    Welcome Message

We wish to welcome all Surgical Trainees together with all members o the Division o Surgery to

the General Surgery Education Program or the year 2012. This program is aimed at providing a

comprehensive series o tutorials, seminars and journal club sessions to Surgical Trainees at all levels.

It also provides participation to all members o the Division o Surgery through the Surgical Forumsand the Annual Surgical Unit Audits. The strength o the program and its success over the past eight

years has been predominantly due to the commitment and dedication by the members o the Division

o Surgery who have volunteered their valuable time or the benet o our trainees.

The program continues to evolve each year based on the general attendance, impact and eedback

received rom both trainees and consultants. A number o changes have been incorporated in to the

program or 2012 to reduce the demand on trainees and surgeons while concentrating the teaching in

to manageable segments. The Saturday Surgical Seminars held once a month on Saturday aternoons

have been an outstanding success with record attendances by trainees and have been open to all

surgical trainees in Victoria. Another notable success was the new SET1 reresher course or trainees

preparing or the surgical sciences and clinical examinations. The program will remain concentrated

    between April and November 2012.

Some o the changes to the Surgical Education program or 2012 will demand an increased and pro-

active role by trainees in successully implementing it. This includes a greater role in dening the

content o the Journal Clubs as well as more emphasis on the Trainee Education Co-ordinator to

ensure a seamless progression o educational activities.

On Friday the 3rd o December 2011 the third Annual Austin Surgical Trainee Dinner was hosted by

the trainees to demonstrate their appreciation o the time and eort imparted to them by the Surgeons

and Fellows o the Austin Hospital. This will be held in 2012 at the same venue on Saturday the 1st o

December. Finally, the Austin Research Prize will be held on Tuesday the 27th o December 2012 and

we encourage all surgical trainees to participate.

We wish to emphasize that the program is open to anyone who is interested rom the Austin and

other centres, including all Surgical Trainees, Unaccredited Registrars, Surgical HMOs, Interns and

International Medical Graduates. The success o this program is dependent on active participation

rom all Surgical Trainees and sta members.

Thank you to all the members o the Division who participate in the surgical education program and we

look orward to another successul year.

INTRODUCTION: All Surgical Trainees are expcted to participate in the Surgical Education

Program. This program provides a continuous series o educational activities or trainees at

SURGICAL TRAINEE REPRESENTATIVE: Two trainee representatives based at the Austin

Hospital are chosen (usually SET4 trainees) to co-ordinate the Surgical Education Program. It

will be the trainee representatives responsibility to communicate with the relevant tutors in

advance to conrm the sessions or make alternative arrangements i necessary. One trainee

would be the designated contact person or SET2+ trainees while the other designated to

cordinate the SET1 and NSET trainees.

ATTENDANCE: Attendance is MANDATORY or all SET trainees based at the Austin Hospital.

All trainees including those on regional rotations are expected to attend the Saturday Surgical

Seminars. Surgical HMOs and Interns are also strongly encouraged to participate.

ABSENCE: Trainees who are unable to attend a particular session are expected to inorm the

    General Surgery Trainee Representatives

Since their inception in 2009 the General Surgery Trainee Representatives have

played a major role in liaising between trainees and their teachers and ensuring the

smooth running o the Surgical Education program. Given the increasing number o

trainees based at Austin Hub two trainees will be assigned the role to be sharedbetween them. The trainee reps will also play an increasing role in determining the

content o the sessions, in particular they will be instrumental in the running o the

    journal clubs.

In addition to their role in coordinating the education sessions they will also mentor

junior surgical trainees and act as a link between trainees and their respective super-

    visors.

    WEDNESDAY

    Surgical Forum

    Time: 0730 0830 (Light Breakast at 0715)

    Day: Wednesday

Venue: Lecture Theatre, Dept o Surgery, Level 8, Lance Townsend Building

Content: The Surgical Forum will be conducted in two parts as described below:

    INVITED LECTURES (February June)

This will be conducted in the rst hal o the year rom the 15th February to the 27th June.

