What is happening?The UK Government’s aim is for more people to have another income, on top of the state pension, when they come to retire. The basic state pension is intended to be a foundation and you may want more.

Employers are now required to enrol their workers automatically into a pension scheme to make it easier for people to start saving. You will therefore be automatically enrolled into the NHS Pension Scheme from your date of commencement with us.

What does this mean for you?Our pension scheme is provided by Scottish Public Pensions Agency. This scheme is a qualifying pension scheme, which means it meets or exceeds the government’s new standards. To opt out, you can obtain the relevant opt out form from SPPA’s website http://www.sppa.gov.uk/Documents/NHS/NHS%20Useful%20Resources/NHS%20Forms/Opting%20out/OPTOUT%20NHS%202013%20V.1.pdf Once you have completed it, send it to the Payrolll Dept (see payslip for details).

If your completed opt out form is received within 1 month of your start date you will be removed from the pension scheme with effect from your start date. Any payments you have already made will be refunded and you will not have become an active member of the scheme on this occasion.

If you want to stop making payments at any time after 1 month of commencing employment you can do so by completing the opt out form. Repayment of pension contributions and the calculation of preserved benefits will be arranged by SPPA. Further information on refunds is available on www.sppa.gov.uk - see NHS factsheet.

If you encounter any difficulties in accessing this form, please contact either your Line Manager in the first instance or a member of the HR Department.

HMRC ProtectionIf you are one of the relatively small number of people who applied for, obtained and still hold a Fixed Protection certificate from HM Revenue and Customs (which protects the total value of all pension benefits you can have at £1.8 million without triggering an excess benefits tax charge) or an Enhanced Protection certificate then you may lose the Fixed or Enhanced Protection if you opt to remain in the scheme. If you currently hold HMRC Fixed or Enhanced Protection you may wish to consider the consequences of further accrual of benefits as this may invalidate your protection. Further information is available from HMRC

A regular reminder Anyone who opts out will be automatically enrolled back into a pension scheme at a later date (usually every three years). This is because your circumstances may have changed and it may be the right time for you to start saving. We will contact you when this happens, and you can opt out if it’s still not right for you.

A commitment from usIf you are under 75, work or usually work in the UK, and earn over £10,000 a year (the amount set by the government for this):

- we must by law continue to maintain your membership of a scheme that meets certain government standards; and

- if your membership of such a scheme ends (and it is not because of something you do or fail to do), we must by law put you into another scheme that meets government standards straightaway.

Questions you may have:

Will the amounts paid into my pension change?Yes, the amounts will automatically increase or decrease accordingly if your earnings (exclusive of overtime payments) go up or down.

If I opt out or stop making payments, can I rejoin a workplace pension scheme at a later date?Yes, you can rejoin a workplace pension scheme. To do so, contact your payroll contact (see payslip for details) in writing by sending a letter, which has to be signed by you. Or, if sending it

2. JOB PURPOSE

To contribute and assist in the management and co-ordination of a range of both complex and routine department investigations to aid in the diagnosis and treatment of cardiac and respiratory diseases. Assist with the provisions of either the echocardiography or pacemaker implant and follow-up services.

To be pro–active in the provision and development of training programmes for both trained technical and assistant technical officers.

6. KEY RESULT AREAS

Clinical Work as an independent practitioner in the supervision of technician led exercise testing

(ETT). This is done partly by adhering to departmental policies but a degree of analytical skills are required to manage patients and available results pre and during test (including interpretation of ECG, BP and patient status).

Work as an independent practitioner in the supervision of Physiologist led Tilt Testing. A high degree of analytical skills are required to manage patients and interpretation of ECG, BP and continual patient status.

Provide written report on outcome of test Analyse 24-hour ECG recordings and produce a technical report. This requires a high

degree of ECG interpretation and knowledge. Work as an independent practitioner performing a range of respiratory physiological

measurements. Providing a report on testing procedures using specialist software packages relating to pulmonary function testing equipment. This may include, Spirometry, lung volumes, CO transfers. Assessment of bronchodilator response. Demonstration of inhaler technique. Postural spirometry. Skin testing for allergies. Training in this field may be required.

Become competent in the specialist fields of pacemaker implant and follow-up +/- echocardiography under the supervision of the Specialist Clinical Physiologist responsible for this field.

