Inspirations from the field The Proverbial Last Straw!It was a proverbial “last straw” thatbrought us to Dr. Arnold Gold’s office,in 1995. A few days before our sonAnthony was to become a first grader, he rushed towards me and the force ofhis impact knocked me against a wall inour house, driving my hand into the skinof my face, narrowly missing my eye.This was only the latest in a number ofphysical “accidents” in which he hurt me,and for which he would later apologize.Anthony had been discharged from twochild-care centers by age four, not foraggression but for not listening to theteacher and/or being uncooperative. Hisbehavior at home was even worse andmy husband, Rick, and I couldn’t un-derstand what caused these outbursts.

Although Anthony had graduated fromkindergarten in May, and was expectedto enter first grade, his behavior sincethen had seriously deteriorated. Antho-ny’s expulsion from summer day campwas a huge blow to him and to us.Dr. Laura Brent, our devoted psycholo-gist, provided invaluable guidance inhelping us get through each week withthis difficult young child. Extendedfamily gatherings, where frequentlyAnthony was the only youngster pre-sent, were the worst situations and hisbehavior would often flare up.

Expanding the CommitmentSince 1988, our organization and our programs have grown andflourished. We now work cooperatively with deans and theirassociates at more than 85% of American schools of medicineand osteopathy.

Two quotations continue to inspire The Arnold P. Gold Foun-dation staff and trustees. The first historic sentiment mirrors

the philosophy and concernfor the patient that we at The Gold Foundation seek to foster:“One of the essential qualities ofthe clinician is interest in human-ity, for the secret of the care of thepatient is in caring for thepatient.”

1999 marked the beginning of an exciting new decade for TheArnold P. Gold Foundation. Our strategic plan emphasizeshumanism in medical education, focusing on improving theresidency experience for young doctors,and promotes replicable models of caringhospital communities so that humanisticpractitioners will have receptive environ-ments in which to work.

A featured lecture at the national meetingof the Association of American MedicalColleges (AAMC) in Washington, D.C. onOctober 24 was devoted to “Humanismand Medicine.” This first annual Arnold P.Gold Foundation lecture, titled “TheIntersection of Science and the Soul,” wasdelivered by the distinguished writer and physician, Jerome Groopman, M.D.,Recanati Professor of Immunology at Harvard Medical School and Chief ofExperimental Medicine at Beth IsraelDeaconess Medical Center in Boston. His inspiring address celebrated a seriesof articles published this year by the prestigious journal Academic Medicine.We thank the Healthcare Foundation ofNew Jersey for underwriting this lectureand reception.

It is clear from the overflow audience atDr. Groopman’s lecture that our message–that humanism in medicine must bereemphasized at all levels in health care–is resonating throughout the health professions.

This attention is due, in large part, to thespecial group of medical school deans whohave championed humanism and imple-mented our programs. Their enthusiasticcooperation and involvement is ultimately

responsible for the rapid expansion of The Arnold P. GoldFoundation’s programs. We are greatly encouraged by theircollaboration and express our gratitude for their active interest,advice and wisdom as we work together to develop effective anduseful tools to foster humanism in medicine.

Another group deserving credit for the success of our programsis you, our donors. Each dollar contributed to The Gold Foun-dation furthers our mission. Our programs reach out to moreindividuals in the health care professions than ever before.The Foundation especially thanks our new Golden Circlemembers, contributors who give an additional $25,000 or more in any given year, and we look forward to welcomingnew members.

It will support research and considerationof a national humanism honor society which will help to iden-tify and recognize those medical students who demonstrateoutstanding qualities associated with humanism in medicine,as well as excellence in scientific achievement.

We are also encouraged by the followingmessage to us from Reed Tuckson, M.D.,Senior Vice President for ProfessionalStandards at the AMA. He suggested wetell students that:

“... this is still the finest profession, but our values are being threatened every day. Do notbe dismayed and stay the course. The physician-patient relationship is the context around which these values are expressed.”

The Proverbial Last Straw!(continued from page 1)The fact that he had tested as verybright at three years old made hisrefusal to follow instructions and hisinability to get everyday tasks finishedvery difficult for us to understand.Shortly after these events, we asked Dr.Brent about testing Anthony for Atten-tion Deficit Disorder, the only diagnosisthat seemed a reasonable explanationfor his behavior. She then referred us to Dr. Gold. His subsequent diagnosisincluded Attention Deficit Hyperactivi-ty Disorder (ADHD) and OppositionalDefiant Disorder (ODD).Dr. Gold recommended several majorchanges, including a year’s delay in thestart of first grade, a combination ofmedication, behavioral therapy andparental counseling, known as the“multimodal approach.” (It involvesclose communication with the schoolsystem.) As a result, Anthony’s behaviorimproved dramatically.We were not seeing full-scale rages andtantrums everyday although they stilloccurred. While there were frequentsetbacks, improvements in emotional

maturity overall were being gained aninch or even a millimeter at a time.Dr. Gold headed the excellent clinicalteam that helped to save our son. Hisplan was implemented with the support

of another criticalteam member, apatient and lov-ing college stu-dent named SaraMoore, who work-ed with us forfour years, as ananny and a tutor.I am happy toreport that fouryears later, achild who enter-ed school a year

