Feature Article: Understanding Urinalysis

ʺAs with blood test results, urinalysis numbers can be a little confusing, but letʹs try and make some sense out of them.ʺ

Urinalysis (UA) is a contraction of the terms ʺurineʺ and ʺanalysis.ʺ It is the physical, chemical, and microscopic analysis of a urine sample. It can be used to evaluate several different medical problems including:

1. Kidney failure 2. Urinary tract infections (UTIs) 3. Kidney and ureteral stones 4. GU malignancy 5. Acid-based disorders 6. Abnormalities of volume status 7. Rhabdomyolysis 8. Response to alkalization therapy

UA results can be divided into three broad categories;

1. Gross Inspection 2. Dipstick 3. Microscopy

In each case I give a typical test result.

Part 1: Gross Inspection Gross inspection includes color and turbidity (how clear the urine is). 1.1 Color Your urine will change color under different circumstances. If you are well hydrated it will be a light color, but if you are dehydrated it will be darker. Thatʹs why it will typically be darker first thing in the morning. Also, urine color can be influenced by several other factors including medical conditions, medications, and even foods. Table 1-1 smmarizes potential causes for different colors of urine. Color Potential Causes

Red Medical Conditions: bleeding, porphyria, factitious disorder Meds: rifampin, phenytoin, phenazopyridine Foods: beets

Orange Medical Conditions: hyperbilirubinemia Meds: rifampin Foods: excessive vitamin A or vitamin B complex

Brown or Black

Medical Conditions: Any condition causing red or orange urine if severe enough Meds: metronidazole, nitrofurantpin, senna, sorbitol

Green

Medical Conditions: Urinary tract infections (UTIs) Meds: methylene blue, propofol, amitriptyline, promethazine, metoclopramide, indomethacin Foods: asparagus, blue food dyes (blue mixed with yellow results in green)

Test Name: URINE COLOR Result: YELLOW

1.2 Turbidity Turbidity means how clear or cloudy the urine looks. Turbid urine may indicate: urinary tract infection (UTI) or precipitated crystals. Test Name: APPEARANCE Result: CLEAR Units: -- Part 2: Dipstick

The dipstick is a reagent strip that is dipped into the urine. This will trigger a series of color changes along its length that indicate the presence and concentration of various substances in the urine. Specific properties tested for include:

1. Specific gravity 2. pH 3. Glucose 4. Heme

5. Protein 6. Leukocyte 7. Nitrites 8. Ketones and 9. Bilirubin

2.1 Specific Gravity Specific gravity is a measure of density. It is the density of urine divided by the density of water as follows:

Specific Gravity = Density of urine/Density of water

Urine is mostly composed of water, but it does contain a few other compounds like electrolytes and urea that make it slightly denser than water. Significant amounts of protein or glucose in the urine will increase its density. If the urine has a specific gravity close to 1.001 it is very dilute. This condition can be caused by excessive hydration, diabetes, or acute tubular necrosis (ATN) where the kidneys have lost the ability to concentrate urine. A specific gravity of 1.035 implies the opposite. This condition can be caused by dehydration, SIADH (syndrome of inappropriate anti-diuretic hormone), CHF, cirrhosis, too much glucose (glycosuria), or too much protein in the urine (proteinuria), or recent administration of IV contrast. A specific gravity that is consistently around 1.010 is an indication of kidney failure.

Test Name: SPECIFIC GRAVITY Result: 1.009 Units: -- Reference Range: (1.009-1.025) 2.2 pH

ʺYou probably already know this, but hereʹs a reminder. Hypo means low, and hyper means high. How cool is that!ʺ

The pH of a solution is a measure of the concentration of hydrogen ions. The pH scale goes from 0 to 14. Solutions with a pH between 0 and 7 are acidic, and solutions with a pH between 7 and 14 are basic. Just to give you a feel for pH values, Table 2-1 gives the typical pH for several common items. The pH of urine should be between 5.0 and 9.0.

Table 2-1 pH for several common substances Checking urine pH can be helpful in:

1. Diagnosing renal tubular acidosis. 2. Monitoring urine alkalinization to prevent precipitation of

myoglobin in rhabdomyolysis, and to aid the elimination of certain drugs like aspirin and methotrexate.

