TerminalOperators:Inputinformationintositespecificcomputersystems,inspectlunchboxes,showsafetyorientation,gradetest,givesbreakstotheSwitchboardOperator,anddositeinspections.TheyarelocatedattheSecurityOperationsControlBuilding.
SwitchboardOperator:AnswercallsforMarathonandtransfersthemtotheappropriateperson,issuesscancardsforvisitorsintothemainoffice,andhandlesdeliveriesfortheoffice.TheyarelocatedtheMarathonMainOfficebuilding
GateGuards:Inspectvehiclesenteringandexitingthesiteandcompletesmonthlyinspections.TheirlocationsareC-GateandSWGate.
WestReceivingGateGuards:Inspectvehiclesenteringandexitingthesite,weighssemi-trucksbringingproductintoortakingoutofthesite,andissuesscancardsfordeliveriesandvisitorsenteringtheRefinery.Theyalsocompleteinspectionrequiredfortheirlocation.
SOCGuards:Monitorcamerasandradios,activateEmergencyProcedureswhenrequired,answertelephones,andcompletemonthlyinspections.TheyarelocatedintheSecurityOperationsControlBuilding.
PatrolGuards:Patrolperimetersandotherassignedlocations,openandclosegates,inspectvehicles,completeinspectionsandgivebreaksattheSOCandCGatewhenneeded.
EscortPatrol:EscortsdeliveriesandchemicaltrucksintotherefineryandassistwiththegateguarddutiesattheC-Gateandpatroldutieswhennecessary.

TrafficControlPatrol:Conductstrafficstops(Radar,StopSigns,&CellPhones)intherefineryandassistswithpatroldutieswhennecessary.EscortsIndianaRailTrainthroughtheplant,stoppingatallintersectionsaheadofthetraintowarntrafficoftheoncomingtrain.
Sergeant:WehavePatrol,WestReceivingGate,andTerminalOperatorSergeants.Theirdutieswouldincludeoverseeingeachoftheirareasofresponsibilityandcompletethetasksforthoseareasalso.
TrainingCoordinator:IsresponsibleforthenewhireandannualSecurityTraining,upkeepofSecurityTrainingmanuals,andalsohasanareatheyareresponsiblefor.
SafetyCoordinator:Conductssafetytrainingfornewhires,annualrefreshertraining,andObservationstrainingforallemployees.IsresponsibleforVPPApplication/AnnualEvaluation,upkeepofSafetyManual,overseecommittees,enterObservationinformationintothesystemandhasanareatheyareresponsiblefor.
Lieutenant:TheSr.Lieutenantoverseesallpersonnel,keepstrackofrequestedinformationindifferentprograms,andthecompanyclientliaison.TheJr.Lieutenantisafieldpositionandhaspersonneltheyareresponsibleforandassistwithpatrolduties,andinspectionsandclientaudits.
ContractorSecurityCoordinator:Overseesallpersonnel.
Ourmostrecentnon-safetyinitiativesandcompanychangeswouldconsistofofficelocationchangesandchangesinthetrainingformat.

DescriptionoftheWorkers
Basicinformationonouremployeesincludes:Age,ouryoungestis19andoldestis76foranaverageof38.65;LengthofServiceshortestis6monthswiththelongestbeing26yearsforanaverageof6years;andasfarastheEducationofouremployeeswegofrom9thgradeto3yearsofcollegewithanaverageofhighschoolorequivalent.
Managementturnedouttobealittledifferent.WithanaverageageoftheManagementstaffbeing43,theyoungestis36andtheoldest51;ouryearswiththecompanyaveragedouttobe17;andouraverageeducationis13years.
TheStateofIllinoisrequiresthatallemployeesbetrainedaminimumof20houruponbeinghiredasaSecurityGuard.Thistrainingisclassroomtraining.WeatStewartSecuritytrainouremployees40hoursofclassroomtraining.Thistrainingincludeslaws,self-defense,procedures,safety,haz-com,defensivedriving,etc.:Aswellas40to320hoursofonthejobtraining.
WealsohaveQuarterlyMeetingsthatincludesSecurityaswellasSafetyTrainingineachofthem,withmonthlySafetyMeetingsanddailyToolboxMeetings.
TheemployeesofStewartSecurityhavedevelopedandrolledoutaVoluntaryStretchingProgramthatwassetupforeachjobdescription.Theyareencouragedtotakeadvantageofstretchingbeforeeachshiftbeginsandwhentheyhaveajobassignmentwouldbedifferentthanwhattheyweredoing.(Examples:shovelingsnow,hangingsigns,etc.)
OurTIERSafetymeetings,toolboxmeetings,anddatabeingpassedtoouremployeesarecompletedinadifferentmannerthanmostothers.Wehaveshiftsaroundtheclockthereforemakingitimpossibletogeteveryonetogetheratthesametime.InourmonthlyTIERmeetingsMarathonsSTEPPacketispassedalongtoouremployees.WeaddourinformationtothismeetingregardingAt-RiskbehavioralthathasbeennoticedintheObservationprocess.Wealsoreview

