Therapeutic perspective of chronic hepatitis B and C in HIV-coinfected patients: from recent advances in immunopathogenesis to the patient March 31 st - April 1 st 2006 Caravella Pinta Hall San Raffaele Congress Center Via Olgettina, 58 Milan Presentation On the threshold of the third decade of HIV infec- tion, the improved survival of patients due to the use of HAART in industrialised countries has led to greater weight being placed on some causes of co-morbidity. On of these is decompensated hepatopathy, which represents the second cause of death after AIDS. Alhough the clinical management of HCV and HBV co-infections is becoming increasingly relevant in the treatment of HIV-positive subjects, there are still many barriers to hepatitic therapy due to both poor virologi- cal responses and possible interactions with antiretro- viral drugs. The results could be improved by strategies aimed at increasing the tolerability of pharmacological combinations as well as the efficacy, define of current therapies. The need to come to terms with highly differed thera- peutic questions has encouraged us to organise a meeting of clinicians and researchers with the aim of defining clinical pathways that will improve the treat- ment of HIV-positive patients. Based on the most recent immunopathogenetic and pharmacological findings, the scientific programme will consider how the immunopathogenesis of viral hepa- titis and HIV infection affects the natural history of co- infections, how retroviral therapy affects HCV and HBV therapy, and how to define the timing and treatment of the two infections in such a way as to ensure a better outcome by avoiding pharmacological interactions and viral drug resistance. In addition to lectures by experts, the meeting will provide an opportunity for practical and interactive dis- cussions of clinical cases and most frequent problems encountered in clinical practice. The aim of this critical review of clinical behaviour is to identify shared thera- peutic pathways on the basis of the most recent thera- peutic and pathogenetic findings. PresentationOn the threshold of the third decade of HIV infec-tion, the improved survival of patients due to the use of HAART in industrialised countries has led to greater weight being placed on some causes of co-morbidity. On of these is decompensated hepatopathy, which represents the second cause of death after AIDS.

Alhough the clinical management of HCV and HBV co-infections is becoming increasingly relevant in the treatment of HIV-positive subjects, there are still many barriers to hepatitic therapy due to both poor virologi-cal responses and possible interactions with antiretro-viral drugs. The results could be improved by strategies aimed at increasing the tolerability of pharmacological combinations as well as the effi cacy, defi ne of current therapies.

The need to come to terms with highly differed thera-peutic questions has encouraged us to organise a meeting of clinicians and researchers with the aim of defi ning clinical pathways that will improve the treat-ment of HIV-positive patients.

Based on the most recent immunopathogenetic and pharmacological fi ndings, the scientifi c programme will consider how the immunopathogenesis of viral hepa-titis and HIV infection affects the natural history of co-infections, how retroviral therapy affects HCV and HBV therapy, and how to defi ne the timing and treatment of the two infections in such a way as to ensure a better outcome by avoiding pharmacological interactions and viral drug resistance.

In addition to lectures by experts, the meeting will provide an opportunity for practical and interactive dis-cussions of clinical cases and most frequent problems encountered in clinical practice. The aim of this critical review of clinical behaviour is to identify shared thera-peutic pathways on the basis of the most recent thera-peutic and pathogenetic fi ndings.
