Joint Venture Allows Maple Knoll Communities to Offer Additional Services

Officials of Maple Knoll Communities and Cedar Village recently announced the formation of a joint venture, Village Home Health and Hospice, combining each organization's home health and hospice services to create a new, fully integrated provider of post-acute care services in Southwestern Ohio. Both non-profit continuing care retirement communities have a long history of serving older adults dating back to 1848 for Maple Knoll Communities and 1883 for Cedar Village.

As of July 1st, Village Home Health and Hospice have brought two organizations together to further their missions. Executive staff from each organization came together to determine how a joint venture could strengthen health care services in our community. Village Home Health and Hospice will blend together Maple Knoll and Cedar Village’s home health and hospice staff and services. The home health and hospice care will be offered at Maple Knoll Village, The Knolls of Oxford, Cedar Village and to individuals living in the Southwestern Ohio community.

Village Home Health and Hospice will provide a complete spectrum of home health and hospice services which will also include nursing services in the home, grief counseling, chaplain services, medication instruction, pain management, wound treatment, caregiver education, physical therapy, occupational therapy, speech therapy and social worker services. Staff members will be available 24 hours a day, 7 days a week.

The venture will allow dedicated and talented staff to provide skilled home health services enabling individuals to recuperate safely in the surroundings where they feel most comfortable ….their own home. Maple Knoll Communities previously offered home health services, and last year alone, they served nearly 500 clients accounting for 16,813 home health visits.

Fall 2016 These services, which were previously limited to their campus and close surrounding areas, will now be able to reach a larger number of older surrounding areas, will now be able to reach a larger number of older adults under the joint venture. The hospice care is specially designed to comfort patients and family members when patients have reached a point in their lives where the priority of care turns to ensuring the patient is at ease and comfortable, rather than curing the illness. Under the direction of a physician, Village Home Health and Hospice’s interdisciplinary team of clinicians will offer a special kind of caring that encompasses the medical, social, emotional and spiritual aspects of treatment.

Medicare, Medicaid, managed plans and most commercial insurances cover skilled home care and hospice care.

"This partnership allows us to provide a higher level of support with specialized programs for the older adults we serve," says Jim Formal, President and CEO of Maple Knoll Communities. "Not only will our current programs be strengthened, but it will add new services which will help us provide an improved and higher level of care in the community."

Bodmann Skilled Nursing Pavilion works hard to provide the comfort and care individuals can depend on for themselves or for a loved one. In order to give residents and family members the peace of mind they deserve, Maple Knoll has implemented specialized patient-centered bedside rounds with their care team.

MKV staff members were made aware of The Christ Hospital’s Acute Care for the Elderly (ACE) unit’s efforts to enhance care for vulnerable elders in the hospital under the leadership of medical director Dr. Jeff Schlaudecker. The ACE unit had a mission to provide interprofessional care with shared physician and nursing leadership for a unit focusing on hospitalized older adults with a focus on patient- and family-centered bedside, nurse driven, interprofessional rounds.

The MKV leadership team was impressed with the multiple initiatives seen, including the ACE unit’s focus on: Fall prevention, nurse-patient communication, bedside shift report, prevention of iatrogenic and nosocomial complications, functional status preservation, and early discharge planning. The MKV leadership team began a dialog about the transferability of ACE ‘lessons learned’ to the patients at MKV.

The leadership team of MKV formed a task force to develop a plan to begin bedside rounds on their patients, and they reached out to the ACE team for assistance. The ACE team was led by the Medical Director and Nurse Practitioner of the unit, and joined by the nursing unit manager, assistant nurse manager, 2 RNs, 2 PCAs, as well as an occupational therapist. The MKV team now includes the Director of Nursing (DON), the Director of Rehab Services, the nursing unit director, and the Vice President of clinical services.

After training and discussions the person centered rounds were implemented into Maple Knoll’s Skilled Nursing Care Plan. Rounds include a thorough review of the patient’s care, progress and treatment plan. During this time staff members are able to address any concerns with each member of the care team.

These specialized bedside rounds will occur once a week or more frequently as needed. Family members are encouraged to join and be present in the review of care and treatment. A member of the team will explain the rounding process, including the personal role in which the patient and family member will take part.

This level of proactive nursing care has already begun to improve communication, nursing staff satisfaction and self-efficacy, and resident satisfaction. Long term goals that are expected from the rounds include lower hospital readmission rates, shortened length of stays and decreased falls. For more information please call 513.782.2423.

