OSTEOGENICS 2012 BONE GRAFTING SYMPOSIUM WORKSHOPS

The final shape and consistency of bone in implant recipient sites can greatly enhance the prosthesis or it can limit the functional and

esthetic result to a significant extent. This hands-on workshop, designed for advanced clinicians, will present a ridge sculpting

technique that differs from conventional particulate grafting techniques in the use multiple tenting screws that ultimately define the

final ridge contours to match the prescribed restorative placement positions. This predictable technique uses wide-head tenting

screws, autogenous bone chips sourced from the ramus or other donor sites, acellular collagen membranes, and membrane-fixation

tacks or screws. Supporting topics include incision-design, autogenous graft harvesting, and flap design for obtaining primary closure.

This ridge grafting technique has been shown to develop mature bone that does not tend to resorb, even over extended periods of

time if implants are not placed within a six-month timeframe. Resulting bone development creates an implant foundation that has a

dense nature, and sites that have been exposed up to three years later demonstrate bone levels that are still level with the screw

heads as positioned at the time of the original surgery. Hands-on exercises will utilize anatomic models, surgical instruments, tenting

screws, membrane-fixation screws, and membranes.

Socket Augmentation Techniques for Minimizing Buccal Bone Resorption and Improving Implant Esthetics Limited Attendance

Preventing ridge resorption after tooth extraction remains a major challenge among clinicians. In this hands-on workshop

designed for intermediate and advanced clinicians, different techniques used for managing extraction sockets will be thoroughly

discussed. Participants will understand extraction socket classification and decision-making for selecting certain techniques.

Recent advancements in using newly developed combination allografts and PTFE membranes to prevent extraction socket

resorption will be presented. Choosing a socket grafting technique to increase the zone of keratinized mucosa in order to

facilitate future implant placement and to enhance esthetics will be illustrated. A hands-on exercise using pig mandibles will

focus on both socket augmentation and ridge augmentation techniques using barrier membranes and particulate bone graft

materials. PTFE membranes, collagen membranes, titanium-reinforced PTFE membranes, PTFE suture, and supporting

materials and instruments will be used during the workshop.

Vertical and Horizontal Ridge Augmentation Techniques using Barrier Membranes and Particulate Graft Materials Limited Attendance

This hands-on workshop on ridge augmentation techniques is designed for the advanced clinician. A predictable

technique for vertical ridge augmentation using titanium-reinforced barrier membranes with membrane fixation will be

presented along with a technique for horizontal ridge augmentation using collagen membranes in a “sausage” technique

published by the author. Supporting topics will include flap design and management, membrane fixation, flap

mobilization, and tension-free closure with a double-layer suturing technique. A predictable approach for implant

dehiscence treatment with GBR and membrane fixation with a periosteal suturing technique will also be presented.

Hands-on components using a pig mandible will include the use of titanium-reinforced barrier membranes, highly

adaptable porcine collagen membranes, membrane fixation system, PTFE sutures, and various other biomaterials and

instruments needed to practice many of the techniques taught.

STEPHEN CALDWELL DDSDr. Stephen Caldwell received his DDS at the University of Texas Dental Branch in Houston, followed by a residency in Periodontal Surgery at the University of Texas Health Science Center. Dr. Caldwell completed a Preceptorship in Implant Surgery at the U T Health Science Center San Antonio as well as the Implant Surgery and Prosthetic Program at the Misch International Implant Institute, where he now serves on the Surgical Faculty. Dr. Caldwell maintains a private practice specializing in Periodontics and Implant Dentistry in El Paso, Texas. He also serves as an Adjunct Assistant Clinical Professor in the department of Periodontics and Implant Surgery at the Kornberg School of Dentistry at Temple University in Philadelphia. Dr. Caldwell is a Diplomate of both the American Board of Oral Implantology /Implant Dentistry and the International Congress of Oral Implantology. Dr. Caldwell has received multiple awards related to his contributions to dentistry, including being named the “Texas Dentist of the Year” in 2010 by the Texas AGD.

SASCHA JOVANOVIC DDS, MSDr. Sascha Jovanovic received his DDS from the University of Amsterdam and both his certificate in periodontics and MS in Oral Biology from UCLA. He formally trained in Implant Dentistry at Loma Linda University and in Prosthodontics at the University of Aachen in Germany. Dr. Jovanovic is currently Lecturer and Course Director in Implant Dentistry at UCLA Continuing Dental Education where he restricts his patient care and research to dental implant therapy and bone and soft tissue reconstruction. He has published 63 articles and book chapters, as well as a textbook titled Color Atlas of Implantology. Dr. Jovanovic serves on several editorial boards for scientific journals and is Editor-in-Chief of Implant Tribune. He is the past-president of the European Association for Osseointegration. Dr. Jovanovic is Chairman and Founder of the gIDE Institute, an online dental education company.

