IMPORTANT INFORMATIONAPPLICABLE TO ALL BENEFITS

Applying to joinServing officers and police staff canapply to join the scheme at any timeby completing a medical underwritingapplication form which is availablefrom the Federation Office. Newstudent officer recruits to the policeservice may join the scheme withoutthe need for the completion of themedical underwriting application formand their first 52 weeks of service arefree of charge. The Trustees and/orPhilip Williams & Co reserve the rightto decline any applications.

Subscription collectionSubscriptions are collected monthlyby deduction direct fromsalary/pension unless that is notpossible when alternativearrangements may be agreed.Monthly subscription payments mustbe maintained in order to remain amember of the schemeand to qualify for any benefits.

InsurersA list of the insurers is available onrequest. Full policy wordings forcertain policy sections are available todownload via QR codes. These mayalso be available at the FederationOffice, on the Federation Website orby visiting the Group Scheme sectionof www.philipwilliams.co.uk

How to cancel your coverIn the event that you need to cancelyour cover please contact theFederation Office.

Cohabiting Partner ExtensionsAny cohabiting partner extension willcease when the Serving member orcohabiting partner reach 65 years ofage (70 years for retired members),whichever is the sooner. Anycohabiting partner cover and/orextension will cease when themember ceases to be a memberof the scheme.

Career breaks, maternityleave, secondment or livingoverseasThose going on a career break,maternity leave, secondment or areliving overseas must contact theFederation Office to identify if covercan be maintained.

Retirement fromthe Police ServiceServing officers upon retirement mayremain in the scheme as a retiredmember by submitting a completedmembership continuation form to theFederation Office prior to retirement.

Individuals are not eligible to join thescheme after their retirement date.

Transfer, resignationor dismissalMembers who transfer, resign or aredismissed from the police service arenot eligible to remain in the schemeand all membership and benefits willcease including any cohabitingpartner extension.

Complaints procedureThis Insurance Scheme is arranged on behalf of the trusteesby Philip Williams and Company who are authorised and regulated by the FinancialConduct Authority (Registration Number 308860). The trustees are responsiblefor organising the policies and dealing with the insurance broker. Any complaintsabout any aspect of the scheme should in the first instance be directed to theFederation Office. The insurance broker will then be asked to investigate thecomplaint and resolve any matter either via the Federation Office, directly withthe member, or through the appropriate underwriting organisation.

FINANCIAL SERVICES COMPENSATION SCHEMEIn the event that an insurer is unable to pay a claim you may be entitled tocompensation from the Financial Services Compensation Scheme if an insurercannot meet its obligations. This depends on the type of insurance and thecircumstances of the claim. Information can be obtained on request, or by visitingthe Financial Services Compensation Scheme website at www.fscs.org.uk

This booklet is a summary only. The benefits are subject to certain exclusions and policy conditions notstated in this booklet which in the event of a claim will be applicable. The benefits are provided strictly underthe terms of the insurance policies taken out by the trustees of the scheme. Subscription to the schemeentitles the member to the benefits provided by the scheme but confers no ownership of any of theunderlying policies which are vested in the trustees. The Insurance Scheme is an Independent Trustmanaged by the Trustees. The Trustees pay the Federation for the Federation Staff/Office to administer theInsurance Scheme on their behalf.The trustees retain the full policy wording detailing all benefits andexclusions which may be viewed on request to the Federation Office.

EXPLANATION OF BENEFITS

Life insuranceOn death of a member or subscribing cohabiting partnerthe cash benefit will be paid. The policy is written inTrust so that if a member dies, the proceeds can be paid,by the Trustees, to the member's dependants quicklyfree of tax and without having to wait for probate.Members should ensure that they have an up-to-datebeneficiary nominated and have lodged the details withthe Federation Office, to assist the Trustees in the eventof a claim. If a member aged 63 or under receives aterminal prognosis of 12 months or less, they may applyto the Trustees for an advance of the death benefit of20% of the relevant sum insured.

Child Death GrantThis benefit is paid upon the death of a dependant childof a member, aged between six months and 17 years.

Permanent total disablement (any occupation)Payable when a member is unable to performany gainful employment and is unable to existindependently, requiring continual supervision foractivities of daily living, as a result of an accident.

Permanent total disablement(usual occupation as a Police Officer)Permanent Total Disablement such that the Memberis unable to perform their usual occupation as aPolice Officer

Accidental Loss of Use and Permanent DisablingInjuries Benefit Variable Benefits paid in the event of:- - Loss of sight in one or both eyes- Loss of use of one or more limbs- Total loss of hearing in one or both ears- Loss of Speech- Temporary Total Disablement

Unsociable Hours BenefitIf, as a result of a period of Temporary TotalDisablement, a Benefit Participant suffers a loss ofUnsociable Working Hours Pay, we will pay 7.5% of thebasic scale hourly rate that would otherwise have beenreceived, up to the following maximum benefits: Constables £60 per weekSergeants £75 per weekInspectors £95 per weekChief Inspectors £95 per week

Policy will not pay for scheduled unsociable hours forthe first 14 days of each period of disablement and ispayable for up to a maximum of 24 weeks.

