Notify the below contact of changes in status: Reaching age 60, Marriage, Divorce, Death, birth or adoption of minor child.

If your marital status has changed and you would like to elect or decline a survival benefit plan, or in the event of a death will need to fill out and send a DD Form 2565-6 Election Change Certificate.

When you turn 60, you can then get your new ID Card. You can come on your 60th birthday or after, but not before. ID card office can be reached at 1-800-477-8227.

You should call Tricare to ensure your medical coverage has started when you get your blue ID card and have turned 60 years old.

MDI Ceremonial Unit of Indiana.

The primary mission(s) of the MDI-CU is to provide Military Funeral Honors (MFH) for any eligible, current or retired (20 years of service), Indiana National Guard (INNG) member for the purpose of preserving the history of the INNG through the demonstration of close order marching; drill and ceremonies; provide escort duties to the Governor and other VIPs as directed; and to provide Military Ceremonial Support at public functions when requested.

Section II. Indiana State Veteran Benefits The State of Indiana offers special benefits for its military Service Members and Veterans including State Income Tax Exemptions, Retired Military Pay and Property Tax Exemptions, Education and Tuition Assistance, Special Military Vehicle Tags, as well as Hunting and Fishing License and State Park privileges. Eligibility for some benefits may depend on residency, military component, and Veteran disability status.

Indiana Military Service Tax Deduction This deduction is equal to the actual amount of military income received (i.e. military pay, retirement pay, and/or survivor's benefits) or $5,000, whichever is less. If both you and your Spouse received military income, you may each claim the deduction for a maximum of $10,000. Who is eligible for the Indiana Military Service Deduction? You are eligible to take a deduction if the income you report on your income tax return includes active or reserve military pay. Also, if you are retired from the military or are the Surviving Spouse of a person who was in the military, you may be able to take this deduction if:

-You were at least 60 years of age by the end of the year;

-You were receiving military retirement or survivor's benefits during the year; AND

-The total benefits received as retirement income were reported on your federal return.

Note: Military income earned while in a combat zone may be exempt (not taxed) on your federal income tax return. If that income is exempt on your federal income tax return, then it will also be exempt (not taxed) for Indiana income tax purposes. Since Indiana isn't taxing this income, your combat zone income is not eligible for a deduction. Important: If you received both military pay and retirement pay or survivor's benefits during the tax year, the total deduction cannot be greater than $5,000 per qualifying person. For example, if you earned $6,000 in military pay the first half of the year and $1,500 in retirement pay the second half of the year, you can deduct only $5,000 of your income. Indiana National Guard and Reserve Component Members Deduction There is a deduction for certain members of the reserve components of the Army, Navy, Air Force, Coast Guard, Marine Corps or the Merchant Marine, or a member of the Indiana Army National Guard or the Indiana Air National Guard. A deduction is available for the income received as a result of service on involuntary orders during the period the above members were deployed and mobilized for full-time service, or during the period the above member's Indiana National Guard unit was federalized.

Indiana Income Tax Deduction on Retired Military Pay Military retirees who are age 60 are entitled to deduct up to $6,250 beginning in 2018 of military or survivor benefits. In 2019 they will be able to deduct $6,250 plus 25% of military retired income, in 2020 $6,250 plus 50%, in 2021 $6,250 plus 75%, and in 2022 all Military retiree pay will be state tax free. Who is eligible for the Indiana Income Tax Deduction on Retired Military Pay? Retired military Service members are eligible for this deduction if:

-The retiree was at least 60 years of age by the end of the year;

-The retiree received military retirement during the year; and

-The total benefits received as retirement income were reported on the federal return.

If the retiree is married and the Spouse is also a military retiree, the amount deducted may be increased to a maximum of $12,500 plus according to the year the applicable percentage.

*Note: If the retiree earned both military pay and retirement pay in the same year, the total deduction may not be more than $6,250 for each qualifying person.

Indiana Military Disability Retired Pay Income Tax Deduction

Disabled retirees receiving disability retirement pay may be eligible to subtract up to $5,200 of their payments from gross income. Who is eligible for the Indiana Military Disability Retired Pay Income Tax Deduction? To qualify for this deduction, you must have:

-Been permanently and totally disabled at the time of retirement;

-Retired on disability before the end of the tax year; and

-Received disability retirement income.

A physician's statement must accompany the deduction sheet for each person claiming the deduction. The amount subtracted is limited to the amount of disability pay received or $100/week, whichever is less. Also, this may have to be reduced by part of the retiree's federal adjusted gross income.

Spouses may also be eligible to subtract up to $5,200 of disability payments (for a maximum of $10,400) if the couple files a joint return and the Spouse meets all of the above requirements.

Indiana State Taxes on U.S. Department of Veterans Affairs Disability Dependency and Indemnity Compensation

Dependency and Indemnity Compensation (DIC) is a tax-free monetary benefit paid to eligible survivors of military Service members who died in the line of duty or eligible survivors of Veterans whose death resulted from a service-related injury or disease.

To qualify for this benefit, the claimant(s) must be the eligible surviving spouse or dependent child of a military Service member or Veteran who:

-Died while serving on active duty, active duty for training, or inactive duty training, -Died as a result of a service-connected injury or disease, -Died as a result of a non-service connected injury or disease, and who was totally disabled from his/her service-connected disabilities for

-at least 10 years immediately preceding death, OR -since the Veteran's release from active duty and for at least five years immediately preceding death, OR -at least one year immediately preceding death if the Veteran was a former prisoner of war.

To complete an application, use VA Form 21P-534EZ, "Application for Dependency and Indemnity Compensation (DIC), Death Pension, and/or Accrued Benefits", and mail or fax it to the VA Pension Management Center (PMC) that serves your state. Check the PMC Locator to find the PMC having jurisdiction over the claim.

