Our locations• SWAMS Clinic- Currently based at the South West Health Campus

• Social, Emotional & Wellbeing Team (SEWB)- based at the SWAMS administration building Wellington Street, Bunbury

• Home and Community Care (HACC) Team- provide community care, support, transport to our HACC clients, based with at Admin in Bunbury

• Manjimup Clinic- One GP clinic per fortnight, plans to trial weekly services due to demand

• Collie Clinic- One GP clinic per fortnight, review of new due to strong existing GP services in the area. View to use room to provide Chronic Disease Clinics, managed by nursing and AHW professionals

• Brunswick Clinic- One GP clinic per week, well accessed by local Aboriginal families.

SWAMS has a 5 year plan, to purpose build a single building. This will accommodate all

Social, Emotional & Well-Being TeamThe SEWB team provides an holistic service to Aboriginal people of all ages impacted by the Stolen Generation and/or Family Violence.

SEWB provides: • Counseling• Advocacy - e.g. Housing, Justice, DCP, Centrelink• Referral - e.g. medical, clinical psych, D&A services• Practical & emotional support

“COW” Clinic On Wheels

Purpose built mobile clinic with fantastic facilities

Suitable for use with General Practice, Child & Maternal Health, Chronic Disease & Allied Health Clinics

SWAMS are currently undergoing a process of community consultation and need analysis to plan a schedule of services to be facilitated through the COW

Where to next…?

SWAMS plans for the next 12 months include:• Improve and expand our Mental Health and Aged Care

services• Increase health promotion activities• Expand our Child Health Team• Increase and develop further partnerships in the South

West• Possibly relocate Bunbury Clinic- larger premises• Achieve ISO 9001 Certification (early 2014)