pertinentobservationcomments.Allnearmisses,firstaids,andOSHArecordablesarealsoreviewed.
SafetyConcerns
AtthattimetherewerenosubstantialeventsthatmadeStewartSecuritythinkaboutbeginningaBBSProgram.HowevermanagementfeltaddingtheBehavioralBasedSafetytoourTraditionSafetyProgramwouldenhanceourprogramandwouldbringtheemployeestogethertoworktowardsacommongoal.
In2005MarathontrainedaboutaquarterofouremployeesintheObservationProcess.Andin2006theyformedtheContractorAdvisoryPaneltohaveaplacewhereallparticipatingfacilitatorscouldcometogetherandworkasacohesiveunittoencouragetheuseoftheObservationProcesswithallcontractemployees.
BBSData
Atourworksiteswehave2generallocationswheretheOSHA300Logispostedaspertheirstandards.AllemployeesaregivenamonthlysafetymeetinggivingthempertinentinformationfromMarathon’sSTEPpacketaswellasStewartSecurity’ssafetyinformation.
Ifwehavehadanearmiss,firstaid,orOSHArecordablewediscusswiththeemployeestheoutcomeoftheinvestigationwithacopyofthereport,minusnamesoftheemployeesinvolved.TheBBSDataisdiscussedregardingwhattheemployeesareseeingandhavetakenthetimetoactivelyfix.
AnytimewesubmitanSOS(SafetyOpportunityShared)toMarathonwefollowupwiththeemployeesotheyarekeptinformed.
Thedataisanimportanttooltousetogivemanagementandtheemployeesapointofreference.Somethingwetrend,lookat,andlearnfrom,becausewithoutthedatawewouldalwaysbereactingtoincidentsnotbeingproactiveinourworkandsafety.Thetopat-riskbehaviorsbothMarathonandStewartsarereviewedmonthlywithallemployees.Ifatanytimeweseeanincreaseinaparticularat-riskwewillreviewitduringourmorningmanagementmeetingandpassthatinformationalongtotheStewartSecurityEmployeesintheformofatoolboxmeeting.Bykeepingtheemployeesinformed,italsoraisesawarenessinthe

scopeoftheirwork.Alsowehaveanewsletterwehandouttoalltheemployeeswherewecanincludetopat-riskbehaviorarticlesorinformation.
Welogandtrendourownsafetyincidentstoreflecttherealityoftheincidentsthatarereported.TheBBSinformationisenteredattheStewartOfficebyafewspecificpeople;nooneelsehasaccesstothatparticularpartofthedatabasesowecanavoidunder-reportingduetofearofrepercussion.
WeaskthatemployeesbetrainedontheObservationprocessbeforetheydoaformal,writtenObservation.StewartSecuritydoesitsowntraining,consistingofHazardAwareness/RecognitionandhowtocompleteObservations.Currentlywehave31trainedObserverswithapproximately9or10atthistimetobetrained.Observationsarereviewedfor“PencilWhipping”andifwefeelthereisaproblemwedoacoachingsessionwiththatObserverontheproperwaytocompleteanObservation.Thisisdeterminedbypatternsinresponses.Wehavealsoconductedrefreshertrainingforouremployees.
DescriptionofBBSProgram
TheCambridgeCenterforBehavioralSciencesuggestedtoMarathonthroughtheAccreditationprocessthatthecontractorcommunitybecomeinvolvedintheBBSProcess.In2005,MarathonstartedtrainingcontractemployeestocompleteObservationprocessandHazardAwareness.In2006,theBBSCAPGroupwasformedwithStewartSecuritybeingoneofelevencompaniestoparticipate.
Thepurposeofthisprogramistoencourageemployeeinvolvementinthesafetyoftheirwork,completeobservationsonouremployeesaswellasMarathonandothercontractors.Totakethedatareceivedfromtheseobservationsandturnthemintopositivelearningexperiencesforallofouremployees.
StewartSecurityemployeesformedaBehavioralBasedSafetyawarenesscommitteeorsteeringcommitteecalledTEAMS(TeachingEmployeesaboutMaximumSafety)inOctoberof2005.Thiscommitteeismadeupofhourlyemployeeswhohaveadesiretoseeourworkforceandworkenvironmentsafeandaccidentfree.OursloganforthiscommitteeisGAS(GuardsAdvancingSafety).“OurmissionistocontinuetoinstructStewartSecurityemployeesonBehavioralBasedSafetyandtokeeptheprocesssimpleandworkable.”Wesetgoalsannuallytoensureacontinuousprocess.Wereviewdata,plantoolbox