Maple Knoll Village Skilled Nursing Pavilion Implements Specialized Patient-Centered

University of Cincinnati and Maple Knoll Village Affiliation Update

For 30 years, The University of Cincinnati has been a part of the Maple Knoll family and the two have shared a mission of caring. In April 2015, we were able to officially join forces and finalize a formal affiliation that will continue to bring both organizations to the forefront in telehealth and health care technology. In 2012, a partnership between University of Cincinnati's College of Nursing and Maple Knoll Village led to the creation of an Innovation Collaboratory Smart House, on the campus of Maple Knoll Village. Since the conception, the interprofessional growth has been on the fast track with the College of Engineering and Applied Science (CEAS), the College of Medicine, and the College of Allied Health.

The Smart House and the various projects continue to bridge care delivery throughout the Maple Knoll Campus and now into Maple Knoll's Senior HUD housing complexes. University of Cincinnati students and faculty have been working with Maple Knoll staff and residents to move forward on a variety of projects and programs at the Maple Knoll campus. Currently students are on Maple Knoll campuses five days per week. Recent student projects occurring within the collaboration include:

Telehealth Robot Testing in Various Settings, Smart House Creation, Smart Wheelchair Project, Hydraulic Medication Cart, Clavinova Connections, Resident Wood Shop Relocation ,Appliance Sensor Suite, Older Adult Fall Prevention, Home Health Staff Safety, Lift Harness System, Audiology Services Screening, Community Health Students Resident Education, Sensor Tracking in a Resident Apartment, Telehealth in Skilled Nursing, Telehealth Community Holistic Assistance Team Creation, Future Workforce Training, Community Health Student Days and a Move-a-thon program for employees.

This unique partnership between a university and retirement community is a foreshadowing of what the future brings as we are experiencing a growing population of older adults that need care. With every month more than a quarter million Americans turn 65. Ohio ranks 6th in the nation in the sheer size of this age population and Ohio's 85 and over population will triple in the next 30 years to reach 650,000. What we like to call the "Silver Tsunami" is fast approaching and the need for effective solutions that maintain the independence of older adults is on the rise.

The affiliation will allow UC to develop and test technology aimed at creating environments that can improve physical and mental status through self-care and independence. This collaboration benefits both the residents and the students through intergenerational sharing of history, technology, and care-coping skills. Maple Knoll will also serve as an incubator for students' innovations and inventions to be tested and evaluated before launching them in real life settings. Residents are not only getting firsthand knowledge of changes in healthcare but they are receiving access to care in a way they never dreamed possible.

Maple Knoll Village Skilled Nursing Completes a Deficiency Free State Survey

One of the highest confirmations of quality service is for a healthcare facility to receive a Deficiency Free Survey from the State Department of Public Health. Maple Knoll Village is pleased to have been found deficiency-free on its 2016 annual State Survey in Skilled Nursing. According to Centers for Medicare and Medicaid Services annual Nursing Home Data Compendium there are less than 10% of Skilled Nursing Facilities that achieve Deficiency Free State Survey in the United States. The survey evaluates the overall quality of life and quality of care of the facility to ensure it is operating by the Department of Health’s standards. Bodmann Pavilion, the Skilled Nursing Facility at Maple Knoll Village, consists of 145 beds offering both semi private and private rooms. Maple Knoll’s team of professionals develops an individual care plan based on various need levels. Services include long and short-term levels of care, skilled rehabilitation, a dementia/memory support unit, physical, occupational and speech therapy, a Hospice care wing and an on-site Physician clinic. Activities are a daily part of resident life at Maple Knoll Village. Independent Living residents have three Activity Directors dedicated to enriching the entertainment and education of residents. These independent Living residents have access to an average of 35 activities per week, both on campus and off. A variety of activities are planned to accommodate residents needs. These include:

Regularly Scheduled Bus Trips: Residents go on a minimum of three outings per week, on average the bus visits local shopping malls and specialty stores, Graeters, Kroger’s, Delhi Garden Center, Walgreens and more.

Did you know that prior to moving in to Maple Knoll Village you can be a part of an elite group known as Club MKV? The group consists of individuals who have completed an application and are on the wait to list to move into Maple Knoll Village at some point in the future.

These individuals are invited to unique events throughout the year, have the opportunity to familiarize themselves with Maple Knoll, meet with current Maple Knoll Village residents and create new friendships. Recently this included a Valentine’s Day social and a informational panel with brunch at The Manor House Restaurant. In addition, members are able to familiarize themselves with the campus prior to moving in. This includes meeting staff, sampling food and trying out new activities.

older adults in the community. Fundraising

efforts include Maple Knoll’s Annual Fund

Campaign which originated in 1989. This

year’s theme, “Reaching for the Stars”,

allowed Maple Knoll Communities to prepare

for today and tomorrow with a high goal of

$1,300,000.

