As we have responsibilities to help you with your training, there are certain responsibilities and obligations that you have as well. This Student Policy Handbook is offered to you as a guide to explain what is expected of you, and your expectations of administration while in attendance at River Valley Cosmetology Institute. These policies are to be adhered to whenever and wherever you are a representation of the River Valley Cosmetology Institute. The Administration, Faculty and Staff

Certified true and correct in content and policy. RIVER VALLEY COSMETOLOGY INSTITUTE LLC River Valley Cosmetology Institute LLC is licensed by the Oklahoma State Board of Cosmetology. We offer an educated staff that can assist the community with instruction, enrollment, and placement. River Valley Cosmetology Institute LLC is approved by the Choctaw Nation Tribe of Oklahoma for the training of students in Cosmetology. We are also approved for training with the Oklahoma Department of Rehabilitation Services.

The main campus of River Valley Cosmetology Institute LLC is located in Poteau Oklahoma, a 5,500 square foot building, accommodating as many as 130 students. The facility consists of one clinic area, one phase 1, 24 workstations, 3 class areas, 4 private facial/wax areas, 1 nail and pedicure clinic, lounge, kitchen, 3 restrooms, a library, and a computer lab, an admissions, advisement and financial aid office. Programs Offered: Basic Cosmetology-1500 HOURS Cosmetologist can work with hair, nails, and facial aspects of the business as a contracted worker all the way to a business owner. Manicurist- 600 HOURS Manicurists works with hands and nails becoming an educated individual of nails and their disorders along with the building of artificial nails. Esthetician- 600 HOURS Estheticians work with the skin and face with a career that includes facials, hair removal, and becoming educated individuals of the organ, skin Master Instructor-1000 HOURS Becomes educated in the art of education of Cosmetology students, can become an educator or school owner. Mater Instructor with Credit Hours- 500 HOURS Required of Cosmetologist that have held a license and practiced Cosmetology for 2 or more years to become a Master Instructor Review Course – 250 HOURS Required of Cosmetologist that have held a license of Cosmetology and have allowed those license to expire for more than a year. School Calendar Students may begin classes monthly River Valley Cosmetology Institute LLC will be closed all legal holidays. And 2 weeks for Christmas which will be posted in advance. All students are required to be in attendance the day before and the day following any and all holidays or student will be subject to suspension for a maximum of 3 days (See suspension policy). In the event that the absence may be excused (See attendance policy excused absence) the student must request it in writing to be reviewed and final decision will be at the discretion of the School Director. Students will be notified by television media of school closing due to inclement weather. If Local Public Schools are closed due to weather, so are we. Media sources include Channel 5 KFSM Fort Smith, Arkansas. Admission Requirements 1. Student must be at least 16 years of age by November 1st of the current year. 2. Proof of age through one of the following: Driver’s license or Birth Certificate. 3. Student must submit proof of a high school diploma or its equivalent; a state-issued credential from secondary school completion of homeschooled. River Valley Cosmetology Institute LLC does not administer the Ability to Benefit Testing. In order to participate in the Title IV programs a student must have a high school diploma, GED or transcript with proof of completion. River Valley Cosmetology does not participate in Title IV funding at this time. 4. 2x3 full face photograph to accompany registration for Oklahoma State Board of Cosmetology. 5. $5.00 money order to register with the Oklahoma State Board of Cosmetology. 6. A Basic Cosmetology License is required for the instructor’s course. Transfer Student Students transferring to another cosmetology school may request a written release from River Valley Cosmetology Institute LLC, as long as the Student’s financial agreement has been met by payment or payment arrangements.

Twenty-four (24) hours per week is required to stay eligible for the River Valley Cosmetology Institute LLC. The fees are listed both in the handbook and on the enrollment contract, note: All monies received are applied in the following order: Application Fee, Kit, Books, Tuition. (Application, Kit and Book fees are non-refundable items once they have been issued. Scholarships and/or tuition waivers may be available at the discretion of the administration. At the discretion of the administration terms are made available for monthly payments. A monthly payment contract will require a down payment, amount determined by the administration. The Basic cosmetology Course requires 1500 clock hours to complete. The Nail Technician and Esthetician Courses require 600 clock hours to complete. The master Instructor Course requires 1000 clock hours to complete unless 500 hours have been credited and approved for study by the Oklahoma State Board of Cosmetology. Students are allowed to exceed the contracted hours by 10% of the time allowed from the start date of the course. Scheduled days of closure are inclusive. (Example: Basic Cosmetology students require 1500 clock hours. 150% of 1500 is 750. 750 hours are allowed for overage, which means the student must finish within 2250 hours to meet the contract requirements.) Students who are in school over the contracted time allowed will be required to pay by the hourly at a rate of $10.00 per hour for all courses, until the course is complete. Students are required to pay tuition in full before 1500 hours are clocked. At 1000 hours students will no longer be eligible to clock hours until tuition balance is current. After tuition balance is current, the student will be allowed to continue classes for the remainder hours needed until 1400 hours are clocked. At that point tuition is to be paid in full before the remaining 100 hours are clocked. After tuition is paid in full the student will then be allowed to clock the remaining hours and State Board Testing can be conducted. Expendable Supplies River Valley Cosmetology Institute LLC furnishes all expendable supplies to the student, excluding supplies for personal use. Students are charged for the use of personal expendable supplies at a reduced rate. Uniforms Students must look professional. Hair and makeup must be done and presentable upon arrival. Students are graded on professional appearance and may be sent home for not complying. Assigned uniform must be worn at all times during classes. Smock must be worn at all times in the building. Oklahoma State Board of Cosmetology state: “No hours shall be credited to any student not in uniform during classroom or clinic practical practice training. 175: 10-3-64. School uniform/dress code. It shall be the responsibility of the managing cosmetology instructor of a cosmetology school to dismiss any student not in compliance with uniform dress code from clinic or theory class until such time as student is in Board approved

