Committees and RolesSteering Committee: The Steering Committee is comprised of executive decision makers responsible for reviewing

and approving the community health improvement process, ensuring quality, and assuring vision is maintained. The Steering Committee 1) identifies key stakeholders to be interviewed for the CHA CHIP informant interviews; 2) convenes the larger stakeholder and partner meetings; 3) oversees the work of all committees to ensure work is completed within the necessary timeframe; and 4) assures completion of the CHA and CHIP.

Core Coordinating Committee: The Core Coordinating Committee serves as the overall steward of the process including the following: 1) Make recommendations to the Steering Committee based on best practices, work from Subcommittees and community groups; 2) Review work from Subcommittees and community groups; 3) Develop roles, responsibilities, framework, timeline, and process plan; 4) Participate in Subcommittee work; and 5) Other responsibilities include capturing the history of decision making.

Data and Research Subcommittee: The Data and Research Subcommittee's responsibilities include identifying community health issues to be addressed within selected communities and developing issue and geographic area specific presentations. Plan development includes identifying key health and human service indicators, quantitative data gathering and review, data analysis, research methods, and research questions. This committee coordinates with the Community Engagement Subcommittee to inform and support qualitative data collection methods and information sharing.

Community Engagement Subcommittee: The Community Engagement Subcommittee is responsible for engaging the community to participate in focus groups, community meetings, stakeholder interviews, surveys, and other activities. This includes marketing of CHA CHIP process and reports and oversight of the implementation and engagement efforts. This team ensures that information is shared and solicited from diverse geographic and socioeconomic areas in our community. This committee coordinates with the Data and Research Committee to gather feedback on qualitative data collection methods and information sharing.

Logistics Subcommittee: The Logistics Team supports all subcommittee meetings including maintaining contact information, scheduling dates and rooms, note taking, and material distribution. It is responsible for maintaining infrastructure to sustain processes by identifying resources and ensuring support for ongoing partnership and community participation.

Partner/Stakeholder Group: The Partner/Stakeholder Group is comprised of partners and stakeholders who review the CHA CHIP process and the deliverables. The Steering Committee convenes these meetings. This group will be presented with information and will provide feedback to ensure all groups are included and pertinent information is solicited.