BRADLEY McALLISTER, DDS, PHDDr. Bradley McAllister obtained his DDS at the University of Washington, and then completed a residency in periodontics at the University of Texas Health Science Center, where he also received his PhD involving research centered on techniques to improve surgical wound healing. Dr. McAllister maintains a private practice specializing in periodontics and implant dentistry in the Portland Metropolitan area. Dr. McAllister is a part-time faculty member for the Department of Periodontology at the Oregon Health Sciences University. He has published over twenty manuscripts in professional journals and has written three book chapters, and currently serves as a reviewer for the Journal of Periodontology and the International Journal of Periodontics and Restorative Dentistry. Dr. McAllister was the 2006 recipient of the American Academy of Periodontology’s Earl Robinson Regeneration Award.

ALVARO ORDOÑEZ DDSDr. Alvaro Ordoñez completed his DDS at the Pontificia Universidad Javeriana School of Dental Medicine in Bogotá, Colombia, and then completed general practice residency training at Blas de Lezo Hospital in Cartagena, Colombia. He completed a research fellowship in TMJ, dental occlusion and orofacial pain at Tufts University in Boston, MA. Dr. Ordoñez maintains a private practice specializing in TMJ disorders, orofacial pain and dental implantology in Coral Gables, Florida. He is a Clinical Assistant Professor at the University of Florida College of Dentistry. Dr. Ordoñez is a Fellow, Master and Diplomate of the International Congress of Oral Implantologists (ICOI), and also serves as the Vice President for Latin America with the ICOI.

MICHAEL PIKOS DDSDr. Michael Pikos graduated with honors from The Ohio State University College of Dentistry. Dr. Pikos completed his residency in Oral & Maxillofacial Surgery at the University of Pittsburgh, Montefiore Hospital. He is a Diplomate of the American Board of Oral and Maxillofacial Surgery, the American Board of Oral Implantology, and the International Congress of Oral Implantologists. After he graduated from the Fellowship Program in Implant Dentistry at Loma Linda University, he was appointed Assistant Professor the following year. Dr. Urban teaches implant dentistry in the graduate program at Loma Linda, and he holds an active license in the state of California. He maintains a private practice in Budapest, Hungary. Dr. Urban has published scientific articles and text book chapters on bone regeneration and soft tissue reconstructive surgery around dental implants. HOM-LAY WANG DDS, MSD, PHDDr. Hom-Lay Wang received his DDS from Taipei Medical College in Taiwan, a DDS and MSD from Case Western Reserve University in Cleveland, Ohio and his PhD in philosophy from Hiroshima University, Japan. Dr. Wang is an Endowed Collegiate Professor of Periodontics, Professor and Director of Graduate Periodontics at the University of Michigan School of Dentistry. He is the author or co-author of 20 book chapters/invited reviews and more than 250 scientific articles. Dr. Wang is a Diplomate and Former Director and Co-Chair of the American Board of Periodontology and Former President of Midwest Society of Periodontology. He has served as an Editor-in-Chief, Associate Editor, Assistant Editor or an Editorial Board Member for over 15 different journals, including The International Journal of Oral & Maxillofacial Implants, Journal of Periodontology, Clinical Oral Implants Research, Journal of Clinical Periodontology, and many others. Dr. Wang’s research focuses on tissue/bone regeneration, advanced bone grafting, sinus augmentation, implant esthetics, as well as oral plastic surgery. STEPHEN CALDWELL DDSDr. Stephen Caldwell received his DDS at the University of Texas Dental Branch in Houston, followed by a residency in Periodontal Surgery at the University of Texas Health Science Center. Dr. Caldwell completed a Preceptorship in Implant Surgery at the U T Health Science Center San Antonio as well as the Implant Surgery and Prosthetic Program at the Misch International Implant Institute, where he now serves on the Surgical Faculty. Dr. Caldwell maintains a private practice specializing in Periodontics and Implant Dentistry in El Paso, Texas. He also serves as an Adjunct Assistant Clinical Professor in the department of Periodontics and Implant Surgery at the Kornberg School of Dentistry at Temple University in Philadelphia. Dr. Caldwell is a Diplomate of both the American Board of Oral Implantology /Implant Dentistry and the International Congress of Oral Implantology. Dr. Caldwell has received multiple awards related to his contributions to dentistry, including being named the “Texas Dentist of the Year” in 2010 by the Texas AGD.