uniform dress code.” Closed toe, low heeled rubber soled shoes are permitted. No mid-drift showing and no low cut shirts allowed. Conduct River Valley Cosmetology Institute LLC strives to treat all students on an adult level. Our goal is to help you become employable; we expect the same respect and courtesies you would give on the job. The habits you form here in school are habits that will remain in the work force. Students must have behavior appropriate to a work place environment. Instructors and/or staff members will be addressed “Ms.” or “Mr.”, and shown the respect you would show an employer. You will also be expected to do your part in keeping the facility and grounds clean and attractive. Discipline should never be an issue as long as you take full advantage of the educational opportunities we have to offer. Always, treat others as you would like to be treated. Safety is always a concern to everyone; therefore strict rules will need to be adhered to, so that your safety and other’s is ensured. Measures will be taken to remove any student who persists in wasting time or endangering other students. Your instructor will review safety practices and guidelines that will apply to your course of study and give you safety exams to insure your understanding. Applying these on a daily basis will enable you to function in the school safely and without constant supervision. Lost and Found Any item found in and around the River Valley Cosmetology Institute LLC campus should be turned into the office where the rightful owner can describe and collect property. Visitors Visitors are only allowed upon approval from your instructor. All visitors must notify the front desk and/or office of their presence in the school first. No loitering is allowed during class hours. No children are allowed to remain in the school until the end of day; all students are required to have prearranged child care in order to attend school. Job Placement Assistance will be given to all qualified students toward job placement. This service does not assure employment, but through instructor contacts and board committees many students will be placed. Students will not be recommended to the industry if they are not qualified. Release of Student Records In accordance with the Family Educational Rights and Privacy Act of 1974, students and parents or guardians of dependent minors have the right to inspect educational records of a student. River Valley Cosmetology Institute LLC requires that you make a request in writing for the copies of your file content and sign a designated form stating that you received your file content upon release. Forms are available in the office for convenience. No third party is allowed to release or view student records without written notarized permission of the student. A copy of the aforementioned act is available though request in the Administration Department as well as being posted for your viewing. Record Retention All student records are kept for a period of three years. All records can be accessed by the student upon request to the admissions director. Material Safety Data Sheets MSDS sheets on all chemical products used in the school are readily available to all students for your safety. Make yourself familiar with these items. Searches

Upon reasonable suspicion, River Valley Cosmetology Institute LLC has the right to search any and all property on the premises and/or request a search of property by the Police Department. This includes and is not limited to a student’s belongings and vehicles in our parking lot. Course Change In the case of course change, the transfer of hours is at the discretion of the Oklahoma State Board of Cosmetology. Suspension Policy We hope that suspension is a situation that we can avoid at all times however in the event of suspension the student will be financially responsible for any days missed. All students are charged $10.00 per hour. Payment must be received before the student is allowed. Equipment, Books, and Tools Students at River Valley Cosmetology Institute LLC will be working with expensive equipment. Part of your training is how to care for and respect that equipment. Students will be expected to keep from excessive wear on all equipment, books, tools and classrooms that are used. Any equipment damaged or broken needs to be reported immediately for removal or repair. Students who willfully destroy or vandalize school property will be required to pay for loss and damages, in addition to suspension or expulsion. We take pride in the equipment and supplies we have to offer and hope that you will accept ownership as well and help maintain the equipment.

FAMILY EDUCATION RIGHTS AND PRIVACY ACT OF 1974 (FERPA)

The FERPA is a Federal law designed to protect the privacy of a student's educational records. The law applies to all schools which receive funds under an applicable program from the U.S. Department of Education. The FERPA gives certain rights to parents regarding their children's education records. These rights transfer to the student or former student who has reached the age of 18 or is attending any school beyond the high school level. Students or former students to whom the rights have transferred are called eligible students. Parents or eligible students have the right to inspect and review all of the student's education records maintained by the school. Schools are not required to provide copies of materials in education records unless, for reasons such as great distance, it is impossible for parents or eligible students to inspect the records personally. The school may charge a fee for copies. Parents and eligible students have the right to request that a school correct the records believed to be inaccurate or misleading. If school refuses to change the records, the parent or eligible student then has the right to a formal hearing. After the hearing, if the school still refuses the correction, the parent or eligible student has the right to place a statement in the records commenting on the contested Information in the records.

