Team Nominations for the 2018 State Championships will need to be submitted in MyNetball prior to the completion of this registration form (updated instructions for 2018 are available on the State Championships page of the Netball NSW website). This registration form will then need to be completed and submitted together with the Team Nomination player list/s downloaded from MyNetball, no later than 5pm Friday 6 April 2018.

4. Official Championship Photography Opt-OutSome Associations will have players participating in the 2018 State Championships who are unable to be photographedby our “photography partners” for legal reasons.

To ensure we are able protect these players we request that each Association ask parents to identify any players who are unable to be photographed. If your Association has players who are not to be photographed please indicate the team in which they will participate in the table below.

5. Umpire AgreementI certify that Netball Association will provide one non-playing badged umpire per teamentered, plus an additional non-playing badged umpire for the duration of the 2018 State Championships Competition. Iunderstand that umpires from my association must be submitted together with the team registration form on Friday 6April 2018 and confirmed by Friday 18 May 2018.