3. Detecting different types of kidney stones. Urine pH is highly dependant on diet, so it should always be used in conjunction with other labs. Test Name: URINE PH Result: 6.0 Units: -- Reference Range: (5.0-9.0)

2.3 Glucose Glucose in the urine is referred to as glycosuria. There are only a few conditions that can lead to glucose in the urine.

1. Hyperglycemia. 2. Proximal tubule dysfunction (Fanconi syndrome).

Test Name: URINE GLUCOSE Result: Neg Units: -- Reference Range: (Neg) 2.4 Heme (Blood) The urine dipstick is highly sensitive for hemoglobin, but it also detects myoglobin. Blood in the urine is not a good thing. It can be caused by:

1. Bleeding in the urinary tract from a urinary tract infection (UTI), kidney stones, genitourinary (GU) malignancy.

2. Rhabdomyolysis (acute muscle breakdown). Test Name: URINE BLOOD Result: Neg Units: -- Reference Range: (Neg) 2.5 Protein The dipstick test for protein is most sensitive for albumin, and not very sensitive for other proteins. Also, it is dependent on urine concentration, so the more concentrated the urine, the more sensitive the test will be. Protein in the urine can be caused by:

1. Diabetic nephropathy 2. Multiple myeloma 3. Rhabdomyolysis 4. Intravascular hemolysis 5. UTI

Test Name: UA PROTEIN Result: 2+ Units: -- Reference Range: (Neg) 2.6 Leukocyte Esterase and Nitrites These two tests should be considered together. They are used to aid in the diagnosis of UTIs. Leukocyte esterase is an enzyme released by white blood cells (leukocytes are one of the five different types of white blood cells) and is used as a measure of white blood cells in the urinary tract. Why would there be white blood cells there? UTI, of course. This is your immune system in action.

You might be wondering why this test isnʹt just called ʺleukocytesʺ (as it is in some home testing kits) instead of ʺleukocyte esteraseʺ. Well, thatʹs because it doesnʹt measure leukocyte concentration directly; it just infers their presence by the enzyme activity related to them. See the difference? The nitrites test detects the presents of bacteria from the enterobacteriacae family, which converts the normally present nitrates found in urine to nitrites. Test Name: LEUKOCYTE ESTERASE Result: Neg Units: --

1. Diabetes 2. Alcoholism 3. Starvation

Test Name: URINE KETONES Result: Neg Units: -- Reference Range: (Neg) 2.8 Bilirubin and Urobilinogen The biochemical processes involved in the liver, kidneys, and gut to produce bilirubin and urobilinogen are quite complicated, but testing positive for these two substances may indicate liver disease. The dipstick test for bilirubin and urobilinogen has limited usefulness, and some hospitals donʹt even report these results. Test Name: URINE BILIRUBIN Result: Neg Units: -- Reference Range: (Neg) Test Name: UROBILINOGEN

For urine microscopy, a lab technician will examine the urine sample under a microscope to quantify:

1. White blood cells 2. Red blood cells 3. Bacteria 4. Crystals 5. Casts

3.1 Red Blood Cells (RBCs) Red blood cells are reported as the number observed in a high-powered field (HPF) of 400x magnification. Three or more RBCs per high-powered field is considered to be abnormal. The presence of dysmorphic RBCs strongly suggests glomerular disease. Many conditions can cause RBCs in the urine including:

1. UTI 2. Renal stone 3. GU malignancy 4. Recent instrumentation (including Foley placement) 5. Coagulopathy 6. Sickle cell anemia 7. Renal tuberculosis 8. Vigorous exercise 9. Contamination with menstrual blood

Test Name: UA - RBC/HPF Result: 0-3 Units: /HPF Reference Range: (0-3) 3.2 White Blood Cells (WBCs) White blood cells are also reported as the number observed in a high-powered (HPF). Five or more WBCs per HPF is considered to be abnormal. Causes of WBCs in the urine include:

1. UTI 2. Indwelling urinary catheter 3. Urologic malignancy 4. Chronic interstitial nephritis 5. Interstitial cystitis 6. Intra-abdominal inflammatory process adjacent to the GU tract 7. Contamination with vaginal secretions

3.3 Bacteria Bacteria are a common finding in urine microscopy and are typically caused by a UTI. 3.4 Crystals Crystals are highly organized, microscopic solids usually composed of a very small number of different ions or molecules. Formation of crystals depends on the concentration of ions and molecules as well as the urine pH. Crystals in the urine are very common and usually nothing to be concerned about. The six most common types of crystals are:

1. Uric acid crystals are most well known as the cause of gout when they form in the joints. When found in the urine they are most associated with tumor lysis syndrome caused by rapid cell death due to chemotherapy.

