DE SLEUTEL De Sleutel is a network for drug treatment and prevention. It is an institution embedded in a non-profit organisation of the Brothers of Charity. It is aimed at care, prevention, employment and research in the field of drugs and drug addiction problems and dependency.

The Sleutel institution consists of a central department, ambulatory facilities and social workplaces, each of them with their own specific characteristics according to the region and in cooperation with other partners active in the drug treatment area in that region. Our multi-modal initiatives are aimed at all aspects of drug abuse and addiction. Our modular approach tackles the complex problem of drug use at various levels, from initial contact and ambulatory crisis reception to long-term rehabilitation and social re-integration. All modules are characterised by specific client profiles and needs. These treatment facilities are spread throughout Flanders and Brussels, linked by central management and a common mission. That mission is to help drug addicts and their environment optimize self-realization and re-integration through quality care and the development of social skills. Brussel negeert zijn zwakste bewoners

Drugshulpverlening in de hoofdstad schiet tekort Reportage.

Het dagcentrum van De Sleutel in Brussel luidt de a larmbel. Het wordt overspoeld door hulpvragen en ka n het werk niet meer aan. ,,De overheid wil geen overlast, maar ze doet er niets aan omdat er g een stemmen mee te halen zijn.'' Een rijtjeshuis in de arme Brusselse gemeente Sint-Joost Ten Node. Een smalle gang, kleine sombere kamertjes. Achteraan op het terras drinken twee West-Vlaamse jonge vrouwen koffie. Verslaafd, dakloos en aangespoeld in Brussel. Een allochtone jongen heeft net een intakegesprek gehad. In de wachtkamer zitten nog twee mannen.

Filippo Regni, afdelingshoofd van het dagcentrum van het drugshulpverleningscentrum De Sleutel in Brussel, luidt de alarmbel. ,,Het water staat ons aan de lippen. We zijn met drieën: een medewerker van een sociale dienst, een psycholoog en ik als arts. Vorig jaar hadden we meer dan 2.000 consultaties. In 2002 hadden we 750 individuele psychologische begeleidingen, vorig jaar waren dat er 1.824. Het aantal doktersconsulten verviervoudigde, het aantal methadonverstrekkingen ook. Bovendien wordt de problematiek van de mensen die hier komen aankloppen, almaar complexer. Ze hebben niet alleen een drugprobleem, maar zijn ook dakloos en/of hebben psychiatrische problemen, zitten in de prostitutie.'' Regni: ,,We werkten vorig jaar met een caseload van 184 procent. Dat is alleen mogelijk dankzij de vrijwilligers. Maar we kunnen niet meer de kwaliteit leveren die we zouden moeten leveren. We kunnen patiënten niet genoeg opvolgen. De partnergroep hebben we moeten stopzetten. Ons werk in de gevangenis hebben we bijna volledig moeten terugschroeven. We moeten met wachtlijsten werken, wat voor deze mensen nefast is. Wie vandaag belt, wil vandaag gezien worden, niet over een paar maanden. Dan is hun motivatie om iets aan hun verslaving te doen, weer weg.'' Het dagcentrum startte in 1995. Sinds 1999 staat Regni, arts met Italiaanse roots, in voor de dagelijkse leiding. ,,We krijgen ook almaar meer te maken met agressie. Drie weken geleden zat ik met een cliënt boven. Plotseling haalde hij een pistool uit zijn zak en zette het tegen mijn hoofd. Hij had problemen met zijn ziekenbond en werkloosheidsuitkering en zei dat het onze schuld was.'' Regni klaagt vooral het versnipperde beleid rond hulpverlening in Brussel aan, het totale gebrek aan coördinatie en de afwezigheid van interesse van politici voor de problemen van een grootstad. Daardoor heeft hij het gevoel dat hij dweilt met de kraan open. ,,Vlaanderen investeert wel in cultuur voor Vlamingen in Brussel en in allerlei communautaire rondes. Maar als er iets gebeurt zoals Joe Van Holsbeek, is het enige antwoord repressie'', zegt Rita Brauwers, projectmedewerkster van De Sleutel. ,,De overheid wil geen overlast, wel een stad zonder bedelaars, daklozen, drugs, prostituees. Maar ze doet er niets aan. Iedereen werkt langs elkaar heen.'' ,,Vrijdag kwam hier een dakloze aan. Hij zat in een opvangcentrum, maar hij gebruikt drugs en had problemen geschopt en was op straat gezet. Nu wilde hij een afkickprogramma. Dat opvangcentrum had onze hulp eerder kunnen inroepen, want nu moet hij afkicken terwijl hij wel op straat zit. We willen wel dweilen, maar wil ook eens iemand naar de kraan kijken.''

