It is my great pleasure to welcome you to Coeur d’Alene for the 116th ISDA Annual Session. I would like to thank all dentists, staff, exhibitors and sponsors for attending this conference and making it a success.

We are pleased to be joined by the Idaho State Orthodontic Society, the Idaho Society of Oral and Maxillofacial Surgeons, the Idaho

Dental Foundation, the International College of Dentists, and for the first time, the Pierre Fauchard Academy.

I hope you will be joining me for the President’s Dinner, courtesy of Consani Seims, Ltd, and Advantage Dental, at the new Hagedone Event Center Friday night. Help me celebrate all ISDA Past Presidents as well as this year’s incoming President, Jack Klure, DDS, incoming Trustees, R. Kim Smith, DDS, SEIDS and Mark Sheppard, DDS, LCDS. Also, please help me congratulate this year’s Component President of the Year Award recipient, Kim Keller, DDS, the President’s Award recipient, Richard Ferguson, DDS, MS, and this year’s Lifetime Achievement Award recipient, Timothy Thompson, DMD.

incoming President

Jack Klure, DDSDr. Jack Klure was born and raised in California. He attended the University of California, Davis, graduating with a Bachelor of Science in Psychology. He was awarded his DDS from Creighton University, Omaha, Nebraska, in 1975.

Jack married the love of his life, Melinda, in 1970. Together they have three married children and five

grandchildren. Jack says he now knows what all the hard work was about, “I am thankful for the blessing of my family.”

Dr. Klure began private practice in California’s central valley. Early in his career he became involved in organized dentistry. He served on the Board of Trustees for the San Joaquin District Dental Society and then on the California Dental Association Peer Review Committee. In 1995, seeking a change in the quality of life, the Klure’s moved to Boise, Idaho. It was a wonderful change both for the family and professionally. In 2007 Dr. Klure relocated his practice to his present location in Meridian. Dr. Klure has continued his involvement in organized dentistry in Idaho serving the Southwest Idaho Dental Society as secretary/treasurer, vice-president and president, the Idaho State Dental Society as president in2002-2003 and slated again for 2012-2013, and currently as delegate to the American Dental Association.

Dr. Klure belongs to a number of dental organizations including: a fellow of the Academy of General Dentistry, a member of the Academy of Cosmetic Dentistry, a fellow of the International College of Oral Implantology, a fellow of the International College of Dentistry, and the Pierre Fauchard Academy. In addition, Jack is very active in his community serving as Dental Director for the Genesis Community Clinic and also providing free dentistry for the needy through a number of outreach programs.

Jack enjoys the outdoors and the multitude of opportunities provided by Idaho to experience God’s creation. He has a passion for classic cars, tinkering and restoring, and he loves cruising on his Harley. But, his priorities in life are his God, his family, and serving in a caring profession.

Mark W. Sheppard, DDSRaised in the South, Dr. Sheppard received his DDS from Louisiana State University School of Dentistry in New Orleans (Class of 1994). Mark and his wife of 24 years, Melanie, have four children. He enjoys spending time with them, winter sports, gardening to name a few activities. He has enjoyed membership in the Intermountain

Odontologic Study Club, a chapter of Seattle Study Club, since its inception.

R. Kim Smith, DDSDr. Kim Smith was born and raised in Preston, Idaho. He graduated from Idaho State University with a B.S. in Zoology, and received his Dental Degree in 1988 from Creighton University in Omaha, Nebraska.

He has been practicing in Pocatello for the past 20 years. Dr. Smith is a member of the American Dental

Association, Idaho State Dental Association, and has served as the President of the Southeast Idaho Dental Society. He is also a member of the Eagle Rock Study Club.

Dr. Smith is married to the former Dana Tueller, who operates Dana Smith Dance Studio. He enjoys all outdoor activities such as water and snow skiing, golfing, and fly-fishing. He is the proud father of one son and three lovely daughters, Josh, Whitney, Sierra and Chyna; and two grandchildren

Dr. Richard E. Ferguson has provided compassionate and highly qualified periodontal care to the Treasure Valley area since 1982.

Dr. Ferguson is a native Idahoan, growing up in Idaho Falls. He attended the University of Utah where he earned a Bachelor of Science degree in biology. Dr. Ferguson earned his Doctor of Dental

Surgery degree (D.D.S.) from the University of Michigan in 1978. Postgraduate training allowed Dr. Ferguson to develop greater expertise in periodontal treatment and he received his Masters of Science degree (M.S.) from the University of Michigan in 1982.