2. Calcium phosphate crystals. In huge quantities, calcium phosphate can lead to renal stones, but microscopic calcium phosphate crystals in the urine typically donʹt suggest any specific health issues.

3. Magnesium ammonium phosphate crystals are seen in UTIs with urease-producing organisms.

4. Calcium oxalate dihydrate crystals may indicate the presence of kidney stones.

5. Calcium oxalate monohydrate crystals usually are not associated with any disease, but may suggest ethylene glycol ingestion.

6. Cystine crystals are associated with cystinuria.

3.5 Casts Casts are long, cylindrical structures formed in the renal tubules due to precipitation of Tamm-Horsfall protein. Their formation is promoted by acidic or concentrated urine. There are several varieties of casts including:

1. Hyaline casts are the most common casts seen in urine. These are not specific to any disease state, but can indicate dehydration.

2. Muddy brown casts strongly suggest acute tubular necrosis (ATN). 3. Waxy casts are non-specific and are seen in a variety of renal

diseases. 4. Fatty casts contain yellow/tan fat globules, and strongly suggest

nephrotic syndrome. 5. Pigment casts contain one of several pigment compounds such as

heme or bilirubin. 6. Granular casts are believed to be from the degeneration of cellular

casts. 7. Red blood cell casts are strongly suggestive of glomerulonephritis. 8. White blood cell casts are strongly suggestive of interstitial

inflammation.

Success Story: My Bumpy Road Back to Health This is the story of my journey from obesity, heart disease, and Stage 3 kidney failure back to good health. Iʹll tell you right up front, I didnʹt do it perfectly, but even with several slips and setbacks, I stuck with the program and (eventually) made some amazing changes in my health. Iʹm just an average person, who has lots of weaknesses and gives in to temptations easily, but I kept getting ʺback on the programʺ and thatʹs how I eventually succeeded on the bumpy road to health. My hope is that reading my story will inspire you along your own journey and give you a little extra motivation. The approach I took was pretty simple. I made a few simple diet and lifestyle changes and cured several major health issues. This approach is not about treating your symptoms; itʹs about getting to the root causes of disease and creating an environment in your body that will allow it to heal itself. Your body is an amazing self-healing machine!

Your body can heal itself if you give it a chance.

All you need to do is get rid of the toxins and provide it with healthy nutrients. This is a very simple approach to creating health!

I want to stress that the program I followed is not just about weight loss - itʹs about health. In fact, itʹs not just about health; itʹs about enjoying life and living with purpose!

Letʹs face it; if you live every day in pain, life is not that enjoyable. If you live with the fear that something awful is about to happen, life seems

hardly worth the effort (I speak from experience). But if you live pain free, with hope for the future and a sense of purpose - now that makes for an

enjoyable life!

For me, weight loss was just a side effect. Besides obesity, my health issues included heart attack and open heart surgery, dangerously high blood pressure, Stage 3 kidney failure, alcoholism, enlarged liver, elevated hemoglobin levels in my blood, a spot on my lungs, neuropathy in my feet, painful arthritis in my shoulders, numbness in my left arm, and partial blindness in my right eye. OK, I was a sick puppy. I felt like I was standing on the edge of a cliff, and was about to fall over. What a precarious situation! By adopting a plant based diet combined with moderate exercise I was able to back away from the edge and put some distance between myself and disaster.