Generally, the school must have written permission from the parent or eligible student before releasing any information from a student's record. However, the law allows schools to disclose records, without consent, to the following parties: School employees who have a need-to-know Other schools to which a student is transferring Parents when a student over 18 is still dependent Certain government officials in order to carry out lawful functions Appropriate parties in connection with financial aid to a student Organizations doing certain studies for the school Accrediting organizations Individuals who have obtained court orders or subpoenas Persons who need to know in cases of health and safety emergencies State and local authorities to whom disclosure is required by the state laws adopted before November 19, 1974 Schools may also disclose, without consent, directory-type Information such as a student's name, address, telephone number, date and place of birth, honors and awards, and dates of attendance. However, the school must tell parents and students of the Information that is designated as directory information and provide a reasonable amount of time to allow the parent of eligible student to request the school not to disclose that information about them. Schools must notify parents and eligible students of their rights under this law. The actual means of notification (special letter, inclusion in a PTA bulletin, student handbook, or newspaper article) is left to each school. Schools must adopt a written policy about complying with the FERPA. Schools must give the parent or eligible student a copy of this policy, on request. If you wish to see your child's education records, or if you· are over 18 or attending college and would like to see your records, you should contact the school for the procedure to follow. If you have any questions about the FERPA, or if you have problems in securing your rights under this Act, you may call.(202) 401-2057 or write to, Family Policy Compliance Office, Department of Education, 400 Maryland Avenue, S.W.,and Room 3017 Washington, DC 20202-4605. Internal Student Complaint/Grievance Policy River Valley Cosmetology Institute, LLC requests that all students or instructors first try to resolve all disputes between each other in an adult and kind manner. If mediation is needed the Master Instructor should be notified and arrangements will be made to resolve the situation. A student, instructor, or interested party may file a complaint against the school in writing and must outline the allegation or nature of the complaint. Upon receipt of the complaint the Director and Staff will review the complaint and provide written response within 10 school days. The decision of the Director and Staff will be final. If the complaint cannot be resolved between the complainant and the River Valley Cosmetology Institute, LLC, the complainant is directed to file in writing first with the licensing agency: Oklahoma State Board of Cosmetology, 2401 NW 23rd Street, Suite 84, Oklahoma City, Oklahoma 73107 (405-521-2441). Then secondly with the accrediting agency: National Accrediting Commission of Cosmetology Arts & Sciences, 4401 Ford Avenue, Suite 1300, Alexandria, Virginia 22302 (703-600-7600). Forms and filing are provided online at either website. Break Rules Break times will be strictly enforced. Day class breaks for 15 minutes in the morning, 30 minutes for lunch, and 15 minutes in the afternoon. Vending and soda machines are made available to the students. Students will keep all trash picked up from the lounge area and tables clean. Working together we can all have a clean and relaxing place to take a break. Lunches are assigned on the books by the instructor. Any student that leaves the building for lunch or to get lunch must clock out due to liability and insurance as well as assurance purposes. All students must notify the instructor before leaving the facility. Use of Tobacco on School Property

Smoking is only allowed in the designated area (behind the building) during designated break times. All cigarette waste will be kept off the grounds and put in the buckets provided for their disposal. Any conflict with these rules will result in the elimination of smoking on school property. Trash is not to be thrown in the smoking buckets. This is a fire hazard. Smoking on school property is considered a privilege. TELEPHONE School phones are for business purposes only. On all calls for students (except emergencies) a message will be taken and returned on break. The use of cell phones is acceptable; however excessive use of cell phones may result in disciplinary actions up to suspension. RECORD OF ATTENDANCE Attendance is tracked through daily sign-in/sign-out sheets. Each student will clock in using the thumb-print clocking system, each student is responsible for clocking in of a morning out for lunch or any other reason the student may leave the building, and out for end of day. As a backup, a time sheet with the student's name is used for general attendance records. Attendance records are kept in a permanent file by the school. Any records promulgated by the Oklahoma State Board of Cosmetology will also be kept by the school. Hours will be calculated monthly and handed out to each student. The student has five days to dispute any discrepancies in the calculated hours. ATTTENDANCE POLICY 1. Students are expected to attend school Monday- Friday 9:00 a.m. - 5.30p.m. Students are required a 30 minute lunch period daily, assigned by the instructor. Lunch period will be staggered. Lunch times may vary depending on clinic activity. Students that do not obey this policy will be clocked out immediately and sent home. 2. Students are expected to check in by 8 :45 a.m. and be in class before 9:00 a.m. for class. If a student arrives after 9:05a.m. they will not be admitted into the building or allowed to clock in until 10:15 a.m 3. Students are expected to notify the school before roll call in the event of expected tardiness or being absent. Communicate all planned absences or tar dies to your instructor immediately. 4. Students who know in advance that they will need to be absent from school must work out some type of arrangements prior to being absent. 5. Students attending must maintain a 75% attendance rate, under VA Benefits must maintain an 75% attendance rate. 6. Students who are absent for 10 consecutive school days, without making contact to the school by phone, written or person to person as to their reason of absence will be dropped from the course. Any monies due to a student who unofficially withdraws from the school shall be refunded within 45 days of a determination by the school that the student has withdrawn without notice. River Valley Cosmetology Institute LLC Determines unofficial withdrawals by monitoring attendance daily. Enrollment or attendance time is defined as the time elapsed between the actual starting date and the student's date of withdrawal. 7. The Basic Cosmetology Course requires 1500 clock hours to complete, the Manicurist Course requires 600 clock hours to complete, the Esthetics Course requires 600 clock hours to complete, and the Master Instructor Course requires 1000 clock hours to complete. The Master Instructor with Oklahoma State Board credit requires 500 clock hours to complete, and Review Hours required are determined by the Oklahoma State Board of Cosmetology at 250 hours. Students are allowed to exceed the contracted hours by 150% of the time allowed from the start date of the course. Scheduled days of closure are inclusive. (Example: Basic Cosmetology students require 1500 clock hours. 750 hours are allowed for the 150% overage, which means the student must finish within 2250 hours.) Students who are in school over the contracted time allowed will be required to pay by the hour at a rate of $10.00 per hour for all courses. 8. Excused absences are for medical, jury duty, military duty, death in family, court appearances. Any other circumstance must be approved by the Human Resource Director. 6 unexcused absences within one month will constitute a 30 day probation period.