These fundraising efforts were for all areas of Maple Knoll, which include the

retirement communities of Maple Knoll Village and The Knolls of Oxford, Maple

Knoll Home Health Services, WMKV 89.3 and 89.9 FM Radio Station, Maple

Knoll Outreach Services for Seniors, Maple Knoll Montessori Child Center, and

the Hemsworth Wellness Center.

John Bloomstrom, Maple Knoll Board Member and Annual Fund Chair, once

again led staff and 28 resident, community and board member volunteers in a

tireless effort to make this goal a reality.

We are happy to report that the 2016 Maple Knoll Communities Annual Fund

Drive brought in $1,598,577 from 1,839 donors.

Maple Knoll Village Main Street Art Gallery Upcoming Art Shows

Maple Knoll Village is dedicated to stimulating the minds of our residents, staff and visitors. One way we are able to do so is through a rotating display of creative works shown in The Main Street Gallery. Centrally located on the campus of Maple Knoll Village, the Main Street Gallery features different art shows every 6-8 weeks. These shows range from single artists to groups and range from oil paintings, watercolor, photography, woodworking and more. All works are for sale and a portion of the proceeds benefit the Maple Knoll Future Care Fund, a benevolent care program for residents in need. The current show features work from the talented Queen City Art Club. Queen City Art Club is made up of artists and art connoisseurs, both men and women, who are interested in exhibiting and promoting art in the Greater Cincinnati, Ohio area. Queen City Art Club was founded in 1976 with the purpose of sharing techniques, talents, artistic growth, and ideas in the whole art spectrum. Members are artists of various media, sculptors, and ceramists. This show will run through September 23rd, 2016. Upcoming exhibits include Hilltop Artists and the Maple Knoll Child Center. Recognizing New Members of The Living Legacy Society

The ability to touch the lives of others in a positive manner is something that we in the field of aging strive for every day. We also witness the positive difference that just one person can make in the life of someone else, a testament to the power of compassion, the power of caring and the power of generosity. At Maple Knoll Communities, we are fortunate to have over 100 caring individuals who have furthered the work of our organization. Alone, each person has made a significant impact, but collectively, they have the power to make miracles happen, by ensuring that Maple Knoll Communities’ mission of providing quality care and services to older adults will endure well into the future for our children and grandchildren. These visionaries are members of a special group called the Living Legacy Society. The “Living Legacy Society” was founded in 1992 to recognize those individuals who make a gift in a special way. This select group consists of special friends who have named Maple Knoll Communities in their estate plans, made a planned gift or contributed $50,000 or more in outright gifts. A unique display has been created to honor individuals who are members of the Living Legacy Society. This colorful display is located in Heritage Hall at Maple Knoll Village. It features an assortment of antique teacups or collector mugs that pay tribute to those donors whose generosity has propelled Maple Knoll Communities into the respected organization it is today. The well thought out teacup display reflects two important traditions of Maple Knoll Communities: the historic Spring Fete and the Anniversary Tea. Both events, which date back to the nineteenth century, allowed the public its earliest glimpses into the mission of the organization, bringing in new friends and supporters throughout our history. This past year, Maple Knoll Communities was thankful to have seven individuals who share in our mission and vision. These individuals were recognized at Maple Knoll Communities’ Annual Dinner in May as the newest members of the Living Legacy Society. They are: John and Helen Anderson, Bill and Wanda Baker, Jerry and Pat Bennett and Ralph Cautley. All of these new members are residents at Maple Knoll Village. It is because of their generosity and dedication that Maple Knoll is able to continue serving older adults in our community. Please stop by the unique display and see others who have shared this commitment to excellence in our community. Gifts of appreciated securities are a smart and simple way to maximize the effectiveness of your charitable giving. If stocks or mutual funds you've held for more than a year have increased in value, you may want to consider using these assets to fund your giving.

By transferring shares of your long-term stock to Maple Knoll Communities, a 501c3 organization, you make a gift to support our mission, avoid the capital gains tax you would pay if the stock were sold, and you may claim a charitable income tax deduction for the appreciated value of the asset.

Gifts of stock can be designated to specific areas or program of Maple Knoll Communities as any other gift would be. Larger gifting options may also provide a naming opportunity.

Using Trusts - Is a Living Trust Right for You?

