METHODOLOGICAL DIFFERENCES BETWEEN HOMEOPATHIC AND CONVENTIONAL CLINICS
On the bas is of our c l inica l exper ience, in th is paper we a im at out l in ing the essent ia l methodological d if ferences between a l lopath ic and homeopath ic c l in ics . The former is evermore based on the formulat ion of nosographic and instrumental d iagnos is and on the appl icat ion of therapeut ic protocols . The lat ter , the medic ine car ing about the man, operates by recognizing in the s ick person the pathogenet ic pic tures of var ious remedies, as they result f rom the provings. Al lopath ic Medic ine deals wi th the man only as i t concerns h is systems, organs , t issues, ce l ls , genes enzymes and operates in an evermore spec ia l ized and analyt ic dimension. I t looks upon d isease as an organic les ion, a struc tura l change in a t issue or a modif icat ion in a b iochemical parameter: that is , i ts perspect ive is a mechanis t ic one. According to what usual ly sa id by Prof . Tomas Pablo Paschero, the s ick person is cons idered as: ”a machine, a phys ical -chemical tool which is in a s tate of dynamic equi l ibr ium wi th the environment and moves in a c losed c ircu it fol lowing mechanical laws and r ig id determin ism of cause and ef fect” . Homeopath ic Medic ine, on the other hand, as a v i ta l is t ic medic ine “regards the human being not as an organism which is only regulated by mechanical laws” s ince “ the psyche, the soul determines the phys io logical work ing, which keeps and permeates the balance among each par t” . Moreover “a l though each atom, each molecule, each cel l and each organ can -as analyt ic unites- preserve their own autonomy both in their form and their funct ion, i t is the g lobal organizat ion, the mutual dependence of the whole in the synthet ic uni ty, tha t g ives meaning to l i fe” . So, who is the s ick person? “He is a man, not h is organs. The organs are not the man and the man is over and before the organs. From the beginning up to the end, d isease and treatment have to go in the same direct ion: f rom the man to the organs, not the oppos ite. Therefore, l i fe is not represented by a s ingle cel l or an iso lated organ, but rather –and at an u ltramolecular level- by the spir i tual force that makes the man to be a human person. W il l and mind govern l i fe and, only i f they can cooperate in a balanced and harmonic way, the man could be heal thy. The homeopath does not ground his therapeut ic act ion on the nosological d iagnos is and on the appl icat ion of a therapeut ic protocol ; he is focused on the s ick person, on the morbid ind ividual i t y that expresses the psycho-bio logical react iv i ty. The homeopath wi l l heal what in every ind ividual , unique and non repeatable human being is wor th of cure“ . (Paschero) In Organon, paragraph 1, Hahnemann states: “The phys ic ian’s h igh and only m ission is to res tore the s ick to health, to cure, as i t is termed” .

As a consequence of these pre l im inary remarks, we had to face the fo l lowing two quest ions: How to show, in a concrete way, the methodological d if ference between homeopath ic and al lopathic medic ine? How to ver i f y therapeut ical ef fect iveness of the remedies? W e intended to ver i f y the pathogenes is and to conf irm the Simi l ia pr inc ip le: “A weaker dynamic af fect ion is permanent ly ex t inguished in the l iv ing organism by a stronger one, i f the lat ter (whi ls t d i f fer ing in k ind) is very s imilar to the former on i ts manifestat ion” (Organon, par .26t h) . From this point of v iew, we analyzed 2748 c l inica l cases, examined at L.U. I .M.O. in the las t f ive years . The cases were recorded by fo l lowing the b iopathographic protocol and the medical f i les were arranged (grouped) according to the v is i t ground and the pathology (see f ig . “Distr ibut ion of Pathologies”)