Military Survivor Benefit Plan (SBP)/ Reserve Component Survivor Benefit Plan (RCSBP)/ Retired Serviceman’s Family Protection Plan (RSFPP) Indiana State Tax Benefits

IC 6-3-2-4 allows a deduction for military retirement pay or survivor’s benefits received as a result of the individual’s active or reserve service in the armed services. For taxable years beginning before January 1, 2018, to qualify for the military service deduction for military retirement or survivor’s benefits, the taxpayer or surviving spouse must be at least 60 years of age on the last day of the tax year and have received military retirement or survivor’s benefits while a resident of Indiana. The allowable adjustment is the amount of military or survivor’s benefits received while a resident of Indiana and included in adjusted gross income. The maximum allowable deduction is $5,000. For taxable years beginning after December 31, 2017, the maximum allowable deduction for military retirement or survivor’s benefits is increased to $6,250. In addition, the minimum age requirement for qualification has been eliminated. This deduction is in addition to the $5,000 deduction for which taxpayers are otherwise entitled for certain military service income under IC 6-3-2-4(a)(1). Indiana Disabled Veterans Property Tax/Excise Tax Deductions Property tax deductions are available to disabled Hoosier Veterans under the following conditions:

Deduction for Totally Disabled Veteran or Veteran age 62 and Partially Disabled: (IC 6-1.1-12-14 ): http://iga.in.gov/static-documents/0/5/3/f/053f0fc4/TITLE6_AR1.1_ch12.pdf

A $12,480 dollar deduction is available to a Veteran who:

-Served at least 90 days of honorable service (can be peacetime service); AND

-Are totally disabled (not necessarily service-connected but the disability must be evidenced by a U.S. Department of Veterans Affairs pension certificate); OR

-Are at least 62 years old and 10% service-connected disabled.

Note: This deduction is not available if the assessed value of the real property owned by the Veteran is in excess of $143,160 for tax year 2016, for tax years 2017 and later the assessed value must not exceed $175,000.

Deduction for Veteran with Partial Disability/ Limitations: (IC 6-1.1-12-13)

A $24,960 tax deduction is available for Veterans who:

-Served honorably in the Armed Forces during any period of wartime. And:

-Are at least 10% service-connected disabled.

-Served honorably during any period of wartime. AND

-Is 100% service-connected disabled OR

-Is at least 62 years of age with at least a 10% service-connected disability.

The Surviving Spouse of the Veteran is eligible for the Veteran's tax deduction. The Spouse must apply in his or her own name. Ref: IC 6-1.1-12-14, IC 6-1.1-12-15. Please see hyperlink above.

If the assessed valuation of the property is less than the deduction, the overage can be applied toward the excise tax on the Veteran's license plates at the rate of $2 per each $100 of deduction.

Deduction for Property Received from a Tax Exempt Organization: (IC 6-1.1-12-14.5 )

For a deduction of property tax for property conveyed to the Veteran at no cost to the Veteran by an organization that is exempt from income taxation under the federal Internal Revenue Code:

Served honorably during any period of wartime for at least 90 days AND

Receive a service connected disability rating of at least 50%

Deductions under this section are determined as follows;

-If the Veteran is totally disabled (100%) then the deduction is equal to 100% of the assessed value

-If the Veteran is 90% Service Connected disabled then the deduction is equal to 90% of the assessed value

-If the Veteran is 80% Service Connected disabled then the deduction is equal to 80% of the assessed value

-If the Veteran is 70% Service Connected disabled then the deduction is equal to 70% of the assessed value

-If the Veteran is 60% Service Connected disabled then the deduction is equal to 60% of the assessed value

-If the Veteran is 50% Service Connected disabled then the deduction is equal to 50% of the assessed value

Indiana Military and Veteran Education Benefits Indiana Wartime Veterans High School Diploma Program

Veterans who served between the following dates may apply to the Indiana Department of Veterans' Affairs for issuance of a high school diploma:

April 6, 1917 and November 11, 1918 (World War I)

December 7, 1941 and December 31, 1946 (World War II)

June 27, 1950 and Jan 31, 1955 (Korea)

or Aug 5, 1964 and May 7, 1975 (Vietnam)

Who is eligible for the Indiana Wartime Veterans High School Diploma Program? Veterans must have:

-Attended public or nonpublic high school in Indiana prior to military service,

-Been a student in good standing,

-Not graduated or received a diploma because of leaving high school for military service, AND

-Been honorably discharged from the Armed Forces of the United States.

Indiana Waiver of Tuition and Fees for Children of Purple Heart Recipients, Deceased or Disabled Veterans: ( IC 21-14-4 )

The biological or legally adopted child(ren) of an eligible disabled Veteran may be eligible for free or reduced tuition at any state-supported post-secondary school or university in the state of Indiana. The free or reduced tuition is limited to a maximum of 124 semester hours of education at an eligible Indiana institution. Who is eligible for the Indiana Waiver of Tuition and Fees for Children of Purple Heart Recipients, Deceased or Disabled Veterans? The Veteran parent must meet the following criteria:

-Served in the U.S. Armed Forces, and received a discharge or separation under other than dishonorable conditions, AND

-Served during a period of war or other hazardous duty which was recognized by a service or campaign medal, OR

-Received the Purple Heart for wounds as a result of enemy action, OR

-Suffered a service-connected death or disability as determined by the Dept. of Veterans' Affairs, AND

-Resided in Indiana for at least 36 consecutive months in his or her lifetime.

Eligible war time dates are:

WWII: December 7, 1941 and December 31, 1946

Korea: June 27, 1950 and Jan 31, 1955

Vietnam: Aug 5, 1964 and May 7, 1975

Gulf War: August 2, 1990 – present

Veteran Parent Enlistment on/before June 30, 2011: If the Veteran parent initially enlisted before June 30, 2011, the following criteria for the child applies:

The child must be eligible for the resident tuition rate in the school which he/she will attend

If the child is legally adopted, he/she must have been adopted before the age of 24.

There is no time limit in which to use the tuition waiver. The benefit will pay 100% of tuition and fees at an eligible Indiana institution as a supplement to other state grants. The benefit may be used at the

undergraduate, graduate and professional degree level. However, tuition and fees will be paid at the undergraduate rate.

Veteran Parent Enlistment on/after July 1, 2011: If the Veteran parent initially enlisted after June 30, 2011, the following criteria for the child applies:

The child must be eligible for the resident tuition rate in the school which he/she will attend

-Possesses the requisite academic qualifications

-Is not more than 32 years old when he/she first applies for and is eligible to receive the benefit.

-If the child is legally adopted, he/she must have been adopted before the age of 18.

-The allotted credit hours (124) must be used within 8 years after the date the child first applied.