Hereʹs how it all started. November 29, 2017, Emergency Room at the VA Medical Center ʺI woke up this morning with my left arm completely numb. Not like when you sleep on it wrong, this numbness didnʹt go away. Also, I was partially blind in my right eye. There was a big black spot obscuring part of the visual field. I thought I had a stroke. So, after getting cleaned up I went directly to the emergency room at the VA medical center. I am obese with extremely high blood pressure, but I thought little of it because I drink a 12-pack of beer every day and just kind of hope nothing bad would happen. Boy, was I wrong! Is this really me? When I was young I could do anything and get away with it. Now, at 73, I have:

1. Dangerously high blood pressure at 228/137. 2. Left arm has gone numb. 3. Partial blindness in right eye. 4. The oncologists said that my blood chemistry indicates I have bone marrow

cancer, and a CAT scan showed that I have an enlarged liver. 5. With an eGFR of 38 mL/min, I am in Stage-3 kidney failure. My primary

care physician said the next thing is dialysis and then death. 6. Itching, rashes and sores all over my body. 7. Ocular migraines. 8. The neuropathy in my feet causes my legs to jerk at night, so I canʹt sleep

very well. Also, the arthritis in my shoulders makes it so I can only lie on my side for a short while. Therefore I flop back and forth all night just waiting for it to be morning. Right now I dread going to bed. This has to change!

9. Because of my excess weight I have trouble walking. I struggle just to walk from the bedroom to the living room without holding on to everything in

sight. At the grocery store I use the shopping cart as a walker. I havenʹt resorted to using their little electric carts, but thatʹs the next step. Also, I find it difficult just to bend over and pick up something off the floor. I used to be a strong hiker, now look at me!

10. The chest X-ray showed a spot on my lungs. 11. The emergency room doctor said I have too much blood and wants to take

some off. What! Bloodletting? I thought they did away with that during the Middle Ages!

12. Am I going to become disabled or die from a stroke? I have some serious health issues, and need to make some major changes. Hopefully it is not too late.ʺ So, hereʹs some straight talk. If you are overweight and have high blood pressure, it is LIKELY that you will have a heart attack or a stroke. Iʹm not trying to use a scare tactic; Iʹm just telling you the way it is. You may be thinking, ʺIt wonʹt happen to meʺ but Iʹm telling you this because Iʹve been there. It can, and probably will, happen to you! If you take action now, you may avoid some of the things I have lived through. For many heart attack victims the first symptom is death. In other words, they donʹt get any early warning signs; they just suffer a massive heart attack and die. Then itʹs too late to make any diet or lifestyle changes; they are already dead.

Iʹm not talking about life and death. Weʹre all going to die.

Hopefully that doesnʹt come as a surprise to you.

What I am talking about is quality of life while you are still alive. If you live a long and healthy life and then go down quickly at the end,

thatʹs a good thing. Itʹs called ʺcompression of morbidity.ʺ

Even if you already have some serious health issues, it may not be too late. You may be able to slow their progress or even reverse these issues by making a few simple diet and lifestyle changes. I did! The Standard American Diet (SAD) that consists of processed foods loaded with fat, salt, sugar, and toxic chemicals causes obesity, diabetes, high blood pressure, heart disease, and cancer. If you wanted to design a diet that would cause disease, you couldnʹt do better than the Standard American Diet.

Fast foods and prepackaged meals taste great, but they will kill you!

It has been shown in several well-documented studies that diabetes, heart disease, and even dementia (type 3 diabetes) can be prevented and even reversed. Given a chance, the human body is an amazing self-healing machine. In fact, your body is healing right now, and it works all the time, 24 hours a day! If it didnʹt, you would die in a short period of time. Sometimes the healing process is obvious like when you cut your finger, but most of the internal healing and immune system functions we are completely unaware of. How do you make changes regarding habits you have been practicing for a lifetime? How do you make these changes if you are like me and have no willpower? I had a heart attack and triple bypass open-heart surgery eight years ago and even that wasnʹt enough to scare me into permanent lifestyle change. It was enough for a few short-term changes, but after a few months I was

right back to my old ways, drinking a 12-pack of beer every day and eating loads of junk food. Different people are motivated in different ways. Fear and sickness were not enough for me. Willpower and toughing it out doesnʹt last very long. The secret for me this time around was to get excited about making these changes and to stay motivated. After several false starts I found that the key to success for me was twelve fold:

1. Sobriety: I couldnʹt follow any health program when I was drinking. I could talk a good story, but couldnʹt put it into action.

2. Healthy Eating: Whole foods, plant based. 3. Mild Exercise: For me it is hiking. 4. A Positive Expectation: I believed from the beginning (without a

doubt) that this program would work and I would achieve my goals. I learned along the way that setbacks are only temporary. In fact setbacks are part of the learning process and make you stronger.