This includes either

Specialist pacemaker skills Provide assistance in theatre with the measurements of thresholds and intrinsic rhythm

parameters, in order to ensure adequate pacemaker function and longevity, particular to each individual patient

Monitor patient and parameters during theatre and respond to any emergency situations which arise

An extensive knowledge of all device algorithms, specific to each manufacturer, is necessary to aid in the decision making of the most appropriate device for implantation to suit the patient’s lifestyle and medical condition. Occasional consultation with Consultant Cardiologist may be required for a more specialist decision.

Participate in technician led follow-up of pacemaker patients. Reprogramming parameters (assessing minimum thresholds, rates, amplitudes etc) as required to suit patients lifestyle and medical condition. This is mostly technicians’ decision, although occasional consultation is required with consultant cardiologists/senior clinical physiologists.

A significant knowledge and experience in this specialised field is required during troubleshooting and tailoring pacemaker parameters to individual needs. This requires interpreting data received with extensive knowledge of device algorithms and clinical requirements of patient

Communicate and discuss directly with Consultant Cardiologists any problems arising during follow-up clinics. Alert Cardiologists to individual cases that may need their intervention or advice.

Teach and training senior/basic grade cardiac physiologists in competency of follow-up and implant to assist them in obtaining BPEG/NASPE accreditation

Provide reassurance and support to patients during implant and at subsequent follow-up clinics

Assist in the development and implementation of protocols and procedures for this specialised area

conditions. Patients may be in/out patients or A/E referrals. Assist in the development and implementation of protocols and procedures for this

specialised area Produce diagnostic reports, which are returned to requesting consultant to become part

of the patient’s case history. Patients are directly managed according to the reports generated. This requires significant clinical and ultrasound knowledge to analyse and interpret complex data to produce an accurate report. Occasional consultation with fellow sonographers/consultant cardiologists is required.

The post holder would communicate and discuss directly with senior medical staff any concerns over abnormalities detected. Alerting cardiologist to individual cases that may need their intervention or advice.

Participate in the quality assurance program, ensuring consistent accurate reports are generated and high quality scan are performed

Train basic grade cardiac physiologist and medical staff (SHOIII to Spr) in both competency of scanning and clinical knowledge to assist them in obtaining accreditation to the British Society of Echocardiography.

Assist consultant with trans-oesophageal examinations by optimising images, recording and measuring data. Preparing and counselling patients before and after examination and monitoring them throughout the procedure.

Perform high quality echocardiograms during the use of contrast injections, ensuring optimal imaging is maintained throughout. Discuss and give opinion on result.

Prioritise own workload according to clinical referrals and other factors. May prioritise the work of more junior staff.

Other clinical duties

Train and provide guidance in the use of departmental protocols to clinical physiologists and attending medical staff, ensuring best practice as recommended by professional bodies.

Initiate life support in case of emergency and assist when cardiac arrest team arrives. Provide a significant input into the training of junior clinical physiologists in the performing

and interpretation of all basic investigations. Train groups of nursing staff to perform electrocardiograms Perform all routine investigations Liaise with ward/porter staff to book patient list.

8. ASSIGNMENT AND REVIEW OF WORK

-Assignment of work is generated by both Head of Department and directly from medical staff.The post holder co-ordinates and prioritises investigations, expediting urgent reports directly to the Cardiologist, Respiratory Consultants and Medical Staff.

-The post holder will be an autonomous practitioner taking responsibility for his or her workload on a daily basis.-Quality assurance checks are performed on echocardiograms to ensure consistent high quality recordings are obtained and inter-observer variability is kept to a minimum with these subjective reports. Training for accreditation to British Society of Echocardiography is part of the training and review within this field.-Technician led pacemaker clinics may be performed with the Chief Clinical Physiologist to ensure standards and consistency are maintained. Training for the BPEG or NASPE examinations form part of the training in this field.-Pacemaker troubleshooting workshops and informal sessions ensure staff are competent and developing within this field-PDP’s and annual appraisal ensures that personal targets are achieved and support is adequate-The post holder would be expected to assist and participate in ECG interpretation workshops

11. COMMUNICATIONS AND RELATIONSHIPSPatientsExcellent communication skills are continually required to explain investigations which may require a significant amount of patient co-operation. This involves clear communication of what is involved, using excellent motivational skills to push the patient to their maximum capacity. These skills will vary depending on the specific investigation and the patients being treated from newborn to elderly, including patients with varying degrees of physical and mental disabilities. A sensitive approach must be adopted when dealing with angry and upset patients and carers.Patients and carers are trained in the correct use of ambulatory equipment.Provide reassurance for test and results and try to alleviate patients concerns.