meetingsandcontestsregardingobservations,encouragetheemployeestoparticipate,andtrytohavefunwiththisprocess.
EmployeeinvolvementistheessentialpartoftheBBSprocess.Howeveremployeeinvolvementisvoluntary.Wedonotrequiretheemployeestoparticipateintheobservationprocessbuthopethatwehaveenoughinformationtoencouragethemtoparticipate.BybeinginvolvedinthisprocesswehopethatbeforeeachjobiscompletedeachemployeelearnstodoamentalJHAtoraisetheirawarenessofthehazardsofthetask.
TheBBSprocessisbackedbythemanagementofStewartSecurity.LarryStewart,President,hasmadefinancialcontributionstoourawardsaswellassmalltokensforourmonthlytoolboxmeetings.He,asanemployer,hastakentheinitiativetopaythecommitteemembersfortheirtimewhenparticipatingonthecommittee.
In2007,StewartSecurityimplementedtheirtrainingprogramthatshowshazardsthattheiremployeeswouldencounter.ApowerpointpresentationisgiventoallpersonnelwhoareinterestedinbeingtrainedtodoObservations.WestartoutwithAwareness/HazardRecognitionTraining.Throughthisprocesstheylearnhowtoproactivelyidentifyandeliminateat-riskbehaviors.Todothistheyusepositivereinforcement.Firstandforemostthesafetyoftheemployeesisourgreatestinterest.Inourtraining,theylearnwhypeopletakechancesorenterintoat-riskactions.Aftertheawarenesstraining,theemployeeslearnhowtocompleteanObservation;whattolookfor,howtocommunicatewhattheysee,howtocompletetheforms,andwhattodoaftertheyhavecompletedtheObservations.
AbasicobservationformisbeingusedbyMarathonandmostcontractorsatthistime.Iftheinformationontheformdoesnotapplytous,wedon’tmarkit.AnObservationismadeonanyemployeewhohasgivenpermissiontobeobserved.Theytakefrom3to5minutestocomplete.Thefirststepoftheobservationistoasktheemployeeifitisokaytocompleteanobservation.Iftheysayyes,proceed.Ifnotthengoontothenextpersonandtryagainsomeotherday.RememberthisisaNONAME,NOBLAMEprocess.Whentheobservationprocessstarts,theywatchwhattheemployeeisdoing,whatjobtheyareperforming,whathazardsarearound,iftheemployeeisperforminganyoftheirjobinanat-riskmanner.Aftertheobservationiscompletetheyhaveapositivediscussiononwhattheydiscoveredwhileobservingtheemployee.Ifanat-risk

behaviorisfound,thediscussionwillturntotryingtoconnectabarriertoit.(Note:AfuturegoalistoconstructourownObservationForm.)
InourObservationprocesswedoofferincentivesforouremployees.Forevery10Observationstheycompletetheymayopttotakea$5.00mealvoucherfromalocallocation.HoweveriftheydonotwishtousethemealvoucherprocesstheymayaccumulateObservationsandaftertheyhavecompleted50theymaycashtheminforagascardorgiftcardvaluedat$25.00.Theymaysaveupandcollectonceayeariftheywant.Theirtotalrollsoverfromyeartoyear.Howeverwedosubtractouttheobservationsonourspreadsheetwhentheywanttocashout.AsanadditionalincentiveLarryhasofferedlunchesforreachingparticipationgoals.

In2005westartedtrackingObservationswhenlookingatthechartabovewewantyoutoseethataswegotbetteracquaintedwiththeworkingsofBBSourreportingofnearmissesforinformationalpurposesincreased.Asyoucanseeuntil2008nonearmisseswerereported.Thenearmissesalsoincludevehicleaccidentswherenoemployeewasinjuredtherewaspropertydamageonly.

Aftertheaccreditationprocesswewereencouragedtotargetbehaviorsmorerelevanttosecuritywork.IhaveattachedtwonewObservationformsthatwerecreatedandapprovedbytheemployeesofStewartSecurity.ThelastformwasupdatedonJanuary8,2015andtheoneweuse.
Weareawarethattheaccreditationteamwasencouragingustolookattheentirerefineryandtheaspectsofourjobsthataffecteveryonewhoenterstherefinery.AfterfurtherdiscussionswehavedecidednottomakevehicleinspectionspartoftheObservationprocesstokeeptheprocesspurewitha“NoName,NoBlame”policy.Inspectionsthatuncoverviolationsmustresultindisciplinaryactionsousingtheseasobservationscouldtainttheobservationprocess.
AnotherrecommendationwasthatweconductvideoobservationfromtheSOCandtheSwitchboardoranylocationthatwouldhaveaccesstovideofeedfromMarathon’sperimetercameras.Belowisagraphtoshowtheincreaseinvideoobservationversusselfandpeertopeerobservations.Videoobservationsarecompletedbyaskingthepersontheyarecompletingtheobservationonifitisokaytocompletetheobservation.Aftercompletingtheobservationtheyasktheobservedtocomeandseethemassoonastheycangetawayfromtheirregulardutiessothattheycanhaveaconversationwiththem.Thefeedbackisnotalwayssoon,certainandpositiveduetothefactthattheobservedmaybetiedupatagateforhourscompletingtheirjobduties.

ButbecauseoftherequestfromtheCambridgeCentertodovideoobservations,observationshaveincreasedandemployeeshavemoreopportunitiesthantheymightnormallyhavegottenpriortotheencouragementofvideoobservations.ThistargetsthestationaryjobsliketheSwitchboard,SOC,andTerminalOperators.