Speakers have been invited by the department to deliver clinical updates on current topics.

A mix o internal and external speakers havs been chosen based on their expertise.

SURGICAL UNIT ANNUAL AUDITS (July November)

This will be conducted in the second hal o the year rom the 4th July to the 21st November.

Each unit within the Division o Surgery is allocated a single day or a presentation o their

    Annual Surgical Audit.

The Austin Surgery Annual Surgical Audits provide an opportunity or all surgical units to

present their annual activity, trends, outcomes and benchmarking. The unit members are

encouraged to present the gures or the unit activity and the general and specic morbidity

and mortality rates or the previous 12 month period. The morbidity should preerably relate

to both the operation and the indication i possible (criteria auditing). All deaths should belisted. Detailed discussion o complications and deaths should be avoided as this would

have been perormed at the weekly surgical audits. However, instances where morbidity

or mortality had resulted in a change in practice should be highlighted and discussed i

necessary. I available, other data on key perormance indicators and comparative data rom

other comparable hospitals would be appreciated. The duration o the audit meeting should

    be approximately 45 minutes.

All units are expected to supply an electronic version o the audit to the Department o

    Surgery archives. (See Kim McAughtry)

It would be ideal i the heads o units would encourage other participating sta to attend

(apart rom the unit members) to their unit audits. This may include ward, theatre and

anaesthetic nursing sta, as well as sta rom other departments such as physiotherapy,

nutrition, etc. In addition, members o similar units rom other Melbourne hospitals may be

    invited and encouraged to attend.

The Surgical Foum (Wednesday 7.30 - 8.30am) now orms part o the protected teachingtime or all surgical HMOs at Austin and thus mandatory attendance is expected.

    Dreams can still come true

The Australian dream o buying your own home is getting harder as house

aordability puts it out o reach.

When it comes to mortgages, escalating prop-

erty prices has meant borrowers need to stump

up more cash as a deposit. Many home buyers

are orced to spend many years saving beore

they have a big enough deposit to buy their own

    home.

To put this in perspective, say a home buyer

plans to purchase a residential property at the

median house price o $600,000. On top o adeposit, mortgage insurance and stamp duty

can add another $20,000 to $40,000 to a prop-

    erty purchase o this value.

To avoid mortgage insurance, many banks re-

quire a buyer to pay a 20 per cent deposit plus

another 5 per cent to cover costs. For the me-

dian house price o $600,000 this equates to

    $150,000, which is a lot o money.

Some banks add the mortgage insurance to

your loan i you dont have the cash upront. This

is problematic because you end up paying more

in interest over the lie o the loan.

To overcome this roadblock to home ownership,

Investec Medical Finance has created innova-

tive lending products, specically designed or

doctors and others in the medical proessions.

Unlike other banks, Investec recognises thatdoctors have a lower risk prole than the aver-

age borrower with a steady earning capacity

    across the economic cycle.

During times o market turmoil, some lenders

have been orced to tighten their lending crite-

ria and decrease the amount they would lend

against the value o the property, which means

they require an even bigger deposit.

As a specialist bank, Investec has the ability to

maintain a consistent credit policy that doesnt

compromise our clients opportunity to borrow

and create wealth - and invest in property.

Investec continues to buck the trend, oering

up to 100 per cent mortgage nance or quali-

ed medical proessionals intending to occupy

the residence, and up to 95 per cent or a resi-

dential investment property without LendersMortgage Insurance.

With steady interest rates and a slowing o

property price growth, now might present an

opportunity to put a plan in place to get your

    oot in the door.

We have a range o nancial options created es-

pecially or medical proessionals. Contact Pen-

The Research Higher Degree program has

granted me the opportunity to take a step back

rom the demanding cut and thrust lie o a busy

surgical registrar, and at the same time enabled

me to urther my career. The experience has

taught me to think analytically and critically,

and I have learnt valuable lessons in study

designs, executions and result preparation or

publication. No doubt the knowledge gained

will aid me to be a better surgeon.