5. Educating Myself: I learned about nutrition and health by watching documentaries and reading books.

6. Embracing the Effort: I had to recognize and embrace the idea that it was going to take some effort. I learned to view the hard or boring parts as just part of the process.

7. Emotional Involvement: Intellect alone doesnʹt do it. Emotional involvement drives these lessons deep into your being.

8. Stress Management: For me stress management consists of relaxation, visualization, and prayer.

9. Staying away from the edge. I canʹt flirt with temptation and get away with it. If I think about some unhealthy behavior long enough, Iʹll end up doing it. So, I have to plot a course that keeps me far away from the edge.

10. Learning to Treat Myself as if I Mattered: This was a tough lesson for me because my feelings of inferiority were part of my personality, but people can change.

11. Gratitude: No matter what your religious beliefs, you probably believe in some kind of Higher Power. I wake up each morning grateful for the opportunity to live another day, and that sets the tone for the entire day.

12. Share Your Journey: Try to help others get motivated and get healthy. Trying to help others (without being obnoxious) is very motivating to me. Thatʹs why I wrote this article.

By adopting a plant based diet combined with sobriety and moderate exercise I have been able to:

1. Bring my blood pressure down to normal, and donʹt need to take any prescription medications.

2. Lose 60 pounds. 3. Eliminate the neuropathy in my feet, and the arthritis in my

shoulders. 4. Start walking again and even do strenuous hikes without any pain in

my legs. 5. Bring my lab numbers back into the normal range with no indication

of kidney failure or cancer. 6. Also, I discovered a greater purpose for living.

What I thought I could accomplish in six months actually took over a year because of my slips and false starts, but getting back on the program right away instead of getting discouraged and quitting eventually paid off! Stories of amazing life-changing recoveries usually come from ʹyoung peopleʹ in their 40s and 50s, but at this writing, I am 75 years old, and Iʹm telling you that you can do this! Today I live pain free and am enjoying many activities that I havenʹt been

able to do for years. Foods With Anti-Cancer Properties

If you hear the words, ʺYouʹve got cancerʺ that is not an automatic death

We all have cancer cells in our bodies, but usually our immune system keeps them in check. Also, there is mounting evidence that genetics doesnʹt play as big of a role as we once thought. Cancer cells love sugar! Give yourself every advantage possible and cut out refined sugars from your diet. These include refined sugar and all highly processed foods that contain high fructose corn syrup and a bunch of other compounds with names that are designed to hide the fact that they are sugar. GMO foods that are ʺroundup readyʺ contain glyphosate, which is a known carcinogen. Herbicides, pesticides, fungicides and other poisons also promote cancer. The antibiotics found in industrially farmed animal products weaken your immune system, which is your only defense against cancer. Leave all that stuff out of your diet. Foods that fight cancer are high in fiber and antioxidants. Some of these include dark leafy greens, garlic, leeks, yellow and green onions, broccoli, brussels sprouts, cauliflower, turmeric, kale, ginger, red cabbage, spinach, beets, zucchini, apples, berries, cacao, lemon and other citrus fruits, grapes, flax seeds, and brown rice.

Recipe: Vegetable Bake Making up a vegetable bake is so simple and delicious. The sky is the limit! Just chop up some of your favorite vegetables, sprinkle on your favorite spices, and pop it in the oven. Here are a few ideas.

