Get Set is a multi-foundation initiative funded by Health Foundation of West-

ern and Central NY, John Oishei Foundation and Peter and Elizabeth Tower Foundation.

Designed to help selected behavioral health, health and human services or-

ganizations in Western New York strengthen their operational infrastructure in order to ensure high quality, affordable services to the vulnerable populations they serve.

BPPN was selected in the 2nd cohort of 6 agencies to participate in Get Set

and began the process in August 2015.

The process afforded us the expertise of 2 Organizational Development con-

sultants from Rochester, Laurie Allan and Carl Hergesell for a two year peri-od. A multilevel workgroup was formed at the Agency comprised of manag-ers, supervisors, line staff, support staff and Board members.

The management team and others began work on a capacity building plan

which covered 8 areas: Mission and Vision, Governance, Environmental Dy-namics, Leadership and Management, Organizational Infrastructure, Commu-nication and Branding, Data-Driven Decision-Making/Planning and Financial Stability/Acuity. The other deliverable for the project is the development of a Value Proposition-an “elevator speech“ for the agency.

There have also been educational sessions offered to the team which includ-

ed Data and Dashboards, Emotional Intelligence and Developing a Value Proposition.

The 13th

Annual “Great Baby Beginnings” was held on July 20th at a new location-Templeton Landing!

Over 150 people attended the event which featured basket auctions, raffles and a live auction. The 2 awardees this year were Dr. Gail Burstein, Erie County Department of Health Commissioner and Mary Powers RN, NNP-BC Neona-tal Outreach Coordinator at Women and Children’s Hospital of Buffalo, Kaleida Health. The event also featured one of our success stories when Ebony Patterson-White spoke about her experience as a 17 year old first time mom in our Home Visiting Program. Ebony spoke about the support she received from her worker Anna Baugh and how she has succeeded since then. The event raised $34,000 and these funds are used for items that our funders do not cover and include such basics as mattresses and box springs for our clients, trainings for our staff and management team including Trauma Informed Care, Compassion Fatigue and Anti-harassment Policy and membership in HR & Non-for-Profit Organization. Attendees were reminded that 90% of donor dollars are used for our programs. Thank you to all that attended and hope to see you next year!

Buffalo Prenatal Perinatal Network MICHC Program

was awarded a grant from the Millennium Performing Provider System (PPS) which is composed of Kaleida and ECMC. The funding is from NYS as part of the Delivery System Reform Incentive Payment Program (DSRIP). The overall goal of DSRIP is to reduce avoidable hospital stays by

25% over 5 years. The projects in DSRIP fall under 3 general categories: system transformation, clinical improvement and population health. BPPN had been an active part of the planning group since 2014 for the maternal child projects with Millennium. Based on a community needs assessment, the MICHC model utilizing Community Health Workers (CHW) was adopted and a work plan developed. The program began in April 2015 and we are reimbursed on a monthly basis on a value based system which includes payments for enrollments, home visits, enrollments in prenatal care, smoking referrals and counseling and timely well child and postpartum visits. The CHW’s continue their street outreach as well as connecting with other community agencies. We are excited to be part of this new initiative and believe our experience over the next few years will better position us to begin negotiations with insurance companies.

Buffalo Prenatal-Perinatal Network, Inc. is a not-for-profit organization

that has been in operation for 26 years. The overall mission of the organization is to coordinate and collaborate with key stakeholders in Erie County/City of Buffalo, to increase access to and utilization of preconception, prenatal, perinatal and postpartum health care services in the community. The agency and its programs work within the community to help identify problems of adequacy, acceptability and accessibility in the delivery of these services, especially to those who are in high-need; to seek support for implementation of programs/services that help attain the agency mission; promote coordination between service providers who serve similar consumers/clients; and to provide informational and educational activities for providers and consumers, to help enable improved birth outcomes for the tar-geted population. The programs operated by Buffalo Prenatal focus on improving maternal and infant health outcomes for high need women and their families, and to help reduce racial and ethnic disparities in those outcomes. Programs operated by Buffalo Prenatal include a New York State Department of Health Maternal Infant Community Health Collaborative (MICHC), a Healthy Families New York Program (also known as the Buffalo Home Visiting Program).
