch rubberized material designed to hold the phone securely, and comes with a built-in battery. The model number “RF-KL12” appears on the front of the product’s packaging, on the packaging barcode, and in white print on the curved inner surface of the product.

"Liebherr" is on the upper center of the refrigerator door and the top interior control panel. The model number can be found on a label located behind the bottom drawer on the left interior side of the single door refrigerator.

HAZARD

The refrigerator's door can detach, posing an injury hazard to consumers.

INCIDENTS/INJURIES

Liebherr has received 16 additional reports of doors detaching on the freestanding refrigerators, including two reports of injuries involving bruising and strains.

The recalled product is a Mophie Juice Pack Air rechargeable external battery which consists of a lithium polymer battery built into a plastic case designed to snap onto the back of an iPod Touch 4G music player. The battery cases come in black, blue or red. Only battery cases with serial numbers that have the first five alphanumeric characters of TR113 through TR120 are subject to this recall. The serial number can be found inside the housing of the product.


HAZARD

The battery case’s integrated circuit switch can overheat, posing a burn hazard to consumers.

INCIDENTS/INJURIES

Mophie has received 110 reports of the product becoming warm to the touch, 44 reports of the product deforming and nine reports of minor burns.

Affected products were sold through Optus. NETGEAR is investigating whether any products sold through other retailers may be affected.

HAZARDS

240V AC will be present on the Earth terminal of the pass-through socket outlet and the Active terminal of the pass-through socket will be grounded to Earth. This will present danger of an electrical shock under the following conditions:

(i) the device plugged into the pass-through socket has a 3-prong AC cable;

(ii) the AC cable is attached directly to the plugged-in product, not through an external AC-DC power adaptor; and

(iii) the plugged-in product has an earthed exposed metal surface.

Under the above circumstances, exposed metal surfaces of the plugged-in product will be at a Voltage of 240V AC. Any person touching the plugged-in product would therefore experience an electric shock.

DEFECTS

Incorrectly attached internal wiring in the AC pass-through circuit during production. Active and Earth wires have been accidentally inverted.

Consumers should discontinue using the product immediately in accordance with the following instructions:

1. Switch off the power point for the white adaptor so there is no electrical current running to the adaptor;

2. Remove the white adaptor from the power point; and

3. Unplug any device plugged into the white adaptor.

Consumers should then follow the directions provided in the new replacement package sent by Optus and use the reply paid satchel provided to send the faulty product back to Optus.

Consumers who have ordered a NETGEAR Powerline kit from Optus on or before 22 November 2011 and have not yet received a replacement kit may contact Optus on 133 937 or email NETGEAR at [email protected].

DESCRIPTION: This recall involves Heath®/Zenith and WirelessCommand® motion sensing wall switches with model numbers listed below. The product replaces a standard household wall switch and is designed to turn off the attached lighting load when motion is no longer detected in the room. The products come in white or ivory. The brand name and model number can be found on a label located on the side of the switch.


HAZARD

When the switches are in the auto mode and the light is off, a small amount of leakage current passes through the electric circuit, including the socket. If consumers fail to disconnect the power at the circuit breaker and make contact with both terminals inside the socket while replacing the bulbs, there is a risk of an electric shock.

Infants as young as 3 months can fall or escape from the seat by arching backward, leaning forward or sideways or rocking.

INCIDENTS/INJURIES

At the time of the 2007 recall announcement, CPSC was aware of 28 falls from the product, three of which resulted in skull fractures to infants who fell or maneuvered out of the product used on an elevated surface. CPSC and Bumbo International are now aware of at least 46 falls from Bumbo seats used on elevated surfaces that occurred prior to the 2007 recall, resulting in 14 skull fractures, two concussions and one incident of a broken limb.

CPSC and Bumbo International Trust of South Africa ("Bumbo International") are urging parents and caregivers to never place Bumbo Baby Seats on tables, countertops, chairs or other raised surfaces. Infants aged 3-10 months old have fallen out of the Bumbo seat and suffered skull fractures and other injuries.

HAZARD

The inner tube accessory can be pulled over a small child's head, posing a strangulation hazard.

INCIDENTS/INJURIES

Build-A-Bear received one report of an incident in which a 3-year-old girl pulled the inner tube over her head and had difficulty removing it.

The black battery packs are 3.25 inches long by 2.3 inches wide by 0.7 inches deep and marked with "Columbia" on the top and "OMNI-HEAT™" on the bottom of the pack. "Part: 054978-001" is printed on the side label.


HAZARD

The batteries have a cell defect which can cause overheating, posing a fire hazard.

INCIDENTS/INJURIES

The firm has received one report of a battery overheating that was discovered at its distribution facility in France. No incidents or injuries have been reported in the United States.

The firm has received 27 reports of cuts and scratches resulting from metal stars that loosened from the heel of the shoe.