“Revocable living trusts" have become popular estate planning tools. Whether a living trust is right for you, however, depends on a number of factors. A living trust may benefit you greatly, or you may experience a negative impact. We reached out to the experts at Mowry, Marty and Bain to gather additional information that may be of benefit to you. A living trust is a trust that you set up during your lifetime to which you transfer most or all of your assets. You get the income from the trust, and also have the right to withdraw principal. You can revoke or cancel the trust at any time during your life. At death the trust becomes irrevocable and its income and assets are disposed of under terms specified by you in the trust papers. Why would you do this? The main advantage of the living trust is that its assets are distributed without going through the court probate process. That avoids a filing fee in probate court. Also, trustee fees generally are lower than nonfamily executors' or personal representatives' fees would be. However, even if probate is avoided there will be the expense of preparing an estate tax return, valuing and transferring assets, and making a formal accounting and settlement. Also, to avoid probate, all probate assets must be included in the living trust. If some are left out, a probate proceeding still would be necessary. As a result, those with living trusts usually also have a will to direct any extra property into the trust. Some of the other benefits and pitfalls to consider are: Quicker distributions: Probating a will and gathering assets into the estate for distribution

can take quite a bit of time. With a living trust, by contrast, all assets already are gathered together, so the trustee can make immediate distributions and continue paying bills as usual.

Protecting minors: Living trusts can help avoid the need to appoint a guardian to represent children's interests, which can cause delay and add to administration costs.

Privacy protection: Since probate records are public, the size of your estate, and the names of beneficiaries and the amounts each received, can come into anyone's possession. The size and terms of a living trust, by contrast, are not necessarily public matters.

Multiple residences: Those with real estate in more than one state can avoid the problems and expense of multiple probate proceedings by putting the out-of-state real estate in a living trust.

Income taxes: If you create a living trust, you will be taxed on its income in much the same way as if you continued to own the property outright.

Estate taxes: It's a fairly common misconception that living trusts save estate taxes, but that's not necessarily the case. The trust assets within a revocable living trust will be subject to estate tax just as if you continued to own them outright. Therefore, basic estate planning tech-niques are important in the context of living trusts as well transfers at death by will.

As we said, living trusts make a lot of sense for some people and none at all for others. You have to consider all of the pluses and minuses as they relate to your particular situation to make an informed choice about a living trust. Maple Knoll Village To Renovate Health Center With Capital Campaign

Maple Knoll Village is currently in the midst of a capital campaign to upgrade the on campus Health Center located in Bodmann Pavilion. The current Health Center provides services to residents in Independent Living, Assisted Living and Skilled Nursing and has not received renovations since its construction in 1977. Under the leadership of University of Cincinnati Physicians and Maple Knoll Village staff, the clinic provides a convenient option for over 750 older adults to have the peace of mind they deserve. Services offered include non-emergency care, wound care, prescription pick up, respiratory care, podiatry, IV therapy, routine testing, audiology, dentistry, dermatology and optometry services. It is a cornerstone of care for older adults who reside at Maple Knoll Village and allows the great convenience of being able to be seen by some of the region’s best medical providers right at home. This long overdue renovation will provide spaces of greater comfort to residents when seeing their doctors or receiving other services from medical specialists. The following renovations are proposed for the Heath Center at Maple Knoll Village: Expansion of existing exam rooms within current floor area including integrated electronic

charting area, accessible exam tables, and new flooring, ceiling, lighting, finishes, furniture, cabinetry and window treatments.

Renovations to existing reception area including an open, friendly and accessible reception

desk; a hospitality station with coffee, tea and water; a new donor focal-wall with contemporary horizontal wood planking; and new carpet flooring, ceiling, lighting, finishes, furniture and window treatments.

Relocation and modernization of existing restrooms including upgrading facilities to full ADA

compliance as well as new ceramic tile flooring, ceilings, lighting, finishes, cabinetry and plumbing fixtures.

Creation within existing floor plan of new training/conference spaces for meetings and collabora-tive education sessions including new carpet flooring, ceiling, lighting, finishes, furniture and inte-grated technology.

General upgrade of all offices within clinic area with new carpet flooring, ceiling, lighting, finishes,

furniture and window treatments. Additional training space for University of Cincinnati Physicians and Nursing students. Advanced technology upgrades in exam rooms and offices. Upgrades to the existing Bodmann lobby entrance including new ceiling, lighting, finishes, furniture, window treatments and a custom artwork installation in place of the previous fountain. Upon completion the Health Center will be named The Gregg Warshaw, M.D. Health Center at Maple Knoll Village after Dr. Gregg Warshaw, a name that is nationally synonymous with innovative and caring geriatric care and expertise. In 2015 he retired as the Director of the Geriatric Medicine Program at the University Of Cincinnati College Of Medicine and as The Medical Director of Maple Knoll Village. For over 30 years, Dr. Warshaw continued to bring forth new levels of compassion, education, research, and collaborations for Maple Knoll Village. The project is a necessary renovation to the Maple Knoll Health Center that will improve quality health care for older adults in the community. Currently, Maple Knoll has raised $522,776 towards the $750,000 goal. To reach this goal Maple Knoll must rely on personal and community support. Gifts of all sizes are needed. Gifts of $1,000 or more will be recognized in a unique donor display in the main hallway. Larger naming opportunities are also available.