The benefit will pay 100% of tuition and fees at an eligible Indiana institution as a supplement to state grants if the Veteran parent suffered a disability with a rating of 80% or more. If the Veteran parent suffered a disability with a rating of less than 80%, the rate of remission for tuition and fees is 20% plus the disability rating (for example, a 50% disability = 70% remission). The benefit may be used at the undergraduate and professional degree level. However, tuition and fees will be paid at the undergraduate rate.

Indiana Children of POW/MIA Service Members, Tuition Waiver: (IC 10-17-7 )

This benefit will pay 100% of tuition and fees for the child(ren) regardless of enlistment date. However, if the parent enlisted on/after July 1, 2011, the allotted 124 credit hours must be used within 8 years after the date the child first applied. The child must have been born or legally adopted and in the legal custody of the Service Member during the period the POW/MIA status was declared.

The following additional criteria apply for POW/MIA Service Members:

-The POW/MIA must have been a resident of Indiana at the time of enlistment; and

-The POW/MIA status must have been declared while serving in the U.S. Armed Forces after January 1, 1960.

Who is eligible for the Indiana Children of POW/MIA Service Members, Tuition Waiver? To be eligible the child of the POW/ MIA must meet the following requirements:

-Must be an individual who was born during the period during which his or her parent was a POW/MIA

-Must have been legally adopted or in the legal custody of his or her parent before and during the period which the parent was a POW/MIA

-Must file the FAFSA annually

-Must not be more than 32 years of age when the person first applies and becomes eligible for benefits under this chapter

-If parent enlisted after June 30, 2011 and student enrolled in college after June 30, 2012, student must maintain SAP as determined by the institution

Indiana National Guard Supplemental Grant (NGSG): Through a partnership with the Indiana National Guard, the Higher Education Commission guarantees 100% of tuition and regularly assessed fees for eligible members of the Indiana Air and Army National Guard. Students can attend either full-time or part-time and receive the NGSG but can only be used in the fall and spring semesters.

Who is eligible for the Indiana National Guard Supplemental Grant (NGSG)?

-Must be a member of the Indiana National Guard in active drilling status**

-Must not have been AWOL at any time during the 12 months prior to their enrollment in college for any academic term**

-Must have all required INNG paperwork submitted and approved to their INNG education office prior to the start of classes (each semester). Forms and submission processes differ by INNG branch. Students should contact their unit commander for details.

**PLEASE NOTE: Students must contact their unit commander for certification and must be recertified each semester to receive the NGSG.

National Guard Scholarship Extension Program : There is a limited scholarship, called the National Guard Scholarship Extension Program, that is available to former Guard members who left the Guard under honorable discharge conditions, used the NGSG in the past, and who served on active duty overseas since Sept. 10, 2001. Former Guardsman who believes they might qualify for this program may review the application and rules at the ScholarTrack website. First time users will have to create an account. Indiana Tuition Fee Exemption for Children and Spouses of Deceased National Guard Service Members: (IC 21-14-7 )

An eligible applicant is exempt from the payment of educational costs for instruction at a state educational institution. The maximum amount that is exempt from fees is 124 credit hours. Who is eligible for the Indiana Tuition Fee Exemption for Children and Spouses of Deceased National Guard Service Members? Any child or spouse whose mother, father or spouse:

Indiana State Employment Veterans Hiring Preference Indiana offers Veterans' preference in State Government employment for Veterans. Preference points awarded to such Veterans on each such examination shall be ten percent (10%) of the total number of points which may be obtained thereon. Indiana Veterans can find assistance through their local county Veterans' Service Offices. Who is eligible for Indiana State Employment Veterans Hiring Preference? To be eligible, a person must have:

-Served on active duty in the U.S. Armed Forces for at least 181 days

-Received an honorable discharge.

Federal Employment Veterans Hiring Preferences By Federal law, Veterans who are disabled or who served on active duty in the United States Armed Forces during certain specified time periods or in military campaigns are entitled to preference over non-Veterans both in hiring into the Federal civil service and in retention during reduction in force. Veterans may be entitled to 5 or 10 points included in any scoring process for examination or interviews.

5-point Hiring Preference: Five points are added to the examination score or rating of a Veteran who served:

-During a war; or

-During the period April 28, 1952 through July 1, 1955; or

-For more than 180 consecutive days, other than for training, any part of which occurred after January 31, 1955, and before October 15, 1976; or

-During the Gulf War from August 2, 1990, through January 2, 1992;

10-point Compensable Disability (CP) Preference: Ten points are added to the examination score or rating of:

A Veteran who served at any time and who has a compensable service-connected disability rating of at least 10 percent but less than 30 percent.

10-Point 30 Percent Compensable Disability Preference (CPS): Ten points are added to the passing examination score or rating of:

A Veteran who served at any time and who has a compensable service-connected disability rating of 30 percent or more.

10-Point Disability Preference (XP): Ten points are added to the passing examination score or rating of:

-A Veteran who served at any time and has a present service-connected disability or is receiving compensation, disability retirement benefits, or pension from the military or the Department of Veterans Affairs but does not qualify as a CP or CPS; or

10-Point Derived Preference (XP): Ten points are added to the passing examination score or rating of spouses, surviving spouses or Mothers of Veterans as described below. This type of preference is usually referred to as "derived preference" because it is based on service of a Veteran who is not able to use the preference. Both a mother and a spouse (including surviving spouses) may be entitled to preference on the basis of the same Veteran's service if they both meet the requirements. However, neither may receive preference if the Veteran is living and is qualified for Federal employment.

Ten points are added to the passing examination score or rating of the spouse of a disabled Veteran who is disqualified for a Federal position along the general lines of his or her usual occupation because of a service-connected disability. Such a disqualification may be presumed when the Veteran is unemployed and

-Is rated by appropriate military or Department of Veterans Affairs authorities to be 100 percent disabled and/or unemployable; or

-Has retired, been separated, or resigned from a civil service position on the basis of a disability that is service-connected in origin; or

-Has attempted to obtain a civil service position or other position along the lines of his or her usual occupation and has failed to qualify because of a service-connected disability.

Uniformed Services Employment and Reemployment Rights Act

Uniformed Services Employment and Reemployment Rights Act (USERRA) protect civilian job rights and benefits for Veterans and members of Reserve components. USERRA also makes major improvements in protecting Service Member rights and benefits by clarifying the law, improving enforcement mechanisms, and adding Federal Government employees to those employees already eligible to receive Department of Labor assistance in processing claims.

