LMS Mission

It is the mission of Louisville Middle School to empower our community of learners with the skills needed to become leaders of the 21st century.

At Louisville Middle School, we envision a learning community where these fundamental values form the foundation of the House of PRIDE:

Positivity propels us to transform challenges into opportunities. Respect compels us to honor the dignity and worth of self and others. Integrity inspires us to do the right thing, especially when no one else is watching.

Determination dares us to approach life with energy and persistence. Empathy calls us to connect with others through compassion and caring.

For the smooth daily operation of our school, here are the basics.

Think PRIDE, and you’ll have them covered. • We shall maintain high personal standards of courtesy, integrity, morality, and honesty in your

relationship with others. • Our conduct shall at all times reflect consideration for the rights and privileges of others; cooperation

with all members of the school community is required. • We shall respect the authority of school staff and officials to keep all of us safe and our system running

efficiently. This includes compliance with school regulations, Boulder Valley School District policies, and Colorado law.

• We will come prepared to learn. Each of us needs to bring all materials, completed assignments, and a positive attitude to class each day. Backpacks must be left in our hall locker. Bring only educational items to school.

• Exhibit positive regard for all students, adults, and school property. Everyone should be treated with common courtesy. We will treat others the way we would like to be treated.

• Be appropriate in behavior, language, and dress. Obscenities, put downs, intimidation, harassment, vulgar language, and gestures will not be tolerated. Materials that display obscenities or vulgarity, including locker decorations, may be confiscated.

• Use of technology, during and after school hours, on and off school property, may result in discipline at school if it interferes with school safety or academic integrity. This includes but is not limited to texting, social interaction sites and e-mail.

ACADEMICS It is the expectation of Louisville Middle School that you use your abilities to the fullest. The staff, along with your parents, is here to assist and support your success. All the information that precedes “academics” in this handbook is here to support your education and safety.

• Louisville Middle School operates on Standards Based semester system. First semester is eighteen weeks long; there are two semesters in the school year. Final grades are issued at the end of each semester.

• The dates for Progress Reports and Semester Report Cards, see the LMS web site: http://lom.bvsd.org/Pages/default.aspx • Progress reports are communicated through the IC portal. Students and parents should check the

portal regularly. Any concerns about grades should be directed to the teacher first, then, if necessary, Student Services (720) 561-7408. Semester report cards will be mailed home in January and June.

• Parents are able to access their student’s academic grades, attendance, and class schedule by logging into the Parent Portal. 6th grade parents will be mailed their Parent Portal ID number in the fall.

• Honor Roll is recognized for students earning a term grade point average of 3.70. HOMEWORK Homework is a regular expectation at Louisville Middle School for all grade levels. It provides the student and teacher with information about the extent to which the subject matter has been mastered. Although homework may not be assigned in every subject every day, assume that you will have some homework on a regular basis. Each teacher has specific expectations for home-work. Please make sure you have a copy of each of your teacher’s class expectations, websites, and homework postings. COURSE SYLLABUS, CURRICULUM MAPS AND GRADING POLICY Each student will receive access to a syllabus, curriculum map, or outline for each course in which he/she is enrolled to communicate course expectations, materials, skills, and content. Parents will receive copies of these on Back to School Night (or they will be available online) and they may email a teacher with questions regarding a specific class. TEXTBOOKS AND MATERIALS Students are responsible for all textbooks and materials issued by teachers. Replacement costs and fines will be assessed when appropriate. Library fines will also be assessed when appropriate. ATTENDANCE Boulder Valley School District / Associated Regulation: JED-R (current at time of print) According to the BVSD Student Rights and Responsibilities Guide regarding Compulsory Attendance, regular attendance in all classes is an integral part of the educational process. Students are expected to be in attendance every day school is in session as required by law and District policy. Participation in the classroom is a significant aspect of a student’s academic performance, and failure to attend is considered to be grounds for academic sanction. Excused Absences mean absences due to temporary illness or injury, and any other reason deemed acceptable by the Principal and in accordance with BVSD policies. Only the LMS Principal, Assistant

