SEALED BIDS FOR GROUP HELTH INSURANCE POLICY 2015-2016

The Punjab Educational Endowment Fund (PEEF) is an initiative of the Government of the Punjab.

It is registered under section 42 of the Companies Ordinance, 1984.

The PEEF invites bids for Group Health Insurance cover for its employees and their dependents (spouse &

children) for the period of one year starting from 01.09.2015. The insurance companies fulfilling the

following criteria may participate in the bidding process:

1. Minimum experience of 05 years in Providing Group Health services in Pakistan.

2. Licensed Group Life & Health Insurance Company through SECP.

3. GST and Tax registered company.

4. Minimum AA rating under PACRA/JCR-VIS any other reputed agency for the year 2014-15.

Other terms & conditions are as follows:

a. Prices quoted shall remain valid for a period of 12 months from the closing date of proposal.

b. Rates quoted should be in PKR inclusive of all applicable taxes.

c. PEEF has right to increase or decrease the number of staff depending upon new induction or

deduction.

d. List of hospitals on the panel of the insurance company in Pakistan should be provided.

e. Nomination of Minimum of 02 medical Officers, of the Company, who shall be responsible to deal

the cases of PEEF.

f. Turnaround time for claims settlement of less than 15 days.

g. Proposal should include List of policy exclusions.

Details of PEEF employees can be obtained from the office of the undersigned during office hours (08:00

AM to 04:00 PM)

Sealed Bids should reach the undersigned on or before Wednesday August 19 2015 not later than 1200

hours.

The bids would be opened at 02:00 PM on the same date in the presence of bidders and /or their authorized

representatives.

PEEF reserve the right to reject all bids at any time before the acceptance of any proposal or bid. The

reasons of rejection shall be communicated to the bidders upon request but PEEF shall not be held liable to

provide the justification of the reasons upon which the bids are rejected.

Instruction to Bidders

&

General Terms and Conditions

1. Only sales tax registered and N.T.N. holder firms may participate in the tender.

2. Rate of each item must be quoted separately inclusive of sales tax, where applicable. It

should clearly be mentioned on the quotation letter that "the rates are inclusive of GST

and all other taxes". If The Quotation letter does not bear the above mentioned statement, it

shall not be considered.

3. Earnest money @ 2% of the total estimated purchase price in the shape of CDR/Bank

Draft of schedule bank should accompany the tender otherwise tender will not be

entertained.

4. The office Tender Committee will open the tenders in the presence of the bidders or their

representatives.

5. 2% earnest money is refundable and will be returned to the bidders after the award of

purchase order to the selected bidder.

6. Quotation filled by the bidders with over writing/ tempering/ cutting/ erasing will be

canceled.

7. Income tax will be deducted at source as per government rules.

8. Quantity of supply may change according to PEEF requirements. The supplier will be

bound to accept the repeat orders as per quoted rates within financial year.

9. Delivery of the desired goods/services will be completed within 30 days after the

issuance of purchase order and agreement between both parties.

10. The supplier is himself responsible for any loss till the stores are inspected.

11. It will be the responsibility of the supplier to take back the supply declared not according

to the specification at his own risk and cost.

12. Each bidder will submit a certificate clearly indicating the amount of bid, amount of

CDR, No and date of CDR.

13. Tender envelop must be signed and stamped by the competent authority of the firm

otherwise tender will not be entertained.

14. PEEF reserves the right to reject all the bids at any time before the acceptance of any

Structure of Benefits Plan

Categories

Description of Benefit A B C D

Hospitalization Any expense incurred by an insured on the non-

pregnancy related hospitalization and surgical

procedure due to any sickness or accidental injuries

subject to a predetermined limit. There is no

restriction on number of days of hospitalization. Day-care surgeries are covered under the

Hospitalization Limit. Examples of day-care surgeries

include cataract operation (cost of IOL included),

stitches due to accidents, etc.

Pre & Post Hospitalization Expense

Benefit:

Pre & Post hospitalization benefit is available

within the basic hospitalization limit for a Rs. 137,500/- Rs. 125,000/- Rs. 93,750/- Rs. 62,500/- maximum of 30 days before or after the (non- pregnancy related) hospitalization to each family

insured per confinement. Please note that no credit

facility is available for Pre/Post expenses from our

approved hospitals. All insured are covered and will be

reimbursed by UIC after the hospitalization due to

sickness or injury only excluding pregnancy related

hospitalization. Pre & Post hospitalization limit is available for

the following treatment, which is directly related to the cause of hospitalization:

Maternity Benefits (Normal)

The maternity Expenses consist of antenatal

expenses (Expenses on consultation, lab test,

medication etc. before delivery). Delivery

Expenses (Expenses on delivery, normal or otherwise) and post-natal Rs. 50,000/- Rs. 37,500/- Rs. 25,000/- Rs. 18,750/-

expenses (Expenses occurring up to 21 days after

and related to the pregnancy.

Maternity Benefits (Cesarian)

The maternity Expenses consist of antenatal Rs. 100,000/- Rs. 75,000/- Rs. 50,000/- Rs. 37,500/-

expenses (Expenses on consultation, lab

test, medication etc. before delivery). Delivery Expenses (Expenses on delivery, normal

or otherwise) and post-natal expenses (Expenses

occurring up to 21 days after and related to the

pregnancy.
