    The work to be performed in the State of Indiana includes the vacuum pressure impregnation of amotor stator and shall last approximately 1 week commencing on or about August 1, 2000.     IS. Radioactive material and contaminated equipment may be stored outside of the process and storagebuilding provided tnt allowable radiation levels in unrestricted areas are not exceeded. This materialmust be stored in either locked DOT intennodal containers as described in Title 49 Code of Federal

    Regulations (CFR) Part 101-3.61 or DOT approved strong tigbt containers. In addition to theserequirements the following criteria and resuictions must be adhered to whenever radioactive materials

    we stored:

    A, Containers used for outside storage of radioactive materials must be capable of withstandirgenvironmental conditions- Wooden containers are not considered suitable for outside storage.

    Materials received in wooden containers mav remain outside for a period not to exceed fiften(15) days, provided the containers are covered with a plastic tarp so as to prevent the infiltrationof rainwater. After 15 days the material must be moved inside or repackaged in a containersutable for outside storage.

    OB. Oupd storage of radioactive materials is only pemitted in paved (asphalt or concrete) ares.Storage on puss. dirt, or gravel is specifically prohibited.

Soil samples must be collected and analyzed at least quarterly along the edges of outside asphaltOr concrete pads where radioactive materials are stored and along the penimeter fence to ensurethat ther is no buildup of radioactive contamination. Data from these samples must be maintainedfor inspection by the Depanment.

    The combined authorizations for processing and storage granted by this license shall :not exceed atotal of 12.115 square fect

    16. The licensee is authorized to store containers which previously contained radioactive material (emptycontainers) outside of the process and storage building. Each empty container in outside storage shall:

    a. be emptied of contents as far as practical;

    b. contain no hazardous material or standing liquid:

    C. be in unimpaired condition and securely closed so that there will be no leakage underconditions normally incident to transportation or storage;

    d. have no radiation levels on the external surface of the package exceeding 0,5 in/hravenage and 2.0 mR/hr hotspot;

    e. have no non-fixed contamination on the external surface of the package exceeding10% of NRC Reg. Guide 1.86 free release limits;

    E. have no internal non-fixed contamination exceeding:

    Beta and gamna emitters 2200 dpmn/cm 2and low toxicity alpha emitters';

    All other alpha emitters; 220 dpm/cmi

    17. The licensee shall not accept either radioactive waste and/or items contaminated or potentiallycontaminated with licensable quantities of radioactive material or radioactive materials or items fromlicenable activities for repackaging, processing, refurbishing, storage pending disposal or disposalunless the shipper of such waste possesses a valid license for delivery issued pursuant to 1200-2-10-.32 of "State Regulations for Protection Against Radiation."

    tB. Wrinte assurunces must be furnished by the facility shipping the radioactive material indicating thatthe facility may accept rumn of the material processed or unprocessed. In addition, for states outsidethe Southeust Compact the state or appropriate Compact must be a signatory to the InterregionalAccess Agreement for Waste Management or assurances shall be obtained from the appropriate stategovernor's office. the state radiation control program, and the appropriate Compact official. if any.

    19. The licensee shall establish in every contractual obligation relating to radioactive materials the abilityto return radioactive materials, processed or unprocessed, to the prior licensed or exempt possessor.

    20. This license does not authorize the disposal of liquid wastes to the sanilary sewerage system. Liquidwastes shall be transferred To Nuclear Materials Management Center for processing and disposal inaccordance with conditions and criteria referenced in Tennessee Radioactive Material License NumberR-10137.

    21. The licensee shal maintain complete and accurate records of the receipt and disposal of radioactivemawtial The licensee shall. for radioactive material no longer useful for any purpose and for myequipment or supplies contaminated with such material for which further use and decontamination amnot planned, define those materials as radioactive waste and treat them as such in accordance with thefollowing provisions;

    A. Radioactive waste material shall not be stored with non-radioactive wast

    D. A written record of all radioactive waste mateial shall be maintained until it has beendowrnined by a suitable survey or radioassay that it has decayed to background levels or until ithas been shipped to an authorized recipient in accordance with applicable regulations.