Absence from Work for Military Duty

USERRA establishes the cumulative length of time that an individual may be absent from work for military duty and retain reemployment rights to five years (the previous law provided four years of active duty, plus an additional year if it was for the convenience of the Government). There are important exceptions to the five-year limit, including initial enlistments lasting more than five years, periodic National Guard and Reserve training duty, and involuntary active duty extensions and recalls, especially during a time of national emergency. USERRA clearly establishes that reemployment protection does not depend on the timing, frequency, duration, or nature of an individual's service as long as the basic eligibility criteria are met.

Disability Accommodation

USERRA provides protection for disabled Veterans, requiring employers to make reasonable efforts to accommodate the disability. Service Members convalescing from injuries received during service or training may have up to two years from the date of completion of service to return to their jobs or apply for reemployment.

Return to Work without Loss of Seniority

USERRA provides that returning service-members are reemployed in the job that they would have attained had they not been absent for military service (the long-standing "escalator" principle), with the same seniority, status and pay, as well as other rights and benefits determined by seniority. USERRA also requires that reasonable efforts (such as training or retraining) be made to enable returning Service Members to refresh or upgrade their skills to help them qualify for reemployment. The law clearly provides for alternative reemployment positions if the Service Member cannot qualify for the "escalator" position. USERRA also provides that while an individual is performing military service, he or she is deemed to be on a furlough or leave of absence and is entitled to the non-seniority rights accorded other individuals on non-military leaves of absence.

Extension of Health and Pension Plans

Health and pension plan coverage for Service Members is provided for by USERRA. Individuals performing military duty of more than 30 days may elect to continue employer sponsored health care for up to 24 months; however, they may be required to pay up to 102 percent of the full premium. For military service of less than 31 days, health care coverage is provided as if the Service member had remained employed. USERRA clarifies pension plan coverage by making explicit that all pension plans are protected.

Indiana Military and Veterans Bureau of Motor Vehicles (BMV) Benefits

Indiana Motor Vehicle Tax Exemption for Disabled Veterans: ( IC 6-6-5-5.2 )

Eligible disabled Veterans may claim a credit against tax imposed upon a motor vehicle owned by the Veteran. Any unused portion after the application to residence property (see the section above entitled Property Tax Exemption) applies next to personal property and lastly as Excise Tax Credit on either motor vehicle excise tax or aircraft license tax. The excise tax shall be reduced in the amount of $2 on each $100 of taxable value or major portion thereof. For the 2016 tax year the Veterans assessed property value must not exceed $143,160 for the 2017 tax year, and later, the Veterans assessed property value must not exceed $175,000. Who is eligible for the Indiana Motor Vehicle Tax Exemption for Disabled Veterans? The Veteran must meet the following requirements:

-The Veteran served in the U.S. Armed Forces during any of its wars and received an honorable discharge,

-The Veteran has a service-connected disability of 10% or more and is evidenced by a pension certificate, award letter of compensation, disability compensation check issued by the Department of Veterans Affairs, or a certificate of eligibility issued by the Indiana Department of Veterans' Affairs, OR

-The individual served at least 90 days in the U.S. Armed Forces, received an honorable discharge, and is either totally disabled or is at least 62 years of age and has a disability rating of at least ten percent

-The Veteran does not own property to which a property tax deduction may be applied under IC 6-1.1-12-13

Surviving Spouses may qualify for this exemption if the deceased Veteran would have been alive and eligible for this credit in 2013, or previously received a credit under this section.

*Note: The Veteran or spouse may claim up to TWO vehicles for the tax credit under this section. The maximum amount of the excise tax liability is $70. An individual may not claim a vehicle credit and a property tax credit at the same time.

Active Duty/Veteran/ Surviving Spouse Indicator for Indiana Driver License/ID Card

Active-duty Service members, Veterans and Surviving Spouses may choose to have an indicator placed on their driver's license or ID Card. This designator is a "V" which is located on the back of the card for Veterans, an "AM" for active Service members and “VS” for Surviving Spouses.

Who is eligible for the Active Duty/Veteran/ Surviving Spouse Indicator for Indiana Driver License/ID Card?

The Veteran must provide a DD214 or other document from the Department of Defense verifying other than a dishonorable discharge to the Bureau of Motor Vehicles. An active-duty Service Member must present a current military Common Access Card (CAC) during your new issuance or renewal application. If you are a Surviving Spouse of a deceased Veteran, you may choose to have a “Veteran Spouse” indicator placed on your driver’s license. To obtain the indicator, you must present proof of the Veteran’s discharge or separation, a marriage certificate, and a death certificate of the deceased Veteran.

If an indicator is added during a new issuance or a renewal transaction, there is no charge. If the indicator is added to an unexpired credential outside of a renewal transaction, there will be a fee to amend your credentials.

Indiana Military Commercial Driver License (CDL) Military Skills Waiver Program

The BMV may waive the required skills test for select Veterans or active duty personnel of the U.S. Armed Forces who were/are truck drivers during their military service, so long as the applicant applies within one year of discharge and meets all requirements. Who is eligible for the Indiana Military Commercial Driver License (CDL) Military Skills Waiver Program? All military applicants must pass the required written test(s); prove that he/she is regularly employed (or was regularly employed) in a military position requiring operation of a Commercial Motor Vehicle (CMV) within one year; and certify that during the two years immediately preceding the application for an Indiana CDL he/she meets the following requirements:

-Has not had more than one license (except for a military license);

-Has not had any license suspended, revoked, or cancelled;

-Has not had any convictions for any type of motor vehicle for the disqualifying offenses contained in § 383.51(b);

-Has not had more than one conviction for any type of motor vehicle for serious traffic violations contained in § 383.51(c); and

-Has not had any conviction for a violation of military, State or local law relating to motor vehicle traffic control (other than a parking violation) arising in connection with any traffic accident and has no record of an accident in which he/she was at fault.

-Was operating a vehicle representative of the CMV the driver applicant operates or expects to operate.