3. DimensionsThe Cardio-Respiratory departments cover Victoria Hospital and Queen Margaret Hospital within NHS Fife. This is on a rotational basis. - Work collaboratively with Clinical Physiologist (Head of echo) to develop protocols and procedures for this specialised area

- Independently produce diagnostic reports, which are returned to requesting consultant to become part of the patient’s case history. Patients are directly managed according to the reports generated. This requires significant clinical knowledge and a vast amount of ultrasound knowledge to analyse and interpret complex data to produce an accurate report. Consultation with fellow sonographers/consultant cardiologists is only required in very difficult/unusual cases.

- The post holder would communicate and discuss directly with senior medical staff any concerns over abnormalities detected. Alerting cardiologist to individual cases that may need their intervention or advice.

- Perform high quality echocardiograms producing ejection fractions for national/international phase II, III and IV clinical trials. Involved in approximately 2-3 clinical trials/year.

- Participate in the quality assurance program, ensuring consistent accurate reports are generated and high quality scan are performed

- Significant in-put into teaching and training senior/basic grade cardiac physiologist and medical staff (SHOIII to Spr) in both competency of scanning and clinical knowledge to assist them in obtaining accreditation to the British Society of Echocardiography.

A significant knowledge and experience in this specialised field is required during troubleshooting and tailoring pacemaker parameters to individual needs. This requires interpreting data received with extensive knowledge of device algorithms and clinical requirements of patient.

Assist in co-ordination of the scheduled pacemaker implant session with theatre, X-ray, wards and Consultants. In addition organising emergency sessions which may be out with normal theatre areas. Ensuring the session is managed effectively.

To provide assistance in theatre with the measurement of thresholds and intrinsic rhythm parameters, in order to ensure adequate pacemaker function and longevity particular to each individual patients needs.

Monitor patients and parameters during theatre. Assist in managing Implantable Cardiovertor Defibrillator (ICD) interrogation in

emergency/clinic situation when device has or suspected to have given in-appropriate therapy/shocks. This requires an in-depth knowledge of patient’s clinical status, drug history and required rhythm. An extensive knowledge of terminology and diagnostics for each individual company’s devices is necessary.

Discuss individual parameters with Clinical Physiologist (Head of ICD and pacing) and or Consultant Cardiologists, alerting Cardiologists to any cases that may need their intervention or advice.

Reassure and council patients with implanted ICD’s to promote confidence and trust in the changes made to their therapy.

Teaching and training senior/basic grade Physiologists in competency of follow-up and implant to assist them in obtaining accredited qualifications (HRUK/NASPE). Assist in organising off site and in-house training for all staff with individual pacemaker companies.

Continuous professional development – undertaking responsibility for assessing, implementing and adapting to new clinical techniques and technologies.

Prioritise own workload. May prioritise the work of more junior staff

Other clinical duties

- Implement, develop and propose changes to departmental protocols, to ensure patients are properly managed, ensuring best practice as recommended by professional bodies

- Train and provide guidance in the use of these protocols to clinical physiologists and medical staff working in cardiology

- Work as an independent practitioner in the supervision of technician led exercise testing (ETT) This is done partly by adhering to departmental policies but a significant degree of analytical skills are required to manage patients and results pre and during test (including interpretation of ECG, BP and patient status). Provide written report on outcome of test.

- Initiate life support in case of emergency and assist when cardiac arrest team arrives. - Provide a significant input into the training of junior clinical physiologists in the performing

and interpretation of all basic investigations- Train groups of nursing staff to perform electrocardiograms and occasionally other basic

investigations (e.g. 24 hour ECG/BP application)- Analyse 24-hour ECG recordings and produce a technical report. This requires a high

7a. EQUIPMENT AND MACHINERY

The post-holder assists with the management of all Cardiac echo machine and all transducers. This equipment is both highly complex and expensive (approx value £100,000). Responsible for any safety issues which arise.

The post holder is also responsible for the safe and proper use of a wide variety of technical equipment within the department from routine ECG machines to more expensive and complex exercise tolerance treadmill and consul and 24 hour analyser system. As a senior member of staff they will investigate and analyse any faults and take action to rectify as appropriate.