Aaron Alford, D.C. Aaron Alford is a chiropractor in private practice in Columbus, Ohio. He earned a B.S. in human biology and his chiropractic degree at Texas Chiropractic College. He has also completed a fellowship at the International Academy of Medical Acupuncture; is a Certified Chiropractic Sports Medicine Specialist; and a Certified Chiropractic Insurance Consultant. Dr. Alford has considerable teaching experience and is currently an adjunct faculty member at Columbus State Community College. Peter Breggin, M.D. Peter R. Breggin, MD is known as ʺThe Conscience of Psychiatryʺ for his many decades of successful efforts to reform the mental health field. His scientific and educational work provides the foundation for modern criticism of psychiatric drugs and electroshock (ECT) and he promotes more caring and effective therapies. He has been a consultant to the Federal Aviation Agency (FAA) on the effects of psychiatric drugs on pilots, and recently was the key expert witness in hearings before the U. S. Congress on psychiatric drug-induced violence and suicide in the military. He has authored more than 50 peer-reviewed scientific articles and more than 20 books including the bestseller Talking Back to Prozac (1994, with Ginger Breggin). Based on his clinical and forensic experience, he presented numerous examples of tragic adverse drug reactions in his book, Medication Madness: The Role of Psychiatric Drugs in Cases of Violence, Suicide and Crime (2008). He summarized the dangers of psychiatric drugs and described how to withdraw from them in Psychiatric Drug Withdrawal: A Guide for Prescribers, Therapists, Patients and Their Families (2013). Dr. Bregginʹs newest book is Guilt, Shame and Anxiety: Understanding and Overcoming Negative Emotions.

Dr. Breggin is a graduate of Harvard (with Honors) and Case Western Reserve Medical School. His psychiatric training included a Teaching Fellowship at Harvard Medical School. After finishing his psychiatric training, he became a full-time consultant at the National Institute of Mental Health (NIMH) for two years. He has taught at many universities. Dr. Bregginʹs private practice is in Ithaca, New York where he treats adults, couples, and families with children. Diane Dodge, M.A. Diane Dodge is a licensed social worker with a B.A. in Psychology and a M.A. from The Ohio State University. She is currently an Adjunct faculty member at Columbus State Community College and Central Ohio Technical College. Diane is an experienced child and family, couples, and group therapist and former clinical researcher. She has made presentations to numerous professional organizations and conferences. Caldwell Esselstyn, M.D. Caldwell B. Esselstyn, Jr., received his B.A. from Yale University and his M.D. from Western Reserve University. In 1956, pulling the No. 6 oar as a member of the victorious United States rowing team, he was awarded a gold medal at the Olympic Games. He was trained as a surgeon at the Cleveland Clinic and at St. Georgeʹs Hospital in London. In 1968, as an Army surgeon in Vietnam, he was awarded the Bronze Star. Dr. Esselstyn has been associated with the Cleveland Clinic since 1968. During that time, he has served as President of the Staff and as a member of the Board of Governors. He chaired the Clinicʹs Breast Cancer Task Force and headed its Section of Thyroid and Parathyroid Surgery.

In 1991, Dr. Esselstyn served as President of the American Association of Endocrine Surgeons, That same year he organized the first National Conference on the Elimination of Coronary Artery Disease, which was held in Tucson, Arizona. In 1997, he chaired a follow-up conference, the Summit on Cholesterol and Coronary Disease, which brought together more than 500 physicians and health-care workers in Lake Buena Vista, Florida. In April 2005, Dr. Esselstyn became the first recipient of the Benjamin Spock Award for Compassion in Medicine. He has published over 150 articles in scientific journals. In 1995 he published his benchmark long-term nutritional research study documenting the use of dietary intervention to arrest coronary artery disease in severely ill patients. That same study was updated at 12 years making it one of the longest longitudinal studies of its type. It is most compelling, as no compliant patients have experienced progression of disease. He has since published articles detailing follow-up at 30 years, and also for additional patients treated since the original group with the same results - compliant patients do not have further events or require additional treatment. Dr. Esselstyn and his wife, Ann Crile Esselstyn, work together to counsel patients both in Cleveland, where they live, and in summer at the farm in upstate New York where Dr. Esselstyn grew up. Dr. Esselstyn concentrates on the medical details, and Ann focuses on healthy foods and how to prepare them. Alan Goldhamer, D.C. Dr. Goldhamer is a graduate of Western States Chiropractic College in Portland, Oregon. After completing his chiropractic education, Dr. Goldhamer traveled to Australia where he became licensed as an osteopathic physician. He completed a residency program at the Arcadia Health Center, an in-patient facility specializing in the therapeutic use of water fasting. Dr. Goldhamer founded the TrueNorth Health Education

