Water and sewer line directional bores and reconnections for utility avoidance. a. Since this work is bored underground, only pictures of final reconnection locations are shown. b. Water bores were installed per the Engineer of Record, Cook-Coggins, Jason Nix, PE. i. Cook-Coggins reviewed and approved the design and all materials supplied for the bore and reconnections. c. Sewer bore design and installation approvals were given by Jeff Foster. d. Testing results for pressure and/or bacterial samples passed for both lines. 2. Road closure and demolition of existing culverts. 3. Installation of box culvert stone base (6” of #57 limestone) 4. Installation of box culvert. 5. Select backfill around and above box culvert and road subgrade (Sand and B9 material). 6. Rip rap placement around box ends and slopes greater than 2’ horizontal to 1’ vertical. (100# stone). 7. Rip rap placement in creek channel (200# stone) 8. Final road base limestone placement (6”) 9. General backfill cover and grading for disturbed areas and side slopes with general grassing and erosion control blankets. Pictures taken during this inspection are attached for clarification. Contractor’s Activities/Observations: Construction during the timeframe of these daily observations included:

1. Water and sewer line directional bores and reconnections for utility avoidance. a. Since this work is bored underground, only pictures of final reconnection locations are

shown. b. Water bores were installed per the Engineer of Record, Cook-Coggins, Jason Nix, PE.

i. Cook-Coggins reviewed and approved the design and all materials supplied for the bore and reconnections.

c. Sewer bore design and installation approvals were given by Jeff Foster. d. Testing results for pressure and/or bacterial samples passed for both lines.

2. Road closure and demolition of existing culverts. 3. Installation of box culvert stone base (6” of #57 limestone) 4. Installation of box culvert. 5. Photo 1 – Existing culvert removed (in upper right of background) and grading/clearing being

performed for uniform subgrade to receive #57 limestone for box culvert.

Photo 2 – Six inches of #57 stone placed, compacted, and grade check to meet plan/existing creek

grades. Stone appeared to interlock and stand up well to subgrade base during compaction efforts.

Photo 11 – Geofabric wrapping at each buried joint to ensure joints are soil tight. Each joint also

received elastomeric seal inside the male-female joint to ensure soil tight connection. Due to numerous previous days of rain, the subgrade next to the box was saturated, so the Engineer

approved the use of sand as a bridge lift to get out of the wet conditions while providing a stable compactable backfill.

Rip rap and erosion control should be installed as a minimum until final grassing is established. It is recommended to have some rip rap at the culvert exit to dissipate and spread out flows going