TARDY POLICY Tardiness will be monitored on a basis. Any student who is continuously tardy will be placed on probation as follows; 6 or more instances in one month will constitute 30 day controlled probation. Controlled Probation is defined as any infraction to probation within the 30 days will be an automatic suspension. Leaving school before the day is complete is referred to as Early Clock Out. 4 unexcused early clock outs will constitute 30 day controlled probation. LEAVE OF ABSENCE One leave of absence will be granted during an enrollment period. A leave of absence cannot exceed 90 days in a twelve month cycle and is only granted at the discretion of the CEO. A minimum time off of school days is required for a leave of absence otherwise determined by the Director. A leave of absence will extend contract period and maximum time allowed by the same number of days in the leave of absence. A student must make written request of leave and reason for leave and mail or fax request to the Corporate Office Attn: April Brown-Huff. INSURANCE It is highly recommended that those students who are not covered by a family insurance and hospitalization plan purchase an accident policy. Information on companies and agents is provided for you in the Administration Department upon request. River Valley Cosmetology Institute owners, staff members, or board members do not assume the responsibility for the payment of hospital or doctor fees. MEDICATON For your safety please list all medications you are taking on your emergency information sheet provided to you by your instructor. All information will be kept confidential and only used to inform a professional in case of injury or accident while attending school GRADING POLICY Students are graded according to the following system: 90-100% Excellent 80-89% Good 75% Satisfactory 74%-Below Unsatisfactory Students are graded on the following: Practical: Professional appearance and conduct, Record Charts Theory: Homework, Workbooks, Tests Evaluations: Practical and Written Final Exam: 75% or higher or repeat until met. A student must maintain a 75% average for satisfactory progress. A final exam is given upon completion of the course. When a student is found guilty of cheating, the instructor will collect the student's paper, mark a zero for the work and result in suspension or expulsion. Home work due dates will be displayed on the board. If a student is absent on the day home work is due arrangements must be made for grading. If a student is tardy on the day home work is due, then it is to be graded as soon as possible or by end of day to receive full credit. If graded on the day after date due the student will only receive half credit. Otherwise zero credit will be recorded in the grade book. To be graded in class: Exam Review Theory Work Book Handouts when issued A student may write out the glossary and definitions, as well as review questions at the end of each chapter and receive 20 points each.

When a chapter requires additional work book out of Practical, that assignment is considered extra credit. NACCAS requires that the Chapter you are on be in plain sight on the board complete with name, number, as well as when homework is due and test to be given. It is the student's responsibility to prepare. GRADUATION REQUIREMENTS Completion of the course, the required clock hours are earned, and a final grade of no less than 75% will earn the student a diploma. The student must be in Satisfactory Progress Status and meet the Financial Agreement in order to graduate. The graduate will then be eligible to register with the State Board of Cosmetology to take the state exam for a license to practice in the state of Oklahoma. LICENSING REQUIREMENTS All graduates are required to pass the Oklahoma State Board of Cosmetology exam to receive a license to practice in the state of Oklahoma. The exams are administered in Oklahoma City, Oklahoma as scheduled by the Oklahoma State Board of Cosmetology. The registration form is completed by the school certifying the graduation requirements are complete. The graduate is responsible for all costs related to the licensing requirements. Test fee is $15.00 and the licensing fee is $ 25.00 made payable to the Oklahoma State Board of Cosmetology. River Valley Cosmetology Institute LLC, information and assistance with testing in other states as well as reciprocity for license through the Director. CANCELLATION AND SETTLEMENT POLICY For applicants who cancel enrollment or students who withdraw from enrollment a fair and equitable settlement will apply. Applicants not accepted by the school shall be refunded all monies paid to the school. If student (or in the case of student under legal age, his/her parent or guardian) cancels the enrollment in writing within three business days of signing the enrollment agreement, all monies collected by the school will be refunded even if the student has begun classes. The “formal cancelation date” will be determined by the postmark on written notification: the date said notification is delivered to the school in person, the date of expulsion by the school, or 30 days after the last day of attendance or the expiration date of an approved Leave of Absence. If a student cancels the enrollment more than three business days after signing the contract but prior to starting classes, a refund of all monies paid to the school less the registration fee in the amount of $100 will be made. For students who enroll and begin classes but withdraw prior to course completion (after three business days of signing the contract), the following schedule of tuition earned by the school applies. In the case of disabling illness or injury, death in the student’s immediate family or other documented mitigating circumstances, a reasonable and fair refund settlement will be made. If permanently closed or no longer offering instruction after a student has enrolled, the school will provide a pro rate refund of tuition to the student or provide course completion through pre-arranged teach out agreement with another institution. If the course is canceled subsequent to a student’s enrollment, the school will either provide a full refund of all monies paid or completion of course at a later time.

