CLINIC NEWS

Student Medication for Next Year

Jamie Hermann, our school nurse, will be in the clinic beginning Thursday,

August 4th, if you would like to stop in and give her your child’s medications for next school

year. The first week of school can be very hectic so this would be a good time to do that. The

St Johns County Medication authorization Form is on our POA website; we need a new one

every school year for each over the counter or prescription medication.

KEEP THE DRESS CODE IN MIND WHEN PURCHASING SCHOOL

CLOTHES OVER THE SUMMER

Shorts and skirts should be no shorter than 4 inches above the knee and

shirts should cover the shoulders. For full details on the District Dress Code

please see the District website at www.stjohns.k12.fl.us. Click on

“Departments”, “Student Services” and then choose either “Student Dress

Code” or “Student Code of Conduct”.

ATTENTION RISING 7th GRADE PARENTS! Students entering 7th grade next school year will need to provide Patriot Oaks Academy with proof of their child’s Tdap vaccine in order to receive a class schedule for the 2016-17 school year.

You may go to your private physician, the St. Johns Co. Health Department (904-825-5055) or a walk-in clinic (Care Spot, Minute Clinic, etc.). If you choose a walk-in clinic, a receipt will NOT be accepted as proof of the Tdap vaccine. The vaccine must be documented on an official Florida shot record. It would be advisable to bring a copy of your child’s shot record with you if you choose to use a walk-in clinic.

Take care of this requirement so your child can receive his/her schedule! If you have any questions, please call POA’s Nurse, Jamie Hermann, 904-547-4065
