Your head of hair is made up of about 150,000 follicles. Around 80% to 90% are in the Anagen or growing phase at any one time. So itʼs normal to shedan average of 100 hairs per day. If, however, youʼre shedding more than 100hairs a day, or if you notice your hair getting thinner and lifeless, it could bedue to a number of factors from poor diet and stress, to age, hormonalchanges or medication, and you may wish to seek medical advice.

Good diet and supplementation with essential fatty acids, vitamins andminerals are important to help maintain healthy hair growth. Whatever weeat goes into our bloodstream. The Hair StructureHair is composed of strongstructural protein called keratin.This is the same kind of proteinthat makes up the nails and theouter layer of skin.

Each strand of hair consists ofthree layers;1. An innermost layer or

medulla which is only present in large thickhairs.

2. The middle layer is known as the cortex. The cortexprovides strength and isresponsible for both the colorand the texture of hair.

3. The outermost layer is knownas the cuticle. The cuticle isthin and colorless and servesas a protector of the cortex.

Human hair grows in a continuouscyclic pattern of growth and restknown as the “Hair Growth Cycle”.

Hair grows from the follicle, or root,underneath the skin. Between startingto grow and falling out years later,each hair passes through differentgrowth phases. Every hair is at adifferent growth phase of the cycle.

The Hair Growth Cycle

AnagenThe growing phase of hairdevelopment lasts two toseven years and determinesthe length of the hair. This is when the hair follicleis attached to the dermalpapilla. The longer this phaselasts, the longer your hair willgrow. As we get older, theAnagen phase gets shorter,so hair may become thinnerafter each cycle.

CatagenThis is the transitional stagethat lasts about 10 days. This is when the hair follicleshrinks and detaches fromthe dermal papilla, whichtakes us into our Telogenphase or resting stage.

MiniaturizationMiniaturization is the technical termgiven to the shortening of theAnagen phase of the hair growthcycle. For many people the length of the Anagen phase shortens everycycle resulting in slower hair growth,becoming thinner, shorter, morebrittle, or weak with each successiveprogression through the hair growth cycle.

Over time, the miniaturizationprocess leads to the appearance ofthin or lacklustre hair – vellus babyhair. In some cases, hair may stopgrowing altogether. Healthy hair ismaintained when the scalp receivesa sufficient blood supply thusachieving the ideal environment forhealthy hair growth. If the follicles donot receive the required nutrientsand oxygen rich blood from theblood stream, they shrink and vellusbaby hairs develop.

Miniaturization of hair follicles canbe recognised by receding hairlinesand male and female patternbaldness. Many treatments work byincreasing the duration of theAnagen phase or reducing theTelogen phase.

TelogenThis phase lasts about threemonths, and as the old hair isno longer attached to itsblood supply, it canʼt berepaired and can appear dulland lifeless. In this phase hairgrowth stops and the old hairdetaches from the hairfollicle. A new hair begins thegrowth phase, pushing theold hair out. Around 10-15per cent of hairs are in theTelogen phase - thatʼs whywe shed hairs all the time.

AgeIn our twenties and thirties we typicallyhave 615 hair follicles per squarecentimeter. The number falls to 485 bythe time we hit fifty and to 435 ateighty. Each hair also becomesthinner, therefore reducing hairvolume.

StressBelieve it or not, the cliché aboutstress leading to hair problems isactually true. In extreme stress, theadrenal gland goes into overdrive, andthe resultant increase in adrenalinesometimes leads to a consequent

increase in the production oftestosterone and dihydrotestosteronewhich, without enough estrogen tocounterbalance it, can lead to hair notbeing as healthy as it should be.

Types ofThinning Hairand Hair LossAlopeciaAlopecia7,8,9 is an autoimmune condition that causeshair to fall out from the scalp or other areas of thebody in small, round, smooth patches because theimmune system mistakenly attacks the hair follicles,inhibiting new hair growth. Alopecia carries aconsiderable disease burden and can destroy aperson's quality of life. About 2% of Americans, or 4.5million people, will have an episode of alopecia duringtheir lifetime14; however, the condition is morecommon in children and young adults than in anyother age group. Almost 66% of people with alopeciaare under age 30 and only 20% are older than age40.14 In addition, alopecia may be hereditary; about20% of those with alopecia have a family history ofthe condition.

There are three types of alopecia – alopecia areata,alopecia totalis, and alopecia universalis. Usuallyoccurring on the scalp, alopecia areata begins as asingle, smooth, circular patch about the size of aquarter. The patch grows back in about 3-6 monthswithout treatment and may even grow back as whitein color. Other times, two or three patches mayappear at one time, which should also grow back, but may be replaced by others.

Other causesHair loss, shedding or thinning canalso be attributed to 6,7:

* Anaemia

* Autoimmune diseases (lupus,Hashimoto's disease)

* Burns

* Infectious diseases (syphilis,ringworm, folliculitis)

* Thyroid diseases (hypothyroidism)

* Radiation therapy

* Ovarian or adrenal gland tumors

* Hormonal changes due tomenopause

* Excessive shampooing and/or blow-drying

* Traction hair loss due to tightly-pulled hair (ponytails, cornrows)

There are generally 2-3 reasons whyyour hair is shedding, there is neverjust one. What happened yesterdaywill not affect your hair today; itgenerally takes 3, 6 or 9 months to notice the effects.

Patterns of femalealopecia can varyconsiderably inappearance.Patterns that mayoccur include:

* Diffuse thinning ofhair over the entirescalp, often withmore noticeablethinning toward theback of the scalp

* Diffuse thinningover the entirescalp, with morenoticeable thinningtoward the front ofthe scalp involvingthe frontal hairline.

entering the Anagen phase.When this happens, more hairsremain in the Telogen phaseand fewer hair follicles areavailable to grow new hair inthe Anagen phase, resulting ina gradual thinning or sheddingof hair. This type of TEprogresses much more slowlyand lasts longer than TEcaused by an environmental"shock".