Page 8: STUDENT EXPECTATIONS AND INFORMATION Info/LMS Student... · Students and parents are responsible for reviewing and knowing the behavior expectations outlined in both this student

6

Principal, or designee may excuse a student’s absence. When you are absent, it is your parent’s/guardian’s responsibility to call the school attendance line at (720) 561-7402 before 9:00 a.m. that day to explain the reason for the absence. Parents may excuse a student in writing within 48 hours. If the school is not notified, students’ absence will remain un-excused. The State and the Department of Health require us to code specific reasons for student absence. We ask parent/guardian to be specific when calling their student absent by not using “ill or appointment.” Please use words such as headache, doctor, dentist, etc. The student has TWO school days to make up missed work from one day’s excused absence. Homework is the individual responsibility of each student, and use of their LMS planner is the primary tool for recording homework and maintaining organization. In addition, homework is often available on individual teacher websites. Early Dismissal during the school day requires a note to the attendance office. You need to bring the note to the Main Office before school starts in the morning. A pass is then given to you. Upon your return to school, bring this pass to the Main Office. Pre-Arranged Absences BEGIN at the Attendance Office. Student’s need to bring a note to the Attendance Office stating: name, grade, dates of absence, and reason. The student will then be given a prearranged absence form to be signed by each of the student’s teachers. The form is then returned to the Main Office to be copied for school records. TARDINESS Tardiness is defined as the appearance of a student after the scheduled time that a class begins and is considered a form of absence. Without proper excuse, tardiness is considered a form of unexcused absence. Minutes of unexcused tardies will be included in the calculation of total unexcused absences.

• Students who are late to school after 8:40 a.m. must check in with the Attendance Office. Students who are late to class and are un-excused will receive consequences from their teacher for time and work missed.

• Tardies are dealt with by each teacher unless they become excessive, in which case they will be referred to the administration. A fifth unexcused tardy may result in an administrative referral. Consequences may include lunch detention, before/after school hours work detail or, in extreme cases, Saturday school.

TRUANCY A student is expected to remain at school and not leave campus once he/she arrives at school.

• If a student is not in class at any time without permission, he/she will be considered truant. Truant students may be suspended. If a class is missed during the school day and the student is still on campus, he/she must make up double the time missed. A student will be allowed to leave only if parents or a person on the emergency card comes to the office to pick up him/her or have made alternate arrangements with the office. If a student must leave the building for any reason, he/she must check out through the Main Office or the Clinic.

• Students who have four unexcused absences from school or from class in any one month or ten unexcused absences from school or from class during any school year are considered ‘habitually truant.’ At this point the student is referred to the districts’ Truancy Officer.

• Unexcused Absences: Truant students will be required to make up missed work and will serve double the amount of class time missed in work detail or detention. The Principal has final discretion to excuse or not excuse an absence or tardy.

BEHAVIOR/EXPECTATIONS PRIDE In general, following PRIDE will help you do the right thing in nearly every situation. In addition to the guidelines, policies and procedures included in this handbook, many teachers will have classroom specific rules and systems. The rules of the House of PRIDE apply everywhere…in the building, on school grounds, to and from school, and on school trips. • It is imperative that all of us feel and be safe at school. We rely on students and staff to come forward to

make a report of any instance that may infringe on the rights of all Pirates to be safe. PLEASE seek the help of a teacher, counselor, administrator or other staff member!

• Minor conflict resolution or peer issues, students may seek assistance from their counselor by going to the counseling office and filling out a request form. Please let Ms. Manders know if it’s urgent.

• Issues that may require discipline or the attention of an administrator, students must come to the office to fill out a “green sheet”.

• Any physical violence or discriminatory language? Go straight to the office. LMS has had a history of dealing with behavior issues that occur in our final four weeks that has been very successful. In dealing with a habitually disruptive incident, or a suspendable offense, instead of taking the student out of the classroom during important academic days, we will remove the student from the festivities of the final day, and possibly the events of the final week. HALL BEHAVIOR Students are permitted in the hall during class periods only if they have a pass issued by the teacher who is responsible for them. You may be requested to show this hall pass to a staff member when out of class.

