Cardiac arrhythmias are a major cause of death (7 million cases annually worldwide; 400,000 in the U.S. alone) and disability. Yet, a noninvasive imaging modality to identify patients at risk, provide accurate diagnosis and guide therapy is not yet available in clinical practice. Nevertheless, there are various applications of electrophysiologic imaging in humans from ECG/CT reconstructions, MRI to tissue Doppler investigations that provide supplimentary diagnostic data to the cardiologist. EP laboratories are experiencing an increase in volume, for both diagnostic and interventional electrophysiology studies, including mapping, ablation, and pacemaker implants. The equipment requirements for these procedures are stringent, include positioning capabilities, and dose management. This book is designed to review all of the current imaging methodologies that assist in diagnosis within the electrophysiology department.

This new textbook wants to offer to neonatologists and pediatricians a modern and complete view of the various problems and aspects of neonatology, currently one of the most complex and advanced fields of pediatrics. The first chapters will be dedicated to the epidemiology of neonatal mortality and morbidity and to the conditions responsible for neonatal risk. A section will be devoted to organizational problems of hospitals and home services for efficient modern neonatal and infant care. Particular attention will be paid to neonatal care, medico-legal questions, examination of newborns and current availability of laboratory facilities and instrumentation for identification of perinatal abnormalities. Neonatal nutritional problems will also be examined, outlining current knowledge of the needs of sick and healthy babies. Conditions jeopardizing fetal health such as diabetes, maternal drug abuse and smoking will be discussed. New advances in neonatal pharmacology will be extensively examined. Various diseases of the neonate involving lungs, heart, gastrointestinal tract, blood, immune system, endocrine system and kidneys will be also  expounded in special chapters. Fetal and neonatal infections will be extensively discussed. Fetal and neonatal neurological abnormalities will be discussed by highly specialized authors. There will be chapters on neonatal ophthalmology, dermatology and orthopedics. Each chapter has beeb written by  a well recognized international expert in his or her field. As the opinion leader in their field, the Author is responsible for giving the most up-to-date information in terms of what is known, what is still being researched, and what has become evidence based medicine. Underlying causes and mechanisms of neonatal diseases will be presented in an immediate form. The use of summaries, tables, and accurately selected guidelines or recommendations that will accompany the text will supply quick references and instant solutions to the concerned neonatologists during their daily practice.

With the time available to surgeons-in-training ever dwindling, there is great emphasis placed on practical learning tools. Mirroring his earlier book on practical procedures in trauma surgery, Prof Giannoudis has produced a reference in more elective techniques. In most medical schools, most emphasis is placed on orthopaedic trauma surgery, with elective techniques often delayed until much later in a surgeon’s training.

The main target of clinical and academic cardiology activities is to optimize the patient‘s management and ultimately their welfare. Professionals manage patients according to their own understanding of disease process; clinicians concentrate on alleviating the symptoms and echocardiographers on identifying the disease. This book helps to ‚cross the barrier‘ and describes the common ground between physiologic disturbances and their management which should ideally form the shared basis for understanding and managing all cardiac problems. A practical review of the many uses of echocardiography in clinical practice, Clinical Echocardiography is designed to integrate and refine the investigation of cardiac disorders within the framework of the pathologic, physiologic and surgical appearances of heart disease. It has been common to consider echocardiographic findings separately from the more physical aspects of heart disease. However, with the development of new imaging modalities such as three- and four-dimensional echocardiography, it has become necessary to consider this imaging as a window on the heart. This book aids the cardiologist, cardiac surgeon and echocardiographer to integrate their knowledge within the overall management of cardiac disease in a clinically relevant manner.

Children with chronic conditions, developmental disorders, and birth defects represent a sizeable minority of American children—as many as one in five. Often their families have financial or other issues limiting their access to appropriate care, thus limiting their adult prospects as well. Compounding the problem, many valuable resources concerning this population are difficult to access although they may be critical to the researchers, practitioners, and policymakers creating standards for quality care and services. In response, the Handbook of Children with Special Health Care Needs assembles research, applied, and policy perspectives reflecting the range of children’s problems requiring special services. Widely studied conditions (e.g., communication disorders, substance abuse) and those receiving lesser attention (e.g., tuberculosis) are covered, as are emerging ideas such as the “medical home” concept of continuity of care. Its interdisciplinary outlook makes the Handbook of Children with Special Health Care Needs a vital, forward-looking text for developmental psychologists, pediatricians, early childhood and special education researchers and practitioners, disability researchers, policymakers, and advocates, and providers for children with special health care needs.

