Vehicle Purchase/Lease Agreement
General Requirements:
• The date on the purchase/lease agreement must be within 6 months of the date of
application. In some cases, the date of delivery of your vehicle may be considered
your date of purchase/lease.
• A purchase/lease agreement confirms the sale or lease of a vehicle and may look
different depending on where the vehicle was purchased/leased.
• You must submit a complete, clear and legible scan/photo with all details visible and
all pages present.
• You must submit a final copy (executed and signed). Note: Review or pro-forma
copies are not accepted. If you do not have the final copy, request it from your
dealership or leasing agency.
• You can submit one of the following documents as a complete purchase/lease
agreement:
o Sample 1: Purchase agreement, if purchased
o Sample 2: Lease Agreement, if leased
o If the vehicle was purchased in a private-party sale, a copy of the current
Certificate of Title listing the new owner’s information will be sufficient.
Alternate Document Requirements:
• If your dealership provided you a Final Price Sheet and a Vehicle Configuration
instead of a multi-page purchase/lease agreement, please submit ONE of the
following document combination options (A or B) as your purchase/lease
agreement:
o Option A:
▪ Sample 3: Motor Vehicle Purchase Agreement: Vehicle Configuration
(with order date and VIN present)
AND
▪ Sample 4: Motor Vehicle Purchase Agreement: Final Price Sheet
o Option B:
▪ Sample 3: Motor Vehicle Purchase Agreement: Vehicle Configuration
(with VIN present, but no order date)
AND
▪ Sample 4: Motor Vehicle Purchase Agreement: Final Price Sheet
AND
▪ Signed Delivery Declaration or signed Terms & Conditions

Sample 1: Purchase Agreement
Scan all pages of your signed purchase/lease agreement and combine them into one file. All
sections, signatures, and pages must be present and legible. This can be a carbon copy or a
digital copy from your dealership.

All sections, signatures, and pages must be present and legible. This can be a carbon copy or
a digital copy from your dealership.

Sample 3: Motor Vehicle Purchase Agreement: Vehicle Configuration
This document must be submitted with Sample 4, your Final Price Sheet, to create a
complete purchase agreement.
This document should have your VIN and an order date present. If your Vehicle
Configuration is missing an order date, please refer to Option B on page 3.

This document must be submitted with Sample 3, your Vehicle Configuration, to create a
complete purchase agreement.

Proof of Vehicle Registration
• Your vehicle must be registered to your current, residential SCE address and your
registration document must be valid. o If your registration lists an out-of-date address, you may also submit a Department of Motor
Vehicles Change-of-Address Form showing that your address is now changed to your current,
residential SCE address.
• Your document must be a complete, legible scan or photo with all details visible.
• Documents that can be used as Proof of Vehicle Registration include:
o Sample 5: Permanent Registration (Registration Card)
o Sample 6: Temporary Registration (New or Used Vehicle Dealer Notice)
o Sample 7: Temporary Registration (Application for Registration of New or
Used Vehicle)

This document is typically found taped to your windshield by your dealer. Unfold this document so
all text is visible.

Sample 3: Temporary Registration (Application for Registration of New or
Used Vehicle)
This document must be signed in the Applicant’s Certification section and must list both
dates at the top.

Electrician Itemized Invoice
This document can vary depending on your home and the licensed electrician you choose to
perform your work. The following information must be visible on your invoice:
o The date the work was initiated
o The address the work took place
o The electrician’s C10 license number
o The itemized amount charged for labor and materials
For this document we accept both digital and written formats, please be sure the license
number is clear and visible on the document or it will not be accepted.

Proof of Permit
This document can vary depending on the city you reside in, but the information presented
on the document should be similar. Information that must be present includes:
o Date of permit
o Job address
o Type of permit, such as electrical
o Permit fees (If fees not listed on permit or electrician invoice, provide additional
receipt of fees paid.)

Proof of Purchase of Level 2 Charging Station
This document can vary depending on how you purchased your Level 2 charging station.
If your vehicle came your charging equipment, such as a mobile connector or charging
cable, please upload documentation showing your vehicle came with the equipment. This
documentation will frequently look like Sample 2.
If you have no documentation showing that your equipment came with the purchase/lease
of your vehicle, please let us know.

Photo of Installed Charging Station
Any charging cables should be plugged into your 240V outlet in your submitted photo. Your
240V outlet does not need to be visible if your EV charging station is fixed to the wall and is
covering the outlet.

this Homeowner/HOA Authorization form. Then have the owner of your home circle the
appropriate title and fill out and sign the second half of this document.
If you own your home and your home is a part of a Homeowners Association (HOA):

Then have a member of your HOA circle the appropriate title and fill out and sign the
second half of this document. Note: If no HOA approval was required for your installation
work, you may instead upload a simple statement confirming this.