• No student may leave the classroom without his/her teacher’s permission AND a pass in hand. This applies to the lunchroom as well.

• Students must walk through the halls in a manner that allows others to pass quickly and safely. • A warning bell will chime when there is one minute remaining in the passing period.

LUNCH EXPECTATIONS As with the rest of your day at LMS, PRIDE governs the lunchroom. In order to maintain the most pleasant environment for all students and staff, please remember RESPECT and EMPATHY as your guiding traits. The Lunch Process:

• Be respectful in line • Settle on a table for lunch and stay seated until the lunch staff has dismissed students to move during

the last 15 minutes of lunch. Then, if your place is clean, once your table has been dismissed, feel free to move to another table or head outside with friends.

• Please leave the table and surrounding area as clean as you found it. • Please treat the lunch staff with kindness and respect, and they will strive to treat you the same. • If you need to see a teacher during lunch, please show your prearranged pass to the lunchroom staff. • All school rules apply to the lunchroom and the “recess” areas, including electronics. • No pushing, shoving, or tackling games are allowed or any other activity that may injure other

individual students. • During nice weather, go outside once you have finished eating. Students should remain visible within

the Sunken Garden area while outside during lunch. The playground equipment is off limits.

Page 10: STUDENT EXPECTATIONS AND INFORMATION Info/LMS Student... · Students and parents are responsible for reviewing and knowing the behavior expectations outlined in both this student

8

• If you have questions, please ask the Assistant Principal. Eating with your peers in the lunchroom is a privilege that is guided by rules of courtesy, cleanliness, and appropriate manners. If you choose to disregard the above expectations, your parents and the administration will need to make alternative lunch arrangements.

BULLYING No student shall engage in bullying, intimidating, or harassing behavior. This includes behaviors in the school, on school grounds, in school vehicles, at a designated school bus stop, or at school activities or sanctioned events, or that happen outside the school day but affect another student’s sense of safety. Bullying is defined as any written or verbal expression, or physical act or gesture, or a pattern thereof that is intended to cause distress upon one or more students. A reasonable balance between the severity and pattern, if any, of the bullying behavior shall be taken into consideration when disciplinary decisions are made. CONFLICTS/ DISCRIMINATION Any student, staff, or community member who witnesses a violation of the non-discrimination policy is expected to take reasonable action to stop the violation at the time the violation occurs and to report the incident to the appropriate personnel immediately.

1. Patrons should initiate complaints by contacting the person at the building, department, or classroom level who is the most direct subject of the complaint.

2. If the complaint concerns a teacher, that teacher would be the initial level. 3. If the complaint concerns a school, the principal would be the appropriate contact. 4. Review at the initial level is informal and should follow a discussion format. The complaint

itself may be oral or in writing. 5. The LMS employee receiving the complaint shall provide a response within three working days after

the discussion. 6. If the LMS patron is dissatisfied by the response of the employee, he or she may bring the matter

before a supervisor or designee for further discussion in accordance with this section. 7. Patrons may then contact the LMS building counselor, and/or assistant principal, who will work

to resolve the issue within 2 working days. 8. Please contact the school principal if the issue remains unresolved.

CAMPUS BUILDING HOURS Main office Hours: 8 a.m. – 4 p.m.

• The building will be open to students at 8:30 a.m. (7:30 a.m. for supervised athletics, clubs, or help sessions) and then the doors are secured at 8:40 a.m. Main Office entrance will be the single point of access during school hours. Visitors must check in there and then will be escorted into the building.

• When the 8:40 a.m. bell rings, all students need to be in class. Dismissal will be at 3:30 p.m. Please drop off and pick up your student on Lafayette St. south of the school. No drop off or pick up on Main St.

• Complimentary supervision of students begins at 8:20 a.m. and ends at 3:40 p.m. Students in the building or on school grounds before 8:20 a.m. and after 3:40 p.m. must be under the direct supervision of an adult.