SASCHA JOVANOVIC DDS, MSDr. Sascha Jovanovic received his DDS from the University of Amsterdam and both his certificate in periodontics and MS in Oral Biology from UCLA. He formally trained in Implant Dentistry at Loma Linda University and in Prosthodontics at the University of Aachen in Germany. Dr. Jovanovic is currently Lecturer and Course Director in Implant Dentistry at UCLA Continuing Dental Education where he restricts his patient care and research to dental implant therapy and bone and soft tissue reconstruction. He has published 63 articles and book chapters, as well as a textbook titled Color Atlas of Implantology. Dr. Jovanovic serves on several editorial boards for scientific journals and is Editor-in-Chief of Implant Tribune. He is the past-president of the European Association for Osseointegration. Dr. Jovanovic is Chairman and Founder of the gIDE Institute, an online dental education company.

BRADLEY McALLISTER, DDS, PHDDr. Bradley McAllister obtained his DDS at the University of Washington, and then completed a residency in periodontics at the University of Texas Health Science Center, where he also received his PhD involving research centered on techniques to improve surgical wound healing. Dr. McAllister maintains a private practice specializing in periodontics and implant dentistry in the Portland Metropolitan area. Dr. McAllister is a part-time faculty member for the Department of Periodontology at the Oregon Health Sciences University. He has published over twenty manuscripts in professional journals and has written three book chapters, and currently serves as a reviewer for the Journal of Periodontology and the International Journal of Periodontics and Restorative Dentistry. Dr. McAllister was the 2006 recipient of the American Academy of Periodontology’s Earl Robinson Regeneration Award.

ALVARO ORDOÑEZ DDSDr. Alvaro Ordoñez completed his DDS at the Pontificia Universidad Javeriana School of Dental Medicine in Bogotá, Colombia, and then completed general practice residency training at Blas de Lezo Hospital in Cartagena, Colombia. He completed a research fellowship in TMJ, dental occlusion and orofacial pain at Tufts University in Boston, MA. Dr. Ordoñez maintains a private practice specializing in TMJ disorders, orofacial pain and dental implantology in Coral Gables, Florida. He is a Clinical Assistant Professor at the University of Florida College of Dentistry. Dr. Ordoñez is a Fellow, Master and Diplomate of the International Congress of Oral Implantologists (ICOI), and also serves as the Vice President for Latin America with the ICOI.

MICHAEL PIKOS DDSDr. Michael Pikos graduated with honors from The Ohio State University College of Dentistry. Dr. Pikos completed his residency in Oral & Maxillofacial Surgery at the University of Pittsburgh, Montefiore Hospital. He is a Diplomate of the American Board of Oral and Maxillofacial Surgery, the American Board of Oral Implantology, and the International Congress of Oral Implantologists. Dr. Pikos is an Adjunct Assistant Professor, Department of Oral & Maxillofacial Surgery at the following institutions: The Ohio State University College of Dentistry, the University of Miami School of Medicine, and Nova Southeastern University College of Dental Medicine. He is also a Courtesy Clinical Associate Professor for both the Department of Periodontology and Department of Prosthodontics at the University of Florida College of Dentistry. Dr. Pikos is an active member and frequent lecturer for the American Association of Oral and Maxillofacial Surgeons, the American Academy of Periodontics, the Academy of Osseointegration, the American Academy of Implant Dentistry and the International Congress of Oral Implantologists. Dr. Pikos is founder of the Pikos Implant Institute. Since 1990, he has been teaching advanced bone and soft tissue grafting courses with alumni that now number more than 2500 from all 50 states and 32 countries. Dr. Pikos maintains a private practice which is limited exclusively to implant surgery in Palm Harbor, Florida.

ISTVAN URBAN DMD, MDDr. Istvan Urban received his DMD degree and subsequently his MD degree from Semmelweis University School of Medicine and Dentistry. He completed a residency program in oral surgery at St. Istvan Hospital in Budapest, Hungary. He completed his internship program in Periodontics at UCLA. After he graduated from the Fellowship Program in Implant Dentistry at Loma Linda University, he was appointed Assistant Professor the following year. Dr. Urban teaches implant dentistry in the graduate program at Loma Linda, and he holds an active license in the state of California. He maintains a private practice in Budapest, Hungary. Dr. Urban has published scientific articles and text book chapters on bone regeneration and soft tissue reconstructive surgery around dental implants. Dr. Urban is an invited speaker internationally in the field of Implant Dentistry.