This refund policy applies to tuition and fees charged in the enrollment agreement. Other miscellaneous charges the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned school property, ect.) will be calculated separately at the time of withdrawal. All fees are identified in the catalog and in this enrollment agreement. If a Title IV financial aid recipient withdraws prior to course completion, a calculation for return of TIV funds will be completed and any applicable returns by the school shall be paid, as applicable, first to unsubsidized Federal Stafford Student Loan Programs; second to subsidized Federal Stafford Student Loan Program; third to Federal Pell Grant Program; fourth to other Federal, State, private or institutional student financial assistance programs; and last to the student. After all applicable returns to Title IV aid have been made, this refund policy will apply to determine the amount earned by the school and owned by the student. If the student has received personal payments of Title IV aid, he/she may be required to refund the aid to the applicable program. Transfer hours from another institution that are accepted by River Valley Cosmetology Institute, LLC. and Oklahoma State Board of Cosmetology count as both attempted and completed hours for the purpose of determining when the allowable maximum time frame has been exhausted. TERMINATION/DROP POLICY The River Valley Cosmetology Institute LLC may or may not terminate for the following reasons: 1. Students who are found to be stealing. 2. The use or possession of any illegal drugs or alcohol before or during school hours. 3. Physical violence towards any other person on school property. 4. Excessive behavioral problems. 5. Students who have been absent for 10 consecutive school days, without making contact to the school by phone, written, or person to person as to their reason of absence. 6. Students who fail to meet the minimum monthly payment for 90 days required for the River Valley Cosmetology Institute, LLC and River Valley Cosmetology Institute, LLC self pay program. 7. Students bringing lethal weapons on school property, includes parking lot. *This policy also applies to all off campus activities when a student is a representation of the River Valley Cosmetology Institute LLC. * All terminations are subject to an appeal. Other excessive circumstances may be subject to termination at the discretion of the administration.

SATISFACTORY ACADEMIC PROGRSS POLICY Satisfactory Progress in attendance and academic work is a requirement for all students and is consistently applied to all students enrolled at River Valley Cosmetology Institute. Evaluation periods are based on contracted (scheduled) hours of attendance. All students must maintain the required academic and attendance progress. This policy applies to every student enrolled in a NACCAS approved program and is provided to applicants prior to enrollment. This policy applies consistently to all students enrolled in a specific program schedule. It includes both cumulative quantitative and qualitative elements evaluated at designated periods throughout program and includes a maximum time frame no longer than 150% of course length. Students will be notified of any evaluation that impacts eligibility for financial aid. Students have access to their SAP evaluations by meeting with the Director of Education or Director of

Financial Aid, upon signing the evaluation the student will be given a copy along with a copy being placed in the students file. All SAP evaluation reports are made available to the students at the evaluation time and a copy is given to the student upon signing and a copy is placed in the students chart. Transfer hours from another institution that are accepted by River Valley Cosmetology Institute, LLC. and Oklahoma State Board of Cosmetology count as both attempted and completed hours for the purpose of determining when the allowable maximum time frame has been exhausted. Attendance Progress Evaluations Students are required to attend a minimum of 67% of the hours possible based on the applicable attendance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum requirements. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 75% cumulative attendance rate, the student will graduate within the maximum time frame allowed. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory or better. If the performance does not meet satisfactory requirements, it is not counted and