CLOSED CAMPUS • The Boulder Valley School District has a “Closed Campus” policy for all middle level schools. You must

remain on school grounds during the school day. LMS school grounds consists within the perimeter of all sidewalks on Main Street, Lafayette Street, and Jefferson Avenue, inclusive of all fields surrounding the school building, Pirates Park and tennis courts, and the entire school building.

• Permission for a student to go to his/her own home for lunch or with his/her own parent to lunch may be given with written permission from his/her parent when presented to the Main Office.

• Lunch is for LMS students only, and lunch visits are not allowed without administrative approval. • Parents wishing to take their student to lunch should reserve this opportunity for special occasions. • Due to the number of students at LMS, birthday celebrations, including balloons and cakes, cannot be

during school hours. VISITORS/GUESTS

• PARENTS: All parents and visitors to Louisville Middle School are required to check in with the Main Office and receive a visitor pass. This pass is to be visible at all times. If a student is needed, we will have the student come to the office. Teachers and staff are instructed not to allow a student to leave his/her classroom with any non-school personnel. This is to insure the safety of our students!

• Parents visiting during school hours should park on Main Street or in the designated visitor spots. • STUDENTS: Due to the number of students at Louisville Middle School, friends/guests are asked to

visit after school hours (3:40 p.m.) with permission from an administrator. • Please make arrangements to meet students from other schools off school grounds and away from

Pirate’s Park. • A special note about student drop off, pick up and parking: Please adhere to parking in the designated

areas only. Dropping and collecting students on the south side of the school on Lafayette is the safest for everyone.

DRESS CODE Dress code revisions for 2015 were made from student input and focus groups. Students are expected to dress appropriately, modestly, and tastefully for the classroom and school activities. Clothing should be conducive to learning at school and should not be distracting. The LMS dress code is as follows:

• Clothing shall not disrupt the learning environment or violate LMS PRIDE. • Don’t show too much skin- if you have a doubt, ask an administrator or a counselor. • Outerwear must cover underwear. • Shoes must be worn at all times (unless directed by the teacher). • Clothing with obscene or questionable language or that advertises violence, drugs, alcohol, and/or

tobacco must not be worn to school. • Hats may only be worn inside LMS on Fridays. Hats may be worn outside. • Students will be provided the opportunity to change into appropriate clothing, contact home, or

borrow clothing from the school. *Teacher/administration interpretation of rules may vary. Administration reserves the right to ask any student to change clothing or send any student home to change if deemed necessary. Repeated dress code violations will be considered insubordination and may result in further discipline or suspension.

Page 12: STUDENT EXPECTATIONS AND INFORMATION Info/LMS Student... · Students and parents are responsible for reviewing and knowing the behavior expectations outlined in both this student

10

ELECTRONICS AND SCREEN USAGE *LMS will not investigate damage to, theft of, or loss of any and all personal electronic devices.

ELECTRONIC SCREEN USAGE • LMS recognizes that technology has its role in a 21st century classroom. Chromebooks and desktops

are available throughout the building and in specialized labs. These are sufficient for our modern middle school educational programming and as such we do not normally want or need students to bring their own computers to school. Exceptions to this must be approved explicitly by school administration.

• LMS Addendum: Teachers have the individual right to allow or disallow various student handheld devices for purposes of multimedia or other academic support. Students are expected to uphold LMS honor code standards with regard to morality, plagiarism, proper care and use, when using these devices and when searching the internet. LMS has a strict Code of Ethics regarding “G” and “PG” only images and vocabulary that is applied at all times to personal conduct and technological devices.

• Misuse of Social Media and Academic Media, may result in suspension of a student’s technology rights and access at school, and additional consequences including but not limited to suspension.

CELL PHONES We understand that parents might want their students to have a phone for communication – and to keep track of a busy teenager after school! We would like to support your communication. During school hours (8:40 a.m. to 3:30 p.m.) cell phones and smart electronic use is prohibited, unless explicitly allowed by teachers in their classroom or in other areas of the school / school environment. If students have an emergency and need to use a phone, they should always ask an adult for permission to use a school phone. If a student is feeling ill, she/he should call from the health room phone. If a student has a phone at school, it should always be turned off, kept secured in his/her locker and out of sight/sound during school hours. This includes all school activities and field trips. LMS will not investigate nor be held liable for inappropriate use of, damage to, theft of, or loss of any and all electronic devices.