Hoosier Veteran License Plates Any resident of the State who was honorably separated from the active Armed Forces can purchase a Hoosier Veteran license plate. Simply take your DD-214 or your Discharge Certificate to your local BMV office and request the special plate. There is a $15 charge for the Hoosier Veteran plate. Indiana Disabled Hoosier Veteran License Plates Disabled Veteran plates are the same as handicap plates in the State of Indiana. These are strictly limited to those individuals who have serious mobility impairment due to a service-connected disability. According to the law: "disabled Veteran" means a Veteran who has a service-connected disability resulting in:

-loss of sight in both eyes or permanent impairment of vision in both eyes; or

-loss of one (1) or both feet or the permanent loss of use of one (1) or both feet; or

-loss of one (1) or both hands or the permanent loss of use of one (1) or both hands; or

-a physical condition that precludes the applicant from being able to walk without pain or difficulty. This must be clearly stated in the VA rating decision. If not, the applicant will be required to furnish a statement from their doctor stating the need for this handicapped license plate OR

-been rated by the United States Department of Veterans Affairs as being at least 50% disabled and is receiving service related compensation from the United States Department of Veterans

Indiana Gold Star Family License Plate

The Gold Star Family license plate is available to the Spouses, Parents, grandparents, great-grandparents, siblings, or Children of anyone who died while serving on duty in the military - active, Reserve or National Guard. There is no additional cost for the Gold Star Family license plate. Regular vehicle registration fees and taxes still apply. To purchase the Purple Heart plate, the Indiana Department of Veterans Affairs (IDOVA) must complete Indiana Bureau of Motor Vehicles, Certification of Eligibility for Veterans License Plates State Form 32584 certifying your eligibility for the plate.

To be eligible for the plate, Family members must provide an official DD Form 1300 from the Department of Defense. In many cases the Department of Defense issues only one copy of the DD Form 1300, normally to the person who is listed as the next-of-kin. In those cases, the next-of-kin will need to be contacted by other Family members and a copy of the form obtained from him/her. This plate will be available at license branches and other customer service locations. You will receive the plate in the mail within 14 days. You will be able to display this plate on passenger cars, trucks up to 11,000 pounds, and recreational vehicles.

National Guard members when on duty or assembled for duty, in case of riot, tumult, breach of peace, insurrection, invasion, public disaster or whenever ordered by the governor, the commanding general of the national guard, or called to the aid of civil authorities, is injured, is disabled, or contracts a disease because of the member's duty or assembly; or without fault or neglect on that member's part, is wounded or disabled while performing any lawfully ordered duty that temporarily incapacitates the member from pursuing the member's usual business or occupation; shall, during the period of incapacity, receive the pay to which the member was entitled while on or assembled for duty, plus any actual necessary expenses for care and medical attention.

(IC 10-16-7-19 ) A National Guard Service member, upon proof of the disability, shall be placed on the roll of invalid pensioners of the state and shall receive out of money in the state treasury not otherwise appropriated, upon the audit of the adjutant general and approval of the governor, the same pension or reward that a person under similar circumstances would receive from the United States

In case of a wound, an injury, or a disease that results in death, the Surviving Spouse, dependent Children, or dependent parent of the member of the Indiana National Guard shall receive the pension and reward dating from the time of receiving the injuries on account of which the pension or reward is allowed. If a member of the Indiana National Guard dies in the active service of the state, the member's reasonable funeral expenses, not exceeding eight thousand eight hundred dollars ($8,800), shall be paid by the state in the manner as the governor directs. A Hoosier Golden Passport provides unlimited admission to all Indiana State owned parks, recreation areas, reservoirs, and forests. It is not valid for historic sites, museums, memorials, and other Department of Natural Resource (DNR) facilities. The pass is good for one calendar year. Who is eligible for the Indiana Golden Hoosier Passport?

Persons eligible for a Disabled American Veteran license plate under IC 9-18.-5-5 (http://iga.in.gov/legislative/laws/2017/ic/titles/009#9-18.5-5) may purchase the Hoosier Golden Passport at DNR facilities around the State. If you have Disabled American Veteran plates on the vehicle, simply drive to the gate and the attendant will sell you the Hoosier Golden Passport. If you do not have the plate but believe you are eligible, you will be given a form to request the plate, which you must send to the Indiana Department of Veterans Affairs for verification.

Former Prisoners of War will receive their Hoosier Golden Passport in the mail after their plates are issued. If you do not receive yours in a reasonable time or have other plates on your vehicle but are entitled to POW plates, contact the Department of Natural Resources at 1-800-622-4931.

Indiana Disabled Veterans' Reduced Fee, Hunting and Fishing Licenses

Disabled Hoosier Veterans may purchase a license to hunt and fish in the State of Indiana for the reduced fee of $2.75 or a 10 year DAV license for $27.50. Who is eligible for Indiana Disabled Veterans' Reduced Fee, Hunting and Fishing Licenses? Indiana residents who have served in the Armed Forces and have a documented service connected disability through the U.S. Department of Veterans Affairs are eligible. Steps to get your DAV license:

-Fill out your application form (applications are available from your County Veteran Service Officer or online).

-Include your USDVA File number.

-Send your application, payment and self-addressed stamped envelope to:

DNR Customer Service Center 402 W. Washington St, Room W160 Indianapolis, IN 46204

Once you receive your certified application, you may use it or your previous license to purchase future licenses and eliminate any delay. Just show your license or certified application to the vendor for proof of your eligibility.

Within two weeks, your signed and sealed certified application will be returned to you. You will need to retain this letter for your future use.

*Note: The DAV license is good for small game hunting and fishing only. Waterfowl, Trout/Salmon, deer, and turkey stamps may be purchased at the regular rate.

Military Service Documents for Indiana Veterans

The Indiana Department of Veterans Affairs (IDVA) and the Indiana State Archives have copies of DD214's on file for many Hoosier Veterans who entered the service from Indiana. Copies of these will be provided upon request to the Indiana Department of Veterans Affairs. The processing time for such a request is usually one or two weeks. The Indiana Department of Veterans Affairs or your County Veterans Service Officer can assist you with the paperwork to obtain other documents from the federal government. In most cases if the Veteran was discharged from 1989 to present the IDVA has copy 6 on file. For discharges prior to 1989 contact the State Archives at 317-591-5222.