    Accountabilitv of radioactive waste material prepared for shipment but not yet shipped from thelicensee's premises shall be maintained by the licensee by an internal record system such that thelicensee is constantly aware of the material's location and the proposed time of shipmentIndividuals who are involved in the shipping of such material and/or the storage of such materialpror to shipment. shall be trained in the precautions necessary for such handling and storage.

    C. For material which has decayed to background levels as determined by radioassay or external levelas measured with appropriately calibrated instruments, records shall indicate that the material wasdetrmined to be no longer radioactive and will indicate the methods and results of the survey oranalysis.

    D. Shipment records of radioactive waste material shall be maintained and the licensee shall requirewritten confirmation from the authorized recipient of such material that this material has been

    22, A. SFled sources authorized by this license with greater than 10 microcurics of alpha emiwugmteial shall be tested for leakage and/or contamination at intervals not to exceed six 16) months.In the absence of a certificate from a transferor indicating that a test has been made within six (6)Months prior to transfer. the scaled source shall not be put into use until tested.

    B. The tests shall be capable of detecting the presence of 0.005 microcuwie of radioactive material onthe test sample. or in the case of radium. the escape of radon at the rate of 0.001 microcwie per 24hours. The test sample shall be taken from the sealed source or from the surface of the device inwhich the sealed source is permanently mounted or stored on which one might expectcontamination to accumulate, Records of leak tests shall be kept in units of microcuries andmaintained for inspection by the Department.

    C. If the test reveals the presence of 0.005 microcurie or more of removable contamination. or in thecae of radium, the escape of radon at the rate of 0.001 microgurie or more per 24 hours, thelicensee shall immediately withdraw the scaled source from use and shall cause it to bedecontaminated and repaired or to be disposed of in accordance with Deparment regulations. Arepon shall be filed within five i5) days of the test with the Division of Radiological Health. 3VFloor. L&C Annex. 401 Church Street. Nashville. TN 3-7243-1532. describing the equipmentinvolved, the test results, and the corrective action taken.

    D- Tests for leakage and/or contamination shall be perfonned by the licensee. or by other personsauthorized by this Deparunent the U. S. Nuclear Regulatory Commission. or another AgreementSine to perform such services.

    25. The following evaluations shall be performed for all process ventilation systems:

    1) Air balance within the RCA at least semi-annually, and following any ventilation system orprocess changes which could potentially alter the effectiveness of the system.

    2) Pnrticulate removal efficiency of the main filtration system HEPA filtec by DOP Or compurubletesting in accordance with pertinent ANSI standards immediately following installation of newHEPA filters or at least semi-annually.

    26. In addition to other requirements of this license or of Chapter l200-2-5-.60 of "State Regulationsfor Protection Against Radiation,2 the licensee shall conduct operations so that radiation levels inunrestricted aes would not cause an individual, assuming an occupancy of one (1). to receive a totaleffective dose equivalent in excess of 500 millirems in one calendar year. These radiation levels shallbe approprialely monitored by the licensee. and records of such monitoring shall be maintained forinspection by the Department. In addition tothese requirements the following criteria and restrictions mnust be adhered to whenever radioactivematerials are stored:

    A. Containers used for outside storage of radioactive materials must be capable of withstandingenvironmental conditions. Wooden containers are not considered suitable for outside storage.Materials received in wooden containers may remain outside for a period not to exceedfifteen (15) days, provided the containers are covered with a plastic tarp so as to prevent theinfiltration of rainwater. After 15 days the material must be moved inside or repackaged in acontainer suitable for outside storage.

    B. Outside storage of radioactive materials is only permitted in paved (asphalt or concrete)areas. Storage on pass, dirt, or gravel is specifically prohibited.

Soil samples must be collected and analyzed at least semi-annually along the edges of outside asphaltor concrete pads where radioactive materials are stored and along the perimeter fence to ensure thatthere is no buildup of radioactive contamination. Data from these samples must be maintained forinspection by the Department.

    The combined authorizations for processing and storage granted by this license shall :not exceed a total of12,1 t5 square feet.

    16. The licensee is authorized to store containers which previously contained radioactive material(empty containers) outside of the process and storage building. Each empty container in outsidestorage shall:

    a. be emptied of contents as far as practical;
