History and Position

Founded in 1984, the Bromley by Bow Centre is a pioneering charity located in the East End of London with an annual turnover of circa £3.5 million and a dedicated & passionate team of around 140 people plus volunteers.

In 2000 the Centre acquired the adjoining three acre park and, on a portion of the land, built the first Healthy Living Centre in the UK. This was an important evolution in the Centre’s model, moving us into primary care. Now the Centre and the medical practice operate together in a highly integrated manner unusually providing a holistic and blended mix of services, although with distinct and separate management and governance structures.

The Centre now has a national and international reputation for its innovative regeneration work in the community. We operate in a location with entrenched economic and social problems. This includes having one of the highest levels of child poverty in the UK, the highest acute mental health admission rate in London and the second lowest employment rate in London; 34% of residents do not have English as their first language compared to 22% in London and 8% nationally.

The Centre’s reputation means that it regularly hosts high profile visits which have included the Royal Family (HRH The Prince of Wales visited us in February 2012), Prime Ministers, Secretaries of State, other Ministers and members of the House of Lords, senior Civil Servants and Business Leaders. In January 2013 Lord Brian Mawhinney endorsed our work during a speech in the House of Lords:

“I urge you to take your colleagues down to the Bromley by Bow Centre and let them see what has emerged from nothing. It is one of the most impressive displays of social entrepreneurship anywhere in Europe.”

Service Delivery Programmes

50+ projects are delivered through six Programme areas:

Community Connections responds to the needs of people who are on a longer, or less specified journey with the Centre or who are perhaps engaging with our services for the first time. The Programme integrates people into the Centre and its associated services. It is designed to increase knowledge and confidence as well as creating involvement in networking and volunteering programmes that assist with community cohesion and integration.

My Life is the Centre’s health and wellbeing programme it focuses on people who are labelled or seen as more vulnerable, significantly residents with physical, mental, sensory, learning and complex disabilities and health conditions. Last year we supported over 250 older people and adults with mental health problems, learning difficulties and physical disabilities, while assisting over 2,600 people to improve their diet, give up smoking and lose weight.

Advice Centre helps over 1,000 people annually to manage their household finances, claim welfare benefits and improve their housing. Debt intervention and energy awareness support is used to give people the control they need over money.

Beyond Business is an award-winning programme that launches and nurtures new social enterprises across Tower Hamlets, Newham and Hackney, providing practical support and advice to ensure their success in the crucial early years of trading. The Programme has a successful track record, celebrating over 40 new social enterprises, generating a combined turnover in excess of £4m and creating over 250 new jobs.

Local People Local Jobs is a responsive employment service that offers a range of intervention and support programmes. Over 500 people a year are supported to overcome barriers to work, find jobs and access training. Our accredited advisor team deliver careers information, advice and guidance across a range of venues.

Capital Skills responds to employer’s needs while upskilling employed and unemployed people through the Centre’s City & Guilds accredited vocational delivery. This includes apprenticeships across Health and Social Care, Business Administration, Customer Service and much more. Courses are also available for entry-level English and basic IT to family learning.

PartnershipsThe Centre has an expansive range of partnership and networking relationships that include Training Providers, Community Groups, Housing Associations, Children’s Centres, Schools and Health Centres.

Centre SupportThe Service Delivery Programmes are supported by a specialist team comprising of Development & External Communications, HR & Personnel, Finance & Accounting, Facilities and Health & Safety, Information Communications &Technology and Hospitality.

Organisation and Management The governing body is a volunteer Board of Trustees, elected by the charity’s membership, who oversee the work of the Senior Management Team, offer advice on direction and strategy, ensure that the Bromley by Bow Centre complies with its legal and reporting obligations, and carries ultimate responsibility for the charity’s financial wellbeing.

The Board comprises of a minimum of 6 and maximum of 23 trustees and includes up to 6 representatives from key collaborating stakeholders. The Board principally operates through three specialist sub-committees.

Responsibility for day to day operations is delegated to the CEO, Rob Trimble, and the Senior Management Team comprising of five Directors with defined operational responsibilities.

Working Environment The culture of the Bromley by Bow Centre is an important component of our success. An important influence is the series of interconnected buildings set in an award winning park in Bromley by Bow. Social prescribing enables primary care services to have a meaningful impact on the social determinants of health. The service provides a pathway mechanism enabling the prescriber to connect with a wide range of services to support client’s health and wellbeing.

Social Prescriber The Bromley by Bow Centre is a pioneering charity located in an award winning three-acre park close to the Olympic complex in the East End, an increasingly vibrant area of London undergoing wide-ranging regeneration. The Social Prescriber Service is an important element of Bromley by Bow Centre’s service delivery, by addressing a range of health, social care and welfare issues the role supports local residents to improve their health and wellbeing.

The role will build relationships with a range of stakeholders, including GP Practices, health professionals and community partners, ensuring the service remains a vital and visible community resource. The successful applicant must have experience of delivering client-focused services, working with local GP surgeries and other health professionals, good communication skills and a genuine interest in helping people to tackle the challenges they face in leading healthy and fulfilling lives. This is a full time position, 35 hours per week, offering a salary in the range of £26,350-£29,350, 30 days’ annual holiday (pro rata) and a workplace pension. You will be joining a dedicated and diverse team of around 120 staff and volunteers, all of whom helped to elevate the Bromley by Bow Centre into The Sunday Times 100 Best Not-For-Profit Organisations to Work for 2014. We are five minutes’ walk from the tube and DLR, with access to major bus routes.

Job Title: Social Prescriber Coordinator

Job Grade: £26,350 - £29,350 per annum

Location: Bromley by Bow Centre

PRIME RESPONSIBILITIES

The role will be responsible for the management and operational delivery of the Social Prescriber project, providing a referral system for GPs and other Healthcare professionals in Tower Hamlets. Working across 6 GP practices in Mile End east Bromley By Bow Network, the role will require supporting clients to access a range of health, social care, welfare advice and other support services. The role will require proactive engagement with healthcare professionals to stimulate referrals into the programme. Clients will be supported on an individual basis to set goals and priorities to improve their health and wellbeing.

DETAILED RESPONSIBILITIES

A. Service Delivery

Act as the central point of contact for all referrals to Social Prescriber service

Provide information, advice and guidance to referred client, including signposting where appropriate

Undertake initial assessments; jointly identify goals and develop personalised plans with clients

Develop and maintain a client database

Work with health professionals to implement feedback mechanisms

Extend and develop the menu of services prescribed in the form of a directory

Liaise with health professionals to increase visibility of the project and to ensure information is up to date and relevant

As for all applicants, disabled candidates who meet the minimum criteria for a post are eligible for interview for the role.