late and who received very few invita-tions to birthday parties that first year inelementary school, recently celebratedhis tenth birthday with 17 classmates.There have been many wonderful mile-stones for Anthony. In the Spring of1998 he was selected to enter the giftedprogram in our school district, one ofonly ten children from approximately150 eligible students. At the end ofthird grade, Anthony was honored with

others as a recipient of the “President’sAward.” In June of 1999, he was accept-ed on a “traveling” soccer team, a majorpersonal accomplishment for him.His capacity to excel in sports was madepossible because of his new ability tolisten and to follow instructions. Antho-ny has had the opportunity to becomeconnected with his peers, his team-mates and his family thanks to person-alized treatment. We thank the entirecompassionate and attentive team andespecially its leader, Dr. Arnold Gold.

Our FoundationPrograms On the Move!Clowning in Russia withPatch Adams

To acquire an overview of medicine andsocial services in the former SovietUnion, The Gold Foundation sponsor-ed two medical students to accompanyPatch Adams, M.D. to Russia inNovember. Donning clown costumesand assuming special new personalitiesfor the project, Amy Auslander andXochitl Olmos, third year medical stu-dents at Columbia University’s Collegeof Physicians & Surgeons, “madefriends” in orphanages and hospitals inMoscow and St. Petersburg.The two students, both of whom haveprovided leadership for The Founda-tion’s humanism in medicine web sitefor nearly two years, kept journals about the trip to post on The Big Picture...Daily Message portion of the web site.Amy’s trip was underwritten by fundsraised at a special event last Spring,hosted by The Foundation’s new Asso-ciates Committee, a group of dynamicyoung individuals interested in our mission.

Supporting the CompassionatePractice of MedicineA goal of The Gold Foundation is toencourage students to create programsand activities that support the compas-sionate practice of medicine. At Colum-bia, the student managed P&S Clubreceived Foundation support for a newevent for second year students,designed to reinforce the values of com-passion and humanism emphasized atfirst year orientation in the White CoatCeremony. To promote teamwork, trust,

communication skills, creative problem-solving and self-challenge, the classleadership invited the entire secondyear class to an outdoor environmentaway from the classroom for one day.

From Book to BedsideStudent Clinician’s Ceremonies (SCC),the new student-generated transitionalprogram for beginning third year stu-dents, was piloted again this summer atfive medical schools for a second year.The orientation programs – preparingstudents for the move from book to bed-side – are being evaluated and thechecklist for future replication of theSCC ceremony is being written. Repre-sentatives from several additional medicalschools have contacted us about partici-pating in the program next summer.

Encouraging Respect forDiversity and CommunicationPediatric residents at New York’s Montefiore Medical Center, under theguidance of Dr. Alex Okun, AssociateProfessor of Clinical Pediatrics at theAlbert Einstein College of Medicine,have joined our Home Visit Program.Other participant schools are the Dr.Jacob Handler program at Columbia,the New Jersey Medical School and theUniversity of Virginia, where Dr. LeighDonowitz initiated the program.The goals of the Home Visit Programinclude sensitizing residents to theiroften culturally diverse clinic patients,providing underserved families withpersonalized medical service by a teamof experts, and encouraging communi-cations between residents and theirmentor role-models.

Hands-on Participation and InvestigationFourteen students at seven U.S. medicalschools received grants to spend thispast summer exploring a wide variety ofpublic health issues. These students,selected by their deans, work closelywith faculty mentors on projectsdesigned to continue when the fellow-ships end.

Our Foundation’s Summer ResearchFellowships differ from most grantsmade available to medical students inthat they support hands-on participa-tion and investigation into communityhealth, rather than basic laboratoryresearch.

This year the medical schools at St.Louis, NYU and Tufts Universitieswere added to four other schools as

recipients of these uniqueresearch fellowships.

Adina Israel at the Universi-ty of Chicago conductedfocus groups to learn moreabout the attitudes andbeliefs of African-Americanmen with HIV and AIDS inhopes of learning ways toincrease patient compliancein often complex medicaltreatments.

At Cornell’s Weill College ofMedicine, Veronica Meneses

and Zephaniah Okeke shared a fellow-ship on a continuing project geared toeducate and prepare minority highschool students interested in studyingmedicine.

Among the four students at ColumbiaUniversity College of Physicians & Surgeons was Elizabeth Munro, whowas awarded the 1999 Helen and FredWaldorf Fellowship. Her project was,“Factors mediating medical adherencein HIV-infected children.”

Associates Committee Grows!Co-chaired by Amelia Gold and Brian Benson, FoundationTrustee and first year medical student at Columbia P&S,the Associates have divided into two subcommittees: a

medical students workinggroup and an event plan-ning committee.The medical studentsgroup is seeking two repre-sentatives from eachschool of medicine andosteopathy in the New Yorkmetropolitan area. They hopeto provide The Foundationwith a medical student per-spective and to help implement

activities for students related to humanism in medicine at theirrespective schools. The event planning committee is pursuingmany creative ideas through which to increase consciousnessabout the need for compassion in medicine, enlarge their net-work and raise funds to support Foundation programs.