De Sleutel pleit in Brussel voor een wijkgezondheidscentrum. Eén loketfunctie voor welzijn, om de versnippering van de hulpverlening tegen te gaan. ,,Daar zijn niet meer middelen voor nodig, want het betekent ook efficiënter werken'', zegt Brauwers. Ze probeerde het probleem aan te kaarten bij de bevoegde politici. ,,Maar je moet wel eerst een cursus bestuurskunde studeren voor je weet bij wie je terecht moet.'' Brauwers ging al aan tafel zitten met Vlaams minister van Welzijn Inge Vervotte en de stad Brussel. ,,Ze delen allemaal onze analyse, hopelijk komen er ook snel maatregelen. Een goed welzijns- en gezondheidsbeleid is ook een goed veiligheidsbeleid.'' Het dagcentrum van De Sleutel is bedoeld voor Nederlandstaligen in Brussel, maar het publiek is heel divers. Ook Franstaligen, Oost-Europeanen, Marokkanen, Iraniërs, Tunesiërs, Iran, ook heel wat illegalen, komen hulp zoeken. ,,We zijn heel laagdrempelig en hebben vier groepen cliënten. Ten eerste de drugsverslaafden die op straat leven en bijna nooit eerder contact hadden met drughulpverlening. Die vangen we medisch en sociaal op en we proberen ze inzicht te geven en te motiveren om in therapie te gaan. Ten tweede diegenen die al gemotiveerd zijn om in therapie te komen. Ten derde mensen aan wie we hervalpreventie geven. Tenslotte een groep bij wie we een diagnose moeten stellen en die we proberen door te verwijzen.'' ,,Druggebruik is erg cultureel gebonden. Marokkanen gebruiken vooral cannabis, alcohol en rohypnol. Italianen, Spanjaarden en Grieken vooral heroïne en cocaïne. Vlamingen vooral amfetamines en cocaïne. Franstaligen heroïne en cocaïne. Oost-Europeanen, Wit-Russen, Oezbeken, zijn de moeilijksten. Ze komen eigenlijk alleen een vervangmiddel halen en begrijpen niet goed wat behandeling inhoudt.'' ,,De allerzwaksten bereiken we niet eens'', zegt Regni. Daarom pleit De Sleutel voor middelen om mobiele drughulpverlening in te zetten. ,,We willen aan de deur gaan kloppen, mensen op straat aanspreken. We vragen ook personeel om aan de mensen die in therapie zijn, een dagbesteding te kunnen aanbieden. En we willen opnieuw in de gevangenis gaan werken.'' Volgens Regni is het beleid van de overheid alleen een beheersingsbeleid. En proberen ze de zaken aan te pakken door toegevingen te doen zoals in Luik, waar men gratis heroïne wil verstrekken die verslaafden dan in een veilige omgevingen kunnen gebruiken. ,,Dat is eigenlijk de boodschap geven dat je die mensen opgeeft. Ik geef toe dat ook hier afkicken niet altijd de grootste prioriteit is. Soms zijn de medische problemen of huisvesting dringender. Maar het maakt altijd wel deel uit van het geheel.