• If a device rings, vibrates or is in use during class, it may be confiscated and brought to the administration.

• If it is the first offense, the student may reclaim the phone at the end of the next school day or 24 hours whichever is the lesser amount. Infraction is logged in main office.

• Upon the second offense, student must check-in and check-out phone for a week and parents will be notified.

• Further or repeated offenses may result in additional discipline. • Headphones may be used in class at teacher discretion and with specific teacher permission. For

safety reasons, headphones are not allowed to be used during passing periods. • Students are reminded that use of technology, during or after school hours, on or off school property,

may result in discipline at school if it interferes with school safety or academic integrity. This includes but is not limited to texting, social media and email.

Page 13: STUDENT EXPECTATIONS AND INFORMATION Info/LMS Student... · Students and parents are responsible for reviewing and knowing the behavior expectations outlined in both this student

11

EXTRA CURRICULAR ACTIVITIES • Students are encouraged to participate in or attend after-school activities and contests. We want

every student to be attached to at least one school activity. Contact Assistant Principal Muñoz for any questions about school activities or athletics.

• A variety of activities, clubs, interscholastic athletics and intramurals are offered for Louisville Middle School students. Most activities will be held in the mornings, (usually from 7:30 a.m.), or afternoons (usually until 5:30 p.m.) If you’re interested in starting a new activity, see Mr. Muñoz.

• Whether you are a participant or spectator, you must display good sportsmanship at all times. Courteous and polite manners are expected at all times during Louisville Middle School activities, at home and away. The Administration reserves the right to remove anyone from participating or observing who refuses to display good sportsmanship or PRIDE.

• Louisville Middle School offers many athletic enrichment activities to all students. Some athletic activities require a current physical to be on file. All athletic activities require a completed registration form – go to Rev Trak on the LMS website or come in to fill out a paper registration.

• No student should be excluded from participation because of financial need. Please contact the Assistant Principal regarding scholarships from our Student Opportunity Fund.

HEALTH CLINIC The clinic is available for students who are ill, injured, or have a health concern. Students MUST have a pass from a teacher to come to the clinic, except in the case of an emergency. Permission to leave school must be obtained from the clinic or office after a parent has been contacted. (Students who are ill, may not call parents to take them home from their cell phones or classroom phone). Only the main office can authorize a dismissal, cell phone calls/text messages placed from the classroom do not suffice. A student may stay in the clinic for 20 minutes and then he/she must return to class, unless he/she is waiting for a parent to take him/her home. It is important that correct and up-to-date information is on your emergency card in case of emergencies. If you need to take medication of any kind, including non-prescription products such as aspirin, the following three items are required, per board policy: 1. A signed copy of the “Parents Request for Giving Medicine at School and Release Agreement,” which is

available from your doctor or the clinic. 2. A doctor’s written order for the medication to be given. 3. Medication must be kept in the clinic in a properly labeled container. LOCKERS

• Lockers are issued to students by Student Services. Lockers may be searched with “reasonable suspicion” at any time. Keep your locker clean, inside and out. Keep your locker combination to yourself for security.

• The school will not be responsible for lost or stolen items. Leave valuables at home. • A $5.00 fee will be charged if the combination has to be changed because it has been shared with

friends. • Students must not mar, write on or alter the appearance of the school locker issued to them. • All backpacks must be kept in your locker until the end of the day. • Lockers that are damaged will be assessed a fee at the end of the school year.

