• 1 day PTA was admitted for hepatectomy, ขณะผาตดไมม complication หลงผาตด แพทยให ambulate ขางเตยงได
• วนน ประมาณ 10.00 น.พยาบาลเหนเดนไปรบประทานอาหารไดปกต
• 12.00 น. พบผปวยเรยกไมตน, พ. ประเมน GCS-> E1V1M5
Case presentation: Case 2 • A Thai man, 57 y.o, Hx. Chest pain 5 d. PTA ไป
รพช. ไดคว PCI จงสงตวมา • 7.30 น. วด BP=200/130 พยาบาลซกประวตไดวาวนนไมได
รบประทานยา เนองจากรบเดนทางมาตรวจ และไมไดเอายามาดวย • 8.30 ขณะรอท า PCI ผปวยบนปวดศรษะ และไดขอยาบรรเทาปวด • 8.40 ขณะพยาบาลน ายาไปใหรบประทาน พบแขน-ขาซกซาย หอย
ลงขางเปลนอน จงบอกใหผปวยนอนดๆ และประเมนไดวาแขน-ขาออนแรง

• 11.00 น. ขณะเขาตรวจอาการปกต, 11.30 ตรวจเสรจ ญาตสงเกตเหนวาปากเบยว ผปวยลกจากเตยงตรวจไมไดเนองจากออนแรงซกขวา

• หลงจากรบประทานลดความดน ตอมาประมาณ 2 hr. อาการปวดศรษะเพมมากขน extern ทอยเวร จงพาไปตรวจท ED
• @ ER BP = 200/110, ผปวยเรมซมลง และหายใจมเสยง stridor
• CT: Pontine hemorrhage

Guideline for Acute ischemic stroke
• Positioning: fowler’s / flat
• BLSRespiratory / oxygenation: SPO2 > 92% / > 94%
Guideline for Acute ischemic stroke • Blood pressure: - SBP ≤ 220/DBP ≤ 120 (except CHF, aortic dissection, AMI, ARF, hypertensive encephalopathy) - SBP>220/DBP 121-140 หางกนอยางนอย 20 min. x 2 times -> Nicardipine / Labetalol -DBP > 140 หางกน 5 min. x 2 times -> Nitropusside, NTG
- Reduce blood pressure by 15% during the first 24 hours after onset of stroke
• SBP <185, DBP<110 before – begin the rtPA treatment
• < 180/105 for at least the first 24 hr. after rtPA treatment.
Guideline for Acute ischemic stroke
• IV fluid: 0.9 %NaCl correct hypovolemia • NPO • Check blood glucose - <60 mg/dL 50% glucose IV push - > 180 RI sc - Body temperature > 38 ลดไข • Seizure • Other conditions
Gold standard: for ischemic stroke
1. IV thrombolysis < 3 / 4.5 hr.
2. Anti-platelet / anti-coagulant < 48 hr.

- Notify physician -> notify neuro (fast track) -> Intubation -> request CT