Indiana Veterans Memorial Cemetery

The Indiana Veterans Memorial Cemetery opened for internments on December 1, 1999. The Cemetery is located adjacent to the Madison State Hospital and Clift Falls State Park in Madison, Indiana. There is no charge for the grave site or use of the Chapel for internment services. Any eligible Veteran will be buried at no cost to the Family. Burial of a Spouse or dependent Child will require the payment of an opening and closing fee. The fee will be determined by the type of grave site. Who is eligible for interment at the Indiana Veterans Memorial Cemetery? Veterans and Members of the Armed Forces:

-Any member of the Armed Forces of the United States who dies on active duty.

-Any Veteran who was discharged under conditions other than dishonorable. With certain exceptions, service beginning after September 7, 1980, as an enlisted person, and service after October 16, 1981, as an officer, must be for a minimum of 24 continuous months or the full period for which the person was called to active duty (as in the case of a Reservist called to active duty for a limited duration). Undesirable, bad conduct and any other type of discharge other than honorable may or may not qualify the individual for Veterans benefits, depending upon a determination made by a VA Regional Office and/or the Indiana Department of Veterans' Affairs. Cases presenting multiple discharges of varying character are also referred for adjudication to a VA Regional Office and/or the Indiana Department of Veterans' Affairs.

-Any citizen of the United States who, during any war in which the United States has or may be engaged, served in the Armed Forces of any Government allied with the United States during that war, whose last active service was terminated honorably by death or otherwise, and who was a citizen of the United States at the time of entry into such service and at the time of death.

Members of Reserve Components and Reserve Officers' Training Corps:

-Reservists and National Guard members who, at time of death, were entitled to retired pay under Chapter 1223, title 10, United States Code, or would have been entitled, but for being under the age of 60. Specific categories of individuals eligible for retired pay are delineated in section 12731 of Chapter 1223, title 10, United States Code.

-Members of reserve components, and members of the Army National Guard or the Air National Guard, who die while hospitalized or undergoing treatment at the expense of the United States for injury or disease contracted or incurred under honorable conditions while performing active duty for training or inactive duty training or undergoing such hospitalization or treatment.

-Members of the Reserve Officers' Training Corps of the Army, Navy, or Air Force who die under honorable conditions while attending an authorized training camp or on an authorized cruise, while performing authorized travel to or from that camp or cruise, or while hospitalized or undergoing treatment at the expense of the United States for injury or disease contracted or incurred under honorable conditions while engaged in one of those activities.

-Members of reserve components who, during a period of active duty for training, were disabled or died from a disease or injury incurred or aggravated in line of duty or, during a period of inactive duty training, were disabled or died from an injury or certain cardiovascular disorders incurred or aggravated in line of duty.

Spouses and Dependents: The spouse or surviving spouse of an eligible Veteran is eligible for interment in the Indiana Veterans Memorial Cemetery even if that Veteran is not buried or memorialized at the Indiana Veterans Memorial Cemetery. In addition, the spouse or surviving spouse of a member of the Armed Forces of the United States whose remains are unavailable for burial is also eligible for burial.

The surviving spouse of an eligible Veteran who had a subsequent remarriage to a non-Veteran and whose death occurred on or after January 1, 2000, is eligible for burial in the Indiana Veterans Memorial Cemetery based on his or her marriage to the eligible Veteran.

The minor children of an eligible Veteran. For purpose of burial in the Indiana Veterans Memorial Cemetery, a minor child is a child who is unmarried and:

-Who is under 21 years of age; or,

-Who is under 23 years of age and pursuing a full-time course of instruction at an approved educational institution.

For purpose of burial in the Indiana Veterans Memorial Cemetery, an unmarried adult child is of any age but became permanently physically or mentally disabled and incapable of self-support before reaching 21 years of age, or before reaching 23 years of age if pursuing a full-time course of instruction at an approved educational institution. Proper supporting documentation must be provided.

Indiana Governor's Memorial Certificate

A special certificate, signed by the Governor, is available to surviving Family members upon the death of an honorably discharged Veteran to commemorate their service. This certificate is available to the men and women who served in the United States Armed Forces, including the National Guard and Reserve.

Indiana County Veterans Burial allowances Each County Auditor is authorized to pay up to an amount not to exceed $1000 for the burial of a Veteran or the Veteran's Spouse, and to pay up to $100 for the setting of a federal headstone or marker. Indiana Military Family Relief Fund (MFRF)

Beginning on January 2, 2007, the Indiana Department of Veterans Affairs (IDVA) began accepting grant applications from the Families of deployed or recently deployed National Guard members and members of the Selected Reserves. Effective July 1, 2016, the Military Family Relief Fund is available to all Veterans that served on active duty during a national conflict or war time period.

The emergency grants may be used by the Families for needs such as food, housing, utilities, medical services, transportation, and other essential Family support expenses which have become difficult to afford. Grants up to $2,500 will be awarded. Grants will be considered by the Indiana Veterans' Affairs Commission, and the applicant will be advised.

*Note: The Director of the Indiana Department of Veterans' Affairs may waive the requirements for emergency cases only upon written request indicating the circumstances justifying such a waiver. Who is eligible for the Indiana Military Family Relief Fund (MFRF)? The requirements are:

-The Service Member must be serving honorably or must have received an honorable discharge.

-Qualified Service Members may be eligible for the fund if they have at least 12-months of qualifying military service, and if a portion was served while on active duty.

-There is a documented need for financial assistance for necessities of daily living that include food, clothing, housing,

-The applicant must sign and date the application and provide all required proof and documentation.

-If your marital status has changed and you would like to elect or decline a survival benefit plan, you will need to fill out and submit a DD Form 2565-6 Election Change Certificate and send copies of marriage license. Section IV. Preparing for Passing The following checklist should be reviewed by military Alumni and their beneficiaries on an annual basis. This checklist is designed to equip you and your loved ones with knowledge and information that may prove helpful. There are also many examples of checklists online and you can find the one that best fits your family. While it may be impossible to truly prepare for the overwhelming emotions and dilemmas that arise with the loss of a loved one, it does help when most of the below issues have been put into place.

__ Create a military file that includes your retirement orders, separation papers, medical records, etc. Make sure your spouse knows the location and telephone number of the nearest military installation.

__ Create a military retired pay file that includes the pertinent information for DFAS

(This file should also include the number of any VA claim still pending and the address of the VA office being used; a list of deductions currently being made from benefits; and the name, relationship and address of the person you have made the beneficiary of any unpaid retired pay at the time of death.)

