    Q&ABy AnDrEA MOriAn

Q: My mothers doctor has said that she

may forget to swallow over time. How can that

    happen?

A: Your concern is a common one, thoughforgetting to swallow is not entirely accurate.

Over time, the myelin that insulates the

neurons in the brain degrades and this lead to a

loss of information as it is conducted from one

area of the brain to another. Tis is why people

have memory loss, difficulty finding their

words and, also, trouble swallowing with many

    chronic or progressive neurological diseases..

he information that helps a person to

swallow is just not conducted eciently and

eectively in some people over time. Tis can

lead to food and liquid entering the airway

and lungs instead of the esophagus. As this

occurs over time, it can lead to aspiration

pneumonia. While that is the rather simplied

version of the cause of a swallowing problem,

the things that this problem leads to are of

much greater concern to most families. For

some, they, unfortunately, do not learn of this

possibility until a family member admits to the

hospital with aspiration pneumonia. Te most

important thing to know is that long before

that happens, your family member has options

and an advance directive can be a powerful

tool to help your family exercise their wishes

even after the disease process has progressed.

Before choosing one option or another, you

can speak with a speech-language pathologist

who is an expert in swallowing disorders. She

should be able to direct you to the most current

evidence and explain the options available to

you and your familys specic situation.

Ask the ExpertWhat is a good caregiver to resident ratio and whyBy JACQuELin hiLTOn, rn

Sundance uses a 6:1 ratio on the morning shift and 7:1 for the remaind

of the day. We simply feel that the typical nursing home ratio of 12:1

overwhelming for both the caregivers and the residents. With a low ratio w

are able to provide them with a more intimate care environment, addressi

personal, social, medical, & family connections. For example, our caregive

dine with our residents to accomplish goals such as assessing eating patter

and any changes to those patterns. In a setting where the ratio is higher, th

are not allowed the time to sit and visit with the residents and observe the

crucial changes. Here at Sundance we have a standard of excellence th

each of us strive to uphold to provide the best possible quality of life for o

    residents.