In this f igure, you can see that a great var iety of persons, suf fer ing f rom dif ferent pathologies –and not only f rom psychosomat ic ones, as i t is of ten held- comes to homeopathic v is i t . From each c l in ica l f i le we took out the prescr ibed remedy (1s t v is i t ) ( f ig . “F irst Prescr ipt ion”) .
I t is to be said, now, that homeopath ic d iagnos is and treatment are grounded on the Simi l ia law. What is impor tant to under l ine is that the Homeopaths do not prescr ibe a remedy on the bas is of the pathology, but of the s imi l i tude between the pat ient ’s symptoms and the pathogenet ic pic ture, as i t resul ts f rom the proving of that specif ic remedy. The fo l lowing cons iderat ions wi l l correspond to the d iagnos is of a remedy and not to the nosographic d iagnos is of a disease. Among var ious homeopath ic remedies, we choose Lycopodium , one of the main polycrests of the homeopathic Mater ia Medica. “Powder of Lycopodium” has been used, in the of f ic ia l pharmacopoeia, for a long t ime “as iner t powder in order to treat the inter tr igo in k ids and in the coat ing of p i l ls , so that they do not st ick together in the pack ing. Actual ly, s ince Lycopodium causes a lo t of erupt ions, his employment for the cure of inter tr igo is a homeopath ic act ion. The analyt ic mind of the of f ic ia l medic ine could not f ind in terest ing such a remedy that cannot be used in accordance to the “oppos ites” law. On the contrary, the synthet ic sp ir i t of Homeopathy f inds Lycopodium to be one of the greatest remedies of our Mater ia Medica” (Hodiamont) .

In f igure “Reasons of Fi rst Prescr ipt ion of Lycopodium” (151 c l in ical cases of Lycopodium and the ground of the f irs t v is i t ) you can see that Lycopodium has been prescr ibed to pat ients suf fer ing f rom a great var iety of pathologies . By showing three c l inica l cases (case A: psor ias is , case B: a l lerg ic asthma and case C: hepat i t is) , we wi l l pract ica l ly exempl i f y how a homeopath ic c l in ica l f i le is organized, how the d iagnos is of the remedy and i ts fo l low-up are formulated. Unfor tunate ly, we could present only a synthes is of our more ar t icu lated act ion.

In these c l in ica l cases, you can observe in pr im is that, as i t concerns famil iar anamnesis (both near and remote), objec t ive examinat ions and col lect ion of laboratory and ins trumental analysis , the homeopath records a lmost the same data as an a l lopath ic doctor does. Nevertheless, the homeopath holds as fundamental step - to get a complete b iopathographic h istory of the pat ient- other impor tant aspects concerning the pat ient ’s pr ivate, soc ia l and emotional l i fe , as wel l as part icu lar e lements on some cruc ia l events , which could have caused (according to our dynamic and vi tal is t ic idea of the human being) a change in that person. The change is responsible for unbalances in the patient’s vital force; it is the cause of the various symptomatic manifestations. In fact, we think that the different symptoms express just one “disease” –always the same- that owing to lasting allopathic treatment goes deeper and deeper.
The complete set of biopathographic data, together with the actualities, allows us to evaluate the patient’s constitutional (miasmatic) predisposition and anticipate the likely evolution of the disease.
After considering the biopathographic history of these 3 clinical cases so that we could understand the dynamic evolution of the morbid state, let us pay attention to the symptomatological actualities. We are going to take into account those symptoms which are -relating to the pathogenesis- consistent, exceptional, unusual, peculiar and which can lead us to the diagnosis of the remedy, by representing the M.V.M.S.. In the fo l lowing tables , the symptoms of the M.V.M.S. are compared wi th those of Lycopodium pathogenes is , as they were recorded in the Pure Mater ia Medica by Hahnemann (and a lso by others, Clarke for example) and in Kent’s reper tory.

SYMPTOMS RETURN 1S T CONTROL: - Pain when urinat ing (see anamnesis) - Strong hemorrhoids (see anamnesis) - I tch al l over the body with furfourus eruptions (see anamnesis)
Obvious ly, both actual i t ies and their evolut ions have been told by the pat ient and objec t ive ly observed by the homeopath dur ing the vis i t . By “amel iorat ion” of a symptom, we intend that i ts f requency and/or i ts in tens it y decrease. By “aggravat ion” of a symptom, we intend that i ts f requency and/or i ts intensi ty increase. The return of one or more symptoms represents the emergency of a past suf ferance which has been cured in an unnatura l way (we say i t was “suppressed”) . By analyzing the evolut ion of a c l in ica l case we can ver i f y the symptoms direc t ion and Her ing law of recovery. Moreover, we can evaluate the evolut ion i tse lf and formulate a prognosis . Now, we are going to show a l ike ly model to ver i f y c l in ica l ef fect iveness of a remedy: Lycopodium , accord ing to the epistemological f ramework of homeopath ic medic ine.