* Abbreviated Anagen, Catagen,and Telogen phases during thehair growth cycle, causing thinnedhair on the scalp and constantshedding of shorter, thinner hairfibers.

Triggers for TE in men and womeninclude16:

* Chronic illness

* Chronic stress

* Hormonal changes due to givingbirth (postpartum alopecia)

* Certain vaccinations

* Crash dieting/improper nutrition

* Physical trauma (car crash, fallingaccidents)

* Surgery

* Certain medications (anti-depressants, hyphenanti-coagulants, anti-inflammatory)

* Toxin exposure

The good news is that TE is reversibleand the hair usually grows back to itshealthy state once the trigger has beenidentified and removed.

Telogen effluvium is when morehairs remain in the Telogen phasetherefore, fewer hair follicles areavailable to grow new hair in theAnagen phase, resulting in a gradualthinning or shedding of hair.

Alopecia totalis is the complete loss ofhair from the scalp while alopeciauniversalis is the complete loss of hairfrom the entire body. In most cases,the hair grows back without treatment,but there are medications availablethat can help the immune systemcease attacking the hair follicles andallow new hair growth. The cause ofalopecia is currently unknown.14

Androgenetic alopeciaThis is an inherited sensitivity to theeffects of androgens (male hormones)on scalp hair follicles. However,women with hair loss due to this causeusually do not develop true baldnessin the patterns that occur in men—forexample, women rarely develop the“cue-ball” appearance often seen inmale-pattern androgenetic alopecia.10

Telogen effluviumA common type of hair thinning orshedding caused when a largepercentage of scalp hairs are shiftedinto the “shedding” phase. This meansthat more hairs are in the resting stagethan usual, slowing down the hairgrowth cycle and resulting in morethan typical hair loss.12-13

The causes of telogen effluvium (TE)may be hormonal, nutritional, drug-associated, or stress associated. The hair growth cycle needs to revertback to the Anagen phase before it willre-grow.

Thinning from TE is usually moresevere at the parting and rarely resultsin recession of the hairline. It isbelieved to develop by either:

* An environmental trigger that"shocks" the hair follicles into adormant state, causing excessiveshedding or thinning of the hair thatis usually noticeable within a fewmonths of the "shock". This type ofTE develops rapidly, but it can alsorecede just as quickly. The hairfollicles return to normal once thetrigger is removed and will begingrowing new hairs quickly. Typically,people with this type of TEexperience hair loss for less than sixmonths and regain their hair within ayear.

* Exposure to a persistent trigger thatcauses the hair follicles to enter theresting phase and remain there foran extended period instead of re-

Recently, someresearch6 wasconducted whichfocused on thepsychological effectsof thinning hair in1,000 women whohad perceived hairloss issues, and 1,000women who did nothave any hair lossissues. This researchshowed that thinninghair can stronglyaffect a womanʼsoverall confidenceand her sex life.According to the survey:

* 96% of women suffering fromthinning hair felt that their hair isconnected to their sense of beauty

* 86% envy women with thick hair

* 55% of women felt they arenegatively judged as a result of theirthinning hair and 45% felt self-conscious in presentations ormeetings

However, because it is in the anagenphase the hair will re-grow once theoffending factor is removed. The twomost common causes of anageneffluvium occur from chemotherapy15

and from radiation therapy.

TrichotillomaniaConsidered an impulse controldisorder, trichotillomania14 is anuncontrollable urge to repeatedly pluckor pull the hair out of the scalp or otherbody areas, leaving uneven, rough,round patches. People suffering fromtrichotillomania engage in thisbehaviour as a way to deal withfeelings of sadness, depression,anxiety, and poor self-esteem. About 4% of the US population hastrichotillomania with women four timesmore likely to suffer from the conditionthan men. Symptoms oftrichotillomania, usually begin inadolescence and the behaviour tendsto cease in about 12 months for mostpeople. For those that have persistenttrichotillomania, treatment with certainmedications combined withbehavioural therapy has shown someeffectiveness.17

Traction alopeciaThis is caused by damaging thedermal papilla and hair follicle byconstant pulling or tension over a longperiod and is often seen in womenwith tight braiding.

FolliculitisThis is an infected hair follicle which if not treated can lead to scarringalopecia. It occurs most commonly in men and it requires specializedtreatment i.e. referral to a Physician, a Dermatologist or Trichologist.

Involutional AlopeciaHair naturally thins a bit due to thenormal aging process as more hairfollicles enter the Telogen phase andthe residual hairs become shorter andless in number. This type of hair lossis known as involutional alopecia.

* 65% of women indicated thatnoticeable hair thinning inhibitsthem from participating in normalactivities, including swimming, orhaving their spouse or significantother caress or touch their hair

When Should YouConsult YourPhysician?Hair loss and shedding can appearsuddenly or gradually over time andpresent itself in many different ways,depending on its underlying cause.Hair loss may not only affect thescalp, but the whole body, too.Depending on the type, the hair losscan be temporary and the hair willgrow back or the loss may bepermanent.

Individuals should consult aphysician if they are experiencing thefollowing symptoms with their hairloss:16

* Losing hair in an unusual pattern

* Losing hair at an early age (in theteens or twenties)

* Losing hair suddenly, not graduallyover time

* Losing hair in large quantities aftercombing or brushing

* Experiencing male patternbaldness (androgenetic alopecia)as a female