On-Duty Assault benefit as a Result ofFirearms or KnivesIf a serving police officer sustains accidental bodilyinjury in the course of duty directly caused by thedischarge of either firearms or shotguns or caused byassault involving a knife and as a consequence of theinjuries is unable to work for a period of at least 14consecutive days immediately after the attack thepolicy will pay: (a) £1,500 as a result of Firearm(b) £750 as a result of knife/sharp instrument injuries

Dental injury and EmergencyThis policy provides cover for Dental Expenses in theevent of:- Dental Injury- Emergency Dental Treatment- Dentist Call-out Fees- Hospitalisation- Mouth CancerDefinitions are shown in the full policy wording.

Making a Dental ClaimNo prior authorisation is required. Undergo thetreatment, pay the dentist direct, and subsequentlysubmit a claim. Please ensure that proof of treatmentand receipts of costs are obtained. Claim forms areavailable from the Federation Office.

Sick Pay Benefit (Regulation 28 cover)If a member suffers a pay cut under regulations orterms of employment, the benefits illustrated on thescheme benefits table will become payable after 26weeks absence.

The benefit ceases on return to work or if the memberretires, resigns, is discharged from the police service,or fails to pay the monthly scheme subscription. Inaddition the benefit shall not be payable if the memberhas been offered reasonable adjusted duties with areturn to full pay and has declined such duties withoutreasonable cause.

Any overpayment due to a reversal in the decision bythe employer which results in a resumption of pay(including any back payment) or due to a failure by themember to inform the insurers of a return to workmust be repaid in a prompt and timely manner.

The benefit will be payable if a member, member's subscribing cohabiting partner or their dependantchild, aged from 30 days to 17 years, suffers from an insured illness and survives for more than 14days from the date of diagnosis or surgery.

Please refer to the policy wording for full definitions of the illnesses covered.

A pre-existing conditions exclusion applies together with other terms and conditions.

For an individual insured person Critical Illness Benefit ispayable once only in respect of conditions in a commongroup. For this purpose certain illnesses belong to morethan one group.

Your Personal Nurse Adviser will be able to assist with:- Help in understanding the condition and all its implications- Explain options for treatment- Help to prepare for consultants appointment- Explain medical terms in simple everyday language- Help families to cope with consequences of illness- Information and advice on home adaptations/specialist equipment- Source suitable equipment & medical aids- Provide information on entitlements from NHS- Helping you to make as speedy a recovery as possible by providing suitable information and helping to navigate into the appropriate services- Provide good long term management of your condition- Simply a listening ear when you are feeling low, or want to talk to someone who is not emotionally involved

At the discretion of the nurse, when clinically necessary in certain circumstances they may be able to arrange extra help.

It covers travel worldwide and inthe United Kingdom.

Your Cover If a Vehicle in which you or your cohabiting partner aretravelling suffers a Breakdown due to a mechanical orelectrical failure, flat tyre, lack of fuel, misfuel, Accident,theft, vandalism or fire, service will be provided. We willprovide cover for any Breakdown in accordance with thepolicy wording.

Covered Vehicle The car, motorcycle, campervan, motorhome (includingdomestic vans up to 3.5 tonnes), or car-derived vanwhich You or your cohabiting partner are travelling in/on at the time of the Breakdown including any towedcaravan or trailer of a proprietary make which is fittedwith a standard towing hitch and doesn’t exceed7 metres/23 feet (not including the length of the A-frame and hitch).

Claims will be validated with the Police Federation, orother such administrative organisation, with costs forclaims from non-eligible persons being the responsi-bility of the claimant.

Cover will apply during the period of insurance andwithin the territorial limits. There is no age limit to thevehicle for breakdowns occurring in the UK. A vehicleage limit of ten years applies outside the UK.

Your mobile phone is covered against the repair orreplacement cost of your mobile phone in the event of:- theft- accidental loss - accidental damage- breakdown whilst in your possession

The liability of the insurer in respect of any one claimand in aggregate for any 12 month period will be therepair or replacement cost of the mobile phone and inany event shall not exceed a total claim cost up to amaximum of £1000 including VAT.

A £75 excess is payable per claim.

The mobile phone should have a fully functioning SIMcard and be no more than 8 years old at the time of theincident as evidenced by the relevant proof ofownership. Upon acceptance of a claim, the insurer mayat its discretion repair or replace your mobile phone.Replacement may be with a refurbished unit or afunctionally equivalent product. This is NOT a new forold policy.

If the claims administrator replaces your mobile phone,your original item becomes the insurer’s property andthe replacement mobile phone is your property, withcoverage for that item continuing for the remainingperiod of insurance.

The maximum liability limit includes cover of up to £750including VAT for unauthorised data usage for a periodof up to 24 hour directly following a valid theft oraccidental loss claim.

Please note any claim involving theft or accidental lossmust be reported to the appropriate police authoritieswithin 48 hours.

In addition to proof of ownership, verification ofmembership will be required before a claim can beprogressed.