the performance must be repeated. At least two comprehensive practical skills evaluations will be conducted during the course of study. Practical skills are evaluated by the practical skills evaluation set forth by the school. Students are must maintain a written grade average of 75% and pass a FINAL written and practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered according to the following scale: 90-100 A Excellent 80-89 B Good 75-79 C Satisfactory 74-below Unsatisfactory STUDENTS MUST MAINTAIN A 75% AVERAGE IN THEORY AND PRACTICAL/CLINIC WORK IN ORDER TO BE CONSIDERED MAKING SATISFACTORY ACADEMIC PROGRESS. ALL EVALUATIONS REFLECT CUMMULATIVE GRADING IN ATTENDANCE AND ACADEMICS. ALL STUDENTS HAVE ACCESS TO THEIR SAP, WHICH IS LOCATED IN THEIR FILE, UPON REQUEST. DETERMINATION OF PROGRESS STATUS Evaluations of progress will be conducted at the end of each scheduled period. Students meeting the minimum requirements for academic and attendance at the evaluation point are considered to be making satisfactory academic progress until the next scheduled evaluation. Students will receive a hard-copy of their Satisfactory Progress Determination at the time of each of the evaluations. Students deemed not maintaining Satisfactory Academic Progress may have title IV funding interrupted, unless the student is on warning or had prevailed upon appeal resulting in a status of probation. WARNING Students who fail to meet minimum requirements for attendance or academic progress are placed on warning and considered to be making satisfactory academic progress while during the warning period. Students will be advised in writing on the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the warning period, the student has still not met both the attendance and academic requirements, he/she may be placed on probation and, if applicable, students may be deemed ineligible to receive Title IV Funding. PROBATION Students who fail to meet minimum requirements for attendance or academic progress after the warning period will be placed on probation and considered to be making satisfactory academic progress while during the probationary period, if the student appeals the decision, and prevails upon appeal. Additionally, only students who have the ability to meet the Satisfactory Academic Progress Policy standards by the end of the evaluation period may be placed on probation. Students placed on an academic plan must be able to meet requirements set forth in the academic plan by the next evaluation period. Students who are progressing according to their specific academic plan will be considered making Satisfactory Academic Progress. The student will be advised in writing of the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the probationary period, the student has still not met both academic and attendance requirements for satisfactory academic progress or by the academic plan, he/she will be determined as not making satisfactory academic progress and if applicable, students will not be deemed eligible for Title IV funds. Re-Establishment of Satisfactory Academic Progress Students may re-establish satisfactory academic progress and Title IV aid, as applicable, by meeting minimum attendance and academic requirements by the end of the probationary period. Interruptions, Course Incompletes, Withdraws If enrollment is temporarily interrupted for a Leave of Absence, the student will return to school in the same progress status as prior to the leave of absence. Hours elapsed during a leave of absence will extend the student’s contract period and maximum time frame by the number of days taken in the leave of absence and will not be included in the student’s cumulative attendance percentage calculation.

Students who withdraw prior to completion of the course and wish to re-enroll will return in the same satisfactory academic progress status as at the time of withdraw. APPEAL PROCESS AND PROCEDURE Students who are not in satisfactory progress after failing to achieve minimum requirements must appeal this determination. The student must submit a written appeal to the Director along with any supporting documentation, reasons why the decision to terminate should be reversed (such as the death of a relative, an injury or illness of the student or child of the student), and a request for a re-evaluation of progress. This written request should state why the student failed to make satisfactory academic progress and give the changes made in the students situation that will allow the achievement of satisfactory academic progress at the next evaluation. This appeal must be received by the Director within five (5) business days of termination. This appeal will then be ruled on by the director and the decision by the director will then be given to the student in writing within 3 business days of the appeal date and a copy of the decision will be placed in the students file. Should a student fail to appeal this decision, the decision to terminate will stand. An appeal hearing will take place within five (5) days of receipt of the written appeal. This hearing will be attended by the student, parent/guardian (if the student is a dependent minor), the student’s instructor, and the Director. A decision on the student’s appeal will be made within three (3) business days by the Director and will be communicated to the student in writing. This decision will be final. Should a student prevail upon his/her appeal and be determined making satisfactory progress, the student will be automatically re-entered in the course and financial aid funds will be reinstated to eligible students. Noncredit and Remedial Course Noncredit and remedial courses do not apply to this institution. Therefore, these items have no effects upon the school’s satisfactory academic progress standards. Transfer Hours With regard to Satisfactory Academic Progress, a student’s transfer hours will be counted as both attempted and earned hours for the purpose of determining when the allowable maximum time frame has been exhausted.

RETURN OF TITLE IV FUNDS When you apply for financial aid, you sign a statement that you will use the funds for educational purposes only. Therefore, if you withdraw before completing your program, a portion of the funds you received may have to be returned. River Valley Cosmetology Institute will calculate the amount of tuition to be returned to the Federal fund programs according to the policies listed below.

RETURN TO TITLE IV FUNDS/R2T4 POLICY

This policy applies to students’ who withdraw official, unofficially or fail to return from a leave of absence or dismissed from enrollment at River Valley Cosmetology Institute. It is separate and distinct from the River Valley Cosmetology Institute refund policy. (Refer to institutional refund policy)

The calculated amount of the return of Title IV funds that is required for the students affected by this policy are determined according to the following definitions and procedures as prescribed by regulations. The amount of Title IV aid earned is based on the amount of time a student spent in academic attendance, and the total aid received; it has no relationship to student’s incurred institutional