Continuing education is a priority for Dr. Ferguson and his staff, thus enabling the practice to stay at the forefront of periodontics. Dr. Ferguson provides optimal care for his patients, including dental implants, cosmetic dental surgery and both non-surgical and surgical periodontal therapy.

Memberships in many professional organizations provide Dr. Ferguson with the means to influence developments in his field, and improve patient care. He is a member of the American Dental Association, the American Academy of Periodontology, the Western Society of Periodontology, and the Intermountain Society of Periodontology. In 2010 he became a Fellow of the International College of Dentists. Dr. Ferguson has also been an active member of the Idaho State Dental Association, serving as President for the 2009-2010 year, and serving on the Idaho Dental Foundation Board beginning in 2011.

Dr. Ferguson lives in Boise with his wife Deborah. They have four children, Matthew, Katie, John, and Daniel. When Dr. Ferguson is not practicing periodontics, he enjoys many Idaho outdoor activities, especially skiing.

Dr. Tim Thompson has practiced General Dentistry in Twin Falls for the past 27 years.

An avowed Oregon State Beaver alum, he received his B.S. degree in General Science from OSU in 1977.

Dr. Thompson has been active in dentistry and dental governance since his dental school days at Oregon Health Sciences University School of Dentistry. He served

as Student Body President, as well as Executive Vice President of the American Student Dental Association during his senior year at OHSU, and received his Doctor of Dental Medicine degree in 1977.

He held the local leadership offices in the South Central Dental Society from 1986-89, followed by a term on the ISU School of Dental Hygiene Advisory Board. He was 11th District representative to the ADA Commission on the Young Professional from 1990-94, and was the SCIDS Trustee to the ISDA Board of Trustees during the same time.

He was elected as one of Idaho’s delegates to the ADA House of Delegates in 1996, where he served three terms on behalf of the ISDA. He feels that his tenure as an ADA Delegate was one of the most fulfilling duties he has had in his experiences in organized dentistry, and he values the opportunity to serve and represent the interests of Idaho dentists to the ADA House very much.

He credits the mentoring of Dr. Steve Bruce, Dr. Jim Robson, Dr. Riley Cutler, and Dr. Scott Kido for their example in helping him find his niche in serving dentistry.

He became Editor of the ISDA Newsletter several years ago, serving in that capacity until 2008.

Most recently, he has gone through the chairs of the ISDA, serving as ISDA President from 2007-08, which he says was “no small challenge….., ” in view of the time of transition experienced by the ISDA during that time

He enjoys the usual “dentist” hobbies of golf and skiing (no more), but especially enjoyed racing stock cars as the “Spindoctor” at the Magic Valley Speedway for four years in the 1990’s. His latest need for speed is in the air, where he is completing his training for his private pilot’s license.

He says, “I am very honored to serve and be honored by Idaho dentists, who I think are some of the best in the country. They value and respect their patients and serve them well. They especially value the need for high standards of professionalism and ethics to be maintained. The solid history of the ISDA is at the core of our profession. If we do not stand together and determine our destiny, it will be determined for us.”

He and his wife/hygienist Sylvia practice together, and share six children and five (soon to be seven) grandchildren. Together, they savor the challenges, blessings, and anticipation every day brings, and would like to thank you all for this honor and the opportunity to serve in this wonderful profession, which has been so good to us all.

Dr. Kim Keller grew up in Nampa, falling in love with the Treasure Valley. He received his undergraduate degree in psychology from Brigham Young University in Provo, Utah, and completed four years of dental training at the UCLA School of Dentistry.

During his studies at UCLA, Dr. Keller met his wife, Shereee, while she was working as an electrical engineer in Southern California. They are blessed with three growing children; Preston, Macy, and Shepard.

After completing his dental degree at UCLA in 1993, he returned to Nampa where he began working with Dr. Marriner Bingham. After Dr. Bingham’s retirement, he continued to work with his staff to serve the dental needs of his patients.

Dr. Keller has been active in the community and profession he loves, serving as a Trustee on the Nampa Library Board (2008 – Present), a Nampa Schools Foundation Board Member (2010 – Present), Idaho Dental Director for DentaQuest (2011 – Present), Idaho Dental Foundation Board Member (2011 – Present), and served as the Western Treasure Valley Dental Society President (2011 – 2012).

His professional mission statement is to constantly strive to give his patients quality care with a personal touch, with an emphasis in preventive, restorative, and aesthetic dentistry. His number one goal is to prevent dental pain.

Course Credit

The number of continuing education credits for each course is equal to the duration of the course in hours. CE completion certificates will be made available online at www.TheISDA.org after the session. It is the participant’s responsibility to verify the requirements of his or her state licensing board and to self-report any CE credits earned.