__ Create an annuities file. This file should have information about the Survivor Benefit Plan (SBP), Reserve Component Survivor Benefit Plan (RCSBP) or the Retired Serviceman's Family Protection Plan (RSFPP), Civil Service annuity, etc. Additional information regarding SBP annuity claims can be obtained from the DFAS-Cleveland office at 800-321-1080.

__ Create a personal document file that has copies of marriage certificates, divorce decrees, adoptions and naturalization papers.

__ Create an income tax file. Include copies of your state and federal income tax returns.

__ Create a property tax file. Include copies of tax bills, deeds and any other related information.

__ Create an insurance policy file. Include life, property, accident, liability and hospitalization policies.

__ In a secure location, maintain a list of all bank accounts (joint or individual). Include the location of all deposit boxes, savings bonds, stocks, bonds and any securities owned.

__ In a secure location, maintain a list of all charge accounts and credit cards. Include account numbers and mailing addresses.

__ Maintain a list of all associations and organizations of which you are a member. Some of them could be helpful to your spouse.

__ Maintain a list of all friends and business associates who may be helpful. Include name, address and phone number.

__ Spend time with your spouse discussing your plans with respect to the type and place of your funeral service. You should decide which cemetery, whether ground burial, or cremation, etc. If your spouse knows your desires, it will resolve some of the questions that might arise at a later date.

__ Visit a local funeral home and pre-arrange your services. Many states will allow you to pre-pay for services.

__ Investigate the decisions that you and your family have agreed upon.

__ Once your decisions have been made and you're comfortable with them, have a will drawn up outlining all your wishes.

__ Insure that your will and all other official documents are maintained in a secure location known by your loved ones. This includes all of your DD214s or Retirement Orders. Section V. Reporting Alumni Deaths

In the event of an Alumni who received federal benefits passes here are the steps to follow to help relieve some of the burden of this stressful time on family members. For any Indiana Alumni who pass make your first step to call our Retiree offices at (317) 964-7048 or (317) 964-7050 and our offices will help you through your transition. Checklist 1. In the event of a retiree or service members passing please call the following

Casualty Offices First

Casualty Assistance Centers will submit a casualty report which will stop the retiree’s retired pay. The next of kin may also telephone Defense Finance and Accounting Service Cleveland at (800)-321-1080 to report the retiree’s death. Any check received after the retiree’s date of death will have to be returned to Defense Finance and Accounting Service Cleveland. Once Defense Finance and Accounting Service Cleveland is notified of the death, all applicable claim forms will be mailed to the beneficiary. These forms will usually arrive within 21-28 days.

2. Survivor Benefit Plan. If the deceased participated in the Survivor Benefit Plan, the surviving spouse or dependent child may be eligible for an annuity. To apply for the annuity, complete DD Form 2656-7, Verification for Survivor Annuity, SF 1199A, Direct Deposit Sign-up Form, and Form W4P, Withholding Certificate for Annuity Payments. These forms, along with a copy of the retiree’s death certificate, should be mailed to Defense Finance and Accounting Service, United States Military Annuitant Pay, Post Office Box 7131, London, KY 40742-7131.

For Indiana National Guard Retiree Surviving spouses contact Retiree Services office at (317) 964-7048 or 7050 and we will process the paper work for Survival Benefit Program if it was elected on the 20-years of service/retired pay packet.

3. Veteran’s Administration.

Report the retiree’s death to the Veteran’s Affairs at (800)-827-1000 and inquire about benefits or visit their website at: http://www.va.gov/.

-Veterans Affairs National Cemeteries. Burial benefits in a Veteran Affairs National Cemetery for eligible veterans and some dependents include the gravesite, a headstone or marker, opening and closing of the grave and perpetual care. Many national cemeteries have a columbaria or gravesites for cremated remains. Contact the Veterans Affairs for further information or visit the National Cemetery Administration website at: http://www.cem.va.gov/.

-Headstones and Markers. Veteran Affairs National Cemetery Administration provides headstones or markers for the graves of veterans anywhere in the world and of eligible dependents buried in military post, state veteran or national cemeteries. When burial occurs in a cemetery other than a military post, state veteran or national cemetery, the headstone or marker must be requested from the Veteran Affairs. It is shipped at government expense, but the cost of placing the headstone or marker is the responsibility of the survivor. To apply, complete VA Form 40-1330, Claim for Standard Government Headstone or Marker available at the Veteran Affairs National Cemetery Administration website: http://www.cem.va.gov/ and mail to Director, Memorial Programs Service (403A), Department of Veterans Affairs, 810 Vermont Avenue NW, Washington, DC 20420. -Presidential Memorial Certificates. Presidential Memorial Certificates express the Nation’s recognition of a veteran’s service. Certificates bearing the signature of the President are issued honoring deceased veterans with honorable discharges. Eligible recipients include next of kin and other loved ones. The award of the certificate to one eligible recipient does not preclude certificates to other eligible recipients. To establish honorable service, the documents listed below may be used to establish honorable active military service. The National Cemetery Administration will accept the following documents, with some exceptions to verify eligibility when requesting a headstone or marker. Contact the local Veteran Affairs office for assistance. Weblink: https://www.va.gov/vaforms/va/pdf/va40-0247.pdf

-Reimbursement of Burial Expenses. Veteran Affairs will pay up to $2,000 toward burial expenses for deaths on or after September 11, 2001. If the Veteran is buried in a Veteran Affairs National Cemetery Administration, some or all of the cost of transporting the deceased may be reimbursed.

-Veteran Affairs will pay up to $749 toward burial and funeral expenses for deaths on or after October 1, 2016 (if hospitalized by Veteran Affairs at time of death), or $300 toward burial and funeral expenses (if not hospitalized by Veteran Affairs at time of death), and a $749 plot-interment allowance (if not buried in a national cemetery). For deaths on or after December 1, 2001, but before October 1, 2011, Veteran Affairs will pay up to $300 toward burial and funeral expenses and a $300 plot-interment allowance. For deaths on or after April 1, 1988 but before October 1, 2011, Veteran Affairs will pay $300 toward burial and funeral expenses (for Veterans hospitalized by Veteran Affairs at the time of death).

-Complete VA Form 21P-530, Application for Burial Benefits (http://www.vba.va.gov/pubs/forms/vba-21p-530-are.pdf) and submit to the nearest Veterans Administration office or contact (800) 827-1000 for further information.