Obvious ly, i t is just one of the l ike ly approaches: there are certa in ly other check systems, which are based on not ions l ike “wel l-being” or on general measurements of the homeopath ic treatment ef fect iveness. Among 151 cases of Lycopodium , we took in to account 54, in which Lycopodium was prescr ibed again in two fur ther check v is i ts . In th is way we can have a bet ter fol low up, make a meaningfu l analys is of remedy ef fect iveness and of Lycopodium pathogenet ic p icture, conf irming the symptoms recorded in the proving ( f ig . ”Repeated Adminis trat ion of Lycopodium”).
Then, we evaluated the c l inica l evolut ion dur ing the f irs t s ix months of treatment (see f ig . “Cl in ical Ef f icacy of Lycopodium: Evaluat ion of Pure Exper imentat ion” and “Results of Lycopodium Act ion on Pathology”) .

The char ts show a posit ive evolut ion in a lmost every case: anyway these data are not sat is fac tory, s ince i t is not poss ib le to see the symptoms direct ion. So, tak ing into account a l l the symptoms analyzed to prescr ibe Lycopodium , we grouped them into pathogenet ic symptoms (ver i f ied by the proving) and cl inical symptoms (d isappeared, thus healed, af ter adminis ter ing Lycopodium but not ver i f ied by the proving).
According to homeopath ic method (see the protocol) , we grouped the symptoms in: menta l, general, phys ical with modal i t ies .
The charts in f igures “Cl in ica l Ef f icacy of Lycopodium – Menta l Symptoms”, “Lycopodium Ef f icacy – General Symptoms” and “Lycopodium Ef f icacy – Physical Symptoms” show a qual i tat ive check and a quant i ta t ive evaluat ion of Lycopodium pathogenet ic symptoms.

Tota l observat ions are 23 and the sum (ameliorated + healed) is 19: thus, the rat io of that symptom ( the ef fect iveness of Lycopodium on that symptom) is 0.83.
A stat is t ic measure of symptoms signif icance in each group or ser ies should be necessary. On this ground and not to make use of ca lculat ion devices, we need to increase our data base wi th other cases in which Lycopodium has been prescr ibed in two fur ther check v is i ts .

The last three char ts conf irm, on a concrete level, the Simil ia law, and thus the therapeut ic ef fect iveness of Lycopodium . On th is subject , we would l ike to remind what Hahnemann said in Organon, par. 27 t h : “The curat ive power of medic ines therefore, depends on their symptoms, s imilar to the d isease but super ior to i t in s trength, so that each individual case of d isease is most sure ly, radical ly, rapid ly and permanent ly annih i lated and removed only by a medic ine capable of produc ing ( in the human system) in the most s imilar and complete manner the tota l i t y of i ts symptoms, which at the same t ime are stronger than the disease”.

Conclusions 1. the grounds for the homeopath ic v is i t are d if ferent 2. in homeopathy: - the homeopath does not ac t on the bas is of a nosographic d iagnos is in
order to apply a correspondent therapeut ic protocol but is rather concerned wi th the s ick person, with that morbid ind ividual i t y which expresses the psycho-bio logical react iv i t y of each indiv idual , unique and non repeatable human being
- i t is poss ib le to make a prognos is on the bas is of g lobal evaluat ion of symptoms
- the 3 c l inica l cases, we presented above, we show that not only the symptoms of the diseases the pat ient suf fered f rom at the 1s t v is i t , but many others d isorders were improved or healed according to the Her ing Law
- the b iopathographic protocol is an usefu l too l s ince i t a l lows to record in a regular way data on ef fect iveness and c l in ica l act ion of homeopath ic remedies, according to the epistemology s tatus of homeopath ic medic ine
3. we could not stat is t ica l ly measure Lycopodium ef fect iveness, but in the 54 cases we analyzed we could not ice an improvement of about 75%, fol lowing i ts adminis trat ion.