Course Description With the increased emphasis on interdisciplinary treatment in recent years, the deficiencies associated with traditional methods of diagnosis and treatment planning have become more evident and problematic. Historically, the treatment plan was primarily dictated by information provided by study casts which were mounted on a sophisticated articulator in centric relation. At that time in history, the primary tools available for treating the complex restorative patient were functional crown lengthening surgery and increasing the vertical dimension of occlusion. The treatment plan was simply based on restorative space, anterior tooth coupling and resistance and retention form of the final preparations, with no focus on placing the teeth in the correct position in the face. Practitioners did not have access to advanced periodontal, orthodontic and orthognathic surgical tools that are currently available. With the advent and common usage of these new treatment modalities, the historical method of diagnosis and treatment planning is no longer adequately serving our profession. It is the purpose of this course to provide a systematic approach to diagnosis and treatment planning with a common language that may be used by the orthodontist, periodontist, and oral and maxillofacial surgeon, as well as the restorative dentist.

About the Speaker J. William Robbins, DDS., MA, maintains a full-time private practice and is Clinical Professor in the Department of General Dentistry at the University of Texas Health Science Center at San Antonio Dental School. He graduated from the University of Tennessee Dental School in 1973. He completed a rotating internship at the Veterans Administration Hospital in Leavenworth, Kansas and a 2-year General Practice Residency at the V.A. Hospital in San Diego, California. Dr. Robbins has published over 80 articles, abstracts, and chapters on a wide range of dental subjects and has lectured in the Untied States, Canada, Mexico, South America, Europe, Middle East and Africa. He coauthored a textbook, Fundamentals of Operative Dentistry – A Contemporary Approach, which is published by Quintessence, and is in its 3rd edition. He has won several awards including the Presidential Teaching Award at the University Of Texas Health Science Center, the 2002 Texas Dentist of the Year Award, and the 2003 Honorary Thaddeus V. Weclew Fellowship Award from the Academy of General Dentistry, and the 2010 Saul Schluger Award given by the Seattle Study Club. He is a diplomate of the Federal Services Board of General Dentistry and the American Board of General Dentistry and is a past president of the American Board of General Dentistry. He is a member of several organizations including the American College of Dentists, the International College of Dentists, the American Dental Association, the American Academy of Restorative Dentistry, the Southwest Academy of Restorative Dentistry, the Academy of General Dentistry, and the Academy of Operative.

Course Description When we think of conflict, we typically think, “what can I do to avoid it?” The reality is, we can’t avoid conflict; it is part of everyday interaction. Contrary to our natural instinct to run away from conflict, or avoid it at all costs, conflict is critically important and it can be quite beneficial. As one of the pioneer generation of acupuncturists in the US, she has cared for patients for several years, in China, Germany, California, and Idaho. Hahn served as a Commissioner on the Idaho Board of Acupuncture between 1999 and 2005. November 2005, she received a Commendation for Distinguished Public service from Idaho’s Governor. She currently serves as a subject matter expert on the National Certification Commission for Acupuncture and Oriental Medicine.

TMJ Disorders: They are Common and They are Treatable; Practical Guidelines for Managing These Patients in Your Own Office

Course Description Nearly one in three patients present with signs or symptoms related to TMJ disorders, but only about one in ten of these patients require clinical intervention. The short-term focus is to control pain and dysfunction, and with the proper diagnosis this can be accomplished in a predictable and practical manner. The long-term treatment objective is to prevent recurrence or progression by identifying the perpetuating factors. If the dental occlusion is a perpetuating factor, care must be taken to establish the stability of the temporomandibular joints before proceeding with occlusal therapy.

This private practice experience has enabled him to develop a PRACTICAL APPROACH TO MANANGING TMJ DISORDERS that can be implemented in every dental office. Dr. Howard has published in numerous dental and medical journals and has contributed several textbook chapters. Dr. Howard is the Director of the Temporomandibular Joint Disorders Clinic in the Department of Dental Medicine at Children’s Hospital in Seattle, where he has been on the medical staff since 1979. While living in Seattle he was an Associate Professor in the Department of Orthodontics at the University of Washington School of Dentistry and is currently a faculty member in the Department of Pediatric Dentistry. Mr. McAllister also has taught continuing legal education seminars for attorneys on issues within his practice, including insurance bad faith litigation.

Melissa Sanchez began her professional liability insurance career as a medical malpractice claims representative; working with hospitals, medical professionals and defense attorneys. She then went to work for a prominent Pacific Northwest medical and dental malpractice carrier for 17 years as a senior marketing representative. Currently she is the Manager for NORDIC, providing professional liability insurance and risk management services to dentists in the northwest.