-Dependency and Indemnity Compensation. To be eligible, the deceased must have died from: a disease or injury incurred or aggravated while on active duty or active duty for training, or an injury incurred or aggravated in line of duty while on inactive duty training; or a disability compensable by Veteran Affairs. All eligible survivors (unmarried surviving spouse, unmarried children under 18, and handicap children between 18-23 attending a Veteran Affairs approved school or low-income parents of deceased service members/veterans) may apply.

-Veterans with a total service-connected disabled at the time of death, but their death was not the result of their service-connected disability, the survivor may qualify if: the veteran was continually rated disabled for 10 years or more immediately preceding death; or the veteran was so rated for five years from the date of military discharge; or was a former prisoner of war who died after September 30, 1999 and who was continually rated totally disabled at least one year preceding death. The discharge must have been under conditions other than dishonorable. For further information, contact (800) 827-1000.

-Veterans Group Life Insurance. Veterans Group Life Insurance is insurance the retiree can elect to continue after retirement. To report death or check if retiree is insured, call (800) 419-1473 or write to Office of Servicemen’s Group Life Insurance (SGLI) 290 West Mount Pleasant Avenue, Livingston, NJ 07039. If the policy number is unknown, provide the veteran’s social security number and date of birth/death. If retiree was insured, they will provide the application for benefits to you. Fill out the application and return along with a copy of the retiree’s death certificate.

4. Social Security Contact (800) 772-1213 to find the office nearest you and inquire about benefits. Social security payments may be made to eligible survivors of deceased retirees if survivors meet one of the following requirements.

-Surviving children under age 18 or those 18 or over who become totally disabled before reaching the age of 22.

-Widow(er) of the deceased at any age if he/she has care of a natural or legally adopted child under 16 or a disabled child.

-Widow(er) on reaching age 60.

-Surviving divorced husband or wife at age 60 if he/she was married to the Soldier for at least 10 years.

-Surviving divorced mother or father who is not married and has in care a natural or legally adopted child who is under age 16 and entitled to social security benefits on the Soldier’s record.

-Dependent parents on reaching age 62 if they were receiving at least one-half of their support from the deceased Soldier.

-Apply at the nearest social security office. Contact (800) 772-1213 to find the office nearest you and inquire about benefits. Make sure you have the following documents: Department Defense form 214, Certificate of Release of Discharge from Active Duty, previous year’s Form W2 Wage and Tax Statement, marriage certificate (original or certified copy), divorce papers (if applicable), birth certificates (original or certified copy), and a copy of the death certificate.

-A lump sum death payment of $255 is payable if the deceased, at time of death, was fully or currently insured for social security payments. The lump sum death payment is paid in the following order of priority:

-The widow(er) living in the same household as the deceased at time of death.

-The widow(er) who was eligible for benefits based on the deceased member’s record for the month of death.

-In equal shares to each child who was eligible for benefits based on the deceased member’s record for the month of death.

Contact the nearest social security office or the above number to see if you qualify. Visit the Social Security Office website for more information at: http://www.ssa.gov/.

5. Life Insurance. Gather all life insurance policies. Check safe deposit boxes,

files or papers kept by the retiree. Civilian insurance policies should have a company or agent to contact. Contact the company or agent, report the retiree’s death and ask for instructions on how to apply for the benefit. Usually the death certificate and a claim form or letter with pertinent information on the deceased and beneficiary is needed in order to pay the claim.

6. ID Cards. Survivors eligible for DD Form 1173, Uniformed Services Identification and Privilege Card should have new cards issued under their new status as soon as possible. You will need a copy of the death certificate, marriage certificate, retirement orders and copies of birth certificates for eligible children. Contact the nearest military installation ID card section for details. They service all branches of the Armed Forces. You may address questions to the Fort Knox Identification Card Section at (502) 624-/3473/1667/1626/7428/7616.

7. Funeral Honors. The Fort Knox Casualty Assistance Center is responsible for

providing military funeral honors to eligible deceased Army personnel within the states of Ohio, Indiana, Kentucky (60 counties), Michigan (69 counties) and West Virginia (47 counties). This includes Active duty, Army Reserve, National Guard, retirees and veterans. Full military funeral honors will be provided for all Active Duty Soldiers and Medal of Honor recipients.

a. A full military funeral honors team consists of nine Soldiers who serve as the firing team (also act as pallbearers at the gravesite), an officer or noncommissioned officer in charge, a chaplain (if required, subject to availability), and a bugler to render Taps.

b. Funeral honors for all other Army veterans will consist of two members to fold and present the flag and present render Taps.

MDI Ceremonial Unit of Indiana.

The primary mission(s) of the MDI-CU is to provide Military Funeral Honors (MFH) for any eligible, current or retired (20 years of service), Indiana National Guard (INNG) member for the purpose of preserving the history of the INNG through the demonstration of close order marching; drill and ceremonies; provide escort duties to the Governor and other VIPs as directed; and to provide Military Ceremonial Support at public functions when requested.

The mission of all ARNG State/Territory Military Funeral Honors Programs is to render professional and dignified MFH IAW service tradition, to all eligible Army Service Members, Retirees, and Veterans when requested by an authorized family member or representative as well as to provide or support MFH to other Branch of Service Members, Retirees, and Veterans when requested by the respective Branch of Service Casualty Assistance Center.

-Call the Office of Personnel Management Retirement Information Office at 888-767-6738 for a step by step walk through.

-Complete the appropriate Application for Death Benefits and follow the steps within the PDF

-Attach a copy of the employee’s death certificate and a copy of the certificate of the marriage to the widow or widower. Send the completed application to:

Designation of Beneficiaries - You can choose any person to receive any life insurance and/or lump sum benefit payable upon your death by completing a Designation of Beneficiary form.

9. Death of an Annuitant.

If a person receiving an annuity from a deceased member’s military retirement and he/she dies, report his/her death to Denver Defense Finance Accounting Service at (800) 321-1080. This will stop the annuity pay. Defense Finance and Accounting Service will mail them a claim for any unpaid money due to the designated beneficiary. Complete the form and return to Defense Finance and Accounting Service for payment. Any check received after the date of death will have to be returned to Defense Finance and Accounting Service.

10. Notify any professional organization that the service member was a member of that they have passed and to share information on the service members memorial.