Surgery and Scientic Research: Why do it?

Research Opportunities & The Academic Research Program

Research activity has progressively increased in importance to the surgical trainee over

the past years as emphasis has increased on a scientic approach to surgical problems.

Increasing demands are made on trainees to conduct, present and publish on surgical

research not only as a component o their accredited training but also to increase their

career opportunities and successul applications or uture ellowships. The Department o

Surgery continues to strongly support research undertaken by trainees either as ull time

research higher degrees or on a smaller scale during their clinical tenure.

Trainees at all levels rom HMOs to advanced SET trainees are encouraged to seek

opportunities early in the year to begin work on at least one research project. I you have

a particular interest in a subspecialty, then eel ree to approach members o that unit ora potential research project. I not, the Department o Surgery will be able to provide a

number o options or small, relevant clinical studies based on our ongoing prospective data

collection service. The aim would be to present the results at the Austin Research Prize

presentation and subsequently to have it published in a relevant peer reviewed journal.

Research or Surgical Trainees: Surgical trainees interested in an academic career will

have strong backing rom the department to enrol ull time in one o the Research Higher

Degrees. In addition we also support all surgical registrars, in both accredited and un-

accredited positions, to undertake serious research concurrent with their clinical work. These

are mostly small project based clinical studies involving data rom our clinical databases,

hospital medical records and where necessary, tissue rom the Victorian Tissue Bank.

Expectations and Outcomes o Research: The departmentalphilosophy supports the timely completion o all research

undertaken by providing the necessary acilities, mentorship,

technical expertise and unding support. In general everyone

who undertakes research activities in the department will

be expected to present their results at local, national or

international meetings and be strongly encouraged to publish

in peer reviewed journals. All ull time research students are

expected to present at the annual Austin Medical Research

Week while clinical research undertaken by surgical trainees

and junior hospital doctors will be encouraged to be presented

at the annual Austin Surgery Research prize.

Research Higher Degree (RHD) Program: The Department o Surgery strongly encourages

the undertaking o RHD by research (PhD, MD M Surg and MSc). The main ocus o such

research is related to hepatobiliary and pancreatic disorders, organ transplantation and

regulatory peptides. Students rom both the medical and biomedical elds are welcome.

There is also strong international interest rom regional centres in this program leading to

international post doctoral students undertaking this course.

Currently active research projects include the treatment o liver metastases by vascular

disrupting agents, macromolecular drugs and hyperbaric oxygen, the role o liver

regeneration in tumour growth, the role o microcirculatory disturbances and hyperbaric

oxygen in severe acute pancreatitis, xenotransplantation and the role o regulatory peptides

in the development o liver, colon, prostate and kidney tumours. In vivo and in vitro models

or each o these projects are well established in the Department. Clinical material and

tumour banks are extensively used. Details are available in our Pathways to Discovery

booklet which can be obtained rom the oce or downloaded rom our web site.

Research or Clinicians: The philosophy in the Department o Surgery encourages scientic

research at all levels in clinical, basic science and translational aspects. We also endeavour

to establish strong links with other departments to develop collaborative research including

the Peter McCallum Institute, and clinical departments such as Anaesthesia, Radiology,

Oncology and Intensive Care. Detailed surgical databases have been developed or both

general surgery and specialist HPB disorders. Data collected within these provide a solid

base or developing short term clinical studies. All such projects will aim to have the results

published in a peer reviewed clinical journal within a year o completion.

Research or Junior Hospital Doctors: Recognizing the need or small project based clinical

studies or junior hospital medical ocers (Interns, HMO 2 and 3) we oer a range o clinical

and mixed projects. These are available to anyone interested in developing a uture career

in surgery. They are structured to be completed within an year to enable a ull time junior

doctor to complete without enrolling or a ormal research degree.