Page 14: STUDENT EXPECTATIONS AND INFORMATION Info/LMS Student... · Students and parents are responsible for reviewing and knowing the behavior expectations outlined in both this student

12

In addition to meeting with students individually, counselors meet with groups of students, conduct classroom and other small group activities, and consult with teachers and parents. Counselors assist in making educational plans, understanding test scores, exploring career options, learning effective study skills, resolving conflicts with friends, family, teachers, and understanding yourself and others. If you wish to see a counselor, contact a counselor or the counseling secretary at (720) 561-7408 to arrange an appointment. During the school day, a pass from a staff member is needed to visit with a counselor. Appointments can be requested before and after school or, if time permits, during passing periods. Appendix of BVSD Law and District Policies

HB1254, passed in 2011 further enforces and supports section 22-21-109, supports anti-bullying school programming, public education regarding the effects of bullying and encourages school to end bullying.

Specifically, schools are to enforce their anti-bullying codes of conduct. ACADEMIC HONESTY AND INTEGRITY District Policy JFA, JFA-R, 4/10/07 SECTION I: INTRODUCTION AND RATIONALE BVSD Mission: the Boulder Valley School District challenges students to achieve their academic, creative and physical potential in order to become responsible, contributing citizens. The Boulder Valley School District recognizes the importance of promoting a learning environment that values academic honesty. In order to foster ethical behavior in our students and thus meet the goals outlined in the BVSD Mission Statement and the graduation requirements prescribed in the New Century Graduate, we believe that educating all students regarding the characteristics of academic integrity is a critical component in successfully preparing them for their future endeavors. When engaged in learning activities, students should demonstrate the discipline necessary to seek guidance from their instructor rather than resorting to inappropriate behaviors that may undermine their own academic and personal development. Therefore, the district will actively support our students in taking ownership of their learning both inside and outside of the classroom. District educators will encourage the development of ethical behavior in their students and support constructive dialogue regarding the characteristics of academic integrity in their classes. This is a shared responsibility of all parties: students, parents, and district educators. This regulation defines those behaviors that constitute unethical academic behavior and outlines the range of consequences schools can consider when infractions occur. Faculty and students should be certain to allocate time to review this regulation and discuss any additional expectations that emerge. This regulation and the associated policy will be printed and distributed to all students annually in the Student Rights and Responsibilities Guide. Students will be expected to acknowledge that they have received information related to academic honesty and that they understand the expectations placed upon them as members of the BVSD academic community. SECTION II: POLICY Boulder Valley School District students are expected to respond to academic challenges with the highest degree of integrity and honesty that support the culture of the community and the characteristics of the New Century Graduate. SECTION III: DEFINITIONS Academic Integrity: Compliance to accepted ethical standards. Academic Dishonesty: Deliberate, unacceptable behaviors that include but are not limited to: misrepresentation,

Students and parents are responsible for reviewing and knowing the behavior expectations outlined in both this student handbook and the BVSD “Students’ and Parents’ Rights and Responsibilities

This includes copying exact words of another writer without proper documenting of source, stealing or passing off words, graphics, and artwork that is not one’s own • Purchasing a paper from a service or other sources • Reproducing another person’s paper and submitting it as one’s own • Having someone else take an exam • Inventing of information or citation in an academic exercise and misquoting or quoting out of context • Submitting a reproduction of another person’s work • Signing of another’s name to an official document and/or submission of information known to be false

Obtaining unauthorized information: Information that is obtained dishonestly: • Copying homework • Working with another student on work that is intended to be done independently • Downloading and submitting term papers from on-line resources • Sharing answers through data storage devices • Using cell phones or PDA devices to take electronic pictures of exams • Text-messaging answers of academic work • Stealing materials from teachers or classmates

Tendering of information: Students giving or selling their work to another person who plans to submit it as his or her own:

• Giving work to another student to be copied • Giving someone answers to an exam during an exam • Discussing an assessment with students who have not yet been assessed on the material • Giving or selling a written assignment to another student

SECTION IV: INFRACTIONS AND CONSEQUENCES Suspected incidents of academic dishonesty should be referred by instructional staff to the school’s administration for investigation. In order to maintain a reasonable level of consistency within a school, consequences will be applied by the school’s administration after consultation with the appropriate instructional staff. The general magnitude of an infraction and the level of premeditation involved should be considered before consequences are determined. Schools should strive to maintain consistency in consequences from infraction to infraction. Possible consequences to infractions may include:
