How many times do some
patients need to hear what
you are saying to them, in
terms of recommendations? A
good friend said ten times, at
least.
This means we need to let them know what we expect
from them and what they can expect from us, over and
over and over. I find myself tiring of repeating my
recommendations, and sometimes I wonder either what
is wrong with me or what is wrong with them. It’s not
nice either way I say it.
All the parents reading this know even more so that
what I am saying rings true. Maybe you can even still
hear your mom saying “How many times do I need to
tell you…” (clean your room, take out the garbage or
not pick a fight with your sibling?) It must’ve worked
then and it still works now. It takes a long time to make
a responsible grown up and I am presuming all of you
who are reading this are responsible grownups. Mostly,
anyway.
Now for the ouchy part; we have heard it and heard it
but when does anything change? What does it take to
get you involved – all of you who are reading this
message? It only takes a bit of your time. You read this
message, then look for one of our phone numbers, call
and say hello, tell us your ideas, your comments, or
your complaint. Please, if you have a complaint, I
would hope you are a Chiro-PAC dues-paying cus-
tomer. Be like a good maintenance patient … don’t just
show up and get involved when it hurts; be proactive,
get involved.

platform. Which reminds me - I’d like to take this

Justin Mitchell. I’m sure I speak for everyone when I
say we wish him a stellar upcoming year with the
PCCS.

What Side Are You On?
No, this is not referring to politics, sports or religion
but rather to your familiarity with the correct manner in
which to identify laterality on a radiograph. The num-
ber of times that images are either incorrectly labeled or
not labeled at all is all too frequent, resulting in a time
consuming and frustrating experience for the radiolo-
gist. Often, the soft tissue anatomy, such as the heart or
liver shadows, are not seen or cannot be relied upon to
determine left from right and, without a side marker,
extremity laterality cannot be determined to any degree
of certainty. The radiologist must arbitrarily designate
a side as a reference point which may result in
confusion at a later date. However, much of this can be
avoided by adhering to simple rules and meticulous
attention to detail.
ALWAYS use a side marker. This includes chest films.
(Remember situs inversus?)
The marker MUST be inside the collimated field of
view but not obscure pertinent anatomy.
Verify that the marker does not shift by patient contact
or with movement of the image receptor or cassette
tray.
Use a marker to indicate the correct extremity.
The marker must indicate the side closest to the image
receptor (film) on lateral and oblique views.
For many practitioners, the correct identification of
oblique views of the cervical and lumbar spines is
confusing. This is unfortunate, since accurate
description of the cervical foramen and lumbar pedicles

visualized on the oblique view, requires correct place-
ment of the side marker. If you use a marker that indi-
cates the position of the patient (i.e. LAO or RPO for
example) the marker position is not as critical. Just
remember that one sees the homolateral IVFs on an
anterior oblique and the contralateral IVFs on the
posterior obliques. When a single right or left marker
is used, just follow this simple rule: Indicate the side of
the patient closest to the image receptor by placing the
marker on the correct side of the patient. That is, place
a right marker on the right side of the patient (or left
marker on the left side of the patient). If you do this on
cervical images, the homolateral IVFs are seen if the
marker appears on the image, under the occiput (behind
the spine). The contralateral IVFs are seen if the mark-
er appears under the mandible (in front of the spine)
regardless if one is positioned as an anterior or a poste-
rior oblique. In the lumbar spine a marker seen posteri-
or to the spine demonstrates the contralateral pedicle
(“Eye of the Scotty Dog”) and the homolateral pedicle
is seen if the marker appears in front of the spine.
When performing motion studies indicate the direction
of the motion. For example, mark images with the
word flexion, extension, rotation, etc.
If films are obtained with the patient in an upright posi-
tion, the word “Upright”, “Standing” or an “Up” arrow
should be used.
FYI, by convention, advanced imaging such as MRI
and CT demonstrates the patient’s right side on the
viewer’s left on axial images.
Understanding and adhering to these simple principles
will improve the accuracy of the interpretation of the
imaging study and make life just a bit less stressful.

Tampa, FL (September 3, 2014) The University of SouthFlorida’s John Mayer, DC, PhD, recently received a $1.3million Department of Homeland Security/Federal EmergencyManagement Agency (FEMA) Assistance of Firefighters grant— an award that will help build upon cumulative researchevaluating the effectiveness of targeted exercise programs toreduce the risk of low back pain and disability in firefighters.The grant total includes a 5 percent extramural funding matchfrom the Florida Chiropractic Research Foundation.
Dr. Mayer is the Lincoln Endowed Chair of Biomechanical &Chiropractic Research and Coordinator of Research for theUSF School of Physical Therapy & Rehabilitation Sciences,Morsani College of Medicine. The new funding supports thethird phase of an ongoing FEMA-funded research projectinvestigating the link between poor back muscular fitness andlow back pain in firefighters and ultimately identifying exerciseinterventions that would be safe and effective in helpingprevent low back injury and illness in this at risk population.
For the latest study, a full-scale randomized controlled pre-vention trial, USF will partner with the Tampa Bay region’sthree largest fire departments, employing a total of 2,000firefighters: Tampa Fire Rescue, Hillsborough County FireRescue, and St. Petersburg Fire Rescue. The researcherswill compare the clinical effectiveness of two interventiongroups – one receiving a directly supervised exercise regimenat the fire station and the other receiving the same exerciseregimen only as a web-based, remote interaction – with acontrol group.
A smaller randomized controlled trial conducted by Dr. Mayerand colleagues published online earlier this year in theAmerican Journal of Health Promotion demonstrated that atwo-time a week, 24-week supervised exercise programtargeting back and core muscles in firefighters was safe andeffective. Firefighters in the supervised exercise programexperienced 12 percent and 21 percent greater gains in backand core muscular endurance, respectively, than didfirefighters performing the usual physical fitness regimen(control group).
Back injuries are the leading cause of permanent disabilityand early retirement among firefighters, who in theirphysically demanding jobs often carry more than 50 poundsof protective gear, haul heavy fire hoses, and lift andmaneuver stretchers around tight corners or contortthemselves to extract accident victims from cars.
“Our research thus far indicates that worksite exercisetraining for back and core muscles is a potentially usefulcountermeasure to reduce risk of low back pain infirefighters,” Dr. Mayer said.

New Development: No Rescreening for Chiropractic Massage Establishment Owners

Chiropractors choosing to accept non-patients for massage andkeeping the massage establishment license will not need to againbe fingerprinted and have a criminal background check by January31, 2015. The Department of Health originally took the position thatany physician fingerprinted and screened prior to January 1,2013 would need to be fingerprinted again, because it was thoughtthat the pre-2013 fingerprint and screening records are not avail-able.
On September 8 the department notified the FCA that the pre-2013records are available and will be used.
The department will be mailing a revised information packet toreflect that physicians who do not accept non-patients for massageare exempt from the massage establishment requirements andshould surrender the massage establishment licenses andphysicians choosing to accept non-patients for massage and main-tain the massage establishment license need not be fingerprint-ed and screened. All physicians are fingerprinted and screenedupon initial licensure. Those records are on file and will be used bythe department.
HB 1065, which took effect as law on July 1, 2014 requires allLMTs and owners of massage establishments to be fingerprint-ed and screened for criminal convictions by January 31, 2014.However, an exemption to the massage establishment licensestatute is granted to physicians who employ massage therapists to
perform massages only on their patients at their place ofbusiness. (The exemption does not apply if the LMT working inyour practice is an independent contractor.)
Physicians who are exempt should voluntarily surrender the mas-sage establishment licenses. The department must assume that aphysician is subject to the massage establishment statute ifhe or she holds the license.
A massage establishment license may be voluntarily surren-dered by sending the original license to Mathew Thompson,Regulatory Supervisor, Board of Massage, Department of Health,4052 Bald Cypress Way Bin-C-06, Tallahassee, FL 32399 with aletter stating:
This massage establishment license # xxx is voluntarily surren-dered. §456.043, Florida Statutes, (2014), does not apply to thispractice, because the practice is owned by (name of doctor(s)owner(s)), DC, and employs massage therapists to work solely onpatients of the practice. See §456.043(13), Florida Statutes, (2014).