charges. Because these requirements deal only with Title IV funds, the order of return of unearned funds do not include funds from sources other than the Title IV programs. Title IV funds are awarded to the student under the assumption that he/she will attend school for the entire period for which the aid is awarded. When student withdraws, he/she may no longer be eligible for the full amount of Title IV funds that were originally scheduled to be received. Therefore, the amount of Federal funds earned must be determined. If the amount disbursed is greater than the amount earned, unearned funds must be returned. The institution has 45 days from the date that the institution determines that the student withdrew to return all unearned funds for which it is responsible. The school is required to notify the student if they owe a repayment via written notice. The school must advise the student or parent that they have 14 calendar days from the date that the school sent the notification to accept a post withdraw disbursement. If a response is not received from the student or parent within the allowed time frame or the student declines the funds, the school will return any earned funds that the school is holding to the Title IV programs. Post-withdraw disbursements may occur within 90 days of the date that the student withdrew. Withdraw Before 60% The institution must perform a R2T4 to determine the amount of earned aid through the 60% point in each payment period or period of enrollment. The institution will use the Department of Educations prorate schedule to determine the amount of the R2T4 funds the student has earned at the time of withdraw. Withdraw After 60% After the 60% point in the payment period or period of enrollment, a student has earned 100% of the Title IV funds he or she was scheduled to receive during this period. The institution must still perform a R2T4 to determine the amount of aid that the student has earned. River Valley Cosmetology Institute, Inc. measures progress in Clock Hours, and uses the payment period for the period of calculation. The Calculation Formula: Determine the amount of Title IV aid that was disbursed plus Title IV aid that could have been disbursed. Calculate the percentage of Title IV aid earned:

a) Divide the number of clock hours scheduled to be completed in the payment period as of the last date of attendance in the payment period by the total clock hours in the payment period.

100% minus percent earned = UNEARNED PERCENT Unearned percent (multiplied by) total institutional charges for the period = AMOUNT DUE FROM THE SCHOOL.

If the percent of Title IV aid disbursed is greater than the percent unearned (multiplied by) institutional charges for the period, the amount disbursed will be used in place of the percent unearned. If the percent unearned (multiplied by) institutional charges for the period are less than the amount due from the school, the student must return or repay one-half of the remaining unearned Federal Pell Grant. Student is not required to return the overpayment if this amount is equal to or less than 50% of the total grant assistance that was disbursed /or could have been disbursed. The student is also not required to return an overpayment if the amount is $50 or less. River Valley Cosmetology will issue a grant overpayment notice to student within 30 days from the date the school’s determination that student withdrew, giving student 45 days to either:

1. Repay the overpayment in full to River Valley Cosmetology Institute or Sign a repayment agreement with the U.S. Department of Education. Order of Return River Valley Cosmetology Institute is authorized to return any excess funds after applying them to outstanding balances on previous periods or other remaining institutional costs to the separate funds in the order of Pell Grant and then student. • A sample of the Institutional Refund Calculation work sheet is available through the office upon student request.

Earned AID: Title IV aid is earned in a prorated manner on a per diem basis (calendar days or clock hours) up to the 60% point in the semester. Title IV aid is viewed as 100% earned after that point in time. A copy of the worksheet used for this calculation can be requested from the financial aid director. In accordance with federal regulations, when Title IV financial aid is involved, the calculated amount of the R2T4 Funds" is allocated in the following order: • Federal Pell Grants

Post Withdraw

If you did not receive all of the funds that you have earned, you may be due a post-withdraw disbursement. River Valley Cosmetology may use a portion or all of your post- withdraw disbursement for tuition and fees (as contracted with 4-States). For all other school charges, 4-States needs your

permission to use the post-withdraw disbursement. If you do not give permission, you will be offered the funds. However, it may be in your best interest to allow the school to keep the funds to reduce your debt at the school. The post-withdrawal disbursement must be applied to outstanding institutional charges before being paid directly to the student. Institution Responsibilities River Valley Csometology's responsibilities in regards to Title IV funds follow: • Providing students information with information in this policy; • Identifying students who are affected by this policy and completing the return of Title IV funds calculation for those students; • Returning any Title IV funds due the Title IV programs.

The institution is not always required to return all of the excess funds; there are situations once the R2T4 calculations have been completed in which the student must return the unearned aid. Overpayment of Title IV Any amount of unearned grant funds that you must return is called overpayment. The amount of grant overpayment that you must repay is half of the grant funds you received or were scheduled to receive. You must make arrangement with River Valley Cosmetology Institute or Department of Education to return the amount of unearned grant funds.

Student Responsibilities in regards to return of Title IV funds Includes: • Returning to the Title IV programs any funds that were dispersed to the student in which the student was determined to be ineligible for via the Return of Title IV funds calculation. • Any notification of withdraw should be in writing and addressed to the appropriate institutional official. • A student may rescind his or her notification of intent to withdraw. Submissions of intent to rescind a withdraw notice must be filed in writing. • Either these notifications, to withdraw or rescind to withdraw must be made to the official records/registration personal at your school.