Center in 1984 and is the Director of the Centerʹs residential health education program. Dr. Goldhamer has supervised the fasts of thousands of participants. Under his direction, the Center has become recognized as a training facility for doctors wishing to gain certification in the supervision of the therapeutic use of fasting. He is on the faculty of Bastyr University where he teaches the course on clinical fasting and has been the principal investigator in several studies. The first, Medical Supervised Water-Only Fasting in the Treatment of Hypertension, appeared in the June, 2001 issue of Journal of Manipulative and Physiological Therapeutics. The second study, Medical Supervised Water-only Fasting in the Treatment of Borderline Hypertension, appeared in the October 2002 issue of the Journal of Alternative and Complementary Medicine. He is currently directing a prospective study with long-term follow-up evaluating the clinical and cost of medical care outcomes in the treatment of diabetes and high blood pressure with fasting and a health promoting diet. Dr. Goldhamer is the author of The Health Promoting Cookbook, which provides detailed nutritional information on how and why to adopt a health promoting diet; he is co-author of The Pleasure Trap.

Michael Huff, M.D. Dr. Michael Huff is a medical doctor who lives in Oxnard, California. He is a past president of the Ventura County Medical Association, and has served as both Director of the Education Department and Chairman of the Department of Family Practice at the St. Johns Regional Medical Center. Dr. Huff has also served as a consultant for the development of Primary Care Knowledge Base Kurzweil Applied Intelligence Voice Recognition System. Eileen Kopsaftis Eileen Kopsaftis, MS PT received her physical therapy degree from Russell Sage College. She has practiced for eighteen years and specializes in manual techniques. Her approach is eclectic with a priority on restoring pain free biomechanics and postural symmetry. The techniques she is trained to perform are Muscle Energy, Myofascial Release, Mulliganʹs Mobilization with Movement, Neurofascial Release, Strain/Counterstrain, Advanced Strain/Counterstrain, Feldenkrais somatic movement, and more. Eileen is a Certified MELT instructor, which is a self-treatment technique that addresses connective tissue and improves balance, postural alignment, and pain. William Lessler, LPCC William Lessler is a Licensed Professional Clinical Counselor and Adjunct Professor of Psychology who has been in private practice for over 10 years. He is a trained and experienced Cognitive Behavioral Therapist. of the deficiencies of the psychiatric model and better options for treating those with psychological disorders. Ron Lewis, Ph.D. Ron Lewis is a Senior Management Engineer at Ohio State University. He also teaches mathematics, statistics, and mechanical and manufacturing engineering at Columbus State Community College. Dr. Lewis has authored several papers published in scientific journals, and is experienced in teaching math, statistics, and technical subjects through distance learning. Pinar Miski, M.D. Dr. Pinar Miski is the Chief of Psychiatry at St. Agnes Hospital in Baltimore, Maryland, responsible for clinical and administrative duties for psychiatric consultation services in an acute care setting. She earned a B.S. in psychology from Penn State University, and a medical degree from Temple University School of Medicine. Dr. Miski has received awards including the Don Ross Award for resident psychotherapy paper, and was appointed Chief Resident for psychiatry services at The University of Maryland. She has been involved in several research projects, authored papers, and made presentations at professional conferences. Ralph Moss, Ph.D. The medical writer Ralph W. Moss, PhD, has written or edited over one dozen books and three film documentaries, most on the topic of cancer research and treatment. The former science writer and assistant director of public affairs at Memorial Sloan-Kettering Cancer Center in New York (1974-1977), for over 30 years Moss has independently evaluated the claims of various cancer treatments, conventional and non-conventional. He currently directs The Moss Reports, a periodically updated library of detailed reports on 200+ varieties of cancer diagnoses. Mossʹs books include Antioxidants Against Cancer, Cancer Therapy, Questioning Chemotherapy, and The Cancer Industry, as well as the award-winning PBS documentary The Cancer War. He also wrote the first article on alternative medicine for The Encyclopedia Britannica Yearbook and the first article on complementary cancer treatments for a medico-legal textbook, Courtroom Medicine: Cancer (Matthew Bender). He is co-editor of the first medical textbook in English on non-conventional treatments for cancer, Complementary Oncology (Thieme, 2005). His articles and scientific communications have appeared