Age-related macular degeneration is the most common cause for the loss of central vision beyond the age of 50 in industrial nations. Triplication of the number of affected patients is expected over the next 25 years. Especially over the last years the standard of knowledge regarding etiology, risk factors, diagnostics and therapy of this retina illness has substantially grown – this will be covered in this up-to-date multi-authored work. Apart from epidemiologically and genetically identified risk factors both the various pathophysiological aspects including the role of the complement system and clinical manifestations including OCT and angiographic characteristics are clearly represented. Furthermore, the different therapeutic approaches are presented and discussed, including proven procedures such as intravitreal anti-VEGF therapy and seeing-aid systems, in addition to the latest and upcoming methods in the area of pharmacology. The volume is well-illustrated and tables and summaries complete the presentation.

Our knowledge of cerebellar functions and cerebellar disorders, called ataxias, is increasing considerably. Studies of the cerebellum are now a central focus in neuroscience. During the last four decades, many laboratories worldwide have dedicated their research activities to understanding the roles of the cerebellum in motor control, cognitive processes and biology of mental processes, behavioral symptoms, and emotion. It is now accepted that the cerebellum acts as a cognitive operator in learning, perception, and attention. Moreover, major improvements in our assessment of in vivo cerebellar architecture using imaging techniques have occurred. A typical example is the accurate description of cerebellar anatomy during fetal development with MRI, a progress which has direct impacts on patient care. These advances have been associated with discoveries of new clinical disorders, in particular in the field of genetic ataxias. More than 20 new genes have been identified these last 10 years. Only for dominant ataxias, more than 30 diseases have now been unravelled. The number of ataxic disorders will increase with aging, the cerebellum being the structure of the brain with the most important loss of neurons with age. More than 300 different cerebellar disorders are encountered during daily practice, but we are missing a single source of information explaining their pathogenesis. Despite the immense amount of knowledge acquired about the cerebellar circuitry these last years, a large book covering the neuroscience of the cerebellum is missing. The goal of this endeavour is to bring up to date information relevant for basic science and also for clinical activities. To reach this goal, the most renowned authors will be gathered in a unique and in-depth book with a format of a handbook. We will emphasize the connections between molecular findings, imaging features, behavioural/neuropsychological aspects, and clinical implications.

Cranial ultrasonography is the most important, accessible, patient friendly, and cheapestneuroimaging technique on the neonatal ward. It provides important information on brain maturation in the (preterm) neonate and enables the detection of frequently occurring brain anomalies. In this second edition of Neonatal Cranial Ultrasonography, the focus is on the basics of the technique, from patient preparation through to screening strategies and the classification of abnormalities. Many new ultrasound images have been included to reflect the improvements in image quality since the first edition. Essential information is provided about both the procedure itself and the normal ultrasound anatomy. Standard technique is described and illustrated, but emphasis is also placed on the value of supplementary acoustic windows. The compact design of the book makes it an ideal and handy reference that will guide the novice but also provide useful information for the more experienced practitioner.

The two previous editions of Applied Physiology in Intensive Care Medicine proved extremely successful, and the book has now been revised and split into two volumes to enhance ease of use. This first volume comprises three elements -- „physiological notes,“ “technical notes,” and seminal studies. The physiological notes concisely and clearly capture the essence of the physiological perspectives underpinning our understanding of disease and response to therapy. The technical notes then succinctly explain some of the basics of “how to” in this technology-centered field of critical care medicine. Finally, a number of seminal studies are provided on diverse topics in intensive care. Applied Physiology in Intensive Care, written by some of the most renowned experts in the field, is an up-to-date compendium of practical bedside knowledge that will serve the clinician as an invaluable reference source on key issues regularly confronted in everyday practice.

This is the first book to be devoted exclusively to rare tumors in children and adolescents, and its aim is to provide up-to-date information on their diagnosis and clinical management. The opening section addresses general issues including epidemiology, risk factors/etiology, biology and genetics, early detection, and screening. It also discusses solutions to assist in the management of rare tumors, such as international networking and internet platforms. In the second section, specific malignancies are described, with practical guidance on diagnostic workup, multimodal therapy, follow-up, and adverse effects. Discussion of differential diagnosis encompasses both frequent and rare tumor types, which should enable the clinician to take rare entities into account during the diagnostic assessment. Each chapter goes on to provide detailed therapeutic guidelines for specific rare tumors. The authors are a multidisciplinary group of specialists who have dedicated themselves to this group of tumors.

The Ilizarov device has revolutionized the treatment of non-healing fractures and the correction of deformities. This book supplies all the information required in order to use the Ilizarov and other external fixation devices optimally; it will serve as an indispensable manual for both trainee and experienced orthopedic surgeons. Biomechanical principles, preoperative preparation, and the use of a system of coordinates to allow safer insertion of K-wires and half pins are thoroughly discussed. External fixation of a variety of fractures in different pathologic settings is then clearly explained in a series of detailed chapters with the aid of high-quality illustrations. Numerous case reports are included to illustrate the results of different treatment methods. In addition, postoperative management and treatment of complications are described. Since the first edition the text has been thoroughly updated, with inclusion of contributions from leading world experts.

