Dear Col l eagues ,

Over the past seventeen years , Partners Heal thCare and i t s hosp i ta l s , l ed by our founding inst i tut ions Br igham and Women’s and Massachuset ts Genera l , have made rea l progress toward improving the hea l th o f underserved communit i es . We’ve made that progress work ing with our ne ighbors .

As we focus on the hea l th o f our communit i es , we must make cer ta in the h igh qual i ty care we provide i s access ib l e , equi tab le , and a f fordab le to a l l we serve . The 21 l i censed and a f f i l i a ted community hea l th centers that we support provide care for more than 350,000 pat i ents each year, and community-based coa l i t ions in Revere and Che lsea – among others – are address ing the i r key i ssues o f concern such as substance abuse , d iabetes , prevent ion , and wel lness for a l l ages .

But there ’s more to do . To ensure the care for our pat i ents i s ava i lab le in the r ight p lace a t the r ight t ime , we need to cont inue to a t tract and support phys ic ians and nurse pract i t ioners who want to work in community hea l th centers . To increase economic opportuni t i es in underserved communit i es , we need to expand tra in ing and career opportuni t i es for young people and adul ts .

We are conf ident that we can meet these cha l l enges together. Partners wi l l cont inue to invest in and work with community-based organizat ions to understand loca l needs and implement community- l ed approaches that l ead to s t ronger, hea l th ier ne ighborhoods . In 2010, Partners ’ f inanc ia l commitment to the community was $186M – up 18% from the pr ior year.

As we look for innovat ive ways to fu l f i l l our miss ion , ph i lanthropy p lays an important ro l e in generat ing power ful new ideas for improving community hea l th . In 2006, a g i f t f rom Bank o f Amer ica was the cata lyst for a s ta tewide , mult i funder loan repayment program that to date has added 115 doctors and nurse pract i t ioners who care for more than 204,000 community res idents . And in 2011, a g i f t f rom the Kra f t Fami ly bui lds on our commitment to enhance capac i ty by increas ing the number o f hea l th pro fess ionals in underserved communit i es . The Kra f t Fami ly Nat ional Center for Leadersh ip and Tra ining in Community Heal th wi l l a t t ract and deve lop new phys ic ian l eaders whi l e support ing c l in ic ians a l ready work ing in our ne ighborhoods .

At Partners Heal thCare , our shared v is ion with the communit i es we serve wi l l guide us as we move forward together and s trengthen our bond o f car ing for and commitment to those in need .

Through innovative statewide partnerships with the Mass League of Community Health Centers, Bank of America, and the Kraft Family we are supporting clinicians who will improve health delivery for low and moderate income people. These programs have supported 115 doctors and nurse practitioners who provide care to more than 204,000 community residents.

Hospital navigators have connected more than 9,000 Par tners patients with community -based primary care. We are connecting patients to the r ight care in the r ight place, and reducing the number of patients seeking costly emergency room care.

Par tners HealthCare has invested more than $83 mil l ion in building and renovating 21 l icensed and af f i l iated community health centers that care for more than 350,000 patients annually. Through investments l ike these, patients are better able to get care at the r ight place, at the r ight t ime.

A $3 mil l ion grant from Par tners HealthCare and the Nor th Shore Medical Center to the Lynn Community Health Center wil l expand its physical space and increase access to primary care for nearly 4,000 children and adults. Partners continues to make significant contributions to support high school and college success through scholarships, coaching, tutoring, and mentoring. Through the Student Success Jobs Program (SSJP) at Brigham and Women’s Hospital more than 80 scholarships have been awarded and 96% of alumni are enrolled in or have graduated from college.

As par t of i ts 2011 bicentennial celebration, Massachusetts General Hospital is suppor ting 26 Boston, Chelsea, and Revere high school s tudents. As Bicentennial Scholars, they wil l receive scholarships and a range of suppor ts to help them enter and graduate from college.

Our hospi ta ls and community -based programs provide community res idents and employees wi th heal th care career t ra in ing and ski l l development oppor tuni t ies . More than 7,500 employees par t i c ipate annual ly in Par tner s workforce programs aimed at career advancement .

More than 45 programs across Par tner s have ass is ted over 42,000 pat ients and community res idents to lead heal th ier l ives through prevent ion ef for ts in areas such as subs tance abuse, domest i c v io lence, cancer, chronic d isease, heal th dispar i t ies , hunger, homelessness , and obes i ty.

Through a Revere -based community coal i t ion cal led Revere C ARES, a lcohol and drug use among high school s tudents dec l ined - the number of young people who repor ted dr inking decreased by 27%, and the number who had ever used mar i juana decreased by 12%.

Technology of fer s innovat ive oppor tuni t ies to engage and educate pat ients . We are us ing ce l lu lar and other technologies to help people bet ter manage their heal th care. Par t i c ipants in a text messaging program for pregnant women in Lynn had a 9% increase in meet ing the recommended levels of prenatal care.

Through Step i t Up! , an innovat ive heal thy l iv ing program suppor ted by Par tner s Heal thCare, Dorches ter e lementar y s tudents increased their phys ical ac t iv i ty. More than 160 s tudents walked over 50 mi l l ion s teps in 13 weeks.

Community -based programs target ing chronic d isease have resul ted in improved heal th outcomes and reduced cos ts of care for pat ients . Par t i c ipants in an MGH-Chelsea Heal th Center Diabetes program exper ienced a 10% reduct ion in their b lood sugar levels .

Manag ing Ch ron i c D i s ea se

Community heal th worker s of Br igham and Women’s Hospi ta l Prevent ion and Access to Care and Treatment (PACT) program have helped par t i c ipants reduce their inpat ient cos ts by 60% and hospi ta l length of s tay by 35% through bet ter access to outpat ient and pharmacy care.