Refund vs. Return to Title IV The requirements for the Title IV program funds when you withdraw are separate from any refund policy that River Valley Cosmetology may have to return to you due to a cash credit balance. Therefore, you may still owe funds to the school to cover unpaid institutional charges. River Valley Cosmetology may also charge you for any Title IV program funds that they were required to return. An applicant not accepted by the River Valley Cosmetology shall be entitled to a refund of all monies paid. A student (or in case of a student under the legal age, his/her parent or guardian) that cancels their contract with the institute must make their request in writing, and the cancellation date will be determined by the post mark date or by the date notification is hand delivered, in person to the Director. All monies due to student are refunded within 45 days of written cancellation or termination. If the student cancels enrollment within three business days of signing an enrollment agreement, all monies collected by the school shall be refunded. If the student cancels enrollment after three business days, but prior to starting class, the student is entitled to full refund less the non refundable enrollment fee of $100. A student on an approved leave of absence must notify the school that he/she will not be returning. The date of withdraw shall be the earlier of the date of expiration of the leave of absence or the date the student notifies the school that he/she will not be returning; or if the student is expelled by the school. River Valley Cosmetology Institute determines unofficial withdrawals by monitoring attendance daily. All refunds are calculated based on the student’s last day of physical attendance and scheduled hours of attendance for enrolled time. Enrollment time is defined as the time elapsed between the actual starting date and the student’s last date of physical attendance. If a program or course is canceled subsequent to a student’s enrollment, and before instruction in the program or course has begun, the school will provide completion of the program. In a situation where completion of the program is not available the school will then provide a full refund of all monies paid. The cost of other items such as kits, books, and supplies or other fees shall not be considered in tuition adjustments. These items become the property of the student when issued and are not refundable. In the event that the school must close permanently and ceases to offer instruction after students have been enrolled, or if a program is canceled after students have enrolled and instruction has begun, the school will refund the tuition to the students at a pro-rata calculation and dispose of school records according to the state law requirements.

STUDENT SERVICES

Advising, group tutoring sessions, private tutoring, and referral to available service help organizations or substance abuse advising are available. The school will offer assistance in placement, academic, individual and career advisement throughout the course. Information is provided to you from the Administrative office. The school makes no provisions for housing. Student Services Board is located in your school.

River Valley Cosmetology Institute may or may not terminate for the following reasons: 1. Students who are found to be stealing. 2. The use or possession of any illegal drugs or alcohol before or during school hours. 3. Physical violence towards any other person on school property. 4. Excessive behavioral problems. 5. Students who have been absent for 10 consecutive school days, without making contact to the school by phone, written, or person to person as to their reason of absence. 6. Students who fail to meet the minimum monthly payment for 90 days required in the 4-States Academy self pay program. 7. Students bringing lethal weapons on school property. Includes parking lot.

*This policy also applies to all off campus activities when a student is a representation of River Valley Cosmetology.

*All terminations are subject to an appeal. Other excessive circumstances may be subject to termination at the discretion of the administration.

SCHOOL RULES 1. All instruction will come from the instructor. One student is not to advise another in any phase of his/her training. Infraction will result in student being clocked out and sent home. 2. Students must not leave school at any time without first notifying the school personnel and checking out. 4. Students are assigned weekly duties. Students will perform all duties specified by school personnel. Infraction will result in student being clocked out and sent home. 5. Students must comply with all instruction, direction, and responsibilities at all times. Students are assigned patrons; student refusing a patron will be clocked out and sent home for the day. 6. Lunch period is assigned by instructor and staggered. It is the students' responsibility to become aware of when their lunch will be. If a student leaves the premises for lunch they must punch out infraction will result in student being clocked out and sent home. 7. Students must keep work stations and other areas clean and sanitary at all times. 8. School phones are for business purposes only. Personal calls will be limited. Messages will be taken for incoming calls. 9. Personal items must be kept in your locker. The school is not responsible for stolen items. Students will be prosecuted and terminated immediately from the program if caught stealing. 10. Students must have hair neatly styled and make-up on before class. Infraction will result in the student being clocked out and sent home. 11. Students will pay student prices for products used on their hair. Services must be paid for before service. 12. Absolutely no PROFANE or VULGAR language at anytime. 13: Spouse and children haircuts will be free and any other family member will be expected to pay full price for any and all services unless otherwise arranged with owner. 14. Personal items will be kept for six weeks after the student has left school. After that period they will be disposed of. 15. Professional ethics shall be practiced at all times. 16. The use of illegal drugs or alcohol before or during school hours is strictly prohibited. Students found within violation of this rule will be terminated immediately. 17. Smoking is allowed in designated area only; never in front of any location. This is a privilege that can be removed. 18. Absolutely no lethal weapons allowed on school property; includes parking lot.

CONSUMER INFORMATION A license in Cosmetology can open a new world of opportunities for any individual, such as owning your own business, working for large corporations and even some medical facilities. Cosmetology license allows individuals to do consultation, implementation and maintenance of all beauty related needs of the client. CAMPUS SECURITY ACT INFORMATION Under the Crime Awareness Campus Security Act of 1990 we are required to provide students with the following safety information about our campus locations. All criminal actions must be reported to an instructor, director, or owner immediately. That individual will assist the student or client in reporting the crime to the local police department or any other appropriate security force. The facilities are open to the

public Monday through Friday according to assigned schedules. The facilities may also be opened for educational or meeting purposes scheduled by the owner. Only staff have keys to the facilities preventing internal crimes to as great extent as possible. A staff member made aware of a crime will notify the rest of the staff as soon as possible, perhaps even prior to notifying the police, depending on the situation. It is critical that all staff be aware of any report of crime and that the local police be notified immediately. This information will be provided to all prospective students. At regular times during training, staff and students will be reminded about security and safety procedures, including crime prevention, personal safety off-campus, fire and tornado procedures, etc. Local Police Speakers will be scheduled throughout the training for all staff and students. We have no reports at any location at this time of murder, rape, robbery, aggravated assault, burglary, or motor vehicle theft. We have no reports of liquor law violations, drug abuse violations, or weapons possession.