This manual for diagnostic cytologists offers detailed guidance on diagnostic problems likely to be encountered in everyday practice. It encompasses exfoliative and aspiration cytology of all major nongynecologic body sites. Each chapter opens with an algorithm that presents the reader with the relevant microscopic findings, the most important additional findings, and the differential diagnostic possibilities and problems in a clear and easily remembered form. Another important feature is the wealth of high-quality color photomicrographs, which clearly document the visual appearances of the most important lesions and highlight the differential diagnostic difficulties. The accompanying text contains helpful general remarks and presents further relevant information on diagnostics, differential diagnostic procedures, and auxiliary methods. Besides established cytologists and pathologists, cytopathologists in training and cytotechnologists will find this book to be a valuable aid.

With this seventh edition, Noback’s Human Nervous System: Structure and Function continues to combine clear prose with exceptional original illustrations that provide a concise lucid depiction of the human nervous system. The book incorporates recent advances in neurobiology and molecular biology. Several chapters have been substantially revised.  These include Development and Growth, Blood Circulation and Imaging, Cranial Nerves and Chemical Senses, Auditory and Vestibular Systems, Visual System, and Cerebral Cortex.  Topics such as neural regeneration, plasticity and brain imaging are discussed.  Each edition of The Human Nervous System has featured a set of outstanding illustrations drawn by premier medical artist Robert J. Demarest.  Many of the figures from past editions have been modified and/or enhanced by the addition of color, which provides a more detailed visualization of the nervous system. Highly praised in its earlier versions, this new edition offers medical, dental, allied health science and psychology students a readily understandable and organized view of the bewilderingly complex awe-inspiring human nervous system.  Its explanatory power and visual insight make this book an indispensable source of quick understanding that readers will consult gratefully again and again.

Computed tomography (CT) is a powerful technique providing precise and confident diagnoses. The burgeoning use of CT has resulted in an exponential increase in collective radiation dose to the population. Despite investigations supporting the use of lower radiation doses, surveys highlight the lack of proper understanding of CT parameters that affect radiation dose. Dynamic advances in CT technology also make it important to explain the latest dose-saving strategies in an easy-to-comprehend manner. This book aims to review all aspects of the radiation dose from CT and to provide simple rules and tricks for radiologists and radiographers that will assist in the appropriate use of CT technique. The second edition includes a number of new chapters on the most up-to-date strategies and technologies for radiation dose reduction while updating the outstanding contents of the first edition. Vendor perspectives are included, and an online image gallery will also be available to readers.

Widespread changes in health care delivery and financing have led to a major increase in the level of office-based care provided by urologists. Office Urology: The Clinician‘s Guide, Second Edition was created to provide a fresh, practical, and concise survey of the key medical and management issues every practicing urologist faces in the office on a daily basis. Here the busy urologist will find easy-to-use clinical articles--written by physicians with demonstrated clinical expertise--that summarize the diagnostic procedures and treatments now widely provided in the office, including intravesical therapy for bladder cancer, minimally invasive treatment of benign prostatic obstruction, transrectal ultrasound and prostate biopsy, and office urodynamics. In addition, there are helpful discussions on marketing the practice, billing and coding techniques, advanced information systems, and important legal issues, as well as expert advice on such neglected topics as genital pain, managing geriatric incontinence, practical dermatology, and counseling patients on the optimal management of their problems. All chapters are updated to reflect important changes and appropriate treatments since the first edition. An entirely new section focuses on general surgical principles for the urologist. Authoritative and practical, Office Urology: The Clinician‘s Guide, Second Edition offers every active urologist a gold-standard handbook of today‘s most appropriate treatments and successful office management techniques.

Reconstructive Colorectal and Anal Surgery presents a didactic discussion of complex colonic, rectal, anal, and perineal problems which require reoperative surgery. The book provides a very practical description of the management towards these problems including preoperative investigation, postoperative follow-up and detailed operative approach. The text is aimed at being relatively didactic with an algorithm approach to these complicated cases using operative photographs and composite explanatory line drawings which are complemented by ‘how-to’ guides in all cases describing the operative technical tips and pitfalls from experienced world-renowned commentators. Given the rapid change in techniques over the last 10 years newer procedures will be incorporated. This book will be essential reading for all colorectal surgeons and trainees, general surgeons and trainees, gynecologists as well as specialist nurses in the OR room.