in such journals as The Lancet, the Journal of the National Cancer Institute, the Journal of the American Medical Association, New Scientist, Anticancer Research, Genetic Engineering News, the Journal of Alternative and Complementary Medicine, and Integrative Cancer Therapies, of which he is Corresponding Editor. For years he has written the monthly ʺWar on Cancerʺ column for the Townsend Letter for Doctors and Patients. His op-ed ʺPatents Over Patientsʺ appeared in the New York Times (4/07). Moss was a founding advisor to the National Institutes of Healthʹs Office of Alternative Medicine (now the National Center for Complementary and Alternative Medicine or NCCAM) and to the NIH Cancer Advisory Panel on Complementary and Alternative Medicine (CAP-CAM). He has been a member of the Advisory Editorial Board of the PDQ System of the National Cancer Institute (NCI). A dynamic public speaker, Moss has been an invited lecturer at many institutions, including Memorial Sloan-Kettering Cancer Center (Grand Rounds, Surgery, 1999), Howard University Medical School (Grand Rounds, Family Practice, 2003), the University of Arizona Medical Center (Grand Rounds, CAM, 2007), the Department of Energy, American Cancer Society, and many other universities, medical schools, and medical society meetings in the US and abroad. Moss has a particular interest in new European developments in integrative oncology. On October 31, 1998, he was made an honorary member and scientific advisor of the German Oncology Society (ʺDGOʺ), the first American to be so honored. Since 1997 he has spoken at ʺMedicine Weekʺ in Baden-Baden six times. In October, 2004 he spoke at the Seventh Pf„ltzer Conference on Integrative Cancer Care in Bad Bergzabern, Germany. The previous year he spoke on the chemoprevention of cancer at Santa Famiglia Hospital in Rome, Italy, and wrote a journal article on his findings from that trip. He also speaks frequently in North America. In June 2004 he gave the Coombs Lecture at Foothills Hospital, Calgary, Canada, and at the CAMera conference of the University of Calgary. In 2005, he gave the Todd Cancer Institute Grand Rounds Lecture at Long Beach (Calif.) Pamela A. Popper, PhD, N.D. Pam Popper serves on the Physicianʹs Steering Committee and the Presidentʹs Board for the Physiciansʹ Committee for Responsible Medicine in Washington D.C. Dr. Popper is Executive Director of Wellness Forum Health. The company offers educational programs for consumer and healthcare providers that facilitate evidence-base, collaborative and informed decision making for health-related matters. She served as part of Dr. T. Colin Campbellʹs teaching team at eCornell, teaching part of a certification course on plant-based nutrition. She has been featured in many widely distributed documentaries, including Processed People and Making a Killing and appeared in the acclaimed documentary Forks Over Knives, which played in major theaters throughout North America in 2011. Julie Rengert, Ph.D. Julie Rengert has a B.S. in biology and a M.S. in biology from the University of Texas and a Ph.D. in educational psychology from Ohio State University. After receiving his medical degree from Creighton University in Omaha, NE, Dr. Scholz completed his Internal Medicine internship and Medical Oncology residency at the University of Southern California (USC) Medical Center. He was awarded a medical oncology fellowship at Los Angeles County/USC Medical Center. Dr. Scholz served as oncology director at the Memorial Campus of the Centinela Freeman Regional Medical Center from 1996-2001. He is past president, co-founder and executive director of the Prostate Cancer Research Institute, a non-profit educational and research institute that focuses on disseminating state-of-the art information about the diagnosis, staging and treatment of prostate cancer. He is an acknowledged expert on management and treatment for prostate cancer using hormone intervention, immunotherapy, chemotherapy and angiogenesis as well as vitamin, herbal and other forms of lifestyle counseling. His affiliations include St. Johnʹs Health Center, Marine Campus of the Centinela Freeman Regional Medical Center and others. Currently, Dr. Scholz is an associate clinical professor in the department of Oncology at USC School of Medicine. Dr. Scholz continues his research projects in the area of prostate cancer and has authored numerous scholarly articles in his area of expertise. Sabrina Seitz Sabrina Seitz is a registered nurse in the cardiovascular catheterization lab at Sacred Heart Medical Center. Kathy Waller, Ph.D. Kathy Waller earned her B.S., M.S, and Ph.D. at Ohio State University and has degrees in microbiology, health education, allied medicine, gerontology, and medical technology. She is a former associate professor at OSU Medical School, teaching Clinical Laboratory Hematology and Clinical Laboratory Immunology; and conducting research.
