The pulley type variant described by Kanatli et al.8 has a double-pulley effect providing counter-traction to the proximal portion of the tendon, increasing anterolateral traction on the superior

labral complex and leading to more labral pathology.

The exposure of the bicipital groove involves retraction of the deltoid and pectoralis muscles superolaterally and retraction of the short head of the biceps brachii and coracobrachialis

muscles medially.