HOM-LAY WANG DDS, MSD, PHDDr. Hom-Lay Wang received his DDS from Taipei Medical College in Taiwan, a DDS and MSD from Case Western Reserve University in Cleveland, Ohio and his PhD in philosophy from Hiroshima University, Japan. Dr. Wang is an Endowed Collegiate Professor of Periodontics, Professor and Director of Graduate Periodontics at the University of Michigan School of Dentistry. He is the author or co-author of 20 book chapters/invited reviews and more than 250 scientific articles. Dr. Wang is a Diplomate and Former Director and Co-Chair of the American Board of Periodontology and Former President of Midwest Society of Periodontology. He has served as an Editor-in-Chief, Associate Editor, Assistant Editor or an Editorial Board Member for over 15 different journals, including The International Journal of Oral & Maxillofacial Implants, Journal of Periodontology, Clinical Oral Implants Research, Journal of Clinical Periodontology, and many others. Dr. Wang’s research focuses on tissue/bone regeneration, advanced bone grafting, sinus augmentation, implant esthetics, as well as oral plastic surgery. Because of his expertise in regeneration, he is the author of an AAP position paper entitled “Periodontal Regeneration”.

THOMAS WILSON, JR DDSDr. Thomas Wilson, Jr. received his DDS degree from the University of Tennessee School of Dentistry and his certificate in periodontics from the University of Washington Graduate School of Dentistry. Dr. Wilson maintains a full-time private practice limited to periodontics in Dallas, Texas. Dr. Wilson is a Diplomate of the American Board of Periodontology. He serves as a Clinical Associate Professor at the University of Texas at San Antonio Dental School. Dr. Wilson has lectured extensively on a national and international level and is the author of a number of professional publications. He is also author of three textbooks, including Dental Maintenance for Patients with Periodontal Diseases, ITI Dental Implants: Planning, Placement, Restoration, and Maintenance, and Periodontal Regeneration Enhanced, and an editor and contributor to the books Advances in Periodontics and Fundamentals of Periodontics. In addition, Dr. Wilson is formally Editor-in-Chief of Quintessence International as well as Associate Editor of the Journal of Periodontology and serves as a reviewer for a number of professional journals.

The final shape and consistency of bone in implant recipient sites can greatly enhance the prosthesis or it can limit the functional and

esthetic result to a significant extent. This hands-on workshop, designed for advanced clinicians, will present a ridge sculpting

technique that differs from conventional particulate grafting techniques in the use multiple tenting screws that ultimately define the

final ridge contours to match the prescribed restorative placement positions. This predictable technique uses wide-head tenting

screws, autogenous bone chips sourced from the ramus or other donor sites, acellular collagen membranes, and membrane-fixation

tacks or screws. Supporting topics include incision-design, autogenous graft harvesting, and flap design for obtaining primary closure.

This ridge grafting technique has been shown to develop mature bone that does not tend to resorb, even over extended periods of

time if implants are not placed within a six-month timeframe. Resulting bone development creates an implant foundation that has a

dense nature, and sites that have been exposed up to three years later demonstrate bone levels that are still level with the screw

heads as positioned at the time of the original surgery. Hands-on exercises will utilize anatomic models, surgical instruments, tenting

screws, membrane-fixation screws, and membranes.

Socket Augmentation Techniques for Minimizing Buccal Bone Resorption and Improving Implant Esthetics Limited Attendance

Preventing ridge resorption after tooth extraction remains a major challenge among clinicians. In this hands-on workshop

designed for intermediate and advanced clinicians, different techniques used for managing extraction sockets will be thoroughly

discussed. Participants will understand extraction socket classification and decision-making for selecting certain techniques.

Recent advancements in using newly developed combination allografts and PTFE membranes to prevent extraction socket

resorption will be presented. Choosing a socket grafting technique to increase the zone of keratinized mucosa in order to

facilitate future implant placement and to enhance esthetics will be illustrated. A hands-on exercise using pig mandibles will

focus on both socket augmentation and ridge augmentation techniques using barrier membranes and particulate bone graft

materials. PTFE membranes, collagen membranes, titanium-reinforced PTFE membranes, PTFE suture, and supporting

materials and instruments will be used during the workshop.

mobilization, and tension-free closure with a double-layer suturing technique. A predictable approach for implant

dehiscence treatment with GBR and membrane fixation with a periosteal suturing technique will also be presented.

Hands-on components using a pig mandible will include the use of titanium-reinforced barrier membranes, highly

adaptable porcine collagen membranes, membrane fixation system, PTFE sutures, and various other biomaterials and